IMPACTED MANDIBULAR CANINES; CASE REPORT (하악 매복 견치의 치과적 접근)

  • Impacted mandibular canines are less common than impacted maxillary canines. The proceeding studies show that impacted mandibular canines occur in approximately 0.1% of patients examined. The causes of impacted mandibular canines are inadequate space, premature loss of the primary dentition, excessive crown length, hereditary factors, functional disturbances of endocrine glands, tumors, and traumas. The basic choices for treatment of those cases are orthodontic migration or surgical removal. This is a case report about those two different treatment methods. One patient was 11 years old female who had treated with surgical approach and the other was 14 years old male who had received orthodontic treatment.

  • Proper treatment of an impacted tooth is required as it causes functional and esthetic disharmony, as well as it can cause root absorption of adjacent teeth. Treatment options for impacted teeth include periodic observation, surgical exposure, orthodontic traction followed by surgical exposure, tooth transplantation, and extraction. Modified Nance appliance, used for orthodontic traction, is clinically useful because it does not require patient cooperation. Through orthodontic traction combined with surgical exposure of impacted maxillary incisors, canines, and molars using modified Nance appliance, adequate results can be obtained.

  • The success of autotransplantation depends on the viability of periodontal ligament in the transplanted tooth. Mechanical injury to periodontal tissues frequently results in dental root resorption and ankylosis, which leads to the failure of transplantation. Enamel matrix derivative(EMD) Which contains several enamel matrix protein (amelogenin family) has been reported to be effective in some periodontal therapies has been recently used to induce periodontal regeneration. EMD promotes proliferation of periodontal ligament cells and is suggested to be useful for transplantation. In this case, we report a clinical case of EMD application in the transplantation of an impacted and immature tooth of a 14 year-old girl to enhance the periodontal regeneration.

  • Maxillary canine impaction is a common eruption problem in children. Impaction frequently involves further complications such as root resorption of adjacent teeth, cyst formation and migration of the neighboring teeth, etc. Various treatment modalities include extraction of preceding deciduous canine, orthodontic traction, and surgical extraction followed by immediate replantation of the extracted tooth at the proper position(autotransplantation). Autotransplantation is considered as the treatment of choice when surgical exposure and subsequent orthodontic traction are difficult or impossible due to unfavorable impacted position. The prognosis of autotransplantation is affected by the degree of apex formation, surgical procedures performed, timing of root canal treatment, and length of stabilization period. In these two cases presented, the patients with unerupted maxillary canine were treated with autotransplantation. One case was thought that guidance of eruption by orthodontic traction was difficult because of its unfavorable impacted position. In the other case, parents didn't agree to treat by orthodontic traction, therefore autotransplantation was done. In both cases, autotransplantation was carried out following root canal treatment and orthodontic treatment, and both cases have demonstrated to be successful to this day.

  • Down syndrome (DS) is a genetic disease known as trisomy 21. Congenital cardiac anomalies are present in about 40% of DS patients. Dental anomalies are also common among DS patients. In DS patients, canine impaction is 10 times more frequent and transposition of maxillary canine and first premolar is 50 times more common than in general population. A female DS patient with congenital heart disease was diagnosed as having a transposed impacted maxillary canine. Sectional fixed appliance with Nance holding arch was used for the orthodontic treatment. After space was regained for the eruption of the canine, orthodontic button was attached using flap operation with closed technique. Traction and alignment of the tooth followed. To prevent endocarditis, prophylactic antibiotics were prescribed for the recommended dental procedures. Total treatment time was 25 months and no complication was found.

  • Dilaceration is most common in maxillary central incisor. Impaction by dilacerated tooth cause orthodontic problem such as tilting of adjacent tooth, midline deviation. Dilaceration is caused by trauma, cyst, other origin and tooth shape, degree of root curvature, location in the alveolar bone, ability of spontaneous eruption should be considered whether treat or not. Labially and lingually dilacerated tooth is difficult to spontaneous eruption. Thus, Prosthetic treatment alternative with extraction, auto transplantation, orthodontic traction with surgical exposure is included within the treatment alternative. These cases are about impacted central incisor with dilacerated root. We use closed eruption technique and guide impacted tooth into normal position by orthodontic traction.

  • Skeletal anchorage is recommended as an orthodontic treatment for an impacted immature permanent tooth. Among these methods, C-tube is relatively safe because it is fixed to the cortical bone of interdental and the lower part of the root with several short miniscrews, which causes less damage to the root in patients of early permanent dentition. As it can be easily bent, the traction direction can be adjusted to favorable bone density sites. However, patient cooperation is important and traction based on physiological force in order to gain root and dentoalveolar tissue development in immature permanent teeth is required. Periodic follow-ups should be mandatory.

  • Autogenous tooth transplantation can be defined as the surgical movement of a tooth from one position in the mouth to another in the same individual. The most common reasons for tooth transplantation include replacement of a missing first molar, transplantation of impacted canines to their normal positions in the arch, and transplantation of premolars in areas of missing teeth, especially in the anterior area of the mouth. The key to successful tooth transplantation is proper selection of graft with adequate root development as well as the design of surgical operation. Root development stage with half to three-quarter-developed roots increase the success rate of autotransplantation. We report the cases of successful autotransplantation which resulted in ideal healing of periodontal ligament, gingiva and alveolar bone. All transplanted teeth presented immature root formation at the moment of the procedure. After surgical procedure, we can observe good healing pattern without endodontic problem.
