A Review of Occupational Therapy Approach for Complex Regional Pain Syndrome (복합부위통증증후군의 작업치료적 접근에 관한 고찰)

  • Complex Regional Pain Syndrome(CRPS) is a chronic neruopathic pain syndrome, most frequently encountered after trauma, nerve injury, fracture, cerebral vascular accident(CVA), spinal injury, and surgery to a limb, and affected by diverse factors such as disease process, intervention method, and other biopsychosocial components. Occupational therapy as a multidisciplinary treatment approach may be potentially useful when simultaneously targeting such biological, psychological, and social components. Biological treatment can focus on increasing functional use of extremity for promoting independence. Psychological intervention can include relaxation/biofeedback training and cognitive-behavioral treatments, and social approach can focus on recreational therapy and vocational rehabilitation. Occupational therapy will encourage functional restoration, pain management, and regaining of psychological components with collaboration between different disciplines.

  • While depression is certainly a prevalent disorder, it is often severe and debilitating and does not always have the good prognosis we have been led to expect. Social approaches to affective disorders have not been subjected to the same level of scrutiny as the interventions used in the management of schizophrenia. Psychosocial Rehabilitation is now at a critical stage. Psychoeducation, social skill training, cognitive remediation, family education, vocational rehabilitation and case management programs are essential for the rehabilitation of chronic depression.

  • Recently many researches support the use of traditional psychiatric treatments in the management of chronic pain. Chronic pain is a significant public health problem and frustrating to everyone affected by it. Psychiatrists offer skills with treatments now recognized as effective in the management of chronic pain. In addition to the diagnosis and treatment of psychiatric co-morbidity, the application of psychological treatments to chronic pain, and the development of interdisciplinary efforts to provide comprehensive health care to the patient disabled with chronic pain, psychiatrists have particular skill in pharmacological treatment that have proven efficacy for a variety of chronic pain conditions. With their expertise in the use of psychoactive medication plus their interest in the personal and family dynamics of patients, psychiatrists have the capacity to be involved in the treatment of patients with chronic pain. So, the author reported three cases of patient with pain disorder associated with psychological factors, and reviewed to propose that psychiatrists in Korea should take an active role in the care of these patients.

  • Because chronic pain disorder may has multiple causes or contributing factors, including physical, psychological, and socio-environmental variables, the treatment of patients with the disorder requires biopsychosocial approaches in a multidisciplinary setting. In treating chronic pain, it is important to address functioning as well as pain, and treatment should be to increase functional capacity and manage the pain as opposed to curing it. Therefore treatment goal should be adaptation to pain or minimizing pain with corresponding greater functioning. Treatment begins with the initial assessment, which includes evaluation of psychophysiologic mechanisms, operant mechanisms, and overt psychiatric comorbidity. Psychiatric treatment of the patients requires adherence to sound pharmacologic and behavioral principles. There are four categories of drugs useful to psychiatrist in the management of chronic pain patients : 1) narcotic analgesics, 2) nonsteroidal antiinflammatory drugs, 3) psychotropic medications, and 4) anticonvulsants, but antidepressants are the most valuable drugs in pharmnacotherpy for them. Psychological treatments tend to emphasize behavioral and cognitive-behavioral modalities, which are divided into self-management techniques and operant techniques. Psychodynamic and insight-oriented therapies are indicated to some patients with long-standing interpersonal dysfunction or a history of childhood abuse.

A Review of Prevention and Treatment of the Crimes on the Characteristics of Mental Illness (정신질환의 특성에 따른 범죄 예방과 치료에 관한 소고)

  • This study reviewed the important preceding studies of prevention and treatment of the crimes and therapies on the characteristics of mental illness, trying to investigate and identify more effective ways to prevent, decrease the crimes of the mentally ill and treat them. The main results were as follows. First, relationships between mental illness and crimes have been inconsistently reported, to investigate environmental stresses and triggers of the mentally ill additionally. Second, the term 'mentally illness' was to be defined more specifically, such as severe psychiatric disabled, antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse. If medication and psychosocial treatments were properly treated to the severe psychiatric disabled, their crime incidence and recidivism will be decrease enough to live with their neighbors in community. But the ways and processes of treating antisocial personality disorder, intellectual disability, anger·impulse control disorder, and drug abuse were very different from it, requiring more intense psychological, medical and biological interventions and social seclusion. The national campaigns and projects for their human rights and community care will be needed to promote the therapeutic effects for them. The limitation and future tasks were discussed.

Depression during Pregnancy and the Postpartum (임신 및 산후 우울증)

  • The pregnancy and postpartum period appear to be a time of heightened vulnerability for the development of major depression in some women. Postpartum depression affects 10% of women within a few weeks immediately postpartum. Postpartum depression is associated with disturbances in the mother-infant relationship, which in turn have an adverse impact on the course of child cognitive and emotional development. Depression during pregnancy is also common, although it has been relatively neglected. Psychopathological symptoms during pregnancy have physiological consequences for the fetus. Understanding the aetiology of perinatal depression requires integrating of multiple psychosocial and biological risk factors. The treatment of depressed pregnant women requires skilled decision making by psychiatrists. Risk-benefit analysis is appropriate method for intervention fur depression in pregnancy. Effective treatments for depression in pregnancy include psychotherapy, antidepressant medication and electroconvulsive therapy. In treatment of postpartum depression, the biological, psychological, and social interventions are included. Prescribing antidepressants(such as fluoxetine), estrogen in severe and chronic cases, and counselling can be effective for improving maternal mood and aspects of infant outcome. Ongoing research is directed to further elucidating neurohormonal and psychosocial contributions to depression during pregnancy or postpartum. Screening for risk factors and symptoms for depression need to be incorporated into antenatal and pediatric clinics.

The positive contribution of Spirit (영성의 긍정적 기여)

  • Lately in the medical world, they realized that the spirit is important for health best of all, and in the WHO, the meaning of health was "The condition of being sound in body, mind", now as they added the spirit to there, it became "The condition of being sound in body, mind, or spirit". Even though we know well about the importance of spirit, but it is difficult to make sure what that mean is. Even in the Psychology, they didn't distinguish clearly the spirit from the mind. In this report, we have revealed the traits of spirit from the classic theory of spirit to the today's, as studying at side of biology and psychology. Especially the spirit have been developed remarkably for a long time at every religion. Recently in the medical world, they connected 'the spirit of Buddhism' to 'MBSR', and 'the spirit of Christianity' to 'Breakout Principle' with medical method. So, we have studied how the spirit give the positive contribution to the Psycho-therapy.

  • Leo Kanner (1943)의 자폐증에 관한 획기적 논문발표 50주년을 맞아, 자폐증의 개념변천, 분류, 의학, 원인설 및 자폐증의 본질을 문헌고찰을 통해 살펴 보았다. 초기에 자폐증을 정신병의 아형으로 보다가 1980년 (DMS-III)을 기점으로 전반적 발달장애로의 개념 변천이 일어났다 원인설도, 초기의 심리${\cdot}$환경설은 지지 받지 못하고 1960년대에는 신경${\cdot}$생물학적 이상이 자폐병리의 기저를 이룸이 분명해 졌고 1970년대에는 지각과 운동, 감각과 인지 통합의 결함, 심각한 언어, 인지의 장해가 일차적인 결함으로 생각 되었다 최근 1980년 후반기 부터 상징적${\cdot}$표상적 인지의 결함, 타인의 감정과 생각의 이해 결함, 사회적${\cdot}$정감적 표현의 결함등 사회${\cdot}$정서발달의 이상이 자폐의 근본적 결함이라는 비교 관찰 연구가 많이 보고되어, 자폐증의 근본적이고 일차적인 결함이 정감적 접촉의 선천적 장애라는 Kanner의 놀라운 임상적 통찰을 증명해 주고 있다. 저자는 이상의 광범위한 문헌 고찰을 통해 자폐장애를 일차성 애착장애로 개념화하고 앞으로 치료, 교육의 방향도 일차적으로 사회${\cdot}$정서발달에 촛점을 두어야 하며, 특히 어머니와의 애착증진 치료가 필요함을 제안하고 있다.

Diagnosis and Treatment of Temporomandibular Disorders (측두하악장애의 진단 및 치료)

  • Temporomandibular disorders(TMD) have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory muscles, and associated structures. Since Dr. Costen, an otolaryngologist, published his article in 1934 claiming that pain in and around the jaw and "related ear symptoms" improved with alteration of the bite, diagnosis and treatment of temporomandibular disorders(TMD) have been within the concept of occlusion. However most of the modern descriptions for TMD no longer include occlusal disorders within their domain. Despite this trend toward the exclusion of occlusal disorders from TMD domain, the historical linkages between TMD and occlusal therapy are still strong. Currently the most popular theories regarding TMD etiology are based on the biopsychosocial model. In the future, treatment modalities should be directed at the pathophysiological processes of joint and muscle pain as well as the psychosocial aspects of chronic pain.

The Past, Present and Future of Korean Research in Psychosmatic Medicine (한국 정신신체의학연구의 과거와 현재 그리고 미래)

  • Psychosomatic medicine is a part of medicine which is to find the effect of psychological, behavioral, and medical factors on the human body and disease. In the early $20^{th}$ century, the idea of psychogenesis had been developed and resulted in the concept of psychosomatic disease which was believed to be caused by psychological factors. However a multifactorial model of illness developed and it allowed illness to be viewed as a result of biopsychosocial interactions. The following have been highlighted by consultation-liaison psychiatry. Psychosomatic medicine has addressed stress and psychiatric factors which affect the etiology, course, and treatment of medical disorders. Moreover it contributes the growth of other related disciplines such as psychoneuroendocrinology, psychoimmunology, behavioral medicine, health psychology and quality of life research. Nowadays, psychosomatic field becomes enlarged because medical and surgical departments have been developed rapidly, and research methods and tools have brought forth rapid progress and advance in medical science. Therefore the author reviews the past and present psychosomatic researches and suggests the future of psychosomatic research in Korea.
