초산모 모유수유를 위한 부부교육프로그램의 효과 (Effects of Couple Breast-feeding Education Program for the Enhancement of Primipara Suyu)

  • 연구목적은 초산모 모유수유를 위한 부부교육프로그램을 개발하고 운영하여 초산모에게 모유수유 자기효능감과 배우자 지지 및 모유수유 만족감에 미치는 효과를 파악하고자 하였다. 연구설계는 비동등성 대조군 전후시차설계를 이용한 유사실험연구였다. 연구대상은 임신 34주 이후에 출산하고 모유수유를 희망한 초산모였다. 모유수유를 위한 부부교육프로그램대상은 출산모 부부였으며, 프로그램은 출산 후 1일째부터 3일 동안 매일 1회 20분~40분간 총 80시간을 운영하였다. 대상자 수는 실험군 32명, 대조군 31명이었으며, 자료수집은 설문지를 사용하였다. 수집된 자료는 SPSS 20.0 Win을 활용하여 $x^2$-test, Fisher's exact test, t-test로 분석하였다. 연구결과에서 부부교육프로그램에 참여한 초산모는 참여하지 않은 초산모보다 모유수유 자기효능감(t=3.44, p=.001), 모유수유 관련 배우자 지지(t=3.03, p=.004), 모유수유 만족감(t=3.64, p=.001) 점수가 더 높았으며 통계적으로 유의하였으므로, 초산모의 모유수유를 위한 부부교육프로그램이 효과적이라고 할 수 있다. 따라서 본 프로그램을 임상실무에서 초산모 모유수유를 촉진하는 교육적 중재로 적용할 것을 권한다.

  • Purpose: This study aimed to verify the effects of prenatal breast self-massage education on first-time mothers' breastfeeding self-efficacy, breastfeeding adaptation, and breastfeeding practice. Method: The experimental group (n=26) received prenatal breast self-massage education focused on self mamma care (SMC) at 34-36 weeks gestation and 2 weeks later face-to-face. The control group (n=25) was recruited immediately following birth and received usual care. Breastfeeding self-efficacy was measured 4 hours after childbirth and 3 days later, and breastfeeding practice, breastfeeding adaptation, and breastfeeding-related characteristics were measured 2 weeks and 4 weeks after childbirth through online survey. The data were analyzed by $x^2$ test and t-test using SPSS/WIN 24.0. Results: There was no significant difference in breastfeeding self-efficacy of the experimental group at 4 hours after childbirth (t=0.83, p=.410) whereas it was statistically significantly higher at 3 days postpartum (t=2.86, p=.006). There were no significant differences in breastfeeding adaptation between the two groups at both 2 weeks (t=1.76, p=.084) and 4 weeks postpartum (t=0.87, p=.388). For breastfeeding practice a statistically significant difference was found at 4 weeks for the experimental group ($x^2=4.77$, p=.036). Conclusion: This antenatal SMC intervention was found to be a feasible intervention for use in clinical practice.

  • Purpose: The purpose of this study was to develop a breastfeeding empowerment program and to investigate the effects of the breastfeeding empowerment program on self-efficacy, adaptation and continuation of breastfeeding for primiparous women. Methods: The 5 session breastfeeding empowerment program was developed and a non-equivalent control group non-synchronized quasi-experiment design was used. Fifty-five participants were assigned to either the experimental group (n=27) or the control group (n=28). Effects were tested using repeated measures ANOVA and $x^2-test$. Results: Scores for self-efficacy, adaptation and continuation of breastfeeding of in the experimental group after program were significantly higher than 1week, 4weeks, 8weeks scores in control group. Conclusion: The effects of the breastfeeding empowerment program for elevating self-efficacy, adaptation and continuation of breastfeeding in primiparous women were validated. Therefore, this program can be recommended for vigorous use in clinical practice.

미숙아 어머니의 모유수유 지속을 위한 자기효능 증진 프로그램의 효과 (A Self-Efficacy Promotion Program for the Continuation of Breastfeeding for Mothers of Premature Infants)

  • Purpose: This study examined the effects of breastfeeding education among preterm mothers. Methods: A nonequivalent control group, repeated measure, quasi-experimental study design was used. The participants comprised 46 preterm mothers recruited from the neonatal intensive care unit of a university hospital who were divided into two groups: 23 in the control group and 22 in the experimental group. The breastfeeding education program intervention to promote preterm mothers' self-efficacy used was devised based on the self-efficacy theory. The intervention program consisted of individual breastfeeding education with demonstrations and discharge education, and telephone follow-up education within one week after being discharged. The participants were surveyed before and after the intervention, and four weeks after being discharged from the newborn intensive care unit. Results: The intervention group showed a significant increase in breastfeeding self-efficacy compared with the control group (F=6.92, p=.003) and showed a significant increase in their breastfeeding attitude score (F=3.49, p=.039). Four weeks after discharge, the breastmilk volume of the intervention group increased significantly (t=-2.87, p=.006), and the same group continued to breastfeed more compared with the control group (χ2=4.50, p=.049). Conclusion: This study suggests that the preterm breastfeeding program had a positive effect on breastfeeding self-efficacy, breastfeeding attitude, and extending the breastfeeding period among preterm mothers.

어머니의 모유수유 만족감에 미치는 영향요인 (Factors Explaining Mothers' Breastfeeding Satisfaction)

  • Purpose: This study was done to investigate the level of breastfeeding satisfaction, the relationship among knowledge about breastfeeding, attitude toward breastfeeding, self-efficacy of breastfeeding, intention of breastfeeding, and perceived social encouragement of breastfeeding and the factors influencing breastfeeding satisfaction in mothers 3~6 months after birth. Methods: For this descriptive research, 209 mothers who had been breastfeeding their babies agreed to participate in this study. Data on mother's satisfaction' knowledge, attitude, self-efficacy, intention, and perceived social encouragement regarding breastfeeding were collected by closed-ended questionnaires on September 14, 2007 at a contest for healthy breast-fed babies. Results: The major findings of this study were as follows: First, there were significant differences in the breastfeeding satisfaction level according to planned length of breastfeeding (F=10.43, p<.001). Second, there was a significant relationship between breastfeeding satisfaction and knowledge about breastfeeding (r=.37, p<.001), attitude toward breastfeeding (r=.62, p<.001), self-efficacy of breastfeeding (r=.66, p<.001), intention of breast feeding (r=.40, p<.001), and perceived social encouragement of breastfeeding (r=.32, p<.001). Finally, the strongest factor affecting mothers' breastfeeding satisfaction was attitude toward breastfeeding ($\beta=.38$). Other factors included self-efficacy of breastfeeding ($\beta=.36$), knowledge about breastfeeding ($\beta=.14$), perceived social encouragement of breastfeeding ($\beta=.10$), and planned length of breastfeeding ($\beta=.10$). Conclusion: This study suggests that nurses should help mothers improve breastfeeding satisfaction through promotion of knowledge about, attitudes toward, and self-efficacy of breastfeeding.

  • Purpose: The purpose of this study was to assess the knowledge and health beliefs about gestational diabetes and to identify the influence on breastfeeding intention of women those who have been diagnosed with gestational diabetes. Methods: A cross-sectional descriptive study was designed. A questionnaire survey was conducted on 270 women who were pregnant and currently diagnosed with gestational diabetes. Data collection was conducted at Internet cafes and breastfeeding clinics where pregnant women were able to participate actively. The data collection period was from November 5 to November 27, 2019 and analyzed using descriptive statistics, independent t-test, χ2 test and multiple logistic regression. Results: The average age of the participants was 34.21±3.73 years. There were 221 women who had breastfeeding intention, and 49 women who did not intend to breastfeed. The higher the perceived susceptibility (OR=2.49, p=.032), benefits (OR=2.62 p=.009), and the self-efficacy, the higher the intention to perform breastfeeding (OR=2.97, p=.004). On the other hand, the higher the perceived severity, the lower the intention to perform breastfeeding (OR=0.35, p=.007). Conclusion: Health beliefs such as perceived susceptibility, perceived benefits, self-efficacy and perceived severity have been shown to affect the breastfeeding intention. Based on these results, we suggest developing a breastfeeding promotion intervention program that improves self-efficacy in gestational diabetics.

임신성 당뇨병 산모를 위한 모바일 기반 모유수유 증진 프로그램의 개발 및 효과 평가 (Development and Effectiveness Evaluation of Comprehensive Mobile-Based, Breastfeeding Promotion Program for Mothers with Gestational Diabetes)

  • Purpose: This study aimed to evaluate the effects of a mobile-based breastfeeding promotion program (M-BFGDM) that helps mothers with gestational diabetes. Methods: Forty-seven mothers participated in the study, of whom 22 were in the experimental group and 25 in the control group. To verify the effects, a lag design before and after the non-equivalence control group was used. The data collection for the experimental group was done before and after the intervention. Results: In the results, breastfeeding knowledge showed a significant difference in the interaction between measurement period and group (χ2 = 8.14, p = .017), whereas breastfeeding intention did not show a significant difference in the interaction (χ2 = 4.73, p = .094). There was no difference in self-efficacy interaction (F = 0.13, p = .856). The breastfeeding method showed no difference in interaction (F = 0.04, p = .952), whereas cross-analysis showed a significant difference in breastfeeding practice rate between the experimental group and the control group at 1 month postpartum (χ2 = 7.59, p = .006). Conclusion: A mobile-based breastfeeding promotion program was developed and applied for gestational diabetic mothers, resulting in an increase in breastfeeding knowledge and an improvement in breastfeeding practice rate one month after childbirth. In addition, M-BFGDM managed to create a breastfeeding practice environment with fewer time and place restrictions. A program study that complements motivation is needed to improve breastfeeding in pregnant diabetic mothers in the future.

  • Purpose: This study aimed to identify the effects of a direct breastfeeding program for premature infants in neonatal intensive care units (NICUs). Methods: This quasi-experimental study was conducted during August 2016 to April 2017. Sixty mothers of premature infants were assigned to the experimental (n = 31) or control groups (n = 29). The program was comprised of breastfeeding education and direct breastfeeding support. The experimental and control groups were provided with education and counseling on breastfeeding at the time of admission and discharge. In the experimental group, the mothers initiated oral feeding with direct breastfeeding and engaged in breastfeeding at least seven times during the NICU stay. The collected data were analyzed by the χ2-test and repeated measures ANOVA using an SPSS program. Results: The experimental group showed a higher direct breastfeeding practice rate (χ2 = 19.29, p < .001), breastfeeding continuation rate (χ2 = 3.76, p < .001), and self-efficacy (F = 25.37, p < .001) than the control group except for maternal attachment. Conclusion: The direct breastfeeding program in the NICU has significant effects on the practice and continuation rate of breastfeeding and breastfeeding self-efficacy. Therefore, this program can be applied in the NICU settings where direct breastfeeding is limited.

산후 24개월 이내 어머니의 수유형태와 예측요인 (Types of Breastfeeding and its Predictors of Mothers in Twenty-four Months after Birth)

  • Purpose: This study was done to investigate the types of breastfeeding visits by mothers twenty-four months after birth and to identify the factors that increased breast feeding rates. Methods: The subjects of this study totaled seven hundred and seventy-four mothers with infants aged 2 years or less who had visited national medical institutions including Sanhujori service facilities, breastfeeding care service facilities, and community health centers. Data was collected from June 22 to July 31, 2009. Data was analyzed using descriptive statistics, $x^2$ test, ANOVA with a post hoc Scheffe test, and multiple logistic regression. Results: Major findings of this study were significant differences of planned length of breastfeeding and breastfeeding self-efficacy in relation to the differing types of breast feeding community visits by mothers. In multivariate logistic regression, baby's age, exclusive breastfeeding at hospital, utilization of breastfeeding care service facility, planned length of breastfeeding, and breastfeeding self-efficacy were independent predictors of the feeding type. Conclusion: In order to increase breastfeeding rates, programs provided by public health care services and medical facilities should start education on breast feeding in the hospital before mothers are discharged and then continue through the use of Sanhujori service facilities and workplace at 3 month and 6 month postpartum.
