Feature-based Non-rigid Registration between Pre- and Post-Contrast Lung CT Images (조영 전후의 폐 CT 영상 정합을 위한 특징 기반의 비강체 정합 기법)

  • In this paper, a feature-based registration technique is proposed for pre-contrast and post-contrast lung CT images. It utilizes three dimensional(3-D) features with their descriptors and estimates feature correspondences by nearest neighborhood matching in the feature space. We design a transformation model between the input image pairs using a free form deformation(FFD) which is based on B-splines. Registration is achieved by minimizing an energy function incorporating the smoothness of FFD and the correspondence information through a non-linear gradient conjugate method. To deal with outliers in feature matching, our energy model integrates a robust estimator which discards outliers effectively by iteratively reducing a radius of confidence in the minimization process. Performance evaluation was carried out in terms of accuracy and efficiency using seven pairs of lung CT images of clinical practice. For a quantitative assessment, a radiologist specialized in thorax manually placed landmarks on each CT image pair. In comparative evaluation to a conventional feature-based registration method, our algorithm showed improved performances in both accuracy and efficiency.

A CLINICAL AND RADIOGRAPHIC STUDY OF DENTIGEROUS CYSTS (함치성 낭의 임상 방사선학적 연구)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the dentigerous cysts in the jaws. For this study, the authors examined and analysed the clinical records and radiographs of 233 patients who had lesions of dentigerous cyst diagnosed by clinical and radiographic or histopathological examinations. And the obtained results were as follows: 1. Dentigerous cysts occurred the most frequently in the 2nd decade(38.2%) and occurred more frequently in males(67.4%) than in females(32.6%). 2. The most common clinical symptom was swelling of the jaw(33.9%), and the lesions were treated by the method of surgical removal. 3. The type of lesions was mainly observed as central type(72.5%), and size of the lesion was most frequently observed 2 - 2.9cm in the widest length. 4. The lesions were most frequently observed well-defined outline with hyperostotic border(49.8%), and smooth margin(73.4%), and homogeneous lesional radiolucency(79.4%). 5. Cortical thinning and expansion of the lesions(82.0%) were observed, and their direction were most frequently observed toward buccal side(64.0%). 6. The effect on the causative tooth were observed as tooth displacement(41.2%) and delayed root development(l9.3%), and the distance between cemento-enamel junction and lesional wall attachment of the causative tooth was mainly observed as below 2mm(79.6%). 7. The effect on the adjacent tooth were observed as loss of lamina dura(66.8%), root resorption(33.9%), and tooth displacement(31.5%). 8. The effects on the adjacent anatomic structures were observed as displacement of the mandibular canal(46.5%) and maxillary sinus or nasal cavity(72.2%).

RELATIONSHIP BETWEEN NASOPHARYNGEAL SPACE AND VELOPHARYNGEAL INCOMPETENCE IN CLEFT PALATE (구개열환자에서 비인두공간과 비인강폐쇄부전과의 연관성)

  • Nasopharyngeal closure is a sphincter mechanism between the activities of the soft palate, lateral pharyngeal wall and the posterior pharyngeal wall, which divides the oral cavity and the nasal cavity. It participates in physiological activities such as swallowing, breathing and pronunciation. In case of an error in this mechanism, it is called a nasopharyngeal incompetence. The causes of this error are defects in (1) length, function, posture of the soft palate (2) depth and width of the nasopharynx, (3) activity of the posterior and lateral pharyngeal wall. The purpose of this study is to analyze the nasopharynx of cleft palate patients using lateral cephalograms and at the same time, evaluate the degree of hypernasality of each vowels to find its relationship with nasopharyngeal incompetence. The following results were obtained: 1. The length of the soft palate was markedly short than normal. 2. The adequate ratio was smaller than the normal value. 3. As the adequate ratio decreased, when articulating vowels, anatomic mVPI increased. 4. When articulating each vowels, anatomic VPI was in proportion with the degree of hypernasality. 5. The degree of hypernasality was greater in high vowels(/i/, /u/) than low vowel(/a/). From the above results, it can be concluded that in cleft palate patients, lateral cephalograms can be used effectively in diagnosing and evaluating nasopharyngeal incompetence. The anatomic structure of the nasopharynx has close relation to the degree of hypernasality.

  • Ectopic eruption of a tooth into the oral environment occurs commonly whereas ectopic eruption of a tooth into other sites is rare. Those that have been reported include the nasal cavity, chin, mandibular condyles, coronoid processes, orbits and maxillary sinus. The etiologic factors of ectopic eruption are developmental disturbances such as cleft palate and teeth displaced by trauma or cysts, maxillary infection, genetic factors, crowding and exceptionally dense bone. In many cases, however, the etiology cannot be identified. Eruption of the teeth into the maxillary sinus is uncommon, however the identification of such teeth can be important since they have the potential to cause considerable morbidity. The definitive treatment is surgical removal of the teeth. A 7 year-old-boy visited the Department of Pediatric Dentistry, College of Dentistry, Yonsei University for treatment of dental caries. The abnormal erupting paths of the left and right maxillary canines were found during routine panoramic radiographic investigations. A panoramic radiograph taken at 13 years old revealed that two maxillary canines were located into the sinus. The teeth were extracted by the Caldwell-Luc approach.

The clinical study for the postoperative tracheal stenosis (수술후성 기관협착증에 관한 임사적 고찰)

  • Many etiological factors playa significant role in the development of tracheal stenosis; too high tracheostomy (Jackson, 1921), too small stoma (Greisen, 1966), the treatment with respirator using cuffed tube (Pearson et al., 1968; Lindholm, 1966; Bryce, 1972) and infection (Pearson, 1968). Although the incidence has been reduced due to development of surgical technique and antibiotics, the frequency of tracheal stenosis which produces symptoms after tracheostomy ranges from 1.5 per cent (Lindholm, 1967). In the management of the stenosis, mild cases are treated by mechanical dilatation with silicon tube or stent (Schmigelow, 1929; Montgomery, 1965) combined steroid (Birck, 1970), and in the cases of stenosis causes, these removed under the are bronchoscopy. But in severe stenosis, transverse resection with subsequent end-to-end anastomosis has been used in recent years (Pearson et al., 1968). During about 10 years, 1967 to 1977, a total of 23 patients with tracheal stenosis complicated among the 1, 514 tracheostomies have been treated in Severance Hospital. Now, we have obtained following conclusions by means of clinical analysis of 23 cases of tracheal stenosis. 1. The frequency of tracheal stenosis was 23 cases among 1, 514 cases of tracheostomy (1.5%). 2. Under the age of 5, these are 12 cases (52.2 %). 3. The sex incidence was comprised of 18 males and 5 females. 4. The duration of tracheostomy ranges from 4 days to 16 months. 5. The primary diseases requiring tracheostomy were following; central nerve system lesions 11 cases, upper air way obstruction 10 cases, extrinsic respiratory failure 2 cases. 6. Severe wound infections were only 2 cases. 7. The methods of treatment applied to tracheal stenosis were following; closed observation only 5 cases, nasotracheal intubation combined steroid 5 cases, T-tube stent combined steroid 3 cases, fenestration op. 4 cases, revision 4 cases and transverse resection and end-to-end anastomosis 2 cases.

Clinical Application of Nasal Intermittent Positive Pressure Ventilation with Bi-level Positive Airway Pressure(BiPAP) (기도 이중 양압(BiPAP)을 이용한 비강 간헐 양압환기의 임상적 적용)

  • Background: Noninvasive ventilation has been used extensively for the treatment of patients with neuromuscular weakness or restrictive chest wall disorders complicated by hypoventilatory respiratory failure. Recently, noninvasive positive pressure ventilation has been used in patients with alveolar hypoventilation, chronic obstructive pulmonary disease(COPD), and adult respiratory distress syndrome. Sanders and Kern reported treatment of obstructive sleep apnea with a modification of the standard nasal CPAP device to deliver seperate inspiratory positive airway pressure(IPAP) and expiratory positive airway pressure(EPAP). Bi-level positive airway pressure(BiPAP) unlike nasal CPAP, the unit delivers a different pressure during inspiration from that during expiration. The device is similar to the positive pressure ventilator or pressure support ventilation. Method and purpose: Bi-level positive airway pressure(BiPAP) system(Respironics, USA) was applied to seven patients with acute respiratory failure and three patients on conventional mechanical ventilation. Results: 1) Two of three patients after extubation were successfully achieved weaning from conventional mechanical ventilation by the use of BiPAP ventilation with nasal mask. Five of seven patients with acute respiratory failure successfully recovered without use of conventional mechanical ventilation. 2) $PaO_2$ 1hour after BiPAP ventilation in acute respiratory failure patients significantly improved more than baseline values(p<0.01). $PaCO_2$ 1hour after BiPAP ventilation in acute respiratory failure patients did not change significantly more than baseline values. Conclusion: Nasal mask BiPAP ventilation can be one of the possible alternatives of conventional mechanical ventilation in acute respiratory failure and supportive method for weaning from mechanical ventilation.

  • This report describes a 5-month follow-up of the efficacy of Herbst oral appliance(OA) in a obstructive sleep apnea syndrome patient who has failed with uvulopalatopharyngoplasty(UPPP) and was unable to tolerate to subsequent nasal continuous positive airway pressure(CPAP) because of mouth air leak. The obstructive sleep apnea and daytime performance were markedly improved by regular home use of OA, and the patient still continues to use OA without complications. It is suggested that OA can be a successful alternative therapeutic modality in patients who are unable to tolerate to nasal CP AP, especially after UPPP.

  • It has been reported that There is a relatively high incidence of congenital cleft lip and palate among congenital anomalies. Children with the cleft lip and palate confront various problems ranging from feeding difficulties as infants to frequent ear infections, compromised speech, aberrant dentofacial development, malocclusion and esthetic impairment with its unfavorable effects on the psychosocial adaptation of the individual. Management of these patients is a process that starts in infancy and continues on into adulthood. Through a team approach, professionals from various fields convene to assess the needs of the child and to assist the parents in dealing with the situation. A pediatric dentist, one of the team, must advise the parents the way of feeding or make an obturator for normal feeding and weight gain of infants with cleft lip and/or palate. An obturator can aid nursing, stimulate orofacial development, help develop the palatal shelves, prevent tongue distortions, prevent nasal septum irritation, decrease chance of ear infections, expand collaped maxillary segments, constrict the expanded anterior part of the maxilla, reposition the premaxilla, and help the parents psychologically. Three cases of infants with cleft lip and palate were managed with the obturator at the Dept. of Pediatric Dentistry, Kyung Hee University. In all cases, infants showed normal feeding and weight gaining after the insertion of the obturator.

A Statistical Study of Foreign Bodies in Food and Air Passages (식도 및 기도 이물의 통계적 고찰)

  • We have analysed the 76 cases of the foreign bodies of our ENT department during the period from Jun. 1973 to Feb. 1983. The following results were obtained and were reported. 1) Distribution of these whole 105 cases, 68 cases (64.8 %) were lodged in esophagus, 13 cases (12.4 %) were in the nasal cavities, 9 cases (8.5 %) were in the oral cavity and throat, 8 cases (7.6 %) were in the air passages, and 7 cases (6.7 %) were in the external auditory canal. 2) The sorts of the esophageal foreign bodies, the coins were the most cases(53 cases - 77.9 %) and other sites were shown of variable kinds. 3) Age distribution of the esophageal foreign bodies were mostly in under 5 years of age (49 cases - 72 %) and in cases of the air passages, all the 8 cases were in under 10. 4) In the localities of the esophageal foreign bodies, first narrowing was the most frequent site (61 cases - 89.7 %), and of air passages, 6 cases were in the left main bronchus, more that of Rt. main bronchus. 5) During the lodgement of esophageal foreign bodies, 58 cases (85.3 %) were visited to our ENT department within 24 hours. In the cases of air passages, most were viaited in 3 to 7 days (6 cases -75 %).

Pulmonary Toxicity Assessment of Aluminum Oxide Nanoparticles via Nasal Instillation Exposure (비강내 점적 노출을 통한 산화 알루미늄 나노입자의 폐독성 평가)

  • Objective: The use of nanoparticle products is expected to present a potential harmful effect on consumers. Also, the lack of information regarding inhaled nanoparticles may pose a serious problem. In this study, we addressed this issue by studying pulmonary toxicity after nasal instillation of Al-NPs in SD rats. Methods: The animals were exposed to Al-NPs at 1 mg/kg body weight (low dose), 20 mg/kg body weight (medium dose) and 40 mg/kg body weight (high dose). To determine pulmonary toxicity, bronchoalveolar lavage (ts.AnBAL) fluid analysis and histopathological examination were conducted in rats. In addition, cell viability was investigated at 24 hours after the treatment with Al-NPs. Results: BAL fluid analysis showed that total cells (TC) count and total protein (TP) concentrations increased significantly in all treatment groups, approximately two to three times. Also, lactate dehydrogenase (LDH) and cytokines such as TNF-alpha and IL-6 dose-dependently increased following nasal instillation of Al-NPs. However, polymorphonuclear leukocytes (PMNs) levels showed no significant changes in a dose dependant manner in BAL fluid. In the cytotoxicity analysis, the treatment of Al-NPs significantly and dose-dependently induced cell viability loss (20 to 30%) and damage of cell membrane (5 to 10%) in rat normal lung epithelial cells (L2). Conclusions: Our results suggest that inhaled Al-NPs in the lungs may be removed quickly by alveolar macrophages with minimal inflammatory reaction, but Al-NPs have the potential to affect lung permeability. Therefore, extensive toxicity evaluations of Al-NPs are required prior to their practical application as consumer products.
