A Case of Primary Writing Tremor Type A (원발성 서자진전 A형 1예)

  • Abnormal writing can result from a large variety of neurologic disorders of motor control. Primary writing tremor(PWT) in its pure form denotes occurrence of a disabling tremor induced by writing alone. Therefore PWT is typical task-specific tremor. PWT could be classified as either type A or type B. In a strict sense, type B PWT is not pure form of PWT because it is not task specific. We describe a case of PWT type A. A 53-year-old right handed policeman complained of a 4 year history of shaking of his right hand that interfered with writing. Tremor appeared during writing but not on adopting the hand position normally used for writing.

Effect of Wogonin on Hydrogen Peroxide Production in Polyinosinic-Polycytidylic acid-induced Human Neuroblastoma SH-SY5Y Cells (Wogonin이 polyinosinic-polycytidylic acid로 유발된 인간 신경모세포종 SH-SY5Y의 hydrogen peroxide 생성증가에 미치는 영향)

  • The purpose of this study is to investigate the modulatory effect of wogonin on hydrogen peroxide production in human blastoma SH-SY5Y cells induced by the synthetic analog of double-stranded RNA [polyinosinic-polycytidylic acid]. Hydrogen peroxide production was measured by dihydrorhodamine 123 (DHR) assay. Wogonin significantly inhibited the polyinosinic-polycytidylic acid (PIC)-induced production of hydrogen peroxide for 0.5, 2, 12, 18, and 24 hr incubation at the concentrations of 10, 25, and 50 ${\mu}M$ in SH-SY5Y (P < 0.05) in dose dependent manner. These results suggest that wogonin has neuroprotective property related with its inhibition of hydrogen peroxide production in PIC-induced neuronal cells.

Clinical Study of Oriental Medical Treatment on a Patient with Alcoholic Polyneuropathy (알코올성 다발성 신경병증의 한방치료 효과)

  • Alcoholic polyneuropathy is a disorder of the peripheral nervous system that interferes with sensory, motor, and autonomic nerve function. This study was to report the effect of Oriental medical treatment on a patient with alcoholic polyneuropathy. Conservative Oriental medical treatment was done to a man of 84 years who was diagnosed as a alcoholic polyneuropathy and suffered from both distal dominant leg weakness & symmetrical paresthesia and ataxic gait. The changes of symptoms, reflexes and sensory test were checked by Toronto Clinical Neuropathy Scoring System(TCNSS). Reflexes, motor and sensory impairments were improved after the Oriental medical treatment. I reported good effects of Oriental medical treatment on alcoholic polyneuropathy. Oriental medical treatment can be helpful to improve the symptoms of alcoholic polyneuropathy.

Study on the Effect of Vitamin E on Cultured Hippocampal Neurons Damaged by Hydrogen Peroxide (과산화수소로 손상된 배양 해마신경세포에 대한 Vitamin E의 영향에 관한 연구)

  • To clerify the cytotoxicity of reactive oxygen species in cultured hippocampal neurons of neonatal mouse, toxic effect was measured by MTT assay after cultured cells were incubated for 3 hours in the media containing 1~40 μM concentrations of H₂O₂. In addition, the protective effect of vitamin E was determined in these cultrures. Cell viability was significantly decreased in a dose-dependent manner after exposure of 10 μM H₂O₂ to cultured mouse hippocampal neurons for 5 hours. In the protective effect of vitamin E, vitamin E prevented the H₂O₂-induced cytotoxicity in these cultures. From these results, it suggests that H₂O₂ has toxic effect in cultured mouse hippocampal neurons and vitamin E has protective effect on the cytotoxicity induced by H₂O₂.

  • Oculomotor nerve palsy is a disorder which causes eyeball movement trouble, dipleopia, dizziness and eyelid ptosis. It is caused by aneurysm, mass, truma, intracranial hypertension, diabetes mellitus, infection and so on. But sometimes we can't establish the cause. We report a case of oculomotor nerve palsy associated with spleen energy(脾氣). We treated a patient with herb medicine and acupuncture. This study suggests that oriental medicine theraphy has a good effect on the oculomotor nerve palsy.

  • 후두기능의 전기생리학적 특징에 대한 관심은 1940년대부터 시작되어 1950년대와 1960년대에는 유럽 및 일본에서 Faborg-Anderson, Hirano 등이 후두근전도의 임상적 적용에 대해서 보고한바 있으며, 1970년대부터는 미국에서도 Bleir, Miller등이 후두근전도의 임상적 역할에 대해 보고하고 있다 이와 같이 후두근전도가 임상에 도입된 지 오래되었고 신경후두학 분야에서 후두근육의 전기생리상태를 알 수 있는 아주 중요한 진단도구이나 아직까지 후두근전도는 양적인 평가가 가능한 표준화된 검사는 아니고 내시경이나 방사선학적 검사처럼 경험이 있는 의사에 의해 판독되어지는 검사로 음성장애가 있는 모든 환자에서 집단검진(screening test)으로 이용될 수 있는 검사도구는 아니다. 후두근전도가 임상에서 유용하게 이용되고 있는 질환은 성대운동 장애와 연축성 음성장애이다. 최근 성대운동 장애에 대한 다양한 수술방법이 개발되면서 수술 방법 및 시기 결정, 예후측정에 후두근전도의 이용이 필수적이 되었으며 또한 연축성 음성장애 환자에서 어떤 후두 근육에 botulinum toxin을 주사할 지를 결정하는 데 이용하고 있다. 이번 강의에서는 위 두 가지 질환을 중심으로 후두근전도의 임상적 유용성을 살펴보기로 하겠다.

Inclusion Body Myositis : Clinical Features and Electrophysiological Findings (봉입체 근염 : 임상과 전기생리학 소견)

  • Sporadic inclusion body myositis (s-IBM) is an aquired slowly progressive inflammatory myopathy with unknown etiology. Although light microscopic abnormalities and characteristic histopathology on muscle biopsy distinguishes from other inflammatory myopathies, vacuolated muscle fibers, intracellular amyloid deposits or tubulofilaments in electromicroscopic findings are not definite in some patients. This review shows the prominently involved muscles in s-IBM and specific or nonspecific electrophysiologic manifestations from reported data for helping the diagnosis of definite-or probable-IBM patients. In lower limbs, the quadriceps is predominantly involved, as is iliopsoas, and tibialis anterior is common. In the upper limbs, the greatest weakness is in forearm finger flexors. Finger extensors, biceps and triceps also are moderately to prominently involved. The majority of patients demonstrate polyphasic MUAPs that are short in duration. An additional striking feature is the concomitant documentation of long-duration, large-amplitude, polyphasic MUAPs. In spite of the frequent mixed myopathic-neurogenic electromyographic findings of IBM, just like that of chronic myositis, asymmetric, slowly progressive weakness of flexor digitorum profundus or quadriceps femoris muscles after age of 50 is very necessary condition for the diagnosis of IBM.

  • 음식물을 씹거나 삼키는 정상적인 하악운동이외에 악안면 근육의 활동에 의해 상, 하악 치아가 접촉하거나, 비정상적인 근육활동이 있을 때 이 현상을 이갈이(Bruxism)이라 합니다. 이갈이 습관은 대개 수면중에 나타나나 깨어 있는 상태에서도 비정상적인 근육수축을 관찰할 수 있읍니다. 이러한 현상을 저작근의 근전도(Electromyogram, EMG)를 기록하거나, 구강내에 장치된 원격계측장치로 치아접촉을 관찰함으로써 정상적인 저작이나 연하작용과 비교할 수 있습니다. 이갈이의 발생빈도는 학자들에 따라 서로 다르게 보고되고 있으며, 수면중 이를 가는 사람들은 대부분 이갈이 습관을 알지 못하며, 단지 10%정도만이 이가는 소리를 알 뿐입니다. 본문에서는 이갈이와 수면중의 생리적 현상과의 관계, 저작근에 미치는 영향과 이갈이 발생에 대한 중치신경의 작용을 검토함으로써 이갈이를 이해하는데 도움이 되고저 합니다.

Clinical Study on a case of Ptosis Patient Diagnosed as Oculomotor Palsy (동안신경마비로 진단된 안검하수 환자에 대한 임상적 고찰)

  • This is a clinical report of a ptosis patient diagnosed as oculomotor palsy. A 47-year-old man with ptosis, diplopia, anorexia, dyspepsia, general weakness was admitted at Wonkwang Oriental Medicine Hospital. He was treated with the herbal medicine Bojungikgi-Tang and with acupunture. Results showed improvement of symptoms and total body conditions. The process used in this sucessful treatment is here described.

  • Attentional processes facilitate cognitive and behavioral performance in several ways. Attention serves to reduce the amount of information to receive. Attention enables humans to direct themselves to appropriate aspects of external environmental events and internal operations. Attention facilitates the selection of salient information and the allocation of cognitive processing appropriate to that information. Attention is not a unitary process that can be localized to a single neuroanatomical region. Before the cortical registration of sensory information, activation of important subcortical structures occurs, which is called as an orienting response. Once sensory information reaches the sensory cortex, a large number of perceptual processes occur, which provide various levels of perceptual resolution of the critical features of the stimuli. After this preattentional processing, information is integrated within higher cortical(heteromodal) systems in inferior parietal and temporal lobes. At this stage, the processing characteristics can be modified, and the biases of the system have a direct impact on attentional selection. Information flow has been traced through sensory analysis to a processing stage that enables the new information to be focused and modified in relation to preexisting biases. The limbic and paralimbic system play significant roles in modulating attentional response. It is labeled with affective salience and is integrated according to ongoing pressures from the motivational drive system of the hypothalamus. The salience of information greatly influences the allocation of attention. The frontal lobe operate response selection system with a reciprocal interaction with both the attention system of the parietal lobe and the limbic system. In this attentional process, the search with the spatial field is organized and a sequence of attentional responses is generated. Affective, motivational and appectitive impulses from limbic system and hypothalamus trigger response intention, preparation, planning, initiation and control of frontal lobe on this process. The reticular system, which produces ascending activation, catalyzes the overall system and increases attentional capacity. Also additional energetic pressures are created by the hypothalamus. As psychophysiological measurement, skin conductance, pupil diameter, muscle tension, heart rate, alpha wave of EEG can be used. Event related potentials also provide physiological evidence of attention during information process. NI component appears to be an electrophysiological index of selective attention. P3 response is developed during the attention related to stimulus discrimination, evaluation and response.
