Intraneural Ganglion Cyst of the Peripheral Nerve: Two Cases Report (말초 신경에 발생한 신경 내 결절종: 2예 보고)

  • Ganglion cysts that occurred within sheaths of peripheral nerves have been documented in literatures, but it is relatively rare condition. The peroneal nerve is the most common site of involvement. Other reported sites of involvement are the radial, ulnar, median, sciatic, tibial and posterior interosseous nerves. We report a case of the intraneual ganglion cyst within peroneal nerve and another case of the intraneural ganglion cyst within a medial plantar nerve that were successfully excised without neurologic complication.

The Effectiveness of Ultrasound-guide Steroid Injection According to Morton's Neuroma Size (모톤씨 신경종 크기에 따른 초음파 유도하 스테로이드 주사 효과의 비교분석)

  • Purpose: The aim of this study was to evaluate the effectiveness of ultrasound-guide steroid injection according to Morton's neuroma size. Materials and Methods: From October 2008 to September 2011, 17 patients (23 cases) diagnosed with Morton's neuroma were investigated. All cases were female and mean age was 52.6 years old. Neuroma were measured by the horizontal and longitudinal length of the mass and underwent ultrasound-guided steroid (5 mg dexamethasone) injection. The efficacy of the injection was determined by Visual Analogue Scale pain score and patient satisfaction(subdivided 4 group-much improved, improved, not improved, aggrevation) Results: 7 of 23(30.4%) cases showed much improved and improved satisfaction and mean longitudinal and horizontal length were $0.71{\pm}0.39cm$ and $0.47{\pm}0.24cm$, respectively. 16 of 23(69.6%) cases showed not improved and aggrevation satisfaction and mean longitudinal and horizontal length were $0.83{\pm}0.42cm$ and $0.54{\pm}0.14cm$, repectively. There was a significant difference in VAS and patient satisfaction in case longitudinal and horizontal length were smaller than 0.5 cm and 0.4 cm. (p<0.05) Conclusion: The ultrasonography is a important modality in diagnosis and treatment of morton's neuroma. Ultrasound-guide steroid injection is effective in case longitudinal and horizontal length were smaller than 0.5 cm and 0.4 cm, respectively.

Distribution of Benign Soft Tissue Tumors Occurred in Extremities and Trunk (사지 및 체간에 발생한 양성 연부조직 종양의 발생 분포)

  • The authors reviewed and analyzed the pathologically confirmed 230 cases of benign soft tissue tumors which had been treated at Department of Orthopaedic Surgery in Seoul Hospital, Hanyang University College of Medicine from February 1984 to November 1997. The following results were obtained. 1. The most common benign soft tissue tumors was ganglion(26.5%) followed by hemangioma(19.6%), lipoma(17.0%), Baker's cyst(13.0%) and neurilemmoma(7.0%) in decreasing order of incidence. 2. Benign soft tissue tumors were found evenly distributed over all age group. Hemangioma and lymphangioma were usually found to occur before 20 years old. 3. Female was affected about 1.3 times more common. But, Baker's cyst and neurilemmoma occurred most frequently in male. 4. In children, the most common benign soft tissue tumors was hemangioma followed by ganglion, lymphangioma, and lipoma. In adult, ganglion, lipoma, Baker's cyst, hemangioma were common. 5. The lower extremity was the predominant site of occurrence(60.0%). 6. Benign soft tissue tumors were taken by excision. Local recurrences were developed in 22(9.6%) out of 230 cases, especially in hemangioma.

  • Neurilemmoma is a benign tumor which arises generally from the Schwann's cells. It is a slowly growing tumor and treated as surgically. We experienced a case of neurilemmoma of the oropharynx in a 28-year-old female who had noticed a gradual swelling of the left side of the oropharynx with swallowing difficulty for 6 months. The tumor was removed successfully intra-orally under the general endotracheal anesthesia and verified pathologically as neurilemmoma.

  • We experienced a case of 63 years old male patient who had synchronous rectus abdominis intramuscular schwannoma and chest wall lipoma. Schwannoma is rare benign tumor which derived from nerve sheath and mainly peripheral nerve of flexor part. The authors report rare synchronous schwannoma and lipoma development.

  • Evolutionary artificial neural networks (EANNs) are towards the near optimal ANN using the global search of evolutionary instead of trial-and-error process. However, many real-world problems are too hard to be solved by only one ANN. Recently there has been plenty of interest on combining ANNs in the last generation to improve the performance and reliability. This paper proposes a new approach of constructing multiple ANNs which complement each other by speciation. Also, we develop a multiple ANN to combine the results in abstract, rank, and measurement levels. The experimental results on Australian credit approval data from UCI benchmark data set have shown that combining of the speciated EANNs have better recognition ability than EANNs which are not speciated, and the average error rate of 0.105 proves the superiority of the proposed EANNs.

  • Ganglion cysts occurring within sheaths of peripheral nerves have been documented, but are relatively rare entities. The peroneal nerve at the level of the knee and proximal tibiofibular joint is most commonly involved. We report a case of the intraneural ganglion cyst in the peroneal nerve without neurologic deficit, which was excised sucessfully.

  • Neurogenic tumors are the most common posterior mediastinal tumors and accounting for $19{\sim}39%$ of all mediastinal tumors and 75% of all posterior mediastinal tumors. Neurofibromatosis is an autosomal dominant disorder with variable expression of tumors, including neurologic tumors of the peripheral nerves, nrve roots, and plexi. A posterior mediastinal neufibroma in neurofibromatosis patients is rare. We report here a case of posterior mediastinal neurofibroma in a patient with type 1 neurofibromatosis.

Is Video-assisted Thoracoscopic Resection for Treating Apical Neurogenic Tumors Always Safe? (흉강 첨부 양성 신경종의 흉강경을 이용한 절제술: 언제나 안전하게 시행할 수 있나?)

  • Background: Mediastinal neurogenic tumors are generally benign lesions and they are ideal candidates for performing resection via video-assisted thoracoscopic surgery (VATS). However, benign neurogenic tumors at the thoracic apex present technical problems for the surgeon because of the limited exposure of the neurovascular structures, and the optimal way to surgically access these tumors is still a matter of debate. This study aims to clarify the feasibility and safety of the VATS approach for performing surgical resection of benign apical neurogenic tumors (ANT). Material and Method: From January 1996 to September 2008, 31 patients with benign ANT (15 males/16 females, mean age: 45 years, range: 8~73), were operated on by various surgical methods: 14 VATS, 10 lateral thoracotomies, 6 cervical or cervicothoracic incisions and 1 median sternotomy. 3 patients had associated von Recklinhausen's disease. The perioperative variables and complications were retrospectively reviewed according to the surgical approaches, and the surgical results of VATS were compared with those of the other invasive surgeries. Result: In the VATS group, the histologic diagnosis was schwannoma in 9 cases, neurofibroma in 4 cases and ganglioneuroma in 1 case, and the median tumor size was 4.3 cm (range: 1.2~7.0 cm). The operation time, amount of chest tube drainage and the postoperative stay in the VATS group were significantly less than that in the other invasive surgical group (p<0.05). No conversion thoracotomy was required. There were 2 cases of Hornor's syndrome and 2 brachial plexus neuropathies in the VATS group; there was 1 case of Honor's syndrome, 1 brachial plexus neuropathy, 1 vocal cord palsy and 2 non-neurologic complications in the invasive surgical group, and all the complications developed postoperatively. The operative method was an independent predictor for postoperative neuropathies in the VATS group (that is, non-enucleation of the tumor) (p=0.029). Conclusion: The VATS approach for treating benign ANT is a less invasive, safe and feasible method. Enucleation of the tumor during the VATS procedure may be an important technique to decrease the postoperative neurological complications.

Co-Infection of the Rat Central Nervous System with Genetically Engineered Strains of Pseudorabies Virus (유전자 조작된 Pseudorabies 바이러스에 의한 흰쥐 중추신경계의 이중감염)

  • 중추신경계의 미주신경동쪽핵(DMV)내 유사핵분열후 신경세포로 외래 유전자를 전달하는 매개체로서 pseudorabies 바이러스(PRV)의 유전자 조작기술은 흰쥐의 결장내로 PRV를 주입시킨 후 복잡한 신경로 추적에 관한 연구에서 하나의 바이러스에 의해 얻어지는 것보다 더욱 유용한 결과산출이 가능하게 하였다. 본 연구에서는 흰쥐의 생체내 실험모델로 하나의 바이러스 또는 이중 바이러스 주입에 PRV의 유전자 조작된 2종 바이러스를 사용하였다. 이 2종의 바이러스는 PRV의 Bartha 종에서 유래되었지만 면역조직화학적으로 검출할 수 있는 동일한 유전산물을 산출할 수 있도록 구성되었다. PRV-BaBlu는 PRV 게놈의 Us 구역 중 gC 자리에 lacZ 유전자를 삽입하여 산출되었는데 $\beta-galactosidase$ 발현은 이 바이러스에 감염된 신경원의 독특한 표시자로 나타났다. PRV-D는 2가지 단계에 의해 조성되었는데 첫째, PRV-Bartha의 Us 구역의 일부 유전자를 제거하고, 야생형인 PRV-Be DNA로 복구시켰는데 이로써 PRV-D는 PRV-Bartha 또는 PRV-Bablu에 존재하고 있지 않는 외피 당단백질인 gE와 gI를 지니게 되었다. 본 연구의 결과는 다음과 같았다. 첫째, PRV-D의 개별적 접종에 의해 얻어진 감염은 PRV-BaBlu에 의한 동일 신경회로의 감염보다 유의하게 빨랐다. 둘째, 유전자 조작된 PRV의 변이종은 변이종 상호간 및 부모 바이러스와 상이하였다. PRV-D는 PRV-Bartha 또는 PRV-BaBlu보다 감염독성이 더 강했고, PRV-BaBlu는 PRV-Bartha보다 감염독성이 약했다. 셋째, 결장을 지배하는 미주신경동쪽핵내 신경원은 변이종 바이러스들을 동시에 접종하였을 경우 이중감염을 나타내었다.
