Arthroscopic Meniscal Repair with Rapid Loc Device (Rapid Loc 기기를 이용한 관절경적 반월상 연골판 봉합술)

  • Purpose: To purpose of this study is to evaluate the clinical and radiological results of arthroscopic meniscus repair using Rapid Loc device. Materials and Methods: A retrospective study was performed on 44 cases which had been turned out longitudinal tear without degeneration. We repaired all cases with Rapid Loc device. Patients were evaluated using clinical examination, Orthopaedische Arbeitsgemeinschaft Knie (OAK) scheme, and MRI. We regarded patients with joint line tenderness, swelling or McMurray positive test as clinical failure. Results: A mean age was 33.3 years and follow-up period was average 15 months. Clinical results were excellent 15 cases(34.1%), good 20 cases(45.5%), fair 7 cases(15.9%), poor 2 cases(4.5%). MRI showed grade 115 cases(33.3%), grade II 22 cases(50%), grade III 7 cases(16.7%). Clinical failures were 8 cases and only one complication was developed. Conclusion: Rapid Loc device showed the excellent results in meniscus repair. We regard it has a lot of advantages in safety, softness, ease, ability to control tension at repair site.

  • Partial thickness rotator cuff tears (PTRCTs) are now a well-established clinical entity, yrt there is no consensus in regards to surgical management. Recommended treatment varies widely from simple debridement of the torn tendon to subacromial decompression to excision of the degenerative tendon and repair. Surgery may be performed using an open, arthroscopically assisted (miniopen), or entirely arthroscopic procedure. The advantages and disadvantages of open and arthroscopic procedures may lie in the balance between precision and morbidity. Visualization of both the articular and bursal surfaces is mandatory to determine the level of rotator cuff disease and formulate a treatment strategy. Because these tear often are secondary finding, recognition of the primary pathology is critical. Treatment of the primary problem usually remedies the partial tear. However, when there is significant PTRCT, an arthroscopic or open repair of the diseased tendon will give the most consistent results.

Arthroscopic Subscapularis Tendon Repair - Preliminary Report of 8 cases study - (관절경하 견갑하건 봉합술 - 8 례에 대한 예비보고 -)

  • Purpose: To validate and introduce the technique of the arthroscopic repair for the treatment of the Subscapulris tendon tear. Materials and Methods: From April 2003 to April 2004, Arthroscopic repairs were performed in 8 patient with subscapularis tendon tear. The mean follow-up period was 11months. two cases involved only subscaplaris tendon tear. Four cases were associated small sized posterosuperir. rotator cuff tear and two cases were in large size. The type of subscapularis tendon showed upper portion full-thickness tear in 6 cases, partial-thickness tear localized at articular surface in 1 case, complete tear in 1 case. The results were analyzed by using subjective satisfaction, inferior movenent of superior displaced humeral head, Constant-Murley functional scoring Results: Constant-Murley scoring was improved from 55 point preoperatively to 75 point postoperatively following 11 months. Mean score was 71 point except 2 cases of large superoposterior rotator cuff tear, Most humeral heads were distaracted postoperatively. The subjective result for the 8 shoulders were very satisfying in 5 cases, satisfying in 1 case and dissatisfying in 2 cases. Therefore satisfactory results were noted in 6 cases (75%) of this overall treatment group. Conclusion: The arthroscopic repair for the subscapularis tendon tear is thought to be available method, which could reduce severe complications following the weakness of deltoid muscle and postoperative pain. Especially Partial Subscapularis tear not associated with superoposterior rotator cuff tear including PASTA lesion was the sutable indication of the Arthroscopic repair.

  • Intracavitary coronary artery is variant anomalous entrance to right ventricular cavity of left anterior descending artery. Since the disease is extremely rare, there has not been any report of it in Korea and is only found in $0.2\~0.3\%$ of all CABG patients. It is very difficult to be diagnosed by preoperative coronary arteriography (CAG) and secure suture is needed for right ventriculotomy after CABG due to bleeding from right ventricle. Horizontal mattress suture with pledget has been recommended but, it could compress the myocardium surrounding ventriculotomy and result in disturbed flow of left anterior descending artery branch and perforating artery. So we used simple interrupted suture and the patient was recovered as other CABG patients without complications.

Result of a Long-Term Follow-Up of Arthroscopic Partial Repair for Massive Irreparable Rotator Cuff Tears Using a Biceps Long Head Auto Graft (봉합 불가능한 광범위 회전근 개 파열에서 상완 이두근 건 장두를 이용한 관절경하 부분 봉합술의 장기 추적 관찰 결과)

  • Purpose: This paper presents the long term follow-up results of arthroscopic partial repair for massive irreparable rotator cuff tears using a biceps long head auto graft. Materials and Methods: Forty-one patients with massive irreparable rotator cuff tear, who underwent arthroscopic repair, were reviewed retrospectively. Patients who underwent arthroscopic partial repair using a biceps long head auto graft were assigned to group 1, and patients in group 2 underwent arthroscopic partial repair alone. Patients with a less than 50% partial tear of the long head biceps tendon were included in this study. The clinical scores were measured using a visual analogue pain scale (VAS) for pain, range of motion (ROM), The University of California, Los Angeles shoulder score (UCLA), American Shoulder and Elbow Surgeons Shoulder Score (ASES), and Korean Shoulder Scoring System (KSS) scores preoperatively and at the final follow-up. The acromiohumeral interval (AHI) was measured using plain radiographs taken preoperatively and at the final follow-up, and re-tear was evaluated using postoperative ultrasound or magnetic resonance imaging at the last follow-up. Results: The mean age of the patients was 62.1±12.7 years, and the mean follow-up period was 90.3±16.8 months. No significant differences in the VAS and ROM (forward flexion, external rotation, internal rotation) were found between the two groups (p=0.179, p=0.129, p=0.098, p=0.155, respectively). The UCLA (p=0.041), ASES (p=0.023), and KSS (p=0.019) scores showed functional improvements in group 1 compared to group 2. At the last follow-up, the measured AHI values were 9.46±0.41 mm and 6.86±0.64 mm in group 1 and 2, respectively (p=0.032). Re-tear was observed in six out of 21 cases (28.6%) in group 1 and nine out of 20 cases (45.0%) in group 2; the retear rate was significantly lower in group 1 than in group 2 (p=0.011). Conclusion: Arthroscopic partial repair for a massive irreparable rotator cuff tear using a biceps long head auto graft has significant clinical usefulness in functional recovery and decreases the re-tear rates after surgery than arthroscopic partial repair alone, showing favorable results after a long-term follow-up.

Pseudoaneurysms of Peripheral Arteries - A Report of 6 Cases- (말초동맥에 발생한 가성동맥류의 외과적 치료 -치험 6례-)

  • We experienced six cases of pseudoaneurysm of the peripheral artery which occurred after stab wound or after diagnostic and operative procedures. Among 6 cases, 4 cases of pseudoaneurysm were developed in the femoral arteries, and others were the subclavian and the axillary artery. Two of 6 cases were combined with previous arterio enous fistula. Doppler imaging and angiogram were performed for the dignosis and an operation. Operative procedures were resection of the aneurysm, ligation of the involved arteries and reconstruction of the artery with the autogenous saphenous vein or the cephalic vein graft. Each operations were successfully performed without any disability.

  • Horizontal mattress suture technique on microvascular anastomosis of rat(body weight : 200-250gm) femoral artery was evaluated. The present study was conducted to compare the horizontal mattress suture with simple interrupted suture on the suture time, patency rate of the sutured vessels, and the histological changes of surgical site of the vessel wall during wound healing period. The mean suture time of the vessel wall with horizontal mattress suture technique was 15min. 49sec. ${\pm}$ 2.14, which is significantly shorter than that of simple interrupted suture technique. The patency rate of the sutured vessel in both groups was statistically not different each other till post-operative 3rd day but patency rate of horizontal mattress suture was higher than that of simple interrupted suture at post-operative 3rd week. The histological findings such as intimal noss, medial degenention and intimal regeneration were similar in both groups.

A Clinical Evaluation of the Esophageal Perforation (식도 천공의 임상적 고찰)

  • Fifteen patients with esophageal perforation were treated at the Department of Thoracic and Cardiovacular Surgery, Chungnam National University Hospital during the period from June, 1985 to September, 1995. The ratio between male and female patients was 9 : 6, their age ranged from 19 years to 71 years old(a erage : 49 years old). The causes of the perforation were various, spontaneous in 4 cases, foreign body in 4 cases, instrumental trauma in ) cases, chest trauma in 1 case, drug ingestion (chlorocalchi) induced in 1 case, tracheostomy induced in 1 case, unknown in 1 case. The perforation sites were intrathoracic esophagus in 9 cases and cervical in 6 cases. The Patients complained of chest or cervi- cal pain in 11 cases, fever in 9 cases, dysphagia in 8 cases and dyspnea in 5 cases. We have performed the following surgical procedures : incision and drainage, primary repair, gastrostomy for cervical esophageal perf'oration and primary repair, primary repair and pleural flap reinforcement, gastrostomy for thoracic esophageal perforation. A patient died of sepsis.

  • Various biceps tenodesis techniques being used, make it difficult to compare the result of reports. First, the biceps tenodesis could be classified according to being performed by open incision or by the arthroscopic procedure. Second, it could be classified as a soft tissue and bony tenodesis according to the tissue which the long head of biceps is fixed with. Third, it could be classified as a proximal and distal tenodesis according to the location which the long head of biceps is fixed with. Fourth, it could be classified according to the implant (interference screw, suture anchor, knotless suture anchor). A decision should be suspended until an appropriate strength of tenodesis is revealed.

Malignant Solitary Fibrous Tumor of the Pleura -one case report- (늑막의 악성 고립성 섬유성 종양 -1례 보고-)

  • 흉막의 고립성 섬유성 종양은 중피하에 존재하는 미분화 중배엽성 기원의 섬유 세포에서 기원하는 종양으로 알려졌다. 이 종양은 대부분 양성으로 보고되고 있으나 병리학적으로 높은 세포 밀도, 유사분열의 수가 많고(10개의 고배율 시야에서 4개 이상), 핵의 다양성, 출혈, 괴사등이 있으며 악성의 판단기준으로 보고된다. 환자는 62세 여자로 호흡곤란 및 기침을 주소로 본원에 입원하였으며 단순 흉부 방사선 및 전산화 단층 촬영상 우측 흉강내에 거대한 종양소견이 보였다. 이 종양은 23$\times$18$\times$12 cm, 2 kg의 크기와 무게를 가졌으며 우측폐 하엽과 중엽은 압박되어있었으나 종양 절개 후 재 팽창됨을 확인하였다. 또한 횡경막과 심하게 유착되어 박리도중 손상이 동반되어 봉합술이 필요하였다. 병리학적으로 종괴는 세포밀도가 높았고 방추형세포의 다발로 이루어져 있었으며 유사분열의 수가 많이(27 mitosis/10HPF)보였다. 면역 조직 화학 검사상 vimentin과 CD34에 양성 반응을 보였다. 이에 악성 고립성 섬유성 종양으로 확진되었다. 본 교실에서는 흉막에 발생한 악성 고립성 섬유성 종양을 치험하였기에 문헌 고찰과 함께 보고한다.
