  • Objective: Ortho-Cellular Nutrition Therapy (OCNT) Experiential treatment for Breast Cancer, lymphedema Patients Methods: The patient is a Korean woman aged 50 years. She was diagnosed with stage 3 breast cancer, the right lymph node was removed, resulting in lymphedema and pain. Results: After nutritional therapy, lymphedema improved. Conclusion: The patient's lymphedema improved, and she regained her daily vitality.

  • Objective: Lymphedema case study through the application of Ortho-Cellular Nutrition Therapy (OCNT) Method: OCNT is implemented to a Korean female patient in her 70s with difficulties in walking due to severe swelling in one of her legs. Results: Lymphedema was improved following implementation of OCNT. Conclusion: Application of OCNT to lymphedema patient can be helpful.

Comparison of Cervical Lymphadenitis as First Presentation of Kawasaki Disease and Acute Unilateral Cervical Lymphadenitis (경부 림프절염로 발현된 가와사키병과 급성 편측 경부 림프절염의 비교)

  • Purpose: This study aimed to identify the differential clinical, laboratory, and imaging characteristics between patients with cervical lymphadenitis as first presentation of Kawasaki disease (CLKD) and those with acute unilateral cervical lymphadenitis (AUCL). Methods: We surveyed 372 patients who visited Pusan National University Children's Hospital because of fever and cervical lymph node enlargement, and underwent neck computed tomography (CT) from January 2010 to December 2014. We compared 28 confirmed cases of Kawasaki disease and 28 cases of AUCL based on a retrospective review of the medical records of the patients. Results: Patients with CLKD and AUCL showed no differential clinical characteristics in terms of the duration of fever, antibiotic use, or the size of lymph nodes. Patients with CLKD had higher white blood cell count, absolute neutrophil count, erythrocyte sedimentation rate, and C-reactive protein levels (P<0.05) than those of patients with AUCL. The presence of retropharyngeal edema on neck CT was similar between the groups (64% vs. 33%, P=0.686). Conclusions: CLKD and AUCL showed no differentiating clinical and radiological characteristics; hence, Kawasaki disease should be the presumptive diagnosis in patients with fever and cervical lymph node enlargements who fail to respond to antibiotic treatment.

A Study on Self-care among the Lymphedema Patients (림프부종 환자의 자가관리에 관한 실태 조사)

  • Purpose: The purpose of this descriptive study was to gain basic data to develop a self-care protocol for the lymphedema patients. Method: The subjects of this study consisted of 115 patients with lymphedema from 8 hospitals and two community health and welfare centers in Busan and Seoul. The data was collected with questionnaire by self reporting of patients between March 2001 and December 2001. Data was analyzed by mean and percentage. Result: For self-care activities in daily life, compliance of 'use skin care preparations', 'use heat and cold', 'protect from local compression on affected limbs', 'protect from insect biting', 'use aids to protect affected limbs', 'take diuretics and take protein diet' did not reach to 50%. For self-care activities related to complex physical therapy, 28.7% of subjects complied with compression garment, 14.8% with manual lymph drainage, and 13.0% with exercise. 20.0% of subjects tried to treat with acupuncture and 13.9% with heat therapy. Conclusion: From this study, it is suggested that patients need to get a self-care education with correct information about self care activities and health care professionals need to develop more convenience self-care techniques of massage and exercise.

A Study on Nursing Care and Self-Care of Lymphedema in Mastectomy Patients (유방절제술 환자의 림프부종 자가관리 및 간호활동)

  • Purpose: The purpose of this study was to provide nursing intervention with basic data extracted through investigating self-care and nursing of lymphedema in patient who have had a mastectomy. Method: The subjects for this study consisted of 214 mastectomy patients in 2 hospitals. The data were collected from December 1, 2004 to February 28, 2005. The instruments used for this study were both the Self-care Practice Scale by Cho, Myoung-Ok et al.(2003) which we modified and the Nursing Evaluation Scale developed by the researchers. Results: The mean score of self-care on lymphedema was $59.96{\pm}12.46$ The mean score of nursing was $25.64{\pm}9.25$ Factors influencing the self-care were the postoperative period (F=17.505, p<.001), the location of the tumor (F=3.826, p=.023), menstruation (t=7.333, p=.007), and complications (F=5.427, p<.001). Conclusion: Self-care and nursing care on the lymphedema management were shown to be lower than expected. Especially, the score of self-care was the lowest in the more than 3 year postoperative period. Therefore, the longer the postoperative period is, the more active management on lymphedema needs to be. Also, education should be given to both nurses and patients on the importance of self-care on lymphedema with mastectomies.

Effects of Manual Lymph Drainage on the Edema and Quality of Life in Lymphedema Patients of Maintenance Phase (유지기 림프부종환자에서 MLD가 부종과 삶의 질에 미치는 영향)

  • Purpose: This study examined the effects of manual lymph drainage (MLD) on edema and the quality of life (QOL) of lymphedema patients in the maintenance phase. Method: Forty five lymphedema patients, who had completed intensive decongestive therapy, were enrolled in this study. All subjects were assigned randomly to two groups: Experimental (MLD) group (n=23) and control (self-MLD) group (n=22). MLD by a specialist was applied with a comfortable pressure for 40~60 minutes in the experimental group and self-MLD was carried out by the control group 5 times a week for 2 weeks. The % edema and a SF-36 questionnaire were used to measure the decrease in edema and the QOL. A paired t-test was used to compare the period, and an independent T-test was used to compare experiment and control groups. Result: The % edema was significantly lower in the experimental group after MLD (p<0.05). The physical, vitality, and mental health scores of the experimental group was significantly higher than those of the control group (p<0.05). Conclusion: TMLD by a specialist in the maintenance phase is effective in reducing the % edema and enhancing the QOL of lymphedema patients. Further studies will be needed to determine if there are differences in the effects of many types of treatment methods in the lymphedema treatment.

Evaluation of Error Factors in Quantitative Analysis of Lymphoscintigraphy (Lymphoscintigraphy의 정량분석 시 오류 요인에 관한 평가)

  • Purpose: Lymphoscintigraphy is absolutely being used standard examination in lymphatic diagnosis, evaluation after treatment, and it is useful for lymphedema to plan therapy. In case of lymphoscintigraphy of lower-extremity lymphedema, it had an effect on results if patients had not pose same position on the examination of 1 min, 1 hour and 2 hours after injection. So we'll study the methods to improve confidence with minimized quantitative analysis errors by influence factors. Materials and Methods: Being used the Infinia of GE Co. we injected $^{99m}Tc$-phytate 37 MBq (1.0 mCi) 4 sylinges into 40 people's feet hypodermically from June to August 2010 in Samsung Medical Center. After we acquired images of fixed and unfixed condition, we confirmed the count values change by attenuation of soft tissue and bone according to different feet position. And we estimated 5 times increasing 2 cm of distance between $^{99m}Tc$ point source and detector each time to check counts difference according to distance change by different feet position. Finally, we compared 1 and 6 min lymphoscintigraphy images with same position to check the effect of quantitative analysis results owing to difference of amounts of movement of the $^{99m}Tc$-phytate in the lymphatic duct. Results: Percentage difference regarding error values showed minimum 2.7% and maximum 25.8% when comparing fixed and unfixed feet position of lymphoscintigraphy examination at 1 min after injection. And count values according to distance were 173,661 (2 cm), 172,095 (4 cm), 170,996 (6 cm), 167,677 (8 cm), 169,208 counts (10 cm) which distance was increased interval of 2 cm and basal value was mean 176,587 counts, and percentage difference values were not over 2.5% such as 1.27, 1.79, 2.04, 2.42, 2.35%. Also, Assessment results about amounts of movement in lymphatic duct within 6 min until scanning after injection showed minimum 0.15%, and maximum 2.3% which were amounts of movement. We can recognize that error values represent over 20% due to only attenuation of soft tissue and bone except for distance difference (2.42%) and amounts of movement in lymphatic duct (2.3%). Conclusion: It was show that if same patients posed different feet position on the examination of 1 min, 1 hour and 2 hours after injection in the lymphoscintigraphy which is evaluating lymphatic flow of patients with lymphedema and analyzing amount of intake by lymphatic system, maximum error value represented 25.8% due to attenuation of soft tissue and bone, and PASW (Predictive Analytics Software) showed that fixed and unfixed feet position was different each other. And difference of distance between detector and feet and change of count values by difference of examination beginning time after injection influence on quantitative analysis results partially. Therefore, we'll make an effort to fix feet position and make the most of fixing board in lymphoscintigraphy with quantitative analysis.

  • Primary intestinal lymphangiectasia is a congenital lymphatic disorder in which intestinal lymphatic channels are dilated and ruptured resulting in loss of protein, lipid, and lymphocyte into the intestine or peritoneum. As a result, hypoalbuminemia, generalized edema, diarrhea are clinically manifested. We report a case of primary intestinal lymphangiectasia with generalized edema which occurred in a 7-year old boy who was treated with lipid restriction diet with medium chain triglyceride oil supplement.

Effects of Decongestive Lymphatic Therapy for Lymphoedema in Women With Breast Cancer (유방암 여성 환자의 림프부종에 대한 림프배액맛사지의 효과)

  • Objective: We have the purpose of evaluating the effects of lymphatic massage on lymphoedema in breast cancer patients. Method: The patient who has in condition of lymphoedema after the breast cancer therapy is taken massages with aroma oil for 5 days. We measured the upper arm circumference and edema index before and after the therapy. We also measure the edema index of normal arm to compare with the abnormal one. Results: We gave 4 patients the decongestive lymphatic therapy and now present the positive conclusion. 3cases of the 4 showed decrease of circumferences and edema index. It means that the edematous condition getting better during these therapy. Conclusion: Lymphatic massage with aroma therapy may have effectiveness on lymphoedema in breast cancer patients. but further advanced clinical trials are needed.

Analysis of Domestic Manual Lymph Drainage (MLD)-related Research Trend (국내 림프드레나지(MLD) 연구에 관한 논문 동향 분석)

  • This study attempted to investigate the research trend of MLD studies which were issued from 2001 to March 2022 by research period/area/method/purpose through the analysis of academic theses and journals and use the results for the growth and of MLD. The study results found the followings: First, in terms of the results by research period, a total of 84 academic papers and journals were published: Phase 1 (15), Phase 2 (47), Phase 3 (22). Second, when analyzed by research area, 'cosmetology' was the highest with 19 papers in terms of the number of studies. In fact, the field of study accounted for the largest portion in all three phases. Third, when analyzed by research method, quantitative research showed the largest percentage in all three phases. Fourth, when analyzed by research purpose, a practical approach was dominant with 81 papers out of the total number of 84 (3 papers in theoretical approach). Concerning a domestic MLD-related research trend, it is an essential step for MLD study establishment and qualitative improvements. Such research analysis of MLD studies is meaningful in that it has not been attempted before. In addition, it is anticipated that the study results would be helpful in searching a direction for the academic growth and development of MLD.
