당뇨병 환자들은 말초신경 손상의 합병증으로서 이상감각과 통증을 느낄 수 있는데 이는 날카로운 느낌, 차갑고 시린 느낌, 무딘 느낌, 살갗이 벗겨진 듯한 예민한 느낌, 화끈거리는 느낌, 따끔하고 가려운 느낌 등 다양하게 나타날 수 있다. 이를 진단하기 위해서 여러 선별검사와 전기생리학적 검사를 활용할 수 있다. 본 연구에서는 신경병통증척도와 미시간 신경병증 선별도구 및 신경전도검사를 통해 이상감감/통증을 호소하는 당뇨병 환자 11명과 호소하지 않는 당뇨병 환자 10명 간에 차이점 및 특징을 분석하였고, 세부 감각양상과 신경전도검사 지표간 상관성을 분석하였다. 그 결과 이상감각/통증을 호소하는 환자군에서 신경병통증척도와 미시간 신경병증 선별검사의 점수가 통계적으로 유의하게 높았고 신경전도검사에서는 주로 정중신경 운동신경과 비골신경이 의미 있는 기능 변화를 보였다. 세부 감각양상에 따른 신경전도검사 지표는 주로 정중신경 운동신경, 후경골신경, 장딴지신경에서 유의한 변화를 보였다. 이러한 결과들은 감각이상과 통증이 실제 말초신경 이상과 관련이 있음을 시사한다. 향후 많은 수의 대상자를 포함한 추가적인 연구가 필요하다.

본 논문의 목적은 당뇨병 및 신경병증의 병력이 없는 17명의 일반 성인을 대상으로 한 정량적 감각검사(QST)와 광용적맥파(PPG)와의 비교를 통해 정량적 감각검사의 임상적 유용성을 파악하는 것이며, 광용적맥파 검사를 이용한 혈당수치에 따른 자율신경계의 부교감신경의 변화를 살펴보는 것이다. 일반인을 대상으로 혈당검사와 광용적맥파 검사를 실시하여 상관관계를 분석한 결과 혈당수치에 따라 자율신경계 부교감신경이 느려지는 상관관계를 보였다. 이상의 결과에서 정량적 감각 검사가 주관적 평가에 의한 검사이기는 하나 검사의 민감도를 신뢰할 수 있다고 보이며 따라서 말초 신경의 평가도구로서 정량적 감각검사가 당뇨병성 신경병증 등의 진단의 민감도를 높이는데 폭넓게 이용될 수 있을 것으로 보이며, 광용적맥파 검사가 심박변화율에 의한 자율신경계를 평가하는 후속연구에 기초자료연구로 이용될 수 있을 것으로 사료된다.

Some patients with leprosy may present with atypical features, such as isolated peripheral neuropathy without skin lesions, or marked proprioceptive dysfunction. We report a 56-year-old female who presented with predominant proprioceptive loss without skin lesion, but was finally confirmed as leprous neuropathy by sural nerve biopsy. It is postulated that large myelinated fibers were affected by chronic immunological reactions triggered by inactive bacterial particles, producing a peripheral neuropathy presenting as predominant proprioceptive sensory loss without typical skin lesions.

Intravenous immunoglobulin (IVIg) is the treatment of choice for many autoimmune neuropathic disorders such as Guillain-Barre syndrome (GBS), chronic inflammatory Demyelinating neuropathy (CIDP), and multifocal motor neuropathy (MMN). IVIg is preferred because the adverse reactions are milder and fewer than the other immune-modulating methods such as steroid, other immunosuppressant such as azathioprine, and plasmapheresis. IVIg also has been used in other autoimmune neuromuscular disorders (inflammatory myopathy, myasthenia gravis, and Lambert-Eaton myasthenic syndrome) and has been known as safe and efficient agent in these disorders. Since IVIg would get more indications and be used more commonly, clinicians need to know the detailed mechanism of action, side effects, and practical points of IVIg.

Neurolymphomatosis, an uncommon manifestation of non-Hodgkin's lymphoma, is lymphomatous infiltration of peripheral nerves. We confirmed the diagnosis of neurolymphomatosis in a 75-year old woman with a history of complete remission of diffuse large B cell type lymphoma on the nasal cavity seven years ago. She complained of painful weakness of left leg and took the electrophysiologic study, extremity ultrasonography, fluorodeoxyglucose PET-CT, and extremity MRI serially. She was diagnosed as neurolymphomatosis by targeted posterior tibial nerve mass biopsy.

요즈음은 인터넷이나 대중매체를 통해 일반인들도 다양한 질병에 대한 많은 정보를 얻을 수 있다. 이에 속하는 것으로 당뇨병은 대표적 만성질환이며 누구나 한번쯤은 들어 본 적이 있는 질환일 것이다. 현대와 같이 고도로 의학이 발달한 상황에서도 만성질환은 완치되는 것이 아니고 평생 관리를 해야하는 질병이기 때문에 많은 환자들이 살아가는 동안 서서히 지치게 되고 관리를 소홀히 하게 됨으로써 당뇨병성 신경병증, 망막증, 신증, 대혈관병증(뇌졸중, 심혈관질환, 말초혈관질환) 등과 같은 합병증의 발생으로 일상생활에 지장을 받게 된다. 따라서 당뇨병환자는 일생 주기적으로 검사와 함께 자신에게 맞는 약을 복용하고 경우에 따라서는 인슐린 주사까지 맞음으로써 지속적인 혈당관리를 하여 합병증의 발생을 최소화 하는 것이 필요하다. 이상에 언급한 다양한 합병증 중 가장 중요한 사망원인은 심혈관질환으로 40세 이후에 발생한 당뇨병환자에서 전체 사망 원인의 50% 이상을 차지한다. 또한 대부분의 당뇨병성 만성합병증이 적극적인 혈당관리로 효과적인 예방이 가능한 반면 심혈관질환과 같은 대혈관합병증은 혈당조절이 잘 되고 있는 환자에서도 당뇨병 발생 기간과 무관하게 발생하고 또 여러 혈관에 걸쳐 다발적이고 광범위하게 발생하므로 원인이 되는 다른 위험요소들에 대한 관리가 함께 이루어져야 한다.

Peripheral neuropathy is a disease of multiple Peripheral nerves. Tingling, pins-and needles, numbness, burning and raw sensation are symptoms of peripheral neuropathy. These symptoms are frequently complained by patients who suffered from obstructive syndrome of Ki and blood (痺證). This case was reported to evaluate the effects of oriental medicine therapy on a patient with obstructive syndrome of Ki and blood (痺證). The subject was a female patient who had obstructive syndrome of Ki and blood (痺證). She complained of calf pain, cold sense, walking discomfort, insomnia, dyspepsia, anorexia etc. We administered the medicine with dispeling the cold pathogenic factor (寒邪), adjusting the constructive and defensive energy (調營衛), tonifying the Ki and blood (補氣血) by stage. After the treatment, the symptoms improved. This case suggests that oriental medicine therapy can be applicable to improve in symptoms with Peripheral neuropathy.

Tingling, pins-and-needles, numbness, burning and raw sensations are the words frequently used by patients to describe such conditions as paresthesia and obstruction syndrome of Ki and blood, Obstruction syndrome of Ki and blood results from the complex elements of wind, cold and dampness and has a process of stagnating Ki, blood and meridian system, The subject was a male patient who had obstructive syndrome of Ki and blood, We administered the medication with dispeling the pathogenic factor, adjusting the constructive and defensive energy, tonifying the Ki and blood by stage. The subject reported reduced pain, an increased range of motion and improved hyperlipidemia.

Diabetic Peripheral Polyneuropathy is one of the most distressing complications of diabetes, Drugs such as narcotic analgesics, tricyclic antidepressants, anticonvulsants and phenothiazines have been used to treat diabetic peripheral polyneuropathy, but these drugs are not very effective for the many patients and the side effects may become intolerable. In oriental medicine, usually the main etiology of diabetic Peripheral Polyneuropathy is insufficiency of Blood due to the state of dry-heat in the human body. Therefore, we use Bogan-tang to treat numbness of Diabetic Peripheral Polyneuropathy. Bogan-tang was administrated twice a day for 15 days, and VAS Scale was performed every day to evaluate numbness and insufficiency of blood. In both cases, numbness and insufficiency of blood were improved without any side effects. This study suggests that Bogan-tang is an effective drug in the treatment of Diabetic Peripheral Polyneuropathy.

Objective We report a case about a Soyangin patient with diabetic neuropathy. His symptoms were alleviated with Sasang Constitutional medicine. Methods A Soyangin patient with diabetic neuropathy was treated by herbal medicine, acupuncture, pharmacopuncture according to Sasang Constitutional Medicine. The progress was evaluated with numerical rating scale(NRS) and blood sugar test(BST). Results Sasang Constitutional Medicine seems to be effective for the symtoms of the patient with diabetic neuropathy in this case. Conclusions This case study shows that Sasang Constitutional Medicine may be an effective treatment for Soyangin's diabetic neuropathy.
