Comparison Study of Textbook and Actual Clinical Incidence Angles in Intra-Oral Radiology Examinations (구내 방사선 검사에서 교과서와 실제 임상의 입사각도 비교연구)

  • Existing Intra-oral radiography examinations have problems with information on the angle of incidence used in textbook and those used in the field, and radiologic examinations result vary depending on the operator's skill. Therefore, this study aimed to suggest an ideal angle of incidence for radiological examination so that the proximal areas between teeth are clearly visible without overlapping. First, patient's dental arch in the exact center, and the angle of incidence of the point where the teeth did not overlap was measured using a protractor tool in program. Second, based on the calculated results, we surveyed patient phantoms for student practice during Intra-oral radiography examination and conducted a survey based on qualitative evaluation using a 5-point Likert scale.When comparing the average value obtained by measuring actual patient data and the angle specified in the existing reference book, there is a statistically significant difference in the average between the two groups at p < 0.05 in the proximal area of all teeth, and even if the gender and age group are different, there is a significant difference. Because it did not deviate from the scope, we were able to obtain results that could be sufficiently presented as guidelines. In addition, in a qualitative evaluation based on the images presented by applying the actual measured data to the phantom, images were obtained in which the adjacent surfaces of teeth do not overlap. In conclusion, this study proposes guidelines for the angle of incidence during Intra-Oral Radiography examination to reduce cone-cut images or images with overlapping proximal area and prevent repeat.

A comparative study of guiding methods for natural head posture in cephalometrics (두부방사선규격사진 촬영 시 유도방법에 따른 자연두부자세의 차이 및 재현성에 관한 연구)

  • The Purpose of this study was to compare the degree of vortical head rotation and to evaluate the reproducibility of natural Head posture (NHP) according to two guiding methods, the head posture aligner (HPA) method and the solf balance posture (SBP) method. The subjects consisted of 30 adults. On the first day. lateral and frontal cephalometric radiographs were obtained through the two guiding methods. One mouth later. lateral and frontal cephalometric radiographs were obtained again through both guiding methods. The degrees of vertical head rotation of both guiding methods were compared and the reproducibility was evaluated for each guiding method. A comparison of the degrees of vortical head rotation for the two methods revealed that the vertical head posture was lower in the SBP method than in the HPA method by an average of $2.79^{\circ}$. All measurements obtained using the HPA and SBP methods with a time interval of one north did not show any significant difierence in lateral and frortal cephalometric radiographs. The results of the present study suggest that the SBP method may be used as an alternative to the HPA method in case the HPA method can not be applied.

  • 목적 : 본 연구는 척수손상환자들 중 신체활동 정도가 급격하게 차이 나는 운동 선수군과 비선수군의 일상생활동작과 골밀도를 비교 분석하는데 그 목적이 있다. 방법 : 본 연구를 위해 20명(선수 10명, 비선수 10명)의 SCI 환자가 실험에 참가하였다. 골밀도 측정을 위해 이중에너지 방사선골밀도 측정기(Lunar Prodigy, GE Healthcare. England)를 이용하여 종골부위(calcaneus)의 골밀도를 측정하였다. 척수손상환자의 일상 생활 기능을 측정하기 위해 자조관리(self care), 호흡과 괄약근 조절 (respiration and sphincter management task), 이동(transfer)의 세 영역으로 나누어진 SCIM II(Spinal Cord Injury Measurement II)을 이용하였다. 척수손상환자들 중 운동 선수군과 비운동 선수군의 일상생활 동작과 골밀도를 비교 하기 위해 SPSS 14.0 통계 프로그램의 independent t-test를 이용하여 통계분석을 실시하였고 일상생활 동작과 골밀도의 상관관계는 Pearson correlation을 실시하였다. 유의수준은 ${\alpha}$=.05로 통계처리 하였다. 결과 : 검사 결과 선수군이 비선수군보다 통계적으로 유의하게 큰 SCIM 점수와 T-score를 보여주었다. 결론 : 일상생활동작을 측정하기 위하여 SCIM II(SpinalCordIndependenceMeasureII) 척도를 사용하였는데 선수군이 비선수군에 비해 유의하게 높은 SCIM II 총점을 나타내어 운동을 통한 훈련이 척수손상환자의 기능적 활동을 향상시킬 수 있다고 사료된다. 그리고 골밀도 측정 결과 선수군이 비선수군에 비해 통계적으로 유의하게 높은 골밀도를 나타내었다. 이 역시 강도 높은 훈련이 척수손상환자에게 나타날 수 있는 신체구성을 강화시켜줄 수 있을 것으로 사료된다. 운동을 통한 재활 중에서 그 강도가 높을수록 신체의 구성적, 기능적 측면 뿐 아니라 신경의 가소성 측면에서도 증가를 기대해 볼 수 있을 것으로 판단된다.

Evaluation of Xerostomia Following 3 Dimensional Conformal Radiotherapy for Nasopharyngeal Cancer Patients (3차원 입체조형 방사선치료를 시행받은 코인두암 환자에서의 구강건조증 분석)

  • Purose: This study is to evaluate the xerostomia following 3-dimensional conformal radiation therapy (3D CRT) in nasopharynx cancer patients using the xerostomia questionnaire score (XQS). Materials and Methods: Questionnaire study was done on 51 patients with nasopharynx cancer who received 3D CRT from Dec. 2000 to Aug. 2005. 3D CRT technique is based on 'serial shrinking field' concept by 3 times of computed tomography (CT) simulation. Total target dose to the primary tumor was 72 Gy with 1.8 Gy daily fractions. Xerostomia was assessed with 4-questions XQS, and the associations between XQS and time elapsed after RT, age, sex, stage, concurrent chemotherapy, and parotid dose were analyzed. Results: Concurrent chemotherapy was given to 40 patients and RT alone was given to 11 patients. The median time elapsed after 3D CRT was 20 ($1{\sim}58$) months and the mean XQS of all 51 patients was $8.4{\pm}1.9\;(6{\sim}14)$. XQS continuously and significantly decreased over time after 3D CRT ($X^2$=-0.484, p<0.05). There was no significant difference in XQS according to sex, age, and stag. However, XQS of concurrent chemotherapy patients was significantly higher than RT alone patients (P=0.001). XQS of patients receiving total mean parotid dose ${\ge}35 Gy$ was significantly higher than <35 Gy (p=0.05). Decreasing tendency of XQS over time after 3D CRT was observed. Concurrent chemotherapy and total mean parotid dose ${\ge}35 Gy$ were suggested to adversely affect radiation-induced xerostomia.

Modification of Late Radiation Response of Rat Salivary Glands by Pentoxifylline and Diltiazem (쥐의 타액선 방사선조사 후 만성반응에 Pentoxifylline과 Diltiazem이 미치는 영향)

  • Purpose : To elucidate the effects of pentoxifylline and diltiazem on the late response of the salivary glands of the rat after irradiation. Materials and Methods : Sixteen Sprague-Dawley rats were divided into 4 groups : (a) irradiation alone (b) irradiation with pentixifylline (PTX) (c) irradiation with diltiazem (DTZ) (d) irradiation with both PTX and DTZ. Irradiation was given in a single fraction of 16 Gy using 4 MV photon energy through an anterior port encompassing the left side of the salivary gland leaving the right side of salivary gland as a control. PTX, 20 mg/kg and/or DTZ, 50 mg/kg were infused intraperitoneally before irradiation, Two rats from each group were sacrificed on the 10th week and the rest was sacrificed on the 16th week after irradiation. Histopathologic examinations were undertaken for each section and the proportion of vacuolated cells out of the total number of cells under light microscopic fields was calculated. The statistical significance in the difference of the proportion of the vacuolated cells among the experimental groups was evaluated by a $x^2$-test. Results : Irradiated salivary glands of the 10th week group revealed markedly increased number of vacuolated cells compared to those of unirradiated control. The proportion of vacuolated cells was significantly reduced in both the PTX group (p value=0.001) and the combined PTX and DTX group compared to those of irradiation alone group. The DTZ alone group did not reveal the significant reduction of vacuolated cells compared to those of irradiation alone group (p value, >0.05). The 16th week groups revealed similar findings to those of the 10th week group, but the degree of chronic inflammatory cell infiltrates and interstitial fibrosis was increased and the number of acinar cells was reduced compared to those of the 10th week group. Conclusions : PTX significantly reduced the late radiation response of salivary glands, but DTZ did not reduce the same degree as PTX did. Taking the positive results of this study into consideration, it seems reasonable to apply PTX into the clinical trial for the head and neck irradiation to reduce the late radiation sequelae of salivary glands in the near future. Dose Comparison between Fast Low Dose C-arm CT and DSA (Fast Low Dose C-arm CT와 DSA의 선량 비교)

  • The average dose of Fast Low Dose C-arm CT used during hepatic arterial chemoembolization was compared with the average dose of DSA, and the exposure dose was analyzed by analyzing the average dose for each test technique in the total accumulated dose. 50 patients were randomly selected at our clinic and compared with Fast Low Dose C-arm CT, DAP and Air Kerma of DSA, and the accumulation of four test techniques (DSA, Fast Low Dose C-arm CT, Roadmap, Fluoroscopy) The proportion of dose (DAP, Air Kerma) was analyzed. For statistical comparative analysis, the corresponding sample T test and ANOVA test (post hoc test: Tukey) were performed using the statistical program SPSS 20.0. Fast Low Dose C-arm CT showed statistically significantly lower average dose (DAP, Air Kerma) than DSA. Reducing the number of tests for DSA can reduce the patient's exposure to medical radiation.

Comparative Evaluation of Single-Energy CT and Dual-Energy CT in Brain Angiography : Using a Rando Phantom and OSLD (뇌혈관조영검사 시 단일에너지 CT와 이중에너지 CT의 비교평가 : 화질 및 유효선량평가)

  • Single source and dual source measurements using anthropomorphic phantoms in which the phantoms are lined up in human body equivalents use OSLD (Optically Stimulated Luminescence Dosimeter), so the effective dose is calculated using OSLD. For hospital images, SNR (Signal to Noise Ratio) and CNR (Contrast to Noise Ratio) were measured in MCA (Middle Cerebral Artery) for single source and dual source, and for phantom images, SNR and CNR were measured for brain parenchyma of single source and dual source. For hospital imaging, SNR and CNR were measured in MCA for both single-source and dual-source, and for phantom images, SNR and CNR were measured for brain parenchyma from single-source and dual-source. As a result of comparing the SNR and CNR of the hospital image and the phantom image, there was no statistical difference. Comparing patient doses in hospital images, the effective dose of the dual source was 53.53% less and the effective dose of the dual energy phantom was 57.94% less. The dose can be increased in other areas, but the cerebrovascular area is useful because the dose is small.

The Usefulness of $^{18}F$-FDG PET/CT for Predicting the Response of Locally Advanced Rectal Cancer to Neoadjuvant Chemoradiation Therapy (국소 진행된 직장암의 $^{18}F$-FDG PET/CT를 이용한 항암방사선치료의 반응성 예측)

  • Purpose: This study aimed at assessing the value of fluorine-18 fluorodeoxyglucose positron emission tomography ($^{18}F$-FDG PET) for predicting the response of locally advanced rectal cancer to neoadjuvant CRT. Materials and Methods: Between August 2006 and January 2008, we prospectively enrolled 20 patients with locally advanced rectal cancer and who were treated with neoadjuvant CRT at the Korea Institute of Radiological and Medical Sciences. The treatment consisted of radiation therapy and chemotherapy, and this was followed by curative resection 6 weeks later. All the patients underwent $^{18}F$-FDG PET/CT both before CRT and 6 weeks after completing CRT. The measurements of the FDG uptake ($SUV_{max}$), the absolute difference (${\Delta}SUV_{max}$) and the percent $SUV_{max}$ difference (response index, $RI_{SUV}$) between the pre- and post-CRT $^{18}F$-FDG PET/CT scans were assessed. The measurements of the metabolic volume, the absolute difference (${\Delta}$metabolic volume) and the percent metabolic volume difference (response index, $RI_{metabolic\;volume}$) were also assessed. Results: Of the 20 patients who underwent surgery, 11 patients (55%) were classified as responders according to Dworak's classification. The post-CRT $SUV_{max}$ was significantly lower than the pre-CRT $SUV_{max}$. However, there were no significant differences in the $SUV_{max}$ and the metabolic volume reduction between the responders and non-responders. We used a minimum $SUV_{max}$ reduction of 67% as the cut-off value for defining a response, with a sensitivity of 45.5%, a specificity of 88.9%, a positive predictive value of 77% and a negative predictive value of 53.8%. Conclusion: Although there were no statistically significant results in this study, other studies have revealed that $^{18}F$-FDG PET/CT has the potential to assess the tumor response to neoadjuvant CRT in patients with locally advanced rectal cancer.

Biological Markers as Predictors of Radiosensitivity in Syngeneic Murine Tumors (동계 마우스 종양의 방사선 감수성 예측인자로서의 생물학적 표지자)

  • Purpose: We investigated whether a relationship exists between tumor control dose 50 ($TCD_{50}$) or tumor growth delay (TGD) and radiation induced apoptosis (RIA) in syngeneic murine tumors. Also we investigated the biological markers that can predict radiosensitivity in murine tumor system through analysis of relationship between $TCD_{50}$, TGD, RIA and constitutive expression levels of the genetic products regulating RIA. Materials and Methods: Syngeneic murine tumors such as ovarian adenocarcinoma, mammary carcinoma, squamous cell carcinoma, fibrosarcoma, hepatocarcinoma were used In this study. C3H/HeJ mice were bred and maintained in our specific pathogen free mouse colony and were $8{\sim}12$ weeks old when used for the experiments. The tumors, growing in the right hind legs of mice, were analyzed for $TCD_{50}$, TGD, and RIA at 8 mm in diameter. The tumors were also analyzed for the constitutive expression levels of $p53,\;p21^{WAF1/CIP1},\;BAX,\;Bcl-2,\;Bcl-X_L,\;Bcl-X_S$, and p34. Correlation analysis was peformed whether the level of RIA were correlated with $TCD_{50}$ or TGD, and the constitutive expression levels of genetic products regulating RIA were correlated with $TCD_{50}$, TGD, RIA. Results: The level of RIA showed a significant positive correlation (R=0.922, p=0.026) with TGD, and showed a trend to correlation (R=-0.848), marginally significant correlation with $TCD_{50}$ (p=0.070). It indicates that tumors that respond to radiation with high percentage of apoptosis were more radiosensitive. The constitutive expression levels of $p21^{WAF1/CIP1}$ and 34 showed a significant correlation either with $TCD_{50}$ (R=0.893, p=0.041 and R=0.904, p=0.035) or with TGD (R=-0.922, p=0.026 and R=-0.890 p=0.043). The tumors with high constitutive expression levels of $p21^{WAF1/CIP1}$ or p34 were less radiosensitive than those with low expression. Conclusion: Radiosensitivity may be predicted with the level of RIA in murine tumors. The constitutive expression levels of $p21^{WAF1/CIP1}$ or p34 can be used as biological markers which predict the radiosensitivity.

Accelerated Hyperfractionated Radiotherapy for Locally Advanced Uterine Cervix Cancers (국소진행된 자궁경부암에서의 가속과분할 방사선치료)

  • Purpose: To assess the efficacy of the use of accelerated hyperfractionated radiotherapy(AHRT) for locally advanced uterine cervix cancers. Materials and Methods: Between May 2000 and September 2002, 179 patients were identified with FIGO stage IIB, IIIB, and IVA cancers. Of the 179 patients, 45 patients were treated with AHRT(AHRT group) and 134 patients were treated with conventional radiotherapy(CRT group), respectively. Patients undergoing the AHRT regimen received a dose of 30 Gy in 20 fractions(1.5 $Gy{\times}2$ fractions/day) to the whole pelvis. Subsequently, with a midline block, we administered a parametrial boost with a dose of 20 Gy using 2 Gy fractions. Patients also received two courses of low-dose-rate brachytherapy, up to a total dose of 85{\sim}90 Gy to point A. In the CRT group of patients, the total dose to point A was $85{\sim}90$ Gy. The overall treatment duration was a median of 37 and 66 days for patients that received AHRT and CRT, respectively. Statistical analysis was calculated by use of the Kaplan-Meier method, the log-rank test, and Chi-squared test. Results: For patients that received cisplatin-based concurrent chemotherapy and radiotherapy, the local control rate at 5 years was 100% and 79.2% for the AHRT and CRT group of patients, respectively(p=0.028). The 5-year survival rate for patients with a stage IIB bulky tumor was 82.6% and 62.1% for the AHRT group and CRT group, respectively(p=0.040). There was no statistically significant difference for severe late toxicity between the two groups(p=0.561). Conclusion: In this study, we observed that treatment with AHRT with concurrent chemotherapy allows a significant advantage of local control and survival for locally advanced uterine cervix cancers.
