Development of Manual Multi-Leaf Collimator for Proton Therapy in National Cancer Center (국립암센터의 양성자 치료를 위한 수동형 다엽 콜리메이터 개발)

  • Multi-leaf collimator (MLC) systems are frequently used to deliver photon-based radiation, and allow conformal shaping of treatment beams. Many proton beam centers currently make use of aperture and snout systems, which involve use of a snout to shape and focus the proton beam, a brass aperture to modify field shape, and an acrylic compensator to modulate depth. However, it needs a lot of time and cost of preparing treatment, therefore, we developed the manual MLC for solving this problem. This study was carried out with the intent of designing an MLC system as an alternative to an aperture block system. Radio-activation and dose due to primary proton beam leakage and the presence of secondary neutrons were taken into account during these iterations. Analytical calculations were used to study the effects of leaf material on activation. We have fabricated tray model for adoption with a wobbling snout ($30{\times}40cm^2$) system which used uniform scanning beam. We designed the manual MLC and tray and can reduce the cost and time for treatment. After leakage test of new tray, we upgrade the tray with brass and made the safety tool. First, we have tested the radio-activation with usually brass and new brass for new manual MLC. It shows similar behavior and decay trend. In addition, we have measured the leakage test of a gantry with new tray and MLC tray, while we exposed the high energy with full modulation process on film dosimetry. The radiation leakage is less than 1%. From these results, we have developed the design of the tray and upgrade for safety. Through the radio-activation behavior, we figure out the proton beam leakage level of safety, where there detects the secondary particle, including neutron. After developing new design of the tray, it will be able to reduce the time and cost of proton treatment. Finally, we have applied in clinic test with original brass aperture and manual MLC and calculated the gamma index, 99.74% between them.

Comparison of Intensity-modulated Radiation Therapy (IMRT), Uniform Scanning Proton Therapy (USPT), and Intensity-modulated Proton Therapy (IMPT) for Prostate Cancer: A Treatment Planning Study (전립선 암 환자의 IMRT, USPT, 및 IMPT 기법에 따른 치료효과 비교)

  • This study assessed compared photon and proton treatment techniques, such as intensity modulated radiation therapy (IMRT), uniform scanning proton therapy (USPT), and intensity modulated proton therapy (IMPT), for a total of 10 prostate cancers. All treatment plans delivered 70 Gy to 95% of the planned target volume in 28 fractions. IMRT plans had 7 fields for the step and shoot technique, while USPT and IMPT plans employed two equally weighted, parallel-opposed lateral fields to deliver the prescribed dose to the planned target. Inverse planning was then incorporated to optimize IMPT. The homogeneity index (HI) and conformity index (CI) for the target and the normal tissue complication probability (NTCP) for organ at risk (OAR) were calculated. Although the mean HI and CI for target were not significantly different for each treatment techniques, the NTCP of the rectum was 2.233, 3.326, and 1.707 for IMRT, USPT, and IMPT, respectively. The NTCP of the bladder was 0.008, 0.003, and 0.002 respectively. The NTCP values at the rectum and bladder were significantly lower using IMPT. Our study shows that using proton therapy, particularly IMPT, to treat prostate cancer could be beneficial compared to 7-field IMRT with similar target coverage. Given these results, radiotherapy using protons, particularly optimized IMPT, is a worthwhile treatment option for prostate cancer.

  • There are various type of particle accelerators such as Kyoungju 100-MeV proton beam accelerator in Korea. And Korea plans to build large particle accelerator such as heavy ion accelerator and 4th generation light source facility. The accelerated high energy particles of these facility produce 2nd neutron after nuclear reaction with target materials. And then these 2nd neutron activate structural materials and surrounding environment. Accordingly, it is important to consider the activation and shielding calculation on design of facility for safety operation. In this study, we tried to calculate and compare the neutron flux from the interaction $^{la}150$ beam with target material(Cu) according to thickness of iron and concrete shielding material by MCNPX 2.7 with nuclear library JENDL/HE 07and la150. To verify the properties of nuclear library, we compared computational results with experimental value. These results can be used for dose evaluation technology in planning of the shielding of large particle accelerator.

  • Cellobiose dehydrogenase(CDH) as a hemoflavoenzyme is secreted out of cell in the cellulose degradation. As CDH strongly bound to amorphous cellulose, it helps cellulose hydrolysis by cellulase. CDH may have an important role of saccharification process for bioethanol production. In this study, Phanerochaete chrysosporium ATCC 32629 was selected for the production of CDH among other strains tested. The optimal temperature and pH of CDH produced by P. chrysosporium ATCC 32629 were ${55^{\circ}C}$ and 4, respectively. To improve the activity of CDH, the mutation of P. chrysosporium was performed using proton beam that has high energy level partially. As a result, P. chrysosporium mutant with the high activity was selected at 1.2 kGy in a range of 99.9% lethal rate. The CDH and $\beta$-glucosidase activities of mutant were 1.4 fold and 20 fold higher than those of wild strain. Therefore, P. chrysosporium mutant with the high activities of CDH and $\beta$-glucosidase was obtained from mutation by proton beam irradiation.

  • keV-neutron capture gamma-ray spectrum of $^{197}Au$(natural gold) sample have been measured in neutron energy range from 10 to 90 keV using the 3-MV pelletron accelerator of the Research Laboratory for Nuclear Reactors at the Tokyo Institute of Technology. Pulsed keV neutrons were produced from the $^7Li(p,n)^7Be$ reaction by bombarding on the $^7Li$ target with the 1.5-ns bunched proton beam. The incident neutron spectrum on the Au sample was measured by a $^6Li$-glass scintillation detector and TOF method. Capture gamma-rays from Au sample were measured by anti-Compton NaI(TI) spectrometer. Five average neutron energy regions were selected to obtain the neutron capture spectrum. Several gamma-ray peaks in the spectrum were found in the present experiment.

Evaluation of Residual Radioactivity and Dose Rate of a Target Assembly in an IBA Cyclotron (IBA 사이클로트론 표적집합체에서의 잔류 방사화 분석 및 선량률 평가)

  • When a cyclotron produces $^{18}F^-$, accelerated protons interact with metal parts of the cyclotron machine and induces radioactivity. Especially, the target window and chamber of the target assembly are the main parts where long-lived radionuclides are generated as they are incident by direct beams. It is of great importance to identify radionuclides induced in the target assembly for the safe operation and maintenance of a cyclotron facility. In this study, we analyzed major radionuclides generated in the target assembly by an operation of the Cyclotron 18/9 machine and measured dose rates after the operation to establish the radiation safety guideline for operators and maintenance personnel of the machine. Gamma spectroscopy with HPGe was performed on samples from the target chamber and Havar foil target window to identify the radionuclides generated during the operation for production of $^{18}F^-$- isotope and their specific activity. Also, the dose rates from the target were measured as a function of time after an operation. These data will help improve radiological safety of operating the cyclotron facilities.

  • Field Programmable Gate Array(FPGA)s are replacing traditional integrated circuits for space applications due to their lower development cost as well as reconfigurability. However, they are very sensitive to single event upset (SEU) caused by space radiation environment. In order to mitigate the SEU, on-board computer of STSAT-3 employed a triple modular redundancy(TMR) and scrubbing scheme. Experimental results showed that upset threshold energy was improved from 10.6 MeV to 20.3 MeV when the TMR and the scrubbing were applied to the on-board computer. Combining the experimental results with the orbit simulation results, calculated bit-flip rate of on-board computer is 1.23 bit-flips/day assuming in the worst case of STSAT-3 orbit.

  • Geant4 is compatible with the Windows operating system in C++ language use, enabling interface functions that link DICOM or software. It was simulated to address the basic structure of the simulation using Geant4/Gate code and to specifically verify the density composition and lung cancer process in the human phantom. It was visualized using the Gate Graphic System, i.e. openGL, Ray Tracer: Ray Tracing by Geant4 Tracing, and using Geant4/Gate code, lung cancer is modeled in the human phantom area in 3D, 4D to verify the simulation progress. Therefore, as a large number of new functions are added to the Gate Code, it is easy to implement accurate human structure and moving organs.

Research for Lateral Penumbra and Dose Distribution When Air Gap Changing in Proton Therapy Case (양성자치료시 Air Gap 변화에 따른 Lateral Penumbra와 선량분포 변화에 대한 비교 및 연구)

  • Purpose: In the treatment of high-energy protons Air gap (the distance between the patient and the exit Beam) Lateral Penumbra of the changes to the increase in the radiation fields can form unnecessary and Increase the maximum dose at the site of treatment and reduced the minimum dose homogeneity of dose distributions can decline. Air gap due to this change in dose distribution compared to investigate studied. Materials and Methods: Received proton therapy at our institution Lung, Liver patients were selected and the size of six other Air gap in Field A and Field B 2, 4, 6, 8, 10 cm Proton external beam planning system by setting up a treatment plan established. Air gap according to the Lateral Penumbra area and DVH (Dose Volume Histogram) to compare the maximum dose and minimum dose of PCTV areas were compared. In addition, the dose homogeneity within PCTV Homogeneity index to know the value and compared. Results: Air gap (2, 4, 6, 8, 10 cm) at each change in field size were analyzed according to the Lateral Penumbra region Field A Change in the Air gap 2~10 cm by 1.36~1.75 cm, the average continuously increased about 28.7% and Field B Change in the Air gap 2~10 cm by 1.36~1.75 cm, the average continuously increased about 31.6%. The result of DVH analysis for relative dose of the maximum dose According to Air gap 2~10 cm is the mean average of 110.3% from 108.1% to a sustained increased by approximately 2.03% and The average relative dose of minimum dose is the mean average of 93.9% percent to 90.8 percent from the continuous decrease of about 3.31 percent. The result of Homogeneity index value to the according to Air gap 2~10 cm is the 2-fold increase from 1.09 to 2.6. Conclusion: In proton therapy case, we can see the increasing of lateral penumbra area when airgap getting increase. And increasing of Dmax and decreasing Dmin in the field are making increase homogeneity index, So we can realize there are not so good homogeneity in the PCTV. Therefore we should try to minimize air gap in proton therapy case.

Comparison of Beam Delivery Modes in Prostate Cancer Proton Therapy: A Treatment Planning Comparison Study (전립선암 환자 양성자치료 시 빔 전달방식에 따른 치료계획 비교)

  • Purpose: After making two plans, the Double Scattering (DS) Mode and The Pencil Beam Scanning (PBS) Mode, of patients on early prostate cancer, we not only compare the dose conformity and the dose homogeneity by analyzing each DVH, CN and HI, but also evaluate normal structures's sparing effect on each mode. Materials and Methods: Planes about nine patients, who did proton therapy, on prostate cancer was setted using the Eclipse proton external beam planning system. The prescription dose, every $2.5 Gy{\times}28$ fractions=70 Gy, was delivered to the PTV. The CN and the HI were getted by anlazing each DVHs for the DS Plan and the PBS Plan. Also, normal structures' %volumes according to dose of the PBS are campared with those of the DS. Results: The average CN of the PTV is increase 16.63% from DS $0.68{\pm}0.07$ to PBS $0.79{\pm}0.01$, and the average IN of the PTV is decrease -22.66 % from DS $0.12{\pm}0.03$ to PBS $0.09{\pm}0.01$. The PBS has litter %Volumes of normal structures than the DS about every patient except Rectum. The average %Volume of Left Femoral Head receiving ${\geq}30$ Gy shows most high decreasing rate, -79.93%, from DS to PBS and the average %Volume of Rectum receiving ${\geq}70$ Gy shows most low decreasing rate, -3.03%, from DS to PBS. Conclusion: Therefore, the PBS is more effective achieving the dose conformity and the dose Homogeneity than DS, and better to reduce unnecessary dose arriving normal structures, especially the femoral heads.
