Analysis of Articles published in the Korean Journal of Thoracic and Cardiovascular Surgery - Use of personal computer in storage and inquiry of article data - (대한 흉부외과학회지에 발표된 논문의 분석 고찰 - 개인용 콤퓨터를 이용한 논문자료의 저장과 조회의 실례 -)

  • Authors designed a computer-based article data management system using a 16 bit IBM personal computer and dBASE IV program and applied it to the management of article data of the Korean Journal of Thoracic and Cardiovascular Surgery. In this system, authors developed a coding system for systemic classification of form and subject of articles and this coding system made the storage and inquiry of data easy and convenient. Using the coding system in this article management system, total 1476 sheets of articles in the Korean Journal of Thoracic and Cardiovascular Surgery were analyzed and briefly described according to their forms and subjects. If this article data management system including the coding system is applied to other Journals related to the department of thoracic and cardiovascular surgery, we can utilize them easily and conveniently for the save of time and effort in the data inquiry or article preparation.

Descending Thoracic Aorto-bifemoral Artery Bypass Grafting in a Leriche's Syndrome (Leriche's 증후군에서 흉부하행대동맥-양측 대퇴동맥 우회술)

  • Extra-anatomic graft bypass is frequently performed instead of standard infrarenal aorto-iliac reconstruction in patients with Leriche syndrome in whom the thrombus extends to the level of the renal arteries. However, many different surgical options are still being attempted due to the unsatisfying long-term graft patency. We performed a descending thoracic aorto-bifemoral bypass graft with 14 and 14-7-7 mm artificial vessels through a posterolateral thoracotomy, a median laparotomy, and a longitudinal inguinal incision in a 48-year-old male who suffered from claudication with Leriche syndrome. After surgery, the patient recovered well and was discharged. The patient walked well without any symptoms during the 6 month follow-up period in the outpatient department. We have concluded that descending thoracic aorto-bifemoral bypass grafting could be considered as an alternative method for patients with Leriche syndrome in whom standard infrarenal aorto-iliac reconstruction is unsuitable.

  • This study was undertaken to evaluate current technique of extracorporeal circulation by analyzing the results in 73 cases. We performed standard cardiopulmonary bypass using Polystan roller pump, bubble[Polystan] and membrane[Cobe] oxygenator. There were 42 males and 31 females. We studied the changes of hemoglobin. hemodynamics, speed of cooling and warming, methods of cannulation and ACT. Arterial cannula was inserted on ascending aorta except for 7 cases of femoral cannulation. Preoperative hemoglobin was 13.2 gm% and this value dropped to 7.5 gm% during perfusion. Blood pressure of 113 mmHg in systolic dropped 57mmHg in 10 minutes of perfusion and became 98 mmHg at the end of perfusion. Initial drop of blood pressure was marked in pediatric patient. Mean cooling time was 19.4 min[0.54 Q /min] and warming time was 34 minutes[0.25 Q /min]. During perfusion, ACT was maintained above 600 sec and 44 patient did not need additional Heparin.

A Case of Portosystemic Shunt in a Domestic Shorthair Cat (한국 집고양이의 문맥전신션트 한 증례)

  • A 3-year-old castrated male domestic shorthair cat was presented with a chief complaint of sudden onset of intermittent seizures occurring five times a day. Physical examination revealed the copper colored iris and loss of menace response at both eyes. Abnormalities of blood works and serum chemistry revealed mild erythrocytosis, severe microcytosis, and threefold increase in ALT activity. Additional liver function tests results were increased bile acid and $NH_3$ concentration. Radiographic study revealed multifocal nodules of the liver and an extrahepatic shunt was noted by ultraonography, which was confirmed by computed tomography as multiple extrahepatic shunts. The cat was scheduled for surgery applying an ameloid ring to occlude the shunt gradually. Diazepam and lactulose were instituted to the patient. However, clinical signs worsened despite medical management with shortened interval of seizures and the patient died due to cardiac arrest.

  • A 62-year-old male was admitted to our hospital complaining of coldness in both legs and discolorization in the right toes. On angiogram, obstruction of right external iliac artery and left common iliac artery was shown. The patient underwent extended extraanatomic arterial reconstruction due to poor general conditions. From right subclavian artery to right popliteal artery, artery bypass was done with 8 mm PTFE and 6 mm PTFE Suprapubic crossover bypass was done with another 8 mm PTFE from 8 mm PTFE in right inguinal area that was anastomosed with right subclavian artery and left common femoral artery The patient was discharged from hospital and OPD follow up was done. Right leg pain and discolorization of entire toes disappeared and were normalized. We report a case of extended extraanatomic arterial reconstruction: subclavian artery-popliteo-crossover-femoral-PEFE-bypass.

  • The purpose of this perspective research is to discuss the potential role of exercise-interventions in COVID-19, terms of prevention and prognosis in the periods of the COVID-19 vaccine. SARCO-CoV-2. COVID-19 was detected as a new virus causing severe cardiovascular and respiratory complications. It emerged as a global public health emergency and national pandemic. It caused more than 1 million deaths in the first 6 months of the pandemic and resulted in huge social and economic fluctuations internationally. Unprecedented stressful situations, such as COVID-19 blue and COVID-19 red impact on many health problems. In healthy individuals, COVID-19 infection may induced no symptoms (i.e., asymptomatic), whereas others may experience flu-like symptoms, such as ARDS, pneumonia, and death. Poor health status, such as obesity and cardiovascular and respiratory complications, are high risk factors for COVID-19 prevention, occurrence, and prognosis. Several COVID-19 vaccines are currently in human trials. However, the efficacy and safety of COVID-19 vaccines, including potential side effects, such as anaphylaxis (a life-threatening allergic reaction) and rare blood clots, still need to be investigated. On the basis of direct and indirect evidence, it seems that regular and moderate physical exercise can be recommended as a nonpharmacological, efficient, and safe way to cope with COVID-19. Physical inactivity and metabolic abnormalities are directly associated with reduced immune responses, including reduced innate, CMI, and AMI responses. Due to prolonged viral shedding, quarantine in inactive, obese and disease people should likely be longer than physical active people. Multicomponent and systemic exercise should be considered for the obese, disease, and elderly people. More mechanism research is needed in this area.

  • A 17-year-old male patient of aortic stenoinsufficiency with a small aortic annulus underwent aortic valve replacement[St. Jude Medical valve 21mm] with Rittenhouse-Man-ouguian procedure[patch enlargement of the area of the noncoronary sinus and anterior mitral leaflet] The used patch was made from tubular dacron graft[the longitudinal length was 35mm, the width 20mm]. The length of incision into anterior mitral leaflet was 10mm and the aortic annulus was enlarged in 10mm, The patient was evaluated with two-dimensional echocardiography at 4 month after operation and the result was excellent.

  • In order to investigate experimentally the clinical effects of Taeksa-Tang that was prescribed to cure the severe dizziness, the water extract of Taeksa-Tang was experimented about the effects on the diuretic action, the serum lipid, the isolated ileum and heart, the blood vessel, the blood pressure and respiration. The results of this study were obtained as follows: The diuretic action was significantly shown in rabbits. The level of total cholesterol, triglyceride and phopholipid in serum of rabbits given high cholesterol diet were significantly decreased. The contraction of the isolated ileum of mice induced by acetylcholine chloride and barium chloride was remarkably inhibited. The vasodilating and hypotensive actions were noted in rabbits. Negative inotropic action on the isolated frog heart was recognized.

Present Statistical Status in Papers in the Korean Journal of Thoracic and Cardiovascular Surgery (대한흉부외과학회지에 게재된 통계적 분석에 관한 고찰)

  • From January 1983 to December 1992, There were 1441 papers in the Korean Journal of Thoracic and Cardiovascular Surgery. Among these papers, 783[54.3%] were original article or clinical analysis and 652[45.2%] were case reports. A total of 319 papers contained some statistical analysis. In 150 cases[47.0%] of these 319 papers, the statistical description was insufficient. Of the correctly described papers, 115[68%] had more than one statistical error. Of course, in many cases the errors were not considered to be severe, but they were often sufficient to raise doubts about some inferences. We suggest that authors should be more careful when they describe and apply statistical methods. If possible, authors should interpret results with statistical specialists. And we also suggest that our society have more extensive statistical refereeing system. This would at least prevent the worst errors from appearing in print. The last suggestion is elementary instruction in statistical methods during preclinical training.

  • Being a medium to help understand the status of the whole body including the Five Vital Organs and the Six Viscera such as the heart, the mac is considered to represent the elasticity of the blood vessel and the condition of the bloodstream in the Oriental medicine. A doctor makes a subjective judgment on a patient's conditions based on the information derived from the diagnosis and the medical examination of the mac wave. Thus he or she needs objective records and make a decision based on them in order to prevent misdiagnosis. This paper set out to suggest the ways to detect the particular parameters of the mac wave with the methods to visualize the mac wave interpretation to detect basic documents(causes) of health status judgment in three-dimension and with the simulation.
