Interaction of Rifampin and Warfarin (리팜핀이 와파린의 항응고 효과에 미치는 영향)

  • Background: It is well known that rifampin decreases the hypoprothrombinemic effect of warfarin by induction of cytochrome P-450 enzyme in healthy volunteer. However, in patients the dosage schedule of warfarin during rifampin therapy is not established. Therefore, patients taking both rifampin and warfarin were reviewed to find out the adequate dosage schedule of warfarin in addition to side effects by interaction of two drugs. Method: Patients taking both rifampin and warfarin were retrieved from patients who were admitted due to heart disease and tuberculosis at Boochun Sejong Hospital from January of 1995 to August of 1999. To decide the adequate dosage of warfarin, the dosage of warfarin before, during, and after rifampin was evaluated in patients who kept adequate hypoprothrombinemic effect of warfarin during rifampin. To decide the adequate dosage schedule of warfarin, the time interval from the beginning of rifampin to normalization of prothrombin time(INR$\geq$1.1) was evaluated. And, the side effects by interaction of two drugs were reviewed. Results: All 12 patients taking both rifampin and warfarin were retrieved. Among them only 6 kept adequate hypoprothrombinemic effect of warfarin during rifampin. The dosage of warfarin during rifampin was $2.4{\pm}0.6$(mean$\pm$standard deviation) times as much as that before rifampin but the dosage after rifampin was the same as that before rifampin. The time interval from the beginning of rifampin to normalization of prothrombin time was $5.8{\pm}2.9$(mean${\pm}$standard deviation) days. 2 out of 12 had complication related to the interaction of rifampin and warfarin, one cerebral embolism just after the beginning of rifampin and the other cerebral hemorrhage just after the discontinuation of rifampin. Conclusion: When both rifampin and warfarin are prescribed, it would be a possible method to be confirmed by prospective study that warfarin be gradually increased about 2 times more than that without rifampin over 1 week or so after the beginning of rifampin and be tapered to the same dosage as that before rifampin when rifampin is discontinued. And, it would be prudent that prothrombin time be monitored frequently during rifampin and warfarin therapy, especially the beginning or discontinuation of rifampin.

User-Perspective Issue Clustering Using Multi-Layered Two-Mode Network Analysis (다계층 이원 네트워크를 활용한 사용자 관점의 이슈 클러스터링)

  • In this paper, we report what we have observed with regard to user-perspective issue clustering based on multi-layered two-mode network analysis. This work is significant in the context of data collection by companies about customer needs. Most companies have failed to uncover such needs for products or services properly in terms of demographic data such as age, income levels, and purchase history. Because of excessive reliance on limited internal data, most recommendation systems do not provide decision makers with appropriate business information for current business circumstances. However, part of the problem is the increasing regulation of personal data gathering and privacy. This makes demographic or transaction data collection more difficult, and is a significant hurdle for traditional recommendation approaches because these systems demand a great deal of personal data or transaction logs. Our motivation for presenting this paper to academia is our strong belief, and evidence, that most customers' requirements for products can be effectively and efficiently analyzed from unstructured textual data such as Internet news text. In order to derive users' requirements from textual data obtained online, the proposed approach in this paper attempts to construct double two-mode networks, such as a user-news network and news-issue network, and to integrate these into one quasi-network as the input for issue clustering. One of the contributions of this research is the development of a methodology utilizing enormous amounts of unstructured textual data for user-oriented issue clustering by leveraging existing text mining and social network analysis. In order to build multi-layered two-mode networks of news logs, we need some tools such as text mining and topic analysis. We used not only SAS Enterprise Miner 12.1, which provides a text miner module and cluster module for textual data analysis, but also NetMiner 4 for network visualization and analysis. Our approach for user-perspective issue clustering is composed of six main phases: crawling, topic analysis, access pattern analysis, network merging, network conversion, and clustering. In the first phase, we collect visit logs for news sites by crawler. After gathering unstructured news article data, the topic analysis phase extracts issues from each news article in order to build an article-news network. For simplicity, 100 topics are extracted from 13,652 articles. In the third phase, a user-article network is constructed with access patterns derived from web transaction logs. The double two-mode networks are then merged into a quasi-network of user-issue. Finally, in the user-oriented issue-clustering phase, we classify issues through structural equivalence, and compare these with the clustering results from statistical tools and network analysis. An experiment with a large dataset was performed to build a multi-layer two-mode network. After that, we compared the results of issue clustering from SAS with that of network analysis. The experimental dataset was from a web site ranking site, and the biggest portal site in Korea. The sample dataset contains 150 million transaction logs and 13,652 news articles of 5,000 panels over one year. User-article and article-issue networks are constructed and merged into a user-issue quasi-network using Netminer. Our issue-clustering results applied the Partitioning Around Medoids (PAM) algorithm and Multidimensional Scaling (MDS), and are consistent with the results from SAS clustering. In spite of extensive efforts to provide user information with recommendation systems, most projects are successful only when companies have sufficient data about users and transactions. Our proposed methodology, user-perspective issue clustering, can provide practical support to decision-making in companies because it enhances user-related data from unstructured textual data. To overcome the problem of insufficient data from traditional approaches, our methodology infers customers' real interests by utilizing web transaction logs. In addition, we suggest topic analysis and issue clustering as a practical means of issue identification.

  • In the intracranial regions, an accurate delineation of the target volume has been difficult with only the CT data due to poor soft tissue contrast of CT images. Therefore, the magnetic resonance images (MRI) for the delineation of the target volumes were widely used. To calculate dose distributions with MRI-based RTP, the electron density (ED) mapping concept from the diagnostic CT images and the pseudo CT concept from the MRI were introduced. In this study, the look up table (LUT) from the fifteen patients' diagnostic brain MRI images was created to verify the feasibility of MRI-based RTP. The dose distributions from the MRI-based calculations were compared to the original CT-based calculation. One MRI set has ED information from LUT (lMRI). Another set was generated with voxel values assigned with a homogeneous density of water (wMRI). A simple plan with a single anterior 6MV one portal was applied to the CT, lMRI, and wMRI. Depending on the patient's target geometry for the 3D conformal plan, 6MV photon beams and from two to five gantry portals were used. The differences of the dose distribution and DVH between the lMRI based and CT-based plan were smaller than the wMRI-based plan. The dose difference of wMRI vs. lMRI was measured as 91 cGy vs. 57 cGy at maximum dose, 74 cGt vs. 42 cGy at mean dose, and 94 cGy vs. 53 at minimum dose. The differences of maximum dose, minimum dose, and mean dose of the wMRI-based plan were lower than the lMRI-based plan, because the air cavity was not calculated in the wMRI-based plan. These results prove the feasibility of the lMRI-based planning for brain tumor radiation therapy.

Occlusal Analysis of the Subjects with Chewing Side Preference Using the T-Scan II System (T-Scan II 시스템을 이용한 습관적 편측저작자들의 교합 분석)

  • While orofacial pain or various dental factors are generally considered as the primary cause of unilateral chewing tendency, there exist several studies indicating that dental factors did not affect the preferred chewing side. The aim of this study was to examine difference of occlusal scheme between the subjects with and without chewing side preference. The difference between the chewing and non-chewing sides in the unilateral chewing group was investigated as well. Computerized, T-Scan II system was used for occlusal analysis. 20 subjects for the unilateral chewing group (mean age of $25.25{\pm}2.84$ years) and 20 subjects for the bilateral chewing group (mean age of $27.00{\pm}5.07$ years) were selected by a questionnaire on presence or absence of chewing side preference and those with occlusal problem or pain and/or dysfunction of jaw were excluded. T-Scan recordings were obtained during maximum intercuspation and excursion movement. The number of contact points, relative occlusal force ratio between right and left sides, tooth sliding area and elapsed time throughout the maximum intercuspation were calculated. Elapsed time for excursion was also investigated. The results of this study shows that the unilateral chewing group had the smaller average tooth contact areas compared with those of the bilateral group (p<0.005). In the unilateral chewing group, the contact areas of non-chewing side are smaller than those of chewing side (p<0.005). The contact areas on their preferred sides were not significantly different with those of right or left side of the subjects without chewing side preference. There was no significant difference in the elapsed time during maximum intercuspation and lateral excursion, the sliding areas and relative of right-to-left occlusal force ratio between the two groups. From the results of this study, it is likely that individuals prefer chewing on the side with more contact areas for efficient chewing.

History and Future Direction for the Development of Rice Growth Models in Korea (벼 작물생육모형 국내 도입 활용과 앞으로의 연구 방향)

  • A process-oriented crop growth model can simulate the biophysical process of rice under diverse environmental and management conditions, which would make it more versatile than an empirical crop model. In the present study, we examined chronology and background of the development of the rice growth models in Korea, which would provide insights on the needs for improvement of the models. The rice crop growth models were introduced in Korea in the late 80s. Until 2000s, these crop models have been used to simulate the yield in a specific area in Korea. Since then, improvement of crop growth models has been made to take into account biological characteristics of rice growth and development in more detail. Still, the use of the crop growth models has been limited to the assessment of climate change impact on crop production. Efforts have been made to apply the crop growth model, e.g., the CERES-Rice model, to develop decision support system for crop management at a farm level. However, the decision support system based on a crop growth model was attractive to a small number of stakeholders most likely due to scarcity of on-site weather data and reliable parameter sets for cultivars grown in Korea. The wide use of the crop growth models would be facilitated by approaches to extend spatial availability of reliable weather data, which could be either measured on-site or estimates using spatial interpolation. New approaches for calibration of cultivar parameters for new cultivars would also help lower hurdles to crop growth models.

Development of Stand Yield Table Based on Current Growth Characteristics of Chamaecyparis obtusa Stands (현실임분 생장특성에 의한 편백 임분수확표 개발)

  • We constructed a stand yield table for Chamaecyparis obtusa based on data from an actual forest. The previous stand yield table had a number of disadvantages because it was based on actual forest information. In the present study we used data from more than 200 sampling plots in a stand of Chamaecyparis obtusa. The analysis included theestimation, recovery and prediction of the distribution of values for diameter at breast height (DBH), and the result is a valuable process for the preparation ofstand yield tables. The DBH distribution model uses a Weibull function, and the site index (base age: 30 years), the standard for assessing forest productivity, was derived using the Chapman-Richards formula. Several estimation formulas for the preparation of the stand yield table were considered for the fitness index, and the optimal formula was chosen. The analysis shows that the site index is in the range of 10 to 18 in the Chamaecyparis obtusa stand. The estimated stand volume of each sample plot was found to have an accuracy of 62%. According to the residuals analysis, the stands showed even distribution around zero, which indicates that the results are useful in the field. Comparing the table constructed in this study to the existing stand yield table, we found that our table yielded comparatively higher values for growth. This is probably because the existing analysis data used a small amount of research data that did not properly reflect. We hope that the stand yield table of Chamaecyparis obtusa, a representative species of southern regions, will be widely used for forest management. As these forests stabilize and growth progresses, we plan to construct an additional yield table applicable to the production of developed stands.

  • With the increasing age of motherhood in recent years, attributed to late marriages due to social or environmental factors, the Down's syndrome screening test using biochemical markers has become essential for pregnant women. The process of diagnosing Down's syndrome pregnancy in the high-risk group subjects involves chromosomal analysis, which is performed on samples obtained through invasive procedures such as chorionic biopsy or amniotic fluid. Thus, to reduce unnecessary invasive tests and lower the risk to mother and fetus, it is important to identify a screening test with low risk and high Down's syndrome detection rate. Recently, as the average age of mothers has increased, numerous inspection agencies have classified high-risk mothers as women over the age of 35 years. This study evaluated a total of 36,436 pregnant women aged between 17 to 46 years, and who requested prenatal screening at an inspection agency in Yongin in 2018. Test (13,690 people) Four tests were conducted by applying the time-resolved fluoroimmunoassay method using the direct sandwich and indirect sandwich technology, and the immunoassay method using the sandwich method. We aimed to confirm the difference in positivity rate with increasing age of the subjects. We believe that in future, data obtained from this study will be very useful for the prevention and treatment of Down's syndrome risk at varied inspection institutions, and for prospective mothers.

Yeoheon Jang Hyun Gwang's Design of Self-organization for Completion of Confucian Truth (여헌(旅軒) 장현광(張顯光)의 실학적(實學的) 설계(設計) - 『역학도설(易學圖說)』과 위기지학(爲己之學) -)

  • This study is an argument that Yeoheon Jang Hyun Gwang's Yeok-Hak-Do-Seol has been on the context of the typical Confucian methodology of self-organization for a sage's or superman's character (爲己之學) and completed the methodology. Therefore the context from Confucius to Southern Song China Era is argued as a context of development and systematization of the methodology. In addition the fact that Zhuxi(朱熹) and his students had systimatization of the methodology with the publications of some important textbooks is also explained. The most stressed thing of this study is Yeoheon's Yeok-Hak-Do-Seol is just the text that succeeded them as the methodology of self-organization and had a special purpose of completion of the methodology not only with comprehensive items of practice but also intuition into the human's life and the world by his mastery of Yeokhak(易學). The viewpoint this study keeps is the Confucian methodology of self-organization is considered for the purpose of development and upholding the Confucian truth, and thus the methodology itself has been regarded as the necessary one that is the closest approximation to a pursuit of Confucian truth. According to this viewpoint we can observe the features of methodology of the pursuit of Confucian truth from the Yeok-Hak-Do-Seol. However the Yeok-Hak-Do-Seol has its purpose merely on a human being's self-organization but also on an enormous enterprise to make the universe peace or sustainability of the world. His stress on the enterprise shows that his methodology is not a merely repetition of the tradition of the Zhuzixue but a creative deveopment of the tradition. The other feature is that his methodology is systematization on the basis of Yeokhak, or the study of Yi-jing(易經). The main method he intensified is easiness and simplification that is the main point which he extracted from Yi-jing as the most important and necessary way of life.

Midterm Results of Bipolar Hemiarthroplasty for Unstable Intertrochanteric Femoral Fractures Using a Type 3C Cementless Stem (불안정성 대퇴골 전자간 골절에 3C형 무시멘트 대퇴 스템을 이용한 고관절 반치환술의 중기 결과)

  • Purpose: Bipolar hemiarthroplasty is used as an alternative to open reduction and internal fixation for unstable intertrochanteric fractures in elderly patients. Recent advances in medical systems and technologies have resulted in increased survival rates after intertrochanteric fractures of the femur, requiring selection of the appropriate femoral stems considering the mid- to long-term duration of survival. Hemiarthroplasty was performed for unstable intertrochanteric fractures using a double tapered quadrilateral femoral stem (C2 stem), and the clinical and radiological results were evaluated as a five-year follow-up post-surgery. Materials and Methods: From January 2004 to December 2013, 43 patients (43 hips) who underwent hemiarthroplasty with a C2 stem were enrolled in this study. Their mean age was 78.6 years (range, 70-84 years), and the mean follow-up period was 85.4 months (range, 60-96 months). During the follow-up period, clinical parameters, such as the changes in pain, walking ability, and functional status, were examined. Radiologically, changes in the proximal femur, such as osteoporosis and bone resorption of cortical bone, were noted. Complications that occurred during the follow-up period, such as dislocation and prosthetic features, were also reviewed. Results: Initially, the pain was relieved postoperatively, but it increased four years after surgery. The walking ability was reduced by two steps in nine patients after 60 months, and the Harris hip score was reduced significantly postoperatively after two to three years. Radiologically, cortical osteoporosis occurred in 14 patients. Five patients developed cortical bone resorption. Four of them showed nonunion of the trochanteric fracture fragments, and three of them suffered reverse oblique fractures. Conclusion: Careful selection considering the general health condition and remaining lifespan of the patient would be necessary for primary hip hemiarthroplasty using a 3C type cementless femoral stem for unstable intertrochanteric fractures in elderly patients with osteoporosis.

Application of unmanned helicopter on pest management in rice cultivation (무인 항공기 이용 벼 병해충 방제기술 연구)

  • This research was conducted to determine the alternative tool of chemical spray for rice cultivation using the unmanned helicopter(Yamaha, R-Max Type 2G-remote controlled system) at farmer's field in Korea. The unmanned helicopter tested was introduced form Japan. In Korea the application of chemicals by machine sprayer for pest management in rice cultivation has been ordinarily used at the farmer's level. However, it involved a relatively high cost and laborious for the small scale of cultivation per farm household. Farm population has been highly decreased to 7.5% in 2002 and the population is expected to rapidly reduce by 3.5% in 2012. In Japan, pest control depending on unmanned helicopter has been increased by leaps and bounds. This was due in part to the materialization of the low-cost production technology under agricultural policy and demand environmentally friendly farm products. The practicability of the unmanned helicopter in terms of super efficiency and effectiveness has been proven, and the farmers have understood that the unmanned helicopter is indispensable in the future farming system that they visualized. Also, the unmanned helicopter has been applied to rice, wheat, soybean, vegetables, fruit trees, pine trees for spraying chemicals and/or fertilizers in Japan Effect of disease control by unmanned helicopter was partially approved against rice blast and sheath blight. However, the result was not satisfactory due to the weather conditions and cultural practices. The spray density was also determined in this experiment at 0, 15, 30, and 60cm height from the paddy soil surface and there was 968 spots at 0cm, 1,560 spots at 15cm, 1,923 spots at 30cm, and 2,999 spots at 60cm height. However, no significant difference was found among the treatments. At the same time, there was no phytotoxicity observed under the chemical stray using this unmanned helicopter, nor the rice plant itself was damaged by the wind during the operation.
