A Case of Giant Malignant Peripheral Nerve Sheath Tumor of Neck (경부의 거대 악성 말초 신경성 종양 예)

  • Malignant peripheral nerve sheath tumor (MPNST) is an extremely rare sarcoma of soft tissue. It is locally invasive, frequently recurred and metastasizes distantly, therefore has a very poor prognosis. the most involved sites are human body and extremities, and MPNST occurs and involved very rarely in the parapharynx of neck. MPNST does not react to radiotherapy and chemotherapy, therefore complete surgical resection is the most important treatment. Since immunohistochemical staining is essential for the final diagnosis, it is common to be confirmed after surgery. We report a case of giant MPNST without symptom in the head and neck involving the right parapharynx with multiple distant metastases in a 74-year-old patient on a review of the literature.

Effects of FK224, a $NK_1$ and $NK_2$ Receptor Antagonist, on Plasma Extravasation of Neurogenic Inflammation in Rat Airways (미주 신경의 전기적 자극으로 유발된 백서의 기도내 혈장 유출에 대한 FK224의 효과)

  • Background: Asthma is an inflammatory disease because there are many inflammatory changes in the asthmatic airways. Axon reflex mechanisms may be involved in the pathogenesis of asthma. Sensory neuropeptides are involved in this inflammation, which is defined as neurogenic inflammation. Substance p, neurokinin A, and neurokinin B may be main neuropeptides of neurogenic inflammation in airways. These tachykinins act on neurokinin receptors. Three types of neurokinin receptors, such as $NK_1$, $NK_2$, and $NK_3$, are currently recognized, at which substance p, neurokinin A, and neurokinin B may be the most relevant natural agonist of neurogenic inflammation in airways. The receptor subtypes present in several tissues have been characterized on the basis of differential sensitivity to substance p, neurokinin A, and neurokinin B. Plasma extravasation and vasodilation are induced by substance p more potently than by neurokinin A, indicating NK1 receptors on endothelial cells mediate the response. But airway contraction is induced by neurokinin A more potently than by substance P, indicating the $NK_2$ receptors in airway smooth muscles. These receptors are used to evaluate the pathogenesis of brochial asthma. FK224 was identified from the fermentation products of Streptomyces violaceoniger. FK224 is a dual antagonist of both $NK_1$ and $NK_2$ receptors. Purpose: For a study of pathogenesis of bronchial asthma, the effect of FK224 on plasma extravasation induced by vagal NANC electrical stimulation was evaluated in rat airway. Method: Male Sprague-Dawley rats weighing 180~450gm were anesthetized by i.p. injection of urethane. Plasma extravasation was induced by electrical stimulation of cervical vagus NANC nerves with 5Hz, 1mA, and 5V for 2 minutes(NANC2 group) and for sham operation without nerve stimulation(control group). To evaluate the effect of FK224 on plasma extravasation in neurogenic inflammation, FK224(1mg/kg, Fujisawa Pharmaceutical Co., dissolved in dimethylsulphoxide; DMSO, Sigma Co.) was injected 1 min before nerve stimulation(FK224 group). To assess plasma exudation, Evans blue dye(20mg/kg, dissolved in saline) was used as a plasma marker and was injected before nerve stimulation. After removal of intravascular dye, the evans blue dye in the tissue was extracted in formamide($37^{\circ}C$, 24h) and quantified spectrophotometrically by measuring dye absorbance at 629nm wavelength. Tissue dye content was expressed as ng of dye per mg of wet weight tissue. The amount of plasma extravasation was measured on the part of airways in each groups. Results: 1) Vagus nerve(NANC) stimulation significantly increased plasma leakage in trachea, main bronchus, and peripheral bronchus compared with control group, $14.1{\pm}1.6$ to $49.7{\pm}2.5$, $17.5{\pm}2.0$ to $38.7{\pm}2.8$, and $12.7{\pm}2.2$ to $19.1{\pm}1.6ng$ of dye per mg of tissue(mean ${\pm}$ SE), respectively(p<0.05). But there was not significantly changed in lung parenchyma(p>0.05) 2) FK224 had significant inhibitory effect upon vagal nerve stimulation-induced airway plasma leakage in any airway tissues of rat,such as trachea, main bronchus, and peripheral bronchus compared with vagus nerve stimulation group, 49%, 58%, and 70%, respectively(p<0.05). Inhibitory effect of FK224 on airway plasma leakage in neurogenic inflammation was revealed the more significant in peripheral bronchus, but no significant in lung parenchyma. Conclusion: These results suggest that FK224 is a selective NK receptor antagonist which effectively inhibits airway plasma leakage induced by the endogenous neurotransmitters relased by neurogenic inflammation in rat airway. Tachykinin receptor antagonists may be useful in the treatment of brochial asthma.

Effect of Gyehyuldeung Treatments in Peripheral Nerve Regeneration of Rat (계혈등(鷄血藤)이 Rat의 말초신경 재생에 미치는 효과)

  • Objective : Gyehyuldeung (GHD) has been widely used in oriental medicine for the treatments of cardiovascular and neurological disorders. Thus, its potential facilitatory activity on axonal regeneration was investigated in the rats. Methods: Sprague-Dawley rats were given crush injury at the sciatic nerve and the changes of axon growth after nerve injury on each nerve injury model were investigated with anti-NF-200 antibody, DiI, GAP-43 protein and Cdc2 protein Results : GHD-mediated enhancement of axonal regeneration after crush injury was measured in both qualitative and quantitative ways by immunofluorescence staining with anti-NF-200 antibody and retrograde tracing of fluorescence dye DiI. GAP-43 protein levels were elevated by GHD treatments in the distal injured sciatic nerve and DRG sensory neurons. The neurite outgrowth of DRG sensory neurons was facilitated by GHD treatment when co-cultured with Schwann cells and astrocytes prepared from injured sciatic nerves and injured spinal cord tissues, respectively. It was observed that Cdc2 protein was up-regulated in co-cultured Schwann cells or astrocytes and Cdc2 protein signals were co-localized to a certain extent with those of phospho-vimentin protein. Conclusions : These results suggest that GHD may play a facilitatory role in axonal regeneration by acting on the injured axons and adjacent non-neuronal cells. The current findings may be useful for the development of therapeutic targets through more specific explorations on molecular interactions between herbal components and endogenous factors.

Therapeutic Effect of Bogijetongtanggammi-bang on Peripheral Nerve Injury (보기제통탕감미방(補氣除痛湯減味方)의 랫드 말초신경 손상에 대한 회복 효과 연구)

  • Objectives : This study was aimed to investigate the therapeutic effect of Bogijetongtanggammi-bang (BJTG) on injury of the peripheral nerve tissues. Methods : Rats were divided into 2 groups. The rats of the first group were injected with Taxol (1.25 mg/kg) to their sciatic nerves, once each. The sciatic nerves of the rats of the second group were crushed by forcept for 30 seconds. Rats were administered with BJTG (400 mg/kg) or 0.9% saline for 5 days. Changes of DRG neurons, Schwann cells, Cdc2, caspase 3. phospho-p44/42 Erk1/2, phospho-vimentin and ${\beta}1$ integrin were observed by fluorescent microscope and analysed in western blot. Results : In Taxol-treated SD rat models, BJTG up-regulated neurite outgrowth, Schwann cells, Cdc2 and phospho-Erk1/2, and down-regulated caspase 3. In pressure-injured rat models, BJTG up-regulated axons of sciatic nerve, Schwann cells, Cdc2, phospho-vimentin, ${\beta}1$ integrin, and down-regulated caspase 3. Conclusions : Taken together, BJTG was promotive of nerve regeneration on SNI as well as Taxol-induced nerve injury. BJTG had a pharmaceutical property enhancing recovery of injured peripheral nerves and could be a candidate for drug development after further research.

Intradermal Alcian-Blue Injection Method to Trace Acupuncture Meridians (경락추적을 위한 피내 알시안 블루 주입방법)

  • Objective In this article, we report on the intradermal Alcian blue staining method for tracing the meridians of acupuncture. Methods 1% Alcian blue solution was injected into acupoints by using a 0.5mL insulin syringe with a 31-gauge needle, then the skin was incised and was observed under a stereoscopic microscope. The specimens were examined by using immunohistochemical methods and were observed under a confocal laser scanning microscope. Results A threadlike structure, which was visualized with Alcian blue, existed in dermis layer and proceeded to hypodermis. In this structure, characteristic alignments of rod- shaped nuclei and $1-2{\mu}m$ sized DNA granules were observed. Furthermore, abundant blood capillary plexuses, peripheral nerve endings, and a corpuscle-like structure(about $300{\mu}m$ in diameter) were visualized in the skin tissues of acupoints. Conclusion It was concluded that the specific threadlike and corpuscle-like structures corresponded to superficial Bonghan duct and corpuscle, respectively.

The Effect of Micro-current Electrical Therapy on Muscle Atrophy and Delayed Wound Healing Process Induced Rat Caused by Traumatic Peripheral Nerve Injury (외상성 말초신경 손상으로 인한 창상 치유 지연 및 근위축이 유발된 소동물에서의 미세전류 자극 효능 평가)

  • This study was carried out to evaluate the effect of micro-current therapy on muscle atrophy and delayed wound healing process caused by traumatic peripheral nerve injury. For this, twenty-five 5-week-old Sprague Dawley rat were used and assigned to five groups including the normal group (NOR, n=5), the wound group (WD, n=5), the wounded and treated with micro-current electrical therapy group (WD+MET, n=5), the sciatic nerve denervated and wounded group (WD+DN, n=5), and the sciatic nerve denervated, wounded and treated with micro-current electrical therapy group (WD+DN+MET, n=5). In order to assess the changes in length of incisional wound for 12 days and the muscle volume for 2 weeks, the ImageJ analysis of macroscopic analysis and micro-CT data were obtained and analyzed. As a result, significant delay in the process in wound healing were observed and micro-current therapy suppress the postponement of healing process. Furthermore, there were significant changes in muscle volume between electrically treated group and non-treated group. These result shows that electrical stimulation may improve the delayed healing process and muscle atrophy at once.

The effect of Environmental Enrichment and Peripheral Nerve Electrical Stimulation on Functional Recovery after Brain in rats (환경강화와 말초신경 전기자극이 뇌손상 백서의 기능회복에 미치는 영향)

  • Purpose: To investigate and analyze effects of environmental enrichment(EE) and nerve stimulation that follows in application times with change of functional recovery(1, 3, 7 and 14 days). Methods: Focal ischemic brain injury was produced in 100 Sprauge-Dawley rats through middle cerebral artery occlusion(MCAO). Neurobehavioral assessment were selected, such as tilting plane testing, horizontal wire testing, vestibulomotor function testing and complex neuromotor function test, then they were randomly divided into five groups; Group I : Sham group, Group II: MCAO group, Group III: MCAO and ES group, Group IV: MCAO and EE group, Group V: MCAO and EE and ES group. Results: In neurobehavioral assessment, group V were significantly difference from other groups on between-subject effects. Conclusion: Our findings suggest that in focal ishemic brain injury, combined environmental enrichment and peripheral nerve electrical stimulation is more improved that the improvement of exercise function recovery than non treatment group.

The Influences of Depression, Anxiety, Social Support and Knowledge of Anticancer Drugs on the Chemotherapy-induced Peripheral Neuropathy among Colorectal Cancer Patients Receiving Oxaliplatin (Oxaliplatin을 투여 받는 대장암 환자의 우울, 불안, 사회적지지, 항암제 지식수준이 말초신경병증에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing chemotherapy-induced peripheral neuropathy among colorectal cancer patients receiving oxaliplatin. Methods: A total of 132 patients hospitalized for chemotherapy were surveyed at K University Hospital in Seoul, Korea. This study was a descriptive causal relationship study using a self-report questionnaire survey method. Correlation and multiple regression analysis between the factors were performed using SPSS 18.0. Results: The regression model was significant (F=31.64, p<.001), which meaned that the experience of chemotherapy-induced peripheral neuropathy among the participants was statistically significant. The factors influencing the chemotherapy-induced peripheral neuropathy were depression (${\beta}=.34$, p<.001), followed by anxiety (${\beta}=.32$, p<.001), medical staff support (${\beta}=-.17$, p=.037) and the level of knowledge of anticancer drugs (${\beta}=-.16$, p=.045). The explanatory power of these factors on the chemotherapy-induced peripheral neuropathy of colorectal cancer patients was 69%. Conclusion: The factors influencing the chemotherapy-induced peripheral neuropathy of colorectal cancer patients receiving oxaliplatin were identified as depression, anxiety, level of knowledge of anticancer drugs and medical staff support.

A Case Report of Peripheral Neuropathy after Induction Chemotherapy before Autologous Peripheral Blood Stem Cell Transplantation in a Patient with Multiple Myeloma with Amyloidosis Treated with Complex Korean Medicine Treatment (아밀로이드증을 동반한 다발골수종 환자의 자가말초혈액조혈모세포 이식 전 유도항암화학요법 후 발생한 말초신경병증에 대한 복합 한의 치험 1례)

  • Introduction: We present a case of multiple myeloma with amyloidosis, which has features of peripheral neuropathy after induction chemotherapy before autologous peripheral blood stem cell transplantation, in a 56-year-old woman with Korean medicine. Case Presentation: For 17 days of hospitalization, the patient with complaints of numbness and a tingling sensation in the hands and feet was treated with acupuncture, herbal medicine. To reduce the symptoms, we provided Korean medicine treatments, including herbal medicine, acupuncture, and moxibustion. The Visual Analog Scale (VAS) was used to evaluate the results of the treatment. Until discharge, the VAS scores decreased for both hands and the foot tingling sensation. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for tingling sensations in a patient with multiple myeloma with amyloidosis. Prospective studies are needed in the future to confirm and expand these findings.

  • Employing clonidine, phenoxybenzamine, and phentolamine, the author attempted to clarify the mechanism of the pressor response in raised intracranial pressure (ICP) in urethaneanesthetized rabbits. Intravenous clonidine inhibited the pressor response in raised ICP as intraventricular (I.V.T ) clcnidine did. Intravenous phenoxybenzamine and phentolamine weakened markedly the pressor response in raised ICP I.V.T. phenoxybenzamine did not affect the pressor response as I.V.T. phentolamine. I.V.T. phenoxybenzamine antagonized the inhibitory effect of I.V.T. clonidine on the pressor response as I.V.T. phentolamine. It is concluded that the central and peripheral sympathetic activity plays an important role in producing the pressor response in raised ICP.
