STUDY ON THE GROWTH OF THE MANDIBLE USING WIDE OPEN LATERAL CEPHALOGRAM (Wide open lateral cephalogram을 이용한 하악골 성장에 관한 연구)

  • In proceeding with orthodontic treatment, the prediction for the shape, growth rate and growth direction of mandible plays a major role to set up the treatment plan and determine its period and prognosis. Various approaches being made so far have shown that the linear and angular measurement using lateral cephalograms are relatively accurate to estimate them. This study was purposed to find the shape of mandible more clearly by preventing the overlap of the Condyle head area which appears in lateral cephalogram, and to estimate its growth rate by comparing the growth quantity and ratio via lateral area measurement. This experimental was performed against 40 patients total, of which Class I of 14, Class II of 9 and Class III of 17 consist. Wide open lateral cephalograms of 40 patients were taken over average period of 4 Year 3 Months, then the linear and angular measurements were carried out with 11 itemized lists. Autocad Rl4 application program was utilized to draw their appearance, measure and compare their lateral area. As a result of study, conclusions were made as follows; 1. Mandibular body length (gonion-menton) tended to increase in order of CIII, CI and CII, and Mandibular body length of CIII group had a tendency to grow twice faster than that of CII group. 2. In lateral items such as Go-Me, A-Cd, B-Cp, E-F and G-H, CIII showed a significant increase on the year-average quantity and rate of the growth, and especially apparent difference was observed in CIII group rather than CII group. 3. For the 4 Year 3 Months period, the year-average growth quantity of lateral area of the mandible was $1.0cm^2$ for Class I, $0.8cm^2$ for Class II and $1.4cm^2$ for Class III, which corresponds to $11.9\%,\;11.8\%\;and\;20.3\%$ of growth ratio respectively. Thus, growth ratio almost 2 times more than other groups was observed in group CIII while growth ratio between group CI and CII has little difference. 4. Considering the results as above, it can be proposed that the difference in size of the mandible between groups is caused by the difference in the growth rate and growth quantity of the mandible, which generated in the middle of growth, rather than the difference in size of congenital Jaw-bone.

Analysis of the hereditary factor in craniofacial morphology and fingerprints in Class III malocclusion (III급 부정교합에서 두개안면 형태와 지문의 유전성향 분석)

  • In growing patients with Class III malocclusion, the various patterns of maxillofacial growth are a key element that affects the success or failure of treatment. Therefore it is important to correctly predict maxillofacial growth before initiating treatment. The purpose of this study was to find out the correlation between the maxillofacial morphology of parents and their Class III children by analyzing lateral cephalograms and hereditary factors. Among Class III preadolescent children, 50 families were obtained. To find out the specific hereditary factors involved, fingerprints were obtained and genetic correlation with the maxillofacial morphology was analyzed. The following conclusions were made. 1. A significant correlation (P<0.05-0.00l) was found in many of the cephalometric measurements between the offspring and their parents. The correlation in the skeleton measurements was higher than in the denture measurements. The father-offspring correlation was higher than the mother-offspring correlation 2. A significant correlation (P<0.05-0.00l) was found in fingerprint units between the offspring and their parents. The mother-offspring correlation was higher than the father-offspring correlation. 3. Between the maxillofacial morphology and fingerprint units, there was significant genetic correlation (P<0.05-0.01). Based on the analysis of genetic correlation, higher correlation was found in the parent-son pairing than the parent-daughter pairing.

Reproducibility of asymmetry measurements of the mandible in three-dimensional CT imaging (전산화단층사진을 이용한 하악골 3차원 영상에서 비대칭진단 계측항목의 재현도에 관한 연구)

  • Objective: The purpose of this study was to evaluate the reproducibility of measurements representing asymmetry of the mandible and to identify which landmarks would be more useful in 3-dimensional (3D) CT imaging. Methods: Facial CT images were obtained from forty normal occlusion individuals. Eighteen landmarks were established from the condyle, gonion, and menton areas, and 25 measurements were constructed to represent asymmetry of the mandible; 8 for ramus length, 12 for mandibular body length, 1 for condylar neck length, 2 for frontal ramal inclination, and 2 for lateral ramal inclination. Inter- and intra-examiner reproducibility of the measurements was evaluated. Results: Inter-examiner reproducibility of the measurements proved to be high except for 3 measurements. Intra-examiner reproducibility also proved to be high except for 2 measurements. Inter- and intra-examiner reproducibility of the measurements including Gonion proved to be low. Conclusions: The results of the present study indicate that the landmarks and measurements constructed in 3D CT images can be used for the diagnosis of facial asymmetry.

Building a Robust 3D Statistical Shape Model of the Mandible (견고한 3차원 하악골 통계 형상 모델 생성)

  • In this paper, we propose a method for construction of robust 3D statistical shape model in the mandible CT datasets. Our method consists of following four steps. First, we decompose a 3D input shape Into patches. Second, to generate a corresponding shape of a floating shape, all shapes in the training set are parameterized onto a disk similar to the patch topology. Third, we generate the corresponding shape by one-to-one mapping between the reference and the floating shapes. We solve the problem failed to generate the corresponding points near the patch boundary Finally, the corresponding shapes are aligned with the reference shape. Then statistical shape model is generated by principle component analysis. To evaluate the accuracy of our 3D statistical shape model of the mandible, we perform visual inspection and similarity measure using average distance difference between the floating and the corresponding shapes. In addition, we measure the compactness of statistical shape model using the modes of variation. Experimental results show that our 3D statistical shape model generated by the mandible CT datasets with various characteristics has a high similarity between the floating and corresponding shapes and is represented by the small number of modes.

  • For af accurate diagnosis and treatment planning of facial asymmetry, the use of 3-dimensional (3-D) image is indispensable. The purpose of this study was to get standard data for the 3-D analysis of facial asymmetry Computerized tomography (CT) was taken in the 60 normal occlusion individuals (30 male. 30 female) who did not have any apparent facial asymmetry. The acquired 2D CT DICOM data were input on a computer, and the reformatted 3-D images were created using a 3-D image software. Twenty three measurements were established in order to evaluate asymmetry; 15 ;omear measurements (6 for ramus length. 1 for condylar neck length, and 8 for mandibular body length) and 8 angular measurements (4 for gonial angle. 2 for frontal ramal inclination. and 2 for lateral ramal inclination) The right aid left difference of each measurement was calculated and analyzed. It is suggested that the right and left differences of the measurements obtained from the study could be used as references for the diagnosis of facial asymmetric patients.

STUDY OF CRANIOCERVICAL POSTURE AND CRANIOFACIAL MORPHOLOGY IN KOREAN YOUNG ADULTS (한국 성인의 두경부자세와 두개안면형태에 관한 두부 X-선 계측학적 연구)

  • The purpose of this study was to estimate correlations of craniocervical posture and craniofacial morphology in Korean young adults. The sample consisted of 50 young adults (25 males and 25 females) who had good profile and Class I molar relationship. The analysis of craniocervical posture and craniofacial morphology was performed on lateral cephalograms taken in natural head position. The results were as follows, 1. The mean and the standard deviation of postural and morphologic variables were obtained. 2. Korean young adult had cervical lordosis of which degree between OPT and CVT shows $3.55{\pm}2.58^{\circ}$ 3. Craniocervical posture and each of vertical ratio, facial prognathism, mandibular rotation showed high correlation. 4. Correlation coefficients between postural variables and each of intermaxillary relation, anteroposterior ratio were low. 5. The head positioning error of natural head position was smaller than the inter-individual variability of postural variables.

  • The whitespotted puffer, Arothron hispidus (Linnaeus, 1785), of the family Tetraodontidae were collected by an Koheung Peninsula in July 2003. This species is color of the body; covered with black and the spots on the body is white color. white. Dorsal fin rays 11~12; pectoral fin rays 17; anal fin rays 10~11; caudal fin rays 5+6=11; vertebrae 8+10=18. Measurement for five specimen (195.0~265.3 mm SL) in hundredths of standard length; head length 36.8%; pectoral fin length 13.4%; dorsal fin length 11.6%; caudal peduncle length 15.0%; caudal peduncle depth 13.7%. This is ventral postclavicle, exposed in hundredths of length 6.14~6.75; verterbrae 18; dorsal pterygiophores formula $7{\ast}1231$; number of dorsal pterygiophores 7.

  • Chin is located on most prominent portion of the face, so make more important esthetic value in impression. So historicaly, many authors had made efforts on correction of deformed chin, termed genioplasty. But those technique have been performed on limited cases, which have normal shape or width of chin. In the cases with more narrow or short chin, location of mental foramen or root of incisors restrict the use of such technique. But modified genioplasty technique we used can be performed without any restriction and change of frontal profile of chin. So we report a case that was performed with modified horizontal step osteotomy for advancement genioplasty, which had been reported by Hinds and Kent, with some complications.

  • Burkitt's lymphoma is a malignant tumor originated from B lymphocyte and a type of non-hodgkin lymphoma. It is clinically classified by endemic, sporadic and immune deficient forms. In the past, it had poor prognosis but in recent years survival rate is 50~80% treated by intensive chemotherapeutic regimens. Mostly it is a lesion of the jaws. Clinical symptoms are severe tooth mobility, displacement, gingival redness and swelling. This case is about a patient with aggressively progressing Burkitt's lymphoma of the jaws. Initial diagnosis was done in dental hospital and it has shown satisfactory outcome by prompt and appropriate treatment.

  • Odontoma, hamartomas of odontogenic orgin, are composed of all the structures that make up teeth, which may lead to interference with the eruption of its associated tooth. On the basis of gross, radiographic, and microscopic features, two types of odontoma are recognized: compound and complex. The etilogy of odontomas is unknown, although local trauma, infection, and gentic factors have been suggested. Odontomas occur central in bone between the roots of teeth and the mauority are asymptomatic. Although these tumors occur frequently and constitute 22% of all odontogenic tumors, are very rare. Erupted odontoma are defined as tumors that it occurs a calcifed mass may be defected on the ridge. The treatment of chico is the surgical removal of the lesion followed by a biopsy. This report presents a case of 8-year-old girl with the delayed eruption of the mandibular first molar by the calcified mass within the operculum covered the central fossa. And it was diagnosed as erupted complex odontoma by excisional biopsy.
