Human endogenous retroviruses and neurologic disorders (인간 내인성 레트로 바이러스와 신경학적 장애)

  • Human endogenous retroviruses (HERVs) are fossil viruses that began to be assimilated into the human genome some 30~40 million years ago, and now constitute nearly 8% of the human genome. These ancient retroviruses have since accumulated mutations that have rendered them defective; thus, they have been termed junk DNA. However, recent research indicates that not all HERVs remain silent passengers. Although they have not been shown to be causative of any human disease, endogenous retroviral sequences may become expressed under select pathological circumstances such as neurological disorders, including multiple sclerosis (MS), schizophrenia, and Amyotrophic Lateral Sclerosis (ALS); viral infections, including human immunodeficiency virus (HIV) and herpesvirus; and multiple types of cancers. This review focused on the possible interactions of HERVs and neurological diseases.

  • Multiple sclerosis (MS) is a demyelinating disorder that affects discrete areas of the CNS, including the optic nerves, in a quite variable relapsing-remitting fashion over a prolonged period of time. Although MS is usually considered to be a disease that affects peoples in early to middle adulthood, children do develop multiple sclerosis. The frequency of MS onset before the age of 15 years is 2.7-5% of all cases, while MS onset during infancy and early childhood was observed to be 0.2-0.7% of all cases. We report here on a Korean case of a relapsing-remitting MS female child who was treated with four rounds of intravenous methylpredinsolone pulse therapy and preventive Interferon-$\beta$-1b ($Betaferon^{(R)}$).

  • Rhabdomyoma has been reported to be the most common type of cardiac tumors in fetuses and children, and multiple cardiac rhabdomyomas almost certainly signify the association with tuberous sclerosis. We report here on a case of Tetralogy of Fallot (ToF) that was associated with multiple rhabdomyomas and tuberous sclerosis. A two-year-old boy, who had undergone systemic-pulmonary shunt during the neonatal period, received total correction of his ToF after the complete regression of the cardiac tumor. The postoperative course was uneventful, and he has been follow-up for 2 months. The boy is currently in an excellent condition.

  • Primary tumors of the heart are uncommon among pediatric patients. Rhabdomyoma is the most frequent cardiac tumor in infants and children, which is commonly associated with tuberous sclerosis. Tuberous sclerosis is a neurocutaneous syndrome affecting the brain, heart, skin, and other organs. Cardiac rhadomyomas are reported in 50-64% of infants with tuberous sclerosis. Tuberous sclerosis involves multiple locations in the atrium, ventricle and septum, and may induce mechanical obstruction of the outflow tract and heart failure depending on the location, number, size, and degree of invasion of tumors. Arrhythmias may also develop in infants with cardiac rhadomyomas, but only a few of these patients require prolonged anti-arrhythmic therapy because arrhythmia often disappears with spontaneous regression of the tumors, and the ultimate prognosis may be decided by the cerebral manifestations.

  • Multiple sclerosis is an autoimmune demyelinating disorder of the nervous system. The ocular manifestation includes optic neuritis, internuclear opthalmoplegia and nystagmus. Upbeat nystagmus is a rare manifestation of multiple sclerosis. We report a patient with relapsing multiple sclerosis who presented with upbeat nystagmus from a circumscribed lesion in the caudal medulla.

  • Despite the government operating various preventive programs to ward off accidents and diseases on business sites, diseases rates are not decreasing, unlike accident rates. In many cases, diseases caused by work have a latent period before symptoms appear or progress over a longer term, making it difficult for workers to prove the causal relation between their work and the diseases. Moreover, data related to the business site are mostly owned by the employer. Even if the employee has access to parts of such data, his lack of medical expertise limits his ability to identify the characteristics of the diseases and how it appears. In August, 2017 the Supreme Court did an about-face with its ruling on the case involving diseases caused by exposure to harmful substances in work environments, by easing the burden of proof on the employees. As such, this study focuses on the case to analyze cases involving diseases that have occurred in work environments and present their implications. In doing so, the study seeks to provide a basic set of data that can help secure the employees' labor rights and rights to health by complementing the current law in relation to recognizing industrial incidents caused by rare diseases and making work environments safer for employees.

  • Cardiac rhabdomyoma is a rare type of benign tumor affecting the heart. There are a few previous reports of intracardiac rhabdomyomas causing ventricular arrythmia. We describe a 1-year-old female tuberous sclerosis patient who was presented with a ventricular tachycardia. Diagnostic echocardiography revealed two masses in the left ventricular outflow tract originating from the ventricular septum. The masses were surgically resected through aortotomy using cardiopulmonary bypass and the masses appeared benign. The pathology was that of a cardiac rhabdomyomas. Postoperative course was uneventful and the ventricular tachycardia was controlled.

LOCALIZED SCLERODERMA IN A CHILD : CASE REPORT (소아 피부 경화증 환자의 치험례)

  • Scleroderma is a connective tissue disease of unknown etiology, but known as a kind of auto-immune disease. It is most common in women especially in $30{\sim}50$ years, and very rare in childhood. It can be classified into two main classes : localized scleroderma & systemic sclerosis. Localized type has better prognosis, and usually involves skin only, or in some cases, the muscle below, except inner organs. Systemic type involves skin, oral mucosa and major internal organs. Involving facial skin, we can see small and sharp nose, expressionless stare and narrow oral aperture. Usually they have Raynaud's phenomenon, and in progress, show mouth opening limitation and sclerosis of tongue and gingiva. It is called CREST syndrome showing calcinosis cutis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangioectasia. Treatment of scleroderma is systemic and localized steroid therapy, use of collagen-link inhibitor (D-penicillamine), immune depressor and etc. Mouth opening limitation can be improved by mouth stretching exercise. We report a 6 years old boy, diagnosed with localized scleroderma who had mouth opening limitation. We could get additional mouth opening, and have done successful restorative treatment of mandibular and maxillary 1st and 2nd deciduous molar under deep sedation with nitrous oxide and enflurane.

Therapeutic Effect of Crocin in Inflammatory Diseases (염증성 질환에 대한 Crocin의 치료 효과)

  • Crocin is a major carotenoid of the Gardenia jasminoides fruit and Crocus sativus stigma (saffron), which are used in various cuisines as flavoring and coloring agents, as well as in phytomedicine for the treatment of several disorders, including headache, fever, edema, fatty liver, viral hepatitis, respiratory disease, menstruation disorders, insomnia, and hypertension. Crocin (C44H64O24) is a chemical diester composed of the dicarboxylic acid crocetin and disaccharide gentiobiose. Many in vitro and in vivo studies have been conducted about the biological and pharmacological function and toxicity of crocin. Crocin has been revealed to have no genotoxicity and pathological manifestation. Crocin acts as an antioxidant, anti-cancer, memory enhancer, anxiolytic, antidepressant, aphrodisiac, anti-atherosclerotic, cardioprotector, and hepatoprotector. Here, an inclusive review of crocin is introduced based on previously explored studies referred to in the literature. Different studies have confirmed the protective role of crocin in the pathogenesis of inflammatory diseases, including inflammatory bowel diseases, gastritis, asthma, atherosclerosis, rheumatoid arthritis, multiple sclerosis, type 1 diabetes, Alzheimer's disease, Parkinson's disease, and depression. It is surmised that crocin suppresses inflammatory, antioxidant, and apoptotic processes through multiple mechanisms. Crocin is considered a safe and effective therapeutic choice for patients with inflammatory conditions, although more research investigating its mechanisms and results acquired in clinical trials are needed.

  • Multiple sclerosis is a chronic, pregressive, demyelinating, disease of the central nervous system. It is named for the formation of disseminated scarlike lesions primarily in the central white mattrer of the brain and spinal cord. These plaques are commonly found in the regions of the optic tracts, third and fourth ventricles, basal ganglia, midbrain, pons, and spinal cord. Multiple sclerosis is an unpredictable disease, typically presenting with an exacerbating-remitting course, although other clinical courses have been recognized. Common clinical findings include disturbances in sensation, muscle strength, tone, fatigue, coordination, vision, communication, bladder and bowel function, and cognitive and behavioral function. Physical therapy of the patient with multiple sclerosis is centered around decreasing symptoms, improving function, prevention secondary complications, and promoting successful psychological adjustment. It requires the comprehensive efforts of a health care team to provide coordinated and continuing care.
