Unmet Medical Service Needs in Family Caregivers of Terminal Cancer Patients (말기암환자 가족간병인의 미충족 의료 분석)

  • Purpose: The unmet medical service needs of caregivers critically influence their caring for terminal cancer patients, but not much research has been done in this regard. Thus, the purpose of this study is to investigate the association between caregivers' characteristics and their unmet medical service needs. Methods: The survey was conducted with 109 family caregivers of terminal cancer patients admitted to four hospice units. The data were collected from March 2014 through December 2014 using a structured questionnaire. The unmet medical service needs were measured using 14 items which were adopted and modified by authors. Results: Seven areas of unmet medical service needs were shown to be significant. A well-educated group showed stronger needs for counsel about cancer screening and complementary-alternative medicine and health supplement food. A never-smoked group was identified with less need for sexual dysfunction counsel. Counsel about family and personal relations was more necessary for current drinkers and current workers, and less necessary for the married. Insurance counsel was more needed for a no-religion group. Occupation counsel was less necessary for healthy patients. Financial support was less necessary for the married group. Conclusion: Based on the results, it is highly recommended to further investigate the unmet medical service needs of family caregivers for terminal cancer patients and causes of the unmet needs.

Needs and Satisfaction of Cancer Patients on the Medical Services in Jeju Special Self-Governing Province (제주지역 암환자의 의료서비스 요구도 및 만족도 분석)

  • Purpose: The purpose of the present study was to investigate needs and satisfaction on the medical services of cancer patients in Jeju Special Self-Governing Province. Methods: Total 174 cancer patients, who visited at the clinic of Jeju National University Hospital, submitted informed consent and participated in this study from July 13 to July 30, 2009. Self questionnaire was used and data were analyzed with Kolmogorov-Smirnov test, Mann-Whitney U test, ANOVA, and Kruskal-Wallis test. Results: Participants expressed the needs of most economical support (3.38 out of 4), followed by counseling of treatment plan (3.22), information of disease (3.07), and disease management except cancer (2.97). Participants were satisfied most with religious counseling (3.41), followed by nursing service support (3.39), employment counseling (3.26), and counseling for family or interpersonal relationships (3.26). The satisfaction of economical support was the lowest (1.98). Satisfaction of men was higher than women, and needs in patients who were living with children was the highest. Patients who were living alone or with children showed the lowest satisfaction about the medical services. There were no significant differences in the general characteristics, however, participants who were older than 60 years of age or had higher income showed lower needs and higher satisfaction. There were no significant differences in the medical characteristics, however, thyroid cancer patients and patients who were treated with radiation therapy or transarterial embolization showed low satisfaction. Conclusion: Cancer patients seemed to need more economical support, information of treatment or disease, and symptom management. Furthermore, there were various needs about the services, depending on family formation or economical support of patients. Therefore, it is certain that patients who were suffering from other cancers, except the 5 major cancers, needed more services. In conclusion, continuous and systemic policy to consider patient's characteristics and needs are needed in community as well as health care system.

Dental management of the patient difficulty in swallowing (섭식연하장애와 치과적 관리)

  • Swallowing disorders or dysphagia, which are difficulties with the act of swallowing, can occur at different phases such as oral, pharyngeal, and esophageal phase in the swallowing process. These disorders can be caused by structural, functional abnormalities or psychological problems. Dentists can actively provide patients suffering from dysphagia with comprehensive professional care, especially from the cognitive stage of food to the pharyngeal stage of the swallow. Many cases of dysphagia can be improved with careful managements based on dental professionals' knowledge, including meticulous evaluation of masticatory and swallowing functions, training on eating and swallowing, and fabrication of palatal or lingual augmentation prosthesis. The important thing is that prevention of these disorders through the oral health care instruction and planned follow-up dental visit at periodic intervals in order to manage the problems caused by anatomical, functional, and psychological reason.

Design of Smart Pillow System for Managing Sleep Apnea (수면무호흡증을 관리를 위한 스마트 베개 시스템의 설계)

  • Specialists have developed pillows that take into account sleep science and ergonomics, such as comfort for lying on your side. This pillow is made of natural latex material, and improved resilience after a certain period of time. A new idea was added to the pillow, which was naturally used for sleep, but could it add additional features for health care. Here, health care targets sleep apnea, which is known to be associated with serious illness. The purpose of this paper is to design a comprehensive service that uses a pressure sensor and a voice sensor to obtain information and to identify abnormal symptoms related to diseases from this information and to refer them to a specialist. It also covers the basic design and implementation to confirm the success of this system. Based on this design, the information obtained will be converted into a DB, and a server system for consultation with a specialist will be completed to upgrade the role of assistive health devices for sleep apnea.

맥진기 연구에 대한 중의사의 인식에 대한 수요조사

  • 한중 맥진기 관련 학술 교류와 맥진기 수출을 위한 전략으로 중국의 광주중의약대학 중의사를 대상으로 맥진과 맥진기에 대한 인식을 조사하여 보고하는 바이다. 임상에서 가장 중요하게 생각하고 활용도가 높은 전통적인 병증으로 장부병증과 팔강병증으로 나타났으며, 한방 진단방법 중 가장 중요하게 생각하고 활용하는 방법으로 문진과 맥진으로서, 그 이유는 진단 효율성과 환자와의 상담으로 나타났다. 또한 임상에서 한방 진단기기의 중요한 이유로는 측정기술과 데이터의 신뢰를 들었으며, 중요하지 않다고 생각하는 이유로는 측정기술과 데이터의 신뢰성 부족과 전통적인 진단법을 보조하기엔 적합하지 못함을 들었다. 진단방법 중에서 맥진의 중요도와 활용도는 75%가 가장 높게 나타났고, 맥진법 중에서 가장 잘 활용하는 부위는 촌구맥진법으로 나타났다. 촌구 맥진시 가장 중요하게 생각하는 요소는 환자의 자세와 손목 높이와 촌관척 부위였으며, 단안(單按)과 총안(總按)의 순서는 총안(總按)후 단안(單按)이 단연 우수하였다. 임상의 중요 맥상으로는 부침(浮沈)을 들었으며, 맥상 형성 기전과 표준화 연구에 가장 적합한 연구 방법론은 위수형세론에 입각한 문헌 연구와 병증별 맥상에 대한 임상 데이터 구축으로 나타났다. 그리고 대표적인 노인 특유의 맥상은 미세약맥이었으며, 그 이유는 노화로 인한 체력저하와 기혈부족을 들었다. 그리고 맥진기와 연계하는 만성 생활습관병 병증지표 연구개발에 가장 적합하다고 판단되는 병증 체계는 장부병증이었으며, 질병은 고혈압과 동맥경화증으로 나타났다. 또한 임상에서의 활용도와 유의성이 높은 맥진기로써의 주된 기능으로는 오장육부 상태 진단을 들었으며, 맥진기의 임상적인 활용도를 높일 수 있는 현대적인 임상 분야는 심혈관 질환이 가장 높게 나타났다. 맥진기 개발에 선행되어야 할 기반연구로는 촌관척 부위 찾기의 기준 확립과 임상 변증과의 관계 확립으로 나타났으며, 제공 받고자 하는 맥상 정보구성은 환자의 맥상명과 맥상 정도와 환자의 맥상에 따른 병증명 제시로 나타났으며, 제공되는 병증 서비스로는 장부병증이 가장 높게 나타났다.

Clinical Applications of Chromosomal Microarray Analysis (염색체 Microarray 검사의 임상적 적용)

  • Chromosomal microarray analysis (CMA) enables the genome-wide detection of submicroscopic chromosomal imbalances with greater precision and accuracy. In most other countries, CMA is now a commonly used clinical diagnostic test, replacing conventional cytogenetics or targeted detection such as FISH or PCR-based methods. Recently, some consensus statements have proposed utilization of CMA as a first-line test in patients with multiple congenital anomalies not specific to a well-delineated genetic syndrome, developmental delay/intellectual disability, or autism spectrum disorders. CMA can be used as an adjunct to conventional cytogenetics to identify chromosomal abnormalities observed in G-banding analysis in constitutional or acquired cases, leading to a more accurate and comprehensive assessment of chromosomal aberrations. Although CMA has distinct advantages, there are several limitations, including its inability to detect balanced chromosomal rearrangements and low-level mosaicism, its interpretation of copy number variants of uncertain clinical significance, and significantly higher costs. For these reasons, CMA is not currently a replacement for conventional cytogenetics in prenatal diagnosis. In clinical applications of CMA, knowledge and experience based on genetics and cytogenetics are required for data analysis and interpretation, and appropriate follow-up with genetic counseling is recommended.

Research on Non-Union Representation for Contingent Worker's Representation Mechanism : Focusing on CSO's Representation Mechanism in Civil Society (비정규 노동자 이해대표 기제로서 비노조 이해대표에 관한 연구 : 시민사회진영 내 CSO를 통한 이해대변 기제를 중심으로)

  • While the unionization of contingent workers is at a standstill, the newly emerging peripheral workers' representation mechanism is CSO(Civil Society Organization). Unlike trade union's representation based on coercive collective bargaining and agreements CSO represents the interests of marginal workers through counseling, advocacy, public relations, and education. In the case of Korea, since the 2000s, labor CSOs, called "contingent centers", have started to spread. As a result of the survey, contingent centers which are contracted out by local governments show the possibility that the representation activities are restricted, because most of their resources are covered from the local government. This is because CSOs have limitations due to a dilemma in the process of representing funders and the represented. Nevertheless, the contingent workers' CSO in Korea shows the possibility of supplementing and supporting the role of trade unions' representation. In this regard, through the solidarity of trade unions and CSOs, it will be possible to expand the representation of unorganized and contingent workers in Korean society.

Effect Analysis of Data Imbalance for Emotion Recognition Based on Deep Learning (딥러닝기반 감정인식에서 데이터 불균형이 미치는 영향 분석)

  • In recent years, as online counseling for infants and adolescents has increased, CNN-based deep learning models are widely used as assistance tools for emotion recognition. However, since most emotion recognition models are trained on mainly adult data, there are performance restrictions to apply the model to infants and adolescents. In this paper, in order to analyze the performance constraints, the characteristics of facial expressions for emotional recognition of infants and adolescents compared to adults are analyzed through LIME method, one of the XAI techniques. In addition, the experiments are performed on the male and female groups to analyze the characteristics of gender-specific facial expressions. As a result, we describe age-specific and gender-specific experimental results based on the data distribution of the pre-training dataset of CNN models and highlight the importance of balanced learning data.

Self-Diagnosing Disease Classification System for Oriental Medical Science with Refined Fuzzy ART Algorithm (Refined Fuzzy ART 알고리즘을 이용한 한방 자가 질병 분류 시스템)

  • In this paper, we propose a home medical system that integrates a self-diagnosing disease classification system and a tele-consulting system by communication technology. The proposed disease classification system supports to self-diagnose the health condition based on oriental medical science using fuzzy neural network algorithm. The prepared database includes 72 different diseases and their associated symptoms based on a famous medical science book "Dong-eui-bo-gam". The proposed system extracts three most prospective diseases from user's symptoms by analyzing disease database with fuzzy neural network technology. Technically, user's symptoms are used as an input vector and the clustering algorithm based upon a fuzzy neural network is performed. The degree of fuzzy membership is computed for each probable cluster and the system infers the three most prospective diseases with their degree of membership. Such information should be sent to medical doctors via our tele-consulting system module. Finally a user can take an appropriate consultation via video images by a medical doctor. Oriental medical doctors verified the accuracy of disease diagnosing ability and the efficacy of overall system's plausibility in the real world.

A Study on the Marketing Strategy for Cosmeceuticals in Pharmacies (기능성화장품의 마케팅전략에 관한 연구 -약국을 중심으로-)

  • The purpose of this research is to examine the level of understanding, current management and degree of satisfaction about 'Cosmeceuticals' among pharmacists running pharmacists. Using this research as a foundation, pharmacies are able to prepare an implementation plan for the marketing of 'Cosmeceuticais'. The level of understanding among pharmacists surveyed was found to be rather low as yet, and it will be necessary to increase their specialized knowledge. Through this analysis of current conditions, several means for pharmacies to implement marketing of 'Cosmeceuticals' are evident. These include expanding the definition of 'Cosmeceuticals' reforming the system for inspecting 'Cosmeceuticals', developing and distributing various specialized 'Cosmeceuticals', initiating training for increasing the level of specialized knowledge of pharmacists, developing public information by corporations and consumer networks, driving coordinated marketing with medical associations, linking marketing with health foods, ensuring display space at pharmacies, and offering knowledgeable consultations and detailed information about the products. This research is significant in that it shows that a plan reforming pharmacy management, which is affected by the environment in pharmacies such as the separation of pharmacies from medical practices, should include the an increase in marketing for 'Cosmeceuticals'. This research will be of great usefulness in forming a foundation for the sales strategy and manufacturing of 'Cosmeceuticals' by pharmacists, for the training of distributors about 'Cosmeceuticals', and for developing a distribution strategy.
