업종별 산업간호사의 의료행위 분석

  • 본 연구는 산업 간호사의 통상증상에 대한 투약 및 처치, 타의료기관에 의뢰내용, 응급조치 내용을 업종별로 비교 분석코저 한다. 연구방법은 산업간호사회에 등록된 산업장 중 서울, 인천, 마산, 창원, 부산, 울산지역에 있는 130개 산업장에 근무하는 산업간호사를 대상으로 설문조사하였다. 연구결과는 다음과 같다. 1) 신체기관별 통상증상에 대한 처치 및 투약은 서비스업과 제조업, 기타제조업 모두에서 소화기계, 호흡기계증상에 대한 처치 및 투약 업무가 가장 많았으며 업종별로 보면 서비스업의 경우 소화불량, 감기, 근육통, 설사, 기침, 두통, 복통, 구토, 변비 등이 80% 이상을 차지하였다. 제조업의 경우 소화불량, 감기, 설사, 두통, 근육통 등이 많았으며 기타제조업의 경우 감기, 두통, 소화불량, 근육통, 설사, 기침, 눈다락지 순으로 나타났다. 2) 근로자를 타의료기관에 의뢰 하는 내용은 즉각적인 수술을 요할때나 골절, 내부장기의 손상, 두뇌손상, 출혈 등으로 나타났다. 3) 업종별 응급조치내용은 서비스업에서는 화상과 쇼크, 출혈이 가장 많았으며 유해물질의 급성중독의 경우가 가장 적었다. 제조업에서는 화상, 골절, 쇼크, 출혈, 유해물질의 급성중독이, 기타제조업에서는 화상, 골절, 쇼크, 출혈의 순으로 많았다. 4) 업종별 구비관리하고 있는 의약품은 서비스업종이 제조업, 기타제조업보다 많은 의약품을 구비하고 있었으나 경미한 통상증상에 대한 의약품으로 나타났다. 이러한 연구결과들을 통해 볼때 제조업, 기타제조업, 서비스업종의 보건관리자가 일차보건의료수준에서의 투약 및 처치를 시행하고 있는 바, 이에 대한 구체적이고 명확한 투약의 범위를 설정하여 보건관리자의 직무를 표준화할 필요가 있으며 서비스업종에 종사하는 근로자의 건강문제와 관련요인을 구체적으로 연구할 것을 제안한다.

  • 본 연구는 와상환자에게 자주 발생하는 낙상, 욕창, 불면증을 영상기술을 통해 인식하여 섬망의 조기진단 모형을 구현한다. 실시간 모니터링을 통해 섬망 잠재환자를 선별하고 집중적인 관리와 치료로 이어질 수 있도록 간호인력을 보조하는 데 주된 목적을 두고 있다. 과활동형 섬망은 파생위험 중 하나인 낙상과, 저활동형 섬망은 원인 요소인 욕창과 묶어 자세인식을 통해 판정한다. 또한 주로 밤에 악화되는 섬망의 특성을 고려해 눈 깜빡임을 통한 불면증 검사를 추가로 반영하였다. 낙상과 욕창을 섬망과 묶어 융복합적인 위험예측 시스템을 구축함과 동시에, 기존의 섬망 사정도구들이 지니는 시공간적 제약을 개선함으로써 간호인력의 부담을 덜어줄 것으로 기대된다.

  • Purpose: This article is aimed at broadening the perspectives of nursing by applying Heidegger's philosophical reasoning and views to nursing beliefs. Methods: Heidegger's main concepts on a human's way of being were the source for examining the encounter between care-receiver and care-giver in nursing. In addition, the paper illustrates the attitudes that nurses must present to their care-receiver in existential nursing. Results: As nurses and care-receivers experience existential crises due to anxiety about death and fear over uncontrollable situations, they both raise questions about the significance of their existences. By putting their deep reflections on these questions into nursing practice, nurses can exist as "Mitdasein" and be open to a number of possibilities in nursing. Conclusion: Nurses must be open to a number of possibilities in nursing by embracing various experiences of life and individuality without criticism, and pursuing the existing lives of their counterparts as well as their owns. They are able to take this attitude by raising fundamental questions about life and the existence of human beings through ceaseless reflections on their experiences, then implementing the result of these reflections in their lives and nursing practices.

Development of an Instrument to Measure the Quality of Care through Patients' Eyes for Hospitalized Child (입원한 아동에게 제공되는 환자의 눈을 통한 간호의 질 측정도구 개발)

  • Purpose: The purpose of this study was to develop an instrument that evaluated QUality Of care Through patients' Eyes for hospitalized child (QUOTE-Child), and examine the validity and reliability. Methods: Preliminary items were reviewed through expert content validity and face validity. To test the validity and reliability of the instrument, the data were collected from 221 care givers of hospitalized children. Data were analyzed using exploratory factor analysis, Pearson correlation coefficient, Cronbach's alpha and Spearman-Brown coefficient. Results: Factor analysis yielded 19 items in four factors including 1) respect, 2) explanation, 3) kindness, and 4) skillfulness, with a cumulative explanatory variance of 70.68%. For criterion-related validity, a significant positive relationship was found between quality of care and pediatric family satisfaction. For internal consistency reliability, the Cronbach's ${\alpha}$ was .93 (importance) and .95 (performance) for the overall instrument. The half split reliabilities were .86, .95 (importance) and .92, .97 (performance). The Cronbach's ${\alpha}$ of 110 data was .92 (importance) and .94 (performance). Conclusion: Researchers and practitioners can use this instrument to systematically assess quality of care for hospitalized children and identify areas of support for hospitalized children and their family.

Knowledge, Perception & Practice of Eye Care Nursing among Nurses in Intensive Care Units (중환자실 간호사들의 눈 간호에 대한 지식, 인식 및 수행 실태)

  • Purpose: The purpose of this study was to identify levels of knowledge, perception, and practice of eye care as part of nursing care in the ICU. Method: A descriptive, cross-sectional study design was used. The participants in this study were 269 registered nurses working in the ICUs of five university hospitals in Seoul, Korea. Between November 2005 and December 2005, data were collected using a semi-structured questionnaire developed by the researchers, and analyzed using the SPSS Program. Results: In general, levels of knowledge of nursing care for the eyes were moderate among ICU nurses, but levels of perception of nursing care for the eyes were very high. Of the respondents, 61.7% reported that they would consult a doctor whenever they found an eye problem. Only 42.4% nurses answered that they provide eye care as part of the daily routine and 43.9% nurses responded that they cleanse the eye lids with wet saline gauze. In regression analysis, the practice of eye care as part of nursing was significantly influenced by perception of nursing care for the eyes, and knowledge of interventions for nursing care for the eyes. Conclusion: These results suggest that education on care of the eyes for ICU nurses and the development of a standardized eye care protocol should be done to improve quality of nursing care in the ICU.

Development of an Eye Cure Protocol for ICU Patients (중환자실 입원 환자의 눈 간호를 위한 근거기반 지침 개발)

  • Purpose: The purpose of this study was to develop an eye care protocol for intensive care unit (ICU) patients. Method: A systematic review was conducted to develop an eye care protocol for ICU patients. Searches were performed using computerized databases (CINAHL, MEDLINE, EBM Review) and citation search from 1996 to January 2007. For the keywords, "eye care", and "randomized controlled trial" were used to identify experimental studies regarding eye care for ICU patients. After reviewing the collected studies, a preliminary eye care protocol algorithm was created. Then, content validity was examined with ophthalmologists and ICU nurses. Results: Six studies were included to serve as a basis for framing of the preliminary algorithm. The final eye care protocol was completed after verifying the preliminary algorithm's content validity. The final eye care protocol was organized in the following manner: 3 items in the assessment stage, 7 items in the no-risk stage, 4 items in the low-risk stage, and 5 items in the high-risk stage. Conclusion: The results indicate that, for ICU patients, nurses can broaden their knowledge regarding ocular diseases, as well as improve their practice-based eye care nursing performance.

Clinical Practice Experience of Men in Nursing as a Student (남자 간호대학생의 임상실습의 경험)

  • The study was to understand the meaning and nature of clinical practice by examining the clinical experience of male nursing college students who have experienced clinical practice and to obtain basic data on how male nurses influence the career setting in clinical practice. to be. Participants in this study were male students who are currently in the Department of Nursing at H University, and three snowmen were selected as 3rd grade students and 3rd grade male students who had clinical practice, and analyzed meaningful statements according to Colaizzi data analysis To understand the meaning of students' clinical experience. The findings were divided into three categories, with six themes and 14 meaningful statements. The male nursing students experienced discrimination on the grounds of 'negative nursing image experienced in ward practice', 'the burden of self to be handled', and 'the charm of nursing found in the special part training experience'. Conflict and confusion over the profession were causing. However, among them, the professionalism of the nurses was confirmed and the pleasure of learning was found, and the results of this study can provide a holistic view of the clinical practice experience of male nursing students. In addition, it can be used as a basic data useful for counseling and guidance on academic adaptation and clinical practice adaptation of male nursing college students, and is expected to be useful for counseling career decision after graduation of male nursing students.

Path Analytic Modeling of Clinical Nurses' Nursing Performance: Focused on Positive Affective Events (임상간호사의 간호업무성과 모형의 경로분석: 긍정정서사건을 중심으로)

  • The aim of this study is to verify the relationships of factors affecting nursing performance of clinical nurses focused on positive affective events. The subjects of this study were 275 clinical nurses from secondary and tertiary general hospitals. Data collection was conducted for two months from May 2021 through an online survey through snowball sampling. Data analyzed was using SPSS 26.0 and AMOS 26.0. The variables affecting job satisfaction included direct effect of positive affective events (β=.65, p<.001), and direct effect of positive affectivity (β=.10, p=.038). Job satisfaction (β=.47, p<.001) had statistically significant direct effects on nursing performance. Positive affective events (β=.32, p=.003) and positive affectivity (β=.05, p=.039) had statistically significant indirect effects on nursing performance. These variables explained for 22% of nursing performance. However, emotional labor had no significant effect on job satisfaction and nursing performance. The results indicate that positive affective events and positive affectivity result in high degree of job satisfaction. Job satisfaction would increase the level of nursing performance. Therefore, in order to improve nursing performance, it is necessary to provide educational opportunities and interventions that can promote positive work environment and positive affectivity.

Dry Eyes and its Associated Factors in the elderly living in the community (지역사회 거주노인의 안구건조증 유병률과 관련요인)

  • This study aimed to identify the prevalence of dry eye, sleep disorder, depression and factors related to dry eye among the elderly living in the community. Study subjects were 149 elderly, 65 years of age or older from nearby people, acquaintances and community elderly-related centers that researchers could access. Data collection period was from June 1 to August 31, 2021. Data were analyzed by frequency, percentage, mean, standard deviation, ANOVA, χ2-test, and multiple logistic regression. The results showed that dry eyes in elderly people were statistically significant according to age (χ2=4.50, p=.034), occupation (χ2=7.14, p=.008), chronic diseases (χ2=6.59, p=.010), drugs (χ2=4.86, p=.027), eye surgery (χ2=6.02, p=.014), and sleep disorders (χ2=7.29, p=.007). Multiple logistic regression revealed that dry eyes were associated with sleep disorders (OR=2.45, 95% CI=1.17-5.13). Therefore, for the management and prevention of dry eyes in the elderly, there is a need for nursing intervention and strategy that can find the causes of sleep disorders and improve the quality of sleep.

Factors Associated with Senile Macular Degeneration in Elders within Communities (지역사회거주 노인의 황반변성 관련 요인)

  • Purpose: This study was conducted to examine the degree of senile macular degeneration in elders aged 65 or older and identify factors associated with senile macular degeneration in elders within communities. Methods: Participants in this cross-sectional descriptive study were 388 elders without cataract, glaucoma, and diabetic retinopathy. Data were collected through face to face interviews using a questionnaire in an urban area from June 2010 to December 2011. To detect senile macular degeneration, the participants' self-tests were carried out with Amsler grid testing. Stepwise multivariate logistic regression was performed to evaluate the effects of health behavior and knowledge about eye health on senile macular degeneration. Results: The proportion of participants with senile macular degeneration was 11.6%. Factors that significantly increased the risk of senile macular degeneration included eye discomfort and history of ophthalmic examination (p<.05). Factors that significantly decreased the risk of senile macular degeneration included regular change of magnifiers, non-use of magnifiers, and knowledge about preventive effects of non-smoking and antioxidant vitamin intake on senile macular degeneration (p<.05). Conclusion: This study showed that programs for preventing senile macular degeneration were necessary for elders within communities and the results of this study can be used for developing those programs.
