Effects of Oral Vitamins on the Elderly with Mild Cognitive Impairment (비타민 보충제 영양강화요법이 경도인지기능장애 노인에게 미치는 효과)

  • As the elderly population increases, the prevalence of various geriatric chronic diseases and dementia diseases is also rapidly increasing; accordingly, dementia is becoming a major concern of our society. In this study, 48 elderly patients with mild cognitive impairment located in K district were selected from a group of experimental groups in one building and a control group in one building and evaluated for blood homocysteine levels and cognitive function changes after 4,8, and 12 weeks of taking vitamin supplements. The Chi-squared test, Fisher's Exact test, independent t-test, repeated measures ANOVA, contrast test, repeated measures ANCOVA, and Wilks' lambda test were utilized to analyze the data. The results revealed that the cognitive function of the experimental group was significantly higher than that of the control group at 12 weeks (p<0.05), and so, the experimental group higher than that of the control group (p<0.01), the effect of taking vitamins was significantly increased, indicating that homocysteine was decreased relative to the control group. Therefore, vitamin supplements may prevent decreases in cognitive functions and dementia among elderly patients with mild cognitive impairment.

Effect of resistance training on joint flexibility and muscle strength of upper extremities of elderly with impaired cognition (탄력저항성운동 프로그램이 인지기능저하 노인의 상지유연성 및 근력에 미치는 효과)

  • The purpose of the study was to examine the effect of resistance training on joint flexibility and muscle strength of upper extremities of institutionalized elderly with impaired cognition. The study design was pretest-posttest control group study and inclusion criteria were elderly aged 65-year older, MMSE score 23 or less, ones who had no serious physical and/or mental problem except impaired cognition, and were capable to carry out resistance training. After consents were obtained participants were randomly assigned. Pre-post evaluation was performed by staff nurses trained beforehand. Among those 4-week study period, experiment was carried out during 5 consecutive days a week for 3 weeks. ROM and extension range of shoulder joints and muscle strength of shoulders and hands for both sides were measured. Flexion, extension, abduction range of right shoulder joint was significantly improved. Flexion and extension muscle strength of left side shoulder and abduction muscle strength of both sides of shoulder were significantly improved. With the study result, it could be concluded that resistance training has therapeutic effects on joint flexibility and muscle strength. More studies adopted longer experimental period to evaluate timing of effect and extinction to refine the protocol are called for.

Effects of Occupational Therapy with Physical Exercise on Demented old Adult's Executive Function (신체활동 작업치료가 치매노인의 실행기능에 미치는 영향)

  • The aim of this study was to investigate the effects of occupational therapy with physical exercise on executive function with dementia. For eight elderly persons with mild or moderate dementia, we executed physical exercise with group activity for 10 weeks from April to June, 2012, one times per week, and 50 minutes each time. Before and after the physical activities, the old adults were tested with Verbal Fluency Test(VFT), Trail Making Test A/B(TMT A/B), Clock Drawing Test(CDT). After the physical exercise, the subjects showed significant changes in the score of CDT(p<0.1). In contrast, there was no significant effect in VFT, TMT A. The results of this study show that occupational therapy with physical exercise program for demented elderly people is a useful therapeutic approach by enhancing the their quality of life through improving and maintaining their executive function.

  • This study aims to provide primary data for R&D on rollator-walking frame through researching the usability test of the rollator-walking frame and the effect of rollator-walking frames on the lives of senior citizens. This study surveyed 60 senior citizens living in Sungnam City in South Korea. As the result of the usability test, we can find users' dissatisfaction with the folding, weight, shape of backrest and material of chair. The survey shows that the frequency of outdoor activity is mostly increased by the convenience of product. However, the inconvenience while using facilities, such as elevators or public transportations, gave a significant impact on the decrease in going out experiences. Therefore, for better outside lifestyle of senior citizens, it is important to improve the function of the product, however, we need to make improvement of urban infrastructure.

Comparison of Perceived Health Status, Social Support and Residential Satisfaction in Longterm Care Hospital and Nursing Homes (요양병원과 요양시설 노인의 건강상태, 사회적 지지 및 거주만족도)

  • Purpose: The purpose of this study was to identify and compare the differences on perceived health status, Activities of Daily Livings (ADL), social support, and residential satisfaction between long-term care hospitals and nursing homes. Methods: Data was collected through questionnaires and interviews conducted from March 29 to April 22, 2011. The subjects were 66 old adults in 3 long-term care hospitals and 53 old adults in 6 nursing homes. Data were analyzed by Pearson's correlation analysis and t-tests. Results: ADL and subjective health status in nursing homes were worse than those in long-term hospitals, but it was not statistically significant (p>.05). Old adults in nursing homes received more emotional support from other residents and staff, and received more instrumental support from staff than those in long-term care hospitals (p<.001). The mean scores of resident satisfaction in long-term care hospitals and nursing homes were 3.53 ($SD={\pm}0.36$) and 3.97 ($SD={\pm}0.44$), respectively. Resident satisfaction in nursing homes significantly was higher than long-term care hospitals (p<.001). Conclusion: Health care personnels in long-term care hospitals should enhance resident satisfaction and social support and need to coordinate long-stay patients with nursing homes.

A Study on Residents' Subjective Evaluation of the Elderly Housing Environment in the Suburbs of Chiba City, Japan (고령자거주요소에 대한 거주자의 주관적 평가연구 - 일본치바시근교의 유료노인시설을 대상으로 -)

  • The Purpose of this study is to examine residents' subjective satisfaction of the elderly housing environment and to suggest some major factors which can be used as basic design guidelines for the elderly housing. as the number of the elderly is growing in Japan For this purpose. data were collected through a questionnaire survey method, Some questions on their hope for the conditions of the elderly housing were asked to 265 old People living in the suburbs of Chiba city. Japan. Based on the findings of the statistical factor analysis, the major factors which affected on the elderly housing environment of the study can be summarized as follows ;(1)Factor1 is correlated with the space design element represented by the elderly dwelling performance and overall efficiency for the dwelling exterior space design (2)Factor2 is relative to the interior environmental element with Physiological and sensitive performance for borne services system. (3)Factor3 can be explained the moving and communication performance for Physical uneasiness of outdoor activities and request of public cultural spaces. (4)Factor4 is relative to the mental rest element with safety accidents for moving activities etc. and psychological isolation for the elderly.

Exploring Physical Activity Promotion Strategies for Older Residents in a Small Town Based on Socio-ecological Model: A Mixed Method Inquiry (사회생태학적 분석을 통한 중소규모 A 도시 거주 노인 신체활동 촉진전략 모색 - 혼합연구기법으로)

  • The purpose of this study was to understand barriers and problems associated with physical activity of older adults in a small town and to provide improvement plans based on socio-ecological model. This study employed three independent phases including survey, accelerometer study, and photovoice. Three phases were analyzed based on nesting design among diverse mixed method designs. Among 134 older participants, 73 older individuals reported that their physical activity per week was less than 150min in the first phase. Among the 73 older individuals, 40 older individuals participated in the second phase. In the second phase, 32 older individuals does not meet at least 150min of moderate-to-vigorous physical activity(MVPA) suggested by DHHS. Finally, 6 older adults participated in the third phase. Five major themes emerged including 1) lack of proper information and service regarding physical activity, 2)lack of support from people close to them, 3)some limits due to chronic disease, 4)lack of appropriate facility, and 5)lack of opportunity due to existing inactive lifestyle. Based on these five themes, this study attempted to provide the improvement plans with socio-ecological model.

A Comparative Study on the Eating Behavior in Inchon : The Elderly Living in Home and the Elderly Nursing Home (인천 지역 가정 거주 노인과 시설 거주 노인의 식행동 비교 연구)

  • This study is investigate the eating behavior between the elderly living at home and the elderly at nursing home in Inchon City. This survey was carried out by questionaires. The result are summarized as the followings: 1. Both the elderly living at home and the elderly at nursing home are not smoking and drinking. They are in good health. 2. The elderly living at home and the elderly at nursing home have diseases like neuralgia, hypertension, diabetes etc. 3. Both the elderly living at home and the elderly at nursing home have methods of health care of a regular eating and a good sleeping. The elderly at nursing home have a regular medical examination but the elderly living at home do not. 4. Health giving drugs are depression of blood pressure, a medicine for the stomach and bowels, an anodyne. And health giving sports are walking and jogging. 5. The elderly living at home and the elderly at nursing home are significant relationship on knowledge of nutrition. 6. The elderly living at home and the elderly at nursing home prefer to taste sweet, boiling, korean foods. 7. elderly living at home have meats once a week and the elderly at nursing home have twice a week. Both the elderly living at home and the elderly at nursing home have fish, fruits, vegetables twice a week. The elderly at nursing home have milks twice a week. 8. The frequency of snack intake is higher the elderly at nursing home than the elderly living at home. 9. The elderly living at home and the elderly at nursing home are satisfied their dietary life.

A Review of Person-centered Care in Nursing Homes (노인요양시설의 인간 중심 케어 (Person-centered care)에 대한 연구)

  • The purpose of this study is to review person-centered care models and the scientific evidence for the impact of person-centered care in U.S. nursing homes, and to analyze the applicability of person-centered care for Korean long-term care system. The most commonly recognized person-centered care models were Eden Alternative, Green House, and Wellspring model. The core components of the three models were autonomy, close relationships, homelike environment, and staff empowerment. Thirty eight empirical studies including 10 model testing studies and 28 studies to examine the impact of person-centered care interventions indicated generally positive outcomes in terms of residents, family, and staff/organization level. The authors analyzed the current situation in the Korean long-term care system with respect to the specific core constructs of person-centered care, and proposed applicable models and practical suggestions in consideration of Korean context. Lastly, future considerations regarding policy and research were discussed.

A comparative study on the current oral health conditions of the elderly at home and welfare facilities (재가노인과 시설노인의 구강건강실태 비교연구 -삼척시에 거주하는 일부노인을 대상으로-)

  • This study investigated the current oral health conditions of the elderly at home and welfare facilities in their age over 65 years around some rural areas in Gangwon province, which would expect the fewer medical benefits even with lower interest than urban areas, despite of relatively high ratio of elder populations, so that it could prepare a basic document necessary to determine certain planned quantification for the benefit of elder's oral healthcare. As of the end of December 2004 both 50 elders at home and 50 elders at welfare facilities were randomly sampled in their age over 65 years in Samcheok city. As a result of this study, it was found that the elders at welfare facilities scored 15 pts. in DMFT index level typical of oral health conditions, which was higher than the elders at home. In addition, the elders at welfare facilities scored 26.0% in the coexistence of immobile bridge and partial denture higher than the elders at home with regard to the presence of intraoral prosthetic appliance. The results of analyzing the difference in the one-year dental visiting experience of respondents hereof showed that the elders at home were relatively more in ratio(62.0%) than those at welfare facilities, while many of the former group(38.0%) had relatively more handicap in masticatory movement than the latter one with regard to the conditions of dental prosthesis in use. Besides, many of the elders at facilities(30% or more) felt subjective symptoms of periodontal disease including bleeding or swelling, which indicates higher ratio than the elders at home. Finally, the elders at home used to brush their teeth at more frequency on a daily basis than those at facilities, while the latter group suffered general body disease more than the former group. Summing up, it is concluded that a formulated oral healthcare system will become more needed in near future than now for the benefit of the elderly living in welfare facilities, while nationwide policy-level supports would be urgent for them in the aspect of national welfare.
