A Study on Traffic Line Efficiency of Health Examination Centers Based on Space Syntax - Focused on the Spatial Cognition of the Testee Taking the National Examination Program (공간구문론에 기초한 건강검진센터 동선효율성 분석 연구 - 국가검진프로그램에 대한 수검자의 공간인지를 중심으로)

  • Purpose: With the increasing national interest in health, the number of health examination centers is growing rapidly, and it is growing as independent medical institutes separated from hospitals. With the growing functions and size of health examination institutes, considerations for testees, who are the most important users of the health examination centers, have taken the back seat. In particular, for health examination programs that take on a sequential traffic line, it is important to be aware of the space of each examination room, but the lack of a scientific evaluation method for this has resulted in great discomforts for testees using the health examination center. Method: Thus, this study proposes risk evaluation indices (RCF TCF, RC3, RR, ARR), and set a standard health examination program based on the national health examination program. This was applied to 11 different sized health examination centers to find their features, and together with identifying the trends of the indices, the following results were deduced. Result: 1) ARR showed a wide-range feature as the number of unit spaces increased, while RR were discovered regardless of the size, thus displaying local features. 2) The increase of ARR is affected more by internal factors in the health examination center than from outside factors. 3) By gender, when separating the basic health examination fields, the connective relation of the comprehensive health examination fields had a big effect on ARR. 4) By becoming larger, the fields of function become independent and the waiting space that results from it increases the number of total movement, so there is space for improvement in this.

The Association of Blood Pressures and Blood Biochemical Properties with BMI in Health Checkup Examinees (종합건강검진 수검자들의 비만수준과 혈압, 혈액학적 및 혈액생화학적 지표와의 관련성)

  • The purpose of this study was to investigate the association of blood pressures, hematological and blood biochemical properties according to the degree of obesity based on differential combinations of BMI in health checkup examinees. Study subjects were 3,731 adults of 20 years and over (2,312 males, 1,419 females), not recognized as taking medicines for or having cardiovascular diseases, who underwent health package check-up at the Korea Association of Health Promotion, Daejeon Branch from Jan. 2007 to Dec. 2009. As a results, the mean values of SBP, DBP, TG, TC, LDL-C, ALT, AST, GGT, ALP were significantly increased with higher level of BMI, but the HDL-C was significantly decreased with higher level of BMI. On correlation of BMI with various factors, the BMI was positively correlated with SBP, DBP, TG, TC, LDL-C and GGT, while the HDL-C was negatively correlated. Based on the result of ordinal logistic regression analysis, the risk ratio to the abnormal level of SBP, DBP, TG, ALT and GGT were significantly increased as BMI increased.

Prevalence of Clustering of Coronary Risk Factors in Health Checkup Examinees (일부 건강검진 수검자들의 관상동맥질환 위험인자의 군집별 유병률)

  • This study was conducted to assess the distribution of coronary risk factors(CRF) and evaluate the risk profile of coronary heart disease by the clustering of the CRF in health checkup examinees. The study sample consists of 3,345 adults aged 30s-60s years(1,917 males, 1,428 females), not recognized as taking medicines for or having cardiovascular diseases, who underwent health package check-up at the health examination center of a university-affliated hospital in Daejon City. The proportion of subjects with 1, 2, 3, 4 and or more risk factors were 27.6%, 26.5%, 16.9% and 9.1%, respectively, therefore, the 80.0% of total subjects had at least one more risk factor. From these findings, this study concluded that the prevalence rates of CRF were relatively high. These data provide further evidence that the early intervention for coronary health prevention and promotion in general adult population is necessary at the population level.

  • The health checkup conducted by the National Health Insurance Corporation is aimed at early detection and prevention of diseases. After the screening, you will receive a result sheet, which is a means for smooth communication between the medical staff and the patient. So far, design improvements have been made to the results. However, it is still pointed out that patients have low understanding and cognitive problems. Therefore, in this study, the opinions of the actual users, the medical examiners, and the designers are investigated to improve the design and to propose convenient checkup result sheets. The research method is to investigate the problems of information design research related to the results of the health examination and the user interviews on the results of the current health examination, and to improve the design of the existing results. In addition, we conducted a questionnaire survey on the results of the existing health checkups and the results of the improved checkups by the researchers, and finally designed them by referring to the results. As a result, I got a positive answer that many of the problems pointed out in the existing results were solved.

  • Recently, health examination centers have been changing from auxiliary medical facilities to key and independent medical facilities. However, it is not easy to improve medical facilities, including health examination centers, due to the variable characteristics of the relationship between humans and space. Therefore, this study was done to develop a pedestrian-based discrete event simulation analysis program to examine the problems and develop methods for improvement. The program was developed to analyze five evaluation indices and the density of examinees. The problems were derived by analyzing the required time, capacity, and queue size for each examination through simulations. We reduced the examination time and moving distance, increased the capacity, and distributed the queues by adjusting the medical services and relocating the examination rooms. The results were then quantitatively verified by simulations.

  • To n the field of nuclear medicine, with regard to checking regular patients, from the moment they register up to the doctor's diagnosis, the person in charge of the checks can find errors in the diagnosis, reexamine, reanalyze the results or save images to PACS. Through this process, the results obtained from the readings are delayed due to checks and additional tests which occur in hospitals, causing patient satisfaction and affected reliability. Accordingly, the purpose is to include visual inspection of the results to minimize error, improve efficiency and increase patient satisfaction. Nuclear medicine and imaging tests from examines at Asan Medical Center, Seoul, from March 2008 to December 2008, were analyzed for errors. The first stage, from January 2009 to December 2009, established procedures and know-how. The second stage from January 2010 until June 2010 conducted Pre-and Post-filtering assessment, and the third stage from July 2010 until October 2010 consisted of cross-checks and attaching stickers and comparing error cases. Of 92 errors, the 1st, 2nd and 3rd stage had 32 cases, and there were 46 cases after the 4th stage, with the overall errors reduced by 74.3% from 94.6%. In the field of general nuclear medicine, where various kinds of checks are performed according to the patient's needs, analysis, image composition, differing images in PACS, etc, all have the potential for mistakes to be made. In order to decrease error rates, the image can continuously Cross-Check and Confirm diagnosis.

Convergence Study on Factors that Influence Cancer Screening Rate in Korea and Japan (한일간 암검진 수검율에 영향을 미치는 정책적 요인에 대한 융합연구)

  • The purpose of this convergence study was to analysis the current status of the cancer screening and background healthcare systems in Korea and Japan. First, Cancer Screening Program is coordinated well with National Health Insurance Service(NHIS) under a unified insurer system in Korea. But in Japan, there are over 3,500 insurer and coordinating a comprehensive strategy for cancer screening promotion has been very difficult. Second, Rate of cancer screening is influenced by public healthcare system. In Korea public healthcare does not cover a wide rage of services. Almost free cancer screening provides high incentive to participation. It is very important to understand the impacts of each healthcare system when designing an efficient cancer screening system.

Analysis of Influencing Factors on Health Examination Acceptance Rate: Focused on the 7th National Health and Nutrition Survey Data (건강검진 수검률에 미치는 영향요인 분석 : 국민건강영양조사 제7기 자료를 중심으로)

  • The purpose of this study is to find a way to increase the examination rate by presenting the current state of national health examination. To this end, demographic factors(Gender, age, income level, education level) and health factors(Smoking, alcohol, obesity, subjective health status) were selected, and whether the selected factors were different for each group, and the examinees according to the number of hospitals and income level were visualized. As a result, except for subjective health status in health factors, population factors, and number of hospitals were all related to the examination. In addition, among the age factors of the demographic factor, those in their twenties and those with a low income level and those who were underweight among the health factors of obesity had a high rate of non-testing. Therefore, it is considered necessary to promote, support, and educate those untested by these groups.

The Precision Test Based on States of Bone Mineral Density (골밀도 상태에 따른 검사자의 재현성 평가)

  • Purpose: ISCD (International Society for Clinical Densitometry) requests that users perform mandatory Precision test to raise their quality even though there is no recommendation about patient selection for the test. Thus, we investigated the effect on precision test by measuring reproducibility of 3 bone density groups (normal, osteopenia, osteoporosis). Materials and Methods: 4 users performed precision test with 420 patients (age: $57.8{\pm}9.02$) for BMD in Asan Medical Center (JAN-2008 ~ JUN-2008). In first group (A), 4 users selected 30 patient respectively regardless of bone density condition and measured 2 part (L-spine, femur) in twice. In second group (B), 4 users measured bone density of 10 patients respectively in the same manner of first group (A) users but dividing patient into 3 stages (normal, osteopenia, osteoporosis). In third group (C), 2 users measured 30 patients respectively in the same manner of first group (A) users considering bone density condition. We used GE Lunar Prodigy Advance (Encore. V11.4) and analyzed the result by comparing %CV to LSC using precision tool from ISCD. Check back was done using SPSS. Results: In group A, the %CV calculated by 4 users (a, b, c, d) were 1.16, 1.01, 1.19, 0.65 g/$cm^2$ in L-spine and 0.69, 0.58, 0.97, 0.47 g/$cm^2$ in femur. In group B, the %CV calculated by 4 users (a, b, c, d) were 1.01, 1.19, 0.83, 1.37 g/$cm^2$ in L-spine and 1.03, 0.54, 0.69, 0.58 g/$cm^2$ in femur. When comparing results (group A, B), we found no considerable differences. In group C, the user_1's %CV of normal, osteopenia and osteoporosis were 1.26, 0.94, 0.94 g/$cm^2$ in L-spine and 0.94, 0.79, 1.01 g/$cm^2$ in femur. And the user_2's %CV were 0.97, 0.83, 0.72 g/$cm^2$ L-spine and 0.65, 0.65, 1.05 g/$cm^2$ in femur. When analyzing the result, we figured out that the difference of reproducibility was almost not found but the differences of two users' several result values have effect on total reproducibility. Conclusions: Precision test is a important factor of bone density follow up. When Machine and user's reproducibility is getting better, it’s useful in clinics because of low range of deviation. Users have to check machine's reproducibility before the test and keep the same mind doing BMD test for patient. In precision test, the difference of measured value is usually found for ROI change caused by patient position. In case of osteoporosis patient, there is difficult to make initial ROI accurately more than normal and osteopenia patient due to lack of bone recognition even though ROI is made automatically by computer software. However, initial ROI is very important and users have to make coherent ROI because we use ROI Copy function in a follow up. In this study, we performed precision test considering bone density condition and found LSC value was stayed within 3%. There was no considerable difference. Thus, patient selection could be done regardless of bone density condition.
