MMPI 와 생리적 특성간의 상관성 분석

  • 스트레스 수준이 논을 것으로 예상되는 공군사관학교 수험생의 신체검사 결과로부터 신체건강 지표인 혈액 내 생화학적 요인들과 정신건강 지표인 다면 인성검사 점수를 추출하여 통계적 상관성 및 유의성을 비교 분석하였다 사관생도들 중 $10\%$의 표본을 추출하여 사관생도와 수험생 집단간의 특성 비교를 위한 비교집단으로 이용하였다. 수험생과 사관생도 집단 간의 특성 요인 비교는 Mann-whitney U 검정과 Kruskal-Wallis 검정을 이용하였다. 수험생 집단 내에서 혈중 요인 및 정신건강 지표인 MMPI 점수는 정상범주에 있었고 두 요인 간 상관성은 발견할 수 없었다. 그러나 사관생도와 수험생 집단 간의 특성 요인들에 대한 유의성 검정 결과, 특히 2학년생도들의 혈액 내 혈구 수 및 활동성 또는 적극성과 관련된 MMPI 하위척도에서 수험생 집단과 매운 유의한 차이를 보이는 특이한 현상을 발견할 수 있었고 이에 대한 해석을 언급하였다.

  • Understanding the biopsychosocial model of illness is crucial for any meaningful advance of health. The maintenance and promotion of health is achieved by different combinations of physical, mental, social and spiritual well-being. Health is not an objective of living. It is not only a state, but also a resource for everyday life. Health is a positive concept that emphasizes personal and social resources, as well as physical capacities. Understanding the biopsychosocial model of health and disease is very important in the medical system. George Engel challenged the medical profession to reconsider a strict biomedical approach to medical education and care, and to embrace a "new medical model," the biopsychosocial model. He argued that humans are at once biological, psychological, and social beings who behave in certain ways that can promote or harm their health. Although understanding the biopsychosocial model of illness is important, Korea's medical system have mainly been focusing on the biomedical model of illness. I would like to highlight the importance of biopsychosocial model of illness for Korea's medical system and real clinical field according to the 20th anniversary of Korean Society of Psychosomatic Medicine.

Psychosomatic Integrative Care for Psychosocial Distress of Patients With Breast Cancer (유방암 환자의 정신사회적 디스트레스에 대한 정신신체의학적 통합치료)

  • Breast cancer is the most prevalent oncological disease among women. Various psychosocial distress is common at the diagnosis, treatment, and posttreatment phase of breast cancer. For the treatment of breast cancer, not only medical treatment but also psychosomatic integrative care will be needed. Patients with breast cancer may lead to increased vulnerability to stress, adjustment disorder, anxiety disorder, and depressive disorder, and these psychiatric diseases and conditions are associated with recurrence or exacerbation of breast cancer. Psychosocial treatment of anxiety and depression could increase the quality of life of patients and decrease the recurrence and progression of breast cancer. In this article, we reviewed 5 clinical breast cancer survivorship guidelines focused on psychosomatic integrative care including psychosocial treatment and alternative treatment for psychosocial distress. Because 5 treatment guidelines were using various definitions of evidence, we confirmed evidence of various psychosocial treatments for patients with breast cancer based on the definition of evidence by the US Preventive Service Task Force (USPSTF) guideline. We also reviewed the effect size of psychosocial treatment for anxiety, depression, mood, and quality of life in patients with breast cancer. This article discusses the barrier to the delivery of psychosomatic integrative care and suggests integrative care planning for breast cancer. Multi-disciplinary teams, patient's needs assessment, information technology support, patient and caregiver engagement, planned periodic monitoring of psychosocial distress by a psychosomatic specialist or consultation-liaison psychiatrist are recommended as key features of a psychosomatic integrated care plan.

  • The purpose of this study was to identify the effect of psychological health and physical health on health-related quality of life(HRQoL) in Korean Echo Generation by using Korea Health Panel Data 2013. The Korea Health Panel Data 2013 were collected from February to October 2013 and 2,261 respondents were analyzed. The data were analyzed by Independent t-test, ANOVA and multiple regression using SPSS WIN 24.0 program. he mean score of HRQoL was 0.98. The effect of unmet medical needs, psychological & physical stress, unmet basic needs, anxiety about the future, depression, suicidal ideation, smoking, sleeping time, hearing problem, eating problem, restriction of activity, and self-rated health status) were significant on HRQoL. Health care providers should consider the effect of psychological and physical health when they design program for the improvement of HRQoL for Korean echo generation in community.

Difference of Somatic Symptoms between Anxiety Disorder and Major Depressive Disorder and Their Domainal Association with Suicidal Idealization, Plan and Attempts (불안 장애와 주요우울장애에서 나타나는 신체 증상과 증상군에 따른 자살 사고, 계획, 행동과의 관계 고찰)

  • Objectives : The aim of this study is to evaluate difference of somatic symptoms of anxiety disorder and major depressive disorder and domainal association with suicidal idealization, plan, and attempts. Methods : A total of 359 adults diagnosed with major depressive disorder and anxiety disorder of last one year participated. Participants interviewed with certain sections of Korean version of Composite International Diagnostic interview of CIDI. Sections of interests includes questionnaires regarding somatic symptoms and suicidal idea, plan and attempts of last one year. Results : Chest pain shows more prevalence in major depressive disorder. Symptoms of Headache and loose stool are more prevalent in anxiety disorder. Difficulty in equilibrium and fainting spells are more common somatic complaints of co-diagnosis states of anxiety disorder and major depressive disorder. Comparing 3 domains of pain symptoms, gastrointestinal symptoms and pseudo-neurological symptoms, pain symptom domains, gastrointestinal symptoms domain shows significant statistic difference between diagnosis. Average somatic symptom numbers of each symptom domains increase through suicidal idealization, plan and attempt, accordingly. Conclusions : Our finding shows some of somatic symptoms are more prevalent at certain diagnosis. Since increasing numbers of somatic complaints of each symptom domains goes with the suicidal idealization to suicidal attempts, proper psychiatric evaluation and consultations are crucial for patients with numerous somatic complaints in non-psychiatric clinical settings.

  • The purpose of this study was investigated to provide basic data for the introduction of nursing intervention necessary for the stress prevention and management by examining stress, mental health and physical symptoms. Subjects for the study were conveniently selected among boys and girls middle schools located in T city of Gangwon-do. A total of 171 students were selected including 1st grade, 2nd grade, and 3rd grade. Data were analyzed by using statistical program SPSS(pc win ver. 17.0). The results of this study as follows; There were significant correlation between stress and mental health, stress and physical symptoms, mental health and pysical symptoms. The stress of middle-school students largely came from academic problems in school life, which is considered to reflect the reality of Korean middle-school students who have the burden of entrance examination for universities. Also, significant correlations among stress, mental health, and physical symptoms suggested that continuous observation and assessment are required for the stress management of middle-school students.

  • Using complex sample design, this article analyzes the difference between sex and age groups of mental health, physical activity, suffering lifestyle diseases, drinking, and smoking using the fifth Korea national health and nutrition examination survey data (2011-2012), and then analyzes the effect of mental health, physical activity, suffering lifestyle diseases, drinking, and smoking on EQ-5D, a measure of health-related quality of life. The results show that mental health, physical activity, suffering lifestyle diseases, drinking and smoking are statistically different among gender and age groups, and that age group, education level, suffering diabetes, recognizing stress, thinking suicide are statistically influential on EQ-5D.

Relationship between Somatization and Mental Health of Registered Nurses (간호사의 신체화 증상과 정신건강의 관계)

  • Objectives : To identify the relationship between somatization, stress, depression, anxiety, and psychological symptoms risk for nurses working in the intensive care unit. Create clinical evidence of psychosomatic medicine research and complement the meaning of somatization. Methods : Seventy of the mental health checkups conducted by the National Mental Health Center among the nurses using tools including Perceived Stress scale, Fatigue Severity Scale, Patient Health questionnaire-15, Korean Beck Depression Inventory, Korean Beck Anxiety Inventory, and Symptom Checklist-90-Revision. Results : 12.9% of the patients experienced more than moderate somatization. There was no statistical relationship between somatization and psychological stress perception, but feeling of anxiety and decreased self-confidence were related to the level of somatization. The group with severe somatization experienced more depression and anxiety. The group with high physical fatigue also had no statistical relationship with psychological stress perception, but had an effect on the feeling of tension, stress, or decreased control. Physical fatigue level was increased by experience of depression, not by anxiety. For psychological symptoms the higher the level of somatization, the higher the obsession and hostility was explored. In the linear regression model, stress, depression, and anxiety accounted for 39.3% of somatization and 16.1% of physical fatigue symptoms. Conclusions : We can estimate the decrease in stress cognitive symptoms, accompanying depression and anxiety, compulsion and hostility as characteristics of somatization. The causal relationship between somatization and psychological symptoms cannot be confirmed in this study, but the interrelationships are observed, can be referred to mediation strategies.

A Study on the Relation between Degree and Physical & Mental Health of Old People in Interpersonal Relationship Network (대인관계 네트워크에서 연결정도와 노인의 신체적 건강 및 정신적 건강과의 관련성 연구)

  • The purpose of this study is to see if we can predict the health of seniors of community by analyzing the connection between social network degree and mental and physical health of old people who live in the areas of Gangwha Island. The subjects of the study were men and women aged 65 or over, a total of 643 that resided in Ganghwa A-county. The survey was conducted on Korean Social Life, Health and Aging Project from the year 2011 to 2012. Regression analysis was carried out using the data. The analysis results were as follows. First, it showed the relationships between income, gender, age out of demographic variables used as control variable and old persons'physical health. The research results showed that physical health was better in case of the higher incomes, men, and lower age. Second, out of demographic variables, educational background, income, age was shown to correlate with mental health. The research results showed that mental health was better in case of the higher incomes, higher educational background, and lower age. Third, in social network including direction, both out-degree and in-degree were shown to predict old people's physical and mental health. The results of this study suggest that not only out-degree but also in-degree should be considered in predicting the health of elderly persons by a person's human relationship. Also, two indicators of degree are meaningful in the dimension of health promotion and welfare of the old in that they can be used for finding isolated individuals that can be physically and mentally vulnerable.

A study on relationship between Physical activity, Diet pattern and Mental health of Korean elderly (한국노인의 신체활동 및 식생활과 정신건강 관련성에 대한 연구)

  • The purpose of this study was to investigate the relationship between physical activity, diet and mental health of elderly and to use the study as basic data for a program development for elderly. This was a secondary data analysis study using the 2015 National Health and Nutrition Survey and the study subjects included 1,484 individuals aged 65 and over. The results showed that depression and stress perception in young-old were higher than old-old's and the suicidal ideation rate of old-old was higher than young-old's. In case of eating patterns, 'poor eating' in old-old was higher than young old's. Regression analysis showed that middle-intensity physical activity and eating patterns were associated with mental health of the elderly. Based on the results, it is necessary to develop a differentiated nursing intervention program suitable for the elderly by age.
