Malignant Solitary Fibrous Tumor of the Pleura -one case report- (늑막의 악성 고립성 섬유성 종양 -1례 보고-)

  • 흉막의 고립성 섬유성 종양은 중피하에 존재하는 미분화 중배엽성 기원의 섬유 세포에서 기원하는 종양으로 알려졌다. 이 종양은 대부분 양성으로 보고되고 있으나 병리학적으로 높은 세포 밀도, 유사분열의 수가 많고(10개의 고배율 시야에서 4개 이상), 핵의 다양성, 출혈, 괴사등이 있으며 악성의 판단기준으로 보고된다. 환자는 62세 여자로 호흡곤란 및 기침을 주소로 본원에 입원하였으며 단순 흉부 방사선 및 전산화 단층 촬영상 우측 흉강내에 거대한 종양소견이 보였다. 이 종양은 23$\times$18$\times$12 cm, 2 kg의 크기와 무게를 가졌으며 우측폐 하엽과 중엽은 압박되어있었으나 종양 절개 후 재 팽창됨을 확인하였다. 또한 횡경막과 심하게 유착되어 박리도중 손상이 동반되어 봉합술이 필요하였다. 병리학적으로 종괴는 세포밀도가 높았고 방추형세포의 다발로 이루어져 있었으며 유사분열의 수가 많이(27 mitosis/10HPF)보였다. 면역 조직 화학 검사상 vimentin과 CD34에 양성 반응을 보였다. 이에 악성 고립성 섬유성 종양으로 확진되었다. 본 교실에서는 흉막에 발생한 악성 고립성 섬유성 종양을 치험하였기에 문헌 고찰과 함께 보고한다.

Catamenial Hemoptysis - Report of one case - (월경성 각혈 - 1예 보고 -)

  • Pulmonary endomertiosis is a rare disorder with the typical symptom of hemoptysis during menstruation (catamenial hemoptysis). We report a case of a 19-year-old woman, gravida 0, with 3-month history of catamenial hemoptysis which was confirmed with chest computed tomography. She was treated by means of thoracoscopic wedge resection for the right lesion and fuperior segmental resection through the left thoracotomy, successively. Preoperative fluoroscopy-guided hooking for thoracosopic target lwsion was helpful in circumstances with one lung anesthesia. Four months of follow-up after an uneventful discharge revealed out no recurrence of catamenial hemoptysis in symptoms and images.

Comparison Between HRCT abd Bronchography for Bronchiectasis (기관지 확장증에서 고해상도 전산화 단층 촬영술과 기관지 조영술의 비교)

  • Bronchiectasis is an irreversible dilatation that may require surgery for successful treatment. We compared High Resolutional Computed Tomography[HRCT]with Bronchography to access the utility of HRCT in diagnosis and determining the extent of Bronchiectasis. We performed a comparative study of HRCT and Bronchography in 10 consecutive patients who were clinically suspected Bronchiectasis were investigated prospectively during last year.A segment-by-segment analysis of the presence, extent, type of bronchiectasis by Amashita classification was done. In 26 segments, Results of HRCT and Bronchogram were both positve, and 119 segments were both negative.But 15 segments were discorded, in 11 segments in positive bronchogram were negative in HRCT and 4 segments in positive HRCT were negative in bronchogram. The diagnostic concordance rate between 2 modalities was 90.6%[145/160]. So, we will be tried to elevate of concordance rate between 2 modalties and applied HRCT in diagnotic tool for bronchiectasis.

  • Pure lipoma, originating from the trachea is a very rara disease entity A-37-ycar-old-male patient had suf'leered from intermittent episodes of dyspnea and has been treated under the diagnosis of bronchial asthma for 6 months. On chest CT scan and bronchofiberscopic examination, a round mass with the pedunculated neck was found in the mid-portion of the membranous portion of the intrathoracic trachea. Under the guide of fiberoptic bronchoscope, the mass was extirpated using polypectomy w re loop and eletrocauterization . He was discharged without any events on third postoperative day of operation and has been well without recurrence for 6 months.

  • Mucoepidermoid carcinoma is an uncommon lesion that accounts for approximately 1% of primary malignant bronchial gland tumors and less than 0.2% of all lung neoplasm. This tumor presents with symptoms of bronchial irritation or obstruction. Distant metastasis is uncommon, therefore complete surgical resection is the treatment of choice. The prognosis of tumor correlates with on the histologic grade of tumor. We experienced mucoepidermoid carcinoma in a 15 year-old girl with symptoms of cough and blood tinged sputum. The patient underwent successful removal of tumor by bilobectomy via explorothoracotomy after chest CT and bronchoscopic biopsy.

Communication Bronchopulmonary Malformation (교통성 폐격리증 -1례 보고-)

  • We have experienced a case of communicating bronchopulmonary malformation. A 35-year-old female patient was admitted for coughing with sputum. About eight years ago, she had treated for bronchiectasis with medication. Chest computed tomography revealed esophagopulmonary communication with upper esophageal dilatation and bronchiectasis of the left lower lobe. A about 3 mm diameteded abnormal feeding vessel from descending thoracic aorta to the left lower lobe was detected at operation. Division and closure of the communication between the esophagus and left lower lobe was performed. Communicating bronchopulmonary malformation is the rare form of pulmonary sequestion and chest computed tomography is one of the useful diagnostic methods for pulmonary sequestion.

  • Few patients with traumatic aortic laceration remain undiagnosed and survive long enough to develop a chronic aneurysm. Such aneurysms are frequently asymptomatic: alternatively, they may manifest chest pain, dysphagia, bronchial irritation, or sudden death. A case of aortobronchial fistula secondary to a chronic post-traumatic aneurysm of the aortic isthmus is presented. Hemoptysis was the main sign. The affected segment of the thoracic aorta was repaired with a Hemashield patch and a left upper lobectomy was performed.

  • Pneumomediastinum, also referred to as mediastinal emphysema or Hamman's syndrome, is defined as the presence of air or gas within the fascial planes of the mediastinum. Superior extension of air into the cervicofacial subcutaneous space via communications between the mediastinum and cervical fascial planes or spaces occurs occasionally, Pneumomediastinum frequently results from blunt tracheobronchial lesions and esophageal injuries. However, in most cases, the origin of pneumomediastinum remains unclear. an some cases, it is attributed to the Macklin effect. We report a case of patient with pneumomediastinum, that presented with Macklin effect on chest computed tomographic scan.

  • Sudden cardiac death is an unexpected death originating from the heart that occurs within an hour of the onset of symptoms. The main cause of sudden cardiac death is arrhythmia; however, diagnosing underlying structural heart disease significantly contributes to predicting the long-term risk. Cardiovascular CT and MR provide important information for diagnosing and evaluating structural heart disease, enabling the prediction and preparation of the risk of sudden cardiac death. Therefore, we would like to focus on the various structural heart diseases that increase the risk of clinically-important sudden cardiac death and the importance of imaging findings.
