Epigenetics와 정신장애 (Epigenetics and Psychiatric Disorders)

  • In the post-genomic era, the mechanisms controlling activation of genes are thought to be more important. Gene-environment interactions are crucial in both development and treatment of psychiatric disorders as they are complex genetic disorders. Epigenetics is defined as a change of gene expression that occurs without a change of DNA sequence and can be heritable by certain mechanisms. Epigenetic changes play essential roles in control of gene activation. DNA methylation, chromatin remodeling and RNAi act as key mechanisms for epigenetic modifications of genes. Here, we review the basic mechanisms of epigenetics and discuss their potential involvement of human diseases, including psychiatric disorders.

AMPA 수용체의 조절이 지니는 정신과적 의의 (Psychiatric Implication for the Regulation of AMPA Receptor)

  • Glutamate receptors are important components of synaptic transmission in the nervous system. Especially, ${\alpha}$-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid (AMPA) receptors mediate most abundant excitatory synaptic transmission in the brain. There is elaborate mechanism of regulation of AMPA receptors including protein synthesis/degradation, intracellular trafficking, exocytosis/endocytosis and protein modification. In recent studies, it is revealed that functional dysregulation of AMPA receptors are related to major psychiatric disorders. In this review, we describe the structure and function of AMPA receptors in the synapse. We will introduce three steps of mechanism involving trafficking of AMPA receptors to neuronal membrane, lateral diffusion into synapses and synaptic retention by membrane proteins and postsynaptic scaffold proteins. Lastly, we will describe recent studies showing that regulation of AMPA receptors is important pathophysiological mechanism in psychiatric disorders.

정신분열병의 최신 뇌영상 연구 (Recent Neuroimaging Study in Schizophrenia)

  • Neuroimaging studies in schizophrenia have remarkably increased and provided some clues to understand its pathophysiology. Here, we reviewed the neuroimaging, studies including volume analysis, functional magnetic resonance imaging (MRI) and diffusion tensor imaging, and findings in both early stage schizophrenia and high-risk group. The reviewed studies suggested that the brain with schizophrenia showed both regional deficits and dysconnectivity of neural circuit in the first episode, even high-risk group as well as chronic schizophrenia. Multimodal neuroimaging or combined approach with genetic, electro-or magneto-encephalographic data could provide promising results to understand schizophrenia in the near future.

  • With a rapid development of neuroscience, the theories related to the pathophysiology of schizophrenia have been changed a lot from a simple hyperdopaminergic one to the various complicated ones. Among these, the theories regarding prefrontal cortex(PFC) pathology as a cause of schizophrenia are gaining more recognition as the results of neuroimaging and neuropsychological tests in schizophrenia consistently report abnormalities in PFC. Therefore, we first reviewed the unique characteristics of PFC in anatomy, neurochemistry and neurophysiology to enhance an understanding of those ones. Secondly, various neurotransmitter, neurodevelopmental and neural network theories of schizophrenia introduced recently were reviewed in terms of PFC pathology.

일주기 유형의 평가와 이해 (Assessment and Understanding of Chronotype)

  • 일주기 유형의 차이는 다양한 관점에서 이해되어야 한다. 기존 연구결과들에 따르면 일주기 유형 중 아침형이 몇몇 정신장애의 보호요인인 반면 저녁형은 위험 요인으로 나타났다. 일반적으로 아침형에서 나타나는 습관들은 저녁형보다 더 건강하다. 성인의 생활양식을 형성하는 과정에 있는 청소년 시기와 생활습관과 관련된 건강 문제가 잦아지는 노년기에 일주기 유형의 중요성이 더욱 크다고 볼 수 있다. 따라서 일주기 유형을 생물-심리-사회적 건강 모델에 포함시켜 보다 효율적인 예방 및 치료적 접근을 설계하는 것이 중요하겠다. 정신장애 치료에서도 일주기 리듬을 고려해야 한다. 일반적으로 취침-기상, 식사, 그리고 일상 활동을 규칙적인 아침형 패턴으로 변화시키는 것이 도움이 될 수 있다. 광치료(light therapy)와 멜라토닌 주입(melatonin administration)과 같은 전략들도 정신장애의 치료 및 재발 방지에 효과적일 수 있다. 일주기 유형 기저의 유전학을 이해하는 것 또한 어떤 사람이 특정한 생활 양식이나 정신장애에 취약한지 구별하는데 도움이 될 수 있다.

한국인 조현병 환자에서 Chromogranin B 유전자와 안구운동 이상의 연합에 대한 연구 (Association Analysis between Chromogranin B Genetic Variations and Smooth Pursuit Eye Movement Abnormality in Korean Patients with Schizophrenia)

  • Objectives According to previous studies, the Chromogranin B (CHGB) gene could be an important candidate gene for schizophrenia which is located on chromosome 20p12.3. Some studies have linked the polymorphism in CHGB gene with the risk of schizophrenia. Meanwhile, smooth pursuit eye movement (SPEM) abnormality has been regarded as one of the most consistent endophenotype of schizophrenia. In this study, we investigated the association between the polymorphisms in CHGB gene and SPEM abnormality in Korean patients with schizophrenia. Methods We measured SPEM function in 24 Korean patients with schizophrenia (16 male, 8 female) and they were divided according to SPEM function into two groups, good and poor SPEM function groups. We also investigated genotypes of polymorphisms in CHGB gene in each group. A logistic regression analysis was performed to find the association between SPEM abnormality and the number of polymorphism. Results The natural logarithm value of signal/noise ratio (Ln S/N ratio) of good SPEM function group was $4.19{\pm}0.19$ and that of poor SPEM function group was $3.17{\pm}0.65$. In total, 15 single nucleotide polymorphisms of CHGB were identified and the genotypes were divided into C/C, C/R, and R/R. Statistical analysis revealed that two genetic variants (rs16991480, rs76791154) were associated with SPEM abnormality in schizophrenia (p = 0.004). 노인 기질성 정신장애 환자의 급성 정신증상에 대한 리스페달 사셰와 할로페리돌 근주의 효과 비교 (Risperdal Sachet and Oral Lorazepam versus Intramuscular Haloperidol and Lorazepam Injection for Acute Psychotic Symptom in the Elderly Patients with Organic Mental Disorder)

  • Objectives : The purpose of present study was to investigate the effect, safety and tolerability of risperdal sachet(oral solution) with lorazepam tablet versus intramuscular haloperidol and lorazepam injection for management of acute psychotic symptom in the elderly with organic mental disorder. Methods : Total 37 patients who have dementia, medical or physical diseases, associated with acute psychotic symptom were randomly assigned to oral treatment with 1mg of risperdal sachet(oral solution) plus 1mg of lorazepam(N=17) or to intramuscular treatment with 2.5mg of haloperidol plus 2mg of lorazepam (N=20). The change of CGI scores was used for the evaluation of efficacy. Results : Mean score improvements at 15, 30, 60, and 120 minutes after treatment were statistically significant at each time point in both groups(p<0.001) and were similar in both groups(p=0.189). Conclusion : A single oral dose of risperdal sachet(oral solution) plus lorazepam was as effective and tolerable as parenterally administered haloperidol plus lorazepam for the rapid control of acute psychotic symptom in the elderly with organic mental disorder.

정신과 신체의 분화와 통합 (The Differentiation and Integration of Mind and Body)

  • 1960, 1970년대 이후 정신과 신체를 적절히 조화시키려는 경향이 나타났고, 최근에는 신경생물학과 유전학의 발달로 인해 더 이상 정신과 신체를 구분해서 생각할 수 없게 되었다. 이에 저자들은 정신-선체의 개념 및 그 관계에 대한 역사를 살펴보고, 최근에 논란되고 있는 정신-신체 모형 세 가지 즉, 생물정신사회적모델, 정신-신체동일이론, 기질적단위이론에 대한 비교 및 분자유전학과 환경과의 관계, 스트레스-체질모델을 고찰하였다.

  • 연구배경: 정신적인 면에 밀접한 관련이 있는 스포츠에 정신과적인 관심은 체계적으로 적용되지 못해왔으며 심리학에 비해 관심의 정도나 다양성은 많이 부족하였다. 이번 스포츠 정신의학의 소개는 기본적 개념과 영역을 간단히 논함으로써 정신과 의사로써 스포츠를 보는 다양한 시각을 제공하고 정신과 의사가 할 수 있는 기본적인 역할에 대해 소개하고자 한다. 결과 및 토론 : 운동선수들의 심리학적 발달 단계를 이해하는 것은 그들의 정신 구조를 알아가는데 많은 도움을 준다. 또한 어린 시절부터 지속되어온 일정한 성향은 시합 중에 혹은 연습 중에 간혹 보일 수 있는 문제점을 신속히 알아내는 데 많은 자료를 제공할 것이다. 운동의 정신 생물학은 운동 선수들의 정신 상태에 영향을 끼칠 수 있는 호르몬이나 신경 전달물질 같은 생물학적인 면을 논하게 된다. 운동 선수의 훈련이나 경기 중 일어날 수 있는 정신적 문제를 크게 5가지 항목으로 나눌 수 있다, choke, 슬럼프, 부상, 대인관계, 훈련딜레마. 선수들도 일반인들과 똑같이 정신과적 모든 질환에 노출되어 있다. 정신치료, 집단치료, 가족치료, 약물 치료 등 정신과 환자에게 사용될 수 있는 모든 치료법은 스포츠정신의학에서 사용될 수 있다. 결론: 비록 정신과 의사들이 영향을 끼칠 수 있는 중요한 부분들은 임상적 상황이 되겠지만 선수들 운동과 인생 전반에 걸쳐 벌어지는 선수들의 역할과 갈등에 관심 가짐으로써, 자신의 운동과 인생을 즐기며 살아가는 더 많은 인본주의적인 선수 출현에 일조했으면 한다.
