Comparison of the accuracy of intraoral scanner by three-dimensional analysis in single and 3-unit bridge abutment model: In vitro study (단일 수복물과 3본 고정성 수복물 지대치 모델에서 삼차원 분석을 통한 구강 스캐너의 정확도 비교)

  • Purpose: The purpose of this study was to evaluate the accuracy of three types of intraoral scanners and the accuracy of the single abutment and bridge abutment model. Materials and methods: In this study, a single abutment, and a bridge abutment with missing first molar was fabricated and set as the reference model. The reference model was scanned with an industrial three-dimensional scanner and set as reference scan data. The reference model was scanned five times using the three intraoral scanners (CS3600, CS3500, and EZIS PO). This was set as the evaluation scan data. In the three-dimensional analysis (Geomagic control X), the divided abutment region was selected and analyzed to verify the scan accuracy of the abutment. Statistical analysis was performed using SPSS software (${\alpha}=.05$). The accuracy of intraoral scanners was compared using the Kruskal-Wallis test and post-test was performed using the Pairwise test. The accuracy difference between the single abutment model and the bridge abutment model was analyzed by the Mann-Whitney U test. Results: The accuracy according to the intraoral scanner was significantly different (P < .05). The trueness of the single abutment model and the bridge abutment model showed a statistically significant difference and showed better trueness in the single abutment (P < .05). There was no significant difference in the precision (P = .616). Conclusion: As a result of comparing the accuracy of single and bridge abutments, the error of abutment scan increased with increasing scan area, and the accuracy of bridge abutment model was clinically acceptable in three types of intraoral scanners.

  • 현재 골 유착성 임플랜트는 안전하고 유용한 보철 수복 분야의 하나로 자리잡아 가고있다. 과거 완전 무치악 환자에게 있어서 저작 기능의 회복이란 면에 중점을 두었던 경향에서 현재 골유착 임플랜트의 높은 성공률에 의해 부문 무치악, 단일치 수복에 까지 다양하게 사용되고 있다. 특히 심미성과 인접치의 손상을 가하지 않는다는 면에서 단일치 수복에서 골유착 임플랜트 치료에 관심이 모아지고 있다. 단일치 수복에 있어서 심미성이 중요한 문제로 대두되면서 이런 수복에 사용될 수 있는 다양한 계통의 임플랜트들이 개발되어 사용되고 있다. 이중 가장 기본적인 Branemark 계통의 CeraOne 지대주, 최근에 개발된 전부도재 지대주인 CerAdapt 전부주조 지대주인 UCLA 계통등은 높은 성공률을 보이며 많이 사용되고 있다. 임상가들에 의해 나사유지형 보철물이 착탈성이란 관점에서 선호되고 있다. 시멘트유지형 보철물에서는 나사공이 없는 온전한 교합면을 유지한다는 면과 교합력을 받는 면적에 있어서 더 넓은 면을 가진다는 점등 더 많은 장점을 가진다고 주장하는 사람도 있다. 임상가들의 기호에 의해 선택되어지는 나사 유지형과 시멘트 유지형에 있어서 교합력이 야기하는 하중하에서 각각의 응력 분포를 분석할 필요성이 요구되었다. 이 연구는 단일치 수복에 사용되는 CeraOne, CerAdapt UCLA 계통에서 각각의 나사 유지형과 시멘트유지형에서의 응력분포를 삼차원 유한요소법으로 분석하고 상부 구조물에 가해지는 수직하중, 수평 하중, 경사 하중에 의해 야기되는 응력을 비교 분석한 것이다. 본 연구의 연구 결과는 다음과 같다. 1. CeraOne, CerAdapt, UCLA 지대주 모두 시멘트유지형보다 나사유지형에서 응력집중이 컸다. 2. CeraOne 시멘트유지형인 1번 모델에서 응력분산이 유리하였고, UCLA 나사 유지형인 6번 모델에서 가장 불리하였다. 3. 모든 모델에서 고정체 경부에서 가장 큰 응력 집중이 있었고 이것은 UCLA 지대주에서 가장 컸다. 4. 상부 구조물에서 주된 응력의 집중은 교합면에서 일어났다. 5. 골은 상부 피질골, 즉 고정체경부와 만나는 부위에서 가장 큰 응력의 집중이 일어났으며 수평, 경사 하중시 응력집중 양상은 힘을 가한 쪽의 반대쪽 고정체 경부에 응력이 집중되는 양상을 보였다. 6. 전체 모델과 골, 고정체 모두에서 수평 하중과 경사 하중시 보다는 수직 하중시 더 적은 응력값을 보였다.

  • Immediate implantation is widely applied as it is recognized as a useful implant treatment in dental clinic due to its various advantages as well as being able to shorten the treatment period. Immediate implant treatment was performed on 5 patients who needed extraction of the mandibular first molar for various reasons, shortening the overall treatment period, and obtaining satisfactory results. Immediate implantation after extraction is a high-level treatment that requires proper initial fixation at the exact location of the extraction socket and the surrounding soft tissues, but is considered to be a clinically very useful prosthetic restoration method

  • Porcelain is the first ceramic material to be introduced into dentistry. Porcelain jacket crown was introduced by Dr. Charles H Land in 1886, which was an excellent aesthetic dental restoration but has not been widely used due to high firing shrinkage and low tensile strength. Then metal-ceramic system, which combines the esthetic properties of ceramics and the mechanical properties of metals, was introduced and nowadays it is still used in dental clinical field. However, the metal-ceramic system has shown some problems, such as increased lightness by reflection of light at opaque layer, shadow beneath the gingival line due to the block-out of light by metal coping, exposure of metal in margin part, bond failure between metal and porcelain, oxidation of metal coping during firing the porcelain, etc. Recently, along with the advance of fabrication methods of dental ceramics, the all-ceramic restorations with high esthetic and mechanical properties has increased and gradually replaced metal-ceramic restorations. Especially, CAD/CAM technology has opened a new era in fabricating the dental ceramic restorations. This overview will take a look at the past, present and future possibility of the dental ceramic materials.

Superimposition: a simple method to minimize occlusal adjustment of monolithic restoration (디지털 장비의 중첩기능을 이용하여 단일체 수복물의 교합조정을 최소화한 증례)

  • The development of translucent zirconia enabled clinicians to choose a monolithic zirconia crown as one treatment modality in the posterior dentition. Careful occlusal adjustments are recommended for monolithic zirconia crowns because grinding zirconia inevitably causes phase transformation, which may deteriorate mechanical properties. intraoral scanners enable the clinician to scan and superimpose a complete tooth structure before preparation onto the prepared abutment. This technique helps to reproduce the original tooth form and occlusion of the patient. In this case report, prostheses were fabricated for patients with cracked or fractured tooth by applying intraoral scanner, Computer aided design-computer aided manufacturing (CAD-CAM) and monolithic zirconia crown to reproduce the occlusion of original tooth and to minimize occlusal adjustment. The clinical results were satisfactory in both esthetic and functional aspects.

  • In the case of single tooth replacement, a fixed prosthesis or a dental implant is the treatment option commonly selected as first choice. However, any amount of sound tooth structure should be removed to prepare the abutment teeth for full coverage retainer. The adjacent tooth damage can be avoided placing a dental implant. However, depending on the patient's oral condition and any other circumstances, it may be impossible or delayed. In this case resin bonded fixed partial denture was selected as an alternative that can restore single tooth loss without much tooth structure removal to the young patients and the patients with gingival recession. We report these patients were satisfied with esthetic and function.

Technical complications of cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks (지르코니아 프레임워크를 이용한 시멘트 유지형 임플란트-지지 단일 크라운과 연결 크라운의 보철적 합병증)

  • Purpose: This study was to assess clinically the success rates and technical complications of cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks. Materials and methods: 75 (single crowns: 51, splinted crowns: 24) cement-retained implant-supported single crowns and splinted crowns with zirconia frameworks which were restored in 67 patients were investigated for the evaluation of the success rates and technical complications. All restorations were cemented with temporary cement. Age, gender, restoration position, opposing teeth, restoration type were assessed as possible factors affecting technical complications. Results: During the mean observation period of 22.2 months, cumulative success rates of all restorations were 66.9 (73.2 - 60.6)%. Retention loss was found in 16 restorations (single crowns: 14, splinted crowns: 2), abutment screw loosening and veneer porcelain fracture were found in each 2 single crowns, respectively. According to a Kaplan-Meier survival analysis of single crowns and splinted crowns, the cumulative success rates were 58.9 (66.6 - 51.2)%, 87.5 (96.1 - 78.9)%, respectively. There was a statistically significant difference. The other possible factors did not have a significant effect on the technical complications. Conclusion: Retention loss was the most frequent technical complication. Abutment screw loosening and veneer porcelain fracture were found rarely in single crowns only. Age, gender, restoration position, and antagonist did not have significant effect on the technical complications. Splinted crowns had a higher success rate than single crowns.

Full mouth rehabilitation with vertical increase in patient with severe tooth wear using monolithic zirconia prosthetic restoration (과도한 치아마모를 보이는 환자에서 수직교합고경의 증가를 동반한 단일구조 지르코니아 보철물 전악수복)

  • Gradual teeth wear is normal physiological progress, severe tooth wear can causes dysfunction, occlusal plane disharmony and esthetic problems. If additional space is needed for prosthetic restoration due to severe attrition, full mouth rehabilitation with increase of occlusal vertical dimension may be considered. In this case, 72 year old male patient with severe worn dentition, irregular gingival zenith and deep anterior bite was treated by full mouth rehabilitation for regaining the space for restoration, improving relationship and esthetic of anterior teeth. Provisional restoration obtained by systemic analysis, diagnosis and re-evaluation for a sufficient time was replicated to final restorations through double scanning technique using monolithic zirconia blocks. Satisfactory functional and esthetic outcomes were obtained.
