  • 현대인들은 다양한 전자기기 휴대폰, 컴퓨터, 전자레인지로부터 발생하는 전자파에 뇌를 노출시키고 있다. 전자기기의 전자파는 뇌에 중대한 손상을 가져올 수 있으며, 조기 노화를 초래할 수 있다고 경고하고 있다. 전자파의 실질적인 위험은 뇌가 열을 받는 것이 아니라 비열방사선이라고 널리는 강도가 낮은 방사선이라고 한다. 휴대폰, 컴퓨터의 모니터에서 발생되는 전자파에 장시간 노출로 인한 각종 전자파 유해가 생기고 여러 가지 신경 계통에 대한 장해가 증가하고 있다. 전자파가 인체에 미치는 영향에 대한 많은 연구가 이루어지고 있으나 서로 상반되는 연구결과가 많이 나타나고 있다.   • 뇌손상에 대한 원인설명으로써 소위 central theory를 들수 있다. 이 방법은 머리의 질량 중심에서 측정된 가속도를 이용한 HIC(Head Injury Criterion)의 값을 계산하여 이를 안전기준의 척도로 삼는 방법으로 이와 같은 해석에 따라 각 자동차 회사에서는 안전기구를 설계 제작하고 있다. 그러나 실제 임상적으로 HIC의 상관관계는 뚜렷하지 못하다. 이런 문제점을 해결할 수 있는 하나의 대안으로써 유한요소모델을 이용한 해석방법을 들 수가 있다. 이 글에서는 뇌의 간략한 해부학적인 해설과 아울러 뇌의 3차원 유한 요소 모델을 이용한 해석방법을 소개하고자 한다.

  • 포천중문의대 차병원 세포유전자연구소의 연구팀은 국내 처음으로 쥐의 배아 줄기세포를 살아있는 쥐의 뇌에 이식하여 손상된 뇌신경의 기능을 회복시킬 수 있는 뇌세포를 만드는데 성공했다. 이 연구팀은 쥐실험과 같은 연구과정을 사람에게 적용하는 실험을 하기 위해 오는 5월 '줄기세포치료이식센터'를 완공할 계획이다.

  • Diffusion tensor images were obtained for middle-aged men according to the AUDIT-K scale. As a result of the fractional anisotropy (FA) value of the gyrus nerve fibers in the brain region measured and analyzed by the Tract-Based Spatial Statics, FA values of general drinkers were measured lower than those of estimated alcohol use disabled people. This was statistically significant. In other words, the use of alcohol significantly affects the anatomic microstructural changes measured in the Gyrus of the brain region, and it may damage the nerve fiber tract and affect the functional abnormalities associated with it.

Cognitive-pragmatic Language Ability Assessment Protocol for Traumatic Brain Injury(CAPTBI): Reliability and Validity (외상성 뇌손상 환자의 인지-화용언어 능력 평가도구 개발을 위한 신뢰도 및 타당도 연구)

  • Traumatic brain injury(TBI) is a brain damage caused by an external physical force. TBI patients have disturbances of functioning including attention, memory, reasoning,, executive function, and pragmatic language. The aim of this study was to develop the cognitive-pragmatic language ability assessment protocol for traumatic brain injury(CAPTBI) and to evaluate reliability and validity. This study was also conducted to investigate domains that contributed to differentiate between the normal and TBI groups. The CAPTBI data were obtained from 226 normal adults and 62 TBI patients(mean age=$43.95{\pm}11.92$, $46.37{\pm}11.87$, M:F=110:116, 48:14). The CAPTBI had high item internal consistency, test-retest reliability, construct validity, and concurrent validity. The normal group performed significantly better than the TBI group in all domains of the CAPTBI and the separate scores for 9 domains. All 9 domains were found to be significant variables to discriminate between the two groups. The most powerful variable was executive function followed by memory, organization, pragmatic language, problem-solving, attention, orientation, reasoning, and visuoperception in order. The CAPTBI could discriminate between the two groups accurately by 95.5%. This result demonstrated that 97.3% of normal adults and 88.7% of TBI patients could be discriminated by CAPTBI. In conclusion, The CAPTBI is appropriate for evaluating and identifying cognitive-pragmatic language disorders in TBI patients.

Exploring the Use of Melody During RAS Gait Training for Adolescents with Traumatic Brain Injury: A Case Study (외상성 뇌손상 청소년 대상 리듬청각자극(RAS) 보행 훈련 시 선율 적용 사례)

  • The purpose of this study was to examine the effects of rhythmic auditory stimulation (RAS) on gait parameters, with and without the presence of a melody, for adolescents with traumatic brain injury (TBI). Three adolescents with TBI received a total of ten individual RAS training sessions. At pre and posttest, spatiotemporal parameters including cadence, velocity and kinematic parameters were measured using the VICON 370 Motion Analysis System. The results showed no significant difference in gait velocity between the two conditions, thus the presence of the melody condition did not impact the outcome of RAS gait training. On the other hand, all participants showed improvement in gait function after RAS training. The cadence, velocity, stride length, and symmetry were increased and the stride time was reduced after training. The motion analysis demonstrated that the movement patterns of hip and knee joints improved, as they were more similar to normal gait, which indicates that the walkings tance became more stable. The research findings indicate that rhythm is the primary factor in mediating gait functions via RAS training. This study also supports that RAS training can effectively improve the gait function for adolescents with TBI.

Effects of Virtual Reality-Based Activities of Daily Living Training on Activities of Daily Living and Rehabilitative Motivation in Patients With Traumatic Brain Injury: A Pilot Study (가상현실 기반의 일상생활활동 훈련이 외상성 뇌손상 환자의 일상생활활동 및 재활동기에 미치는 효과 : 예비연구)

  • Objective : The purpose of this study was to investigate the effects of virtual reality-based activities of daily living (ADL) training on ADL and rehabilitative motivation in patients with traumatic brain injury. Methods : This study was performed using a pre-post design with seven traumatically brain injured patients. Subjects were subjected to virtual reality-based ADL training for 30 minutes a day, 2 to 3 times a week for 4 weeks. Evaluation was conducted before and after the intervention using the Korean Modified Barthel Index (K-MBI), Cognitive Functional Independence Measure (C-FIM), and Volitional Questionnaire (VQ). Changes before and after intervention were analyzed by Wilcoxon signed-rank test, and correlations were analyzed using Spearman's coefficient. Results : After intervention, patients with traumatic brain injury showed significant improvements in K-MBI (p<.05). There was no significant change in total C-FIM score and VQ score (p>.05). Total C-FIM score correlated significantly with VQ score (p<.05, r=.755). The social cognition domain of C-FIM had a significant correlation with VQ score (p<.05, r=826). Conclusions : Virtual reality-based ADL training can improve ADL performance, but further research is needed to determine whether improvements in social cognition and rehabilitative motivation are possible.

Differential Response Style on the Personality Assessment Inventory according to Compensation-Seeking Status in Patients with Traumatic Brain Injury (외상성 뇌손상 환자에서 보상추구 여부에 따른 성격평가질문지 반응 양식의 차이)

  • Objectives : This study examined the characteristics and differences of PAI(Personality Assessment Inventory) profile between compensation-seeking(CS) and treatment-seeking(TS) patients with traumatic brain injury(TBI) and assessed the clinical meaning of the characteristics and differences of profiles between the two groups. Methods : 36 TBI patients who visited the Wonkwang University Hospital were selected. The patients were categorized as compensation-seeking TBI patients(n=22) and treatment-seeking TBI patients(n=14). The PAI scales and subscales were used to compare differences between two groups. t-verification for each variable and comparison analysis were performed. Results:In validity scales, CS group showed significantly higher NIM scores and lower PIM scores than TS groups. In full scales, CS group showed significantly higher SOM, ANX, ARD, DEP, and SCZ scores than TS group. In subscales, CS group showed significantly higher SOM-S, ANX-A, ARD-P, DEP(-C, A, P), (MAN-I), PAR-H, SCZ(-T, P), BOR(-A, N), and ANT-S scores than TS groups. In supplementary scales, CS group showed significantly higher SUI, NON and AGG-P, and lower RXR scores than TS group. Conclusions:There were significant differences in PAI scales with validity scales, some full and subscales according to compensation seeking status in TBI patients. The CS patients tended to exaggerate their symptoms on PAI, and showed higher scores representing somatic preoccupation and emotional distress. These results show the usefulness of PAI in reflecting the significant psychological differences between two groups.

Comparison of the Effects of Abdominal Massage and Lower Extremity Exercise using Survival Analysis in Rehabilitation Patients of Brain Injury (생존분석을 이용한 복부마사지와 하지관절운동의 뇌병변 재활환자 배변완화시기 효과 비교)

  • Constipation is the most common digestive disorder, and it occurs frequently in the early stages and affects the prognosis on rehabilitation stage in the patients with brain injury. The purpose of this study is to evaluate the bowel habits of brain-injured patients undergoing enteral nutrition immediately after admission to rehabilitation and transmission, and to investigate the effects of abdominal massage and lower extremity exercise. The study design is a quasi-experimental study to investigate the effect of a bowel care intervention (abdominal massage and lower extremity exercise) applicable to patients with brain lesions. As a result of the study, the number of bowel movements was significantly higher in the intervention group (p=.030), and the use of suppositories in the control group was statistically significantly higher than that in the intervention group (p=.004). The time of constipation relief was 1.73 days for the experimental group and 4.61 days for the control group, indicating that there was a difference in the time of constipation relief between the two groups (p<.001). Abdominal massage and lower extremity exercise were effective as nursing interventions to relieve constipation and prevent constipation in patients with brain injury from the early stage of rehabilitation.

  • This study investigated the role of excitatory amino acid systems in the initiation of organophosphate-induced seizures and brain damages in rats through quantitative in vivo microdialysis. Microdialysates were collected from the hippocampus of rat brain, treated with diisopropylfluorophosphate (DFP; 2.67 mg/kg, s.c.) alone, and/or atropine sulfate (15 mg/kg, i.m.) and procyclidine (30 mg/kg, i.m.). The protective effects of atropine, a muscarinic blocker, and/or procyclidine, a N-methyl-D-aspartate and cholinergic antagonist, against DFP were examined. DFP treatment increased the levels of aspartate (Asp) and glutamate (Glu) significantly in the hippocampal persuate with the induction of seizures. Treatment of procyclidine could effectively block the increase of Asp and Glu levels. Atropine treatment showed no significant anticonvulsive effects against DFP-induced seizures. The increases of Asp and Glu levels by DFP were also completely blocked through the combined treatment of atropine and procyclidine. Histopathological findings on the hippocampus confirmed the above results. More effective protection was observed through the treatments of procyclidine alone or of both procyclidine and atropine than atropine alone against DFP-induced brain damage. Procyclidine was shown to be effective in DFP-induced seizures.
