특이 체형 패턴 보정을 위한 기초 연구 -뇌병변 장애인을 중심으로- (A Basic Study of Pattern Alteration for People with Body Deformities -Focusing on the Brain Injured Handicapped-)

  • The feeling of self-confidence, the sense of well-being and of social acceptance that comes from the wearing clothes that is functional, appropriate, and attractive, is important to all the people, but to those with physical limitations, it is much more important. Finding well-fitting clothing can be a big challenge for them. This study focus on the cut of present available ready-to-wear garments and commercial patterns giving better fit and greater comfort and satisfaction. Subjects were limited to the people with body deformities caused by the brain injury. This can also meet the needs of silver groups in the aging society under the terms of human values and human rights. This study is based on the questionaires and interviews to identify the problems of ready-to-wear or commercial patterns, purchasing behaviors, and the demand for the functional clothes. This study focused on following objectives; 1) to discover what the problems are which confront the people with body deformity, 2) to find the problems in ready-to-wear garments or commercial patterns, 3) to provide information of pattern alteration f3r people with body deformities. A major limitation to the study is that the subjects numbers are limited and the distribution is not even. For the future study, large database with wide varieties of age and fitting test are suggested.

휠체어 여성 장애인의 체형 연구 (A Study on the Body Type of Wheelchair using disabled Women)

  • The purpose of this study is to analyze the physical characteristic by directly measuring the wheelchair using disabled women and to classify Body Type of a disabled person. The subjects were 103 disabled women of wheelchair used women and between 20 - 55 years of age The result of this study is as follow. 1. There was a remarkable difference in the physical characteristic of wheelchair using disabled women due to their cause of disability. The cause of disability was classified into the 4 groups; Poliomyelitis, Spiral Cord Injury, Muscular Dystrophy, Cerebral Palsy. 2. The result came out to be 4 factors by the factor analysis on the data of an anthropometric measurement to the wheelchair using disabled women and also cluster analyzing the factor scores, the experiment was concluded to 4 types. Type 1 came out to be a short height with the shortest legs. Type 2 was body shape which had a corpulent upper body and wide shoulder but with short and slim lower body. Type 3 has the shortest height, weak and lean upper body, small shoulders Type 4 has the largest stature and legs 3.   • In patients with cerebral palsy, dental treatments, which require absolute patient's corporation, have many difficulties because of muscle spacity, involuntary muscle movements and spasm. Especially in endodontic treatments which very meticulous instrumentation is needed, these muscle discordances may lead to unexpected accidents such as tissue damage while filing, over-instrumentation and swallowing instruments. Also, taking radiographs for measuring canal length is often hindered by walking and movement disorders. This paper is to present a clinical case of one-visit root canal treatment using electrical apex locator under general anesthesia in a cerebral palsy patient.

그룹홈 중고령 발달장애인의 생활만족도 영향요인 연구: 청년기 발달장애인과의 비교를 중심으로 (A Study on the Factors of Life Satisfaction of the Adults with Developmental Disabilities)

  • 본 연구는 그룹홈 거주 중고령 발달장애인의 생활만족도에 영향을 미치는 요인을 분석하고 있다. 이를 위해 서울시 그룹홈 종사자들의 대리응답조사와 발달장애인 당사자 대상으로 한 심층면접조사를 병행하였으며, 최종적으로 387명의 자료가 확보되었다. 분석결과에 따르면, 청년기 발달장애인들의 생활만족도는 직원 직무수행 수준이 높을수록, 지역사회활동 수준이 높을수록 높게 나타났다. 이에 비해 중고령 발달장애인들의 생활만족도는 활동피로감 수준이 낮을수록, 이용자 선택 수준이 높을수록, 직원 직무수행 수준이 높을수록, 사회활동 참여수준이 높을수록 높게 나타났다. 그룹홈의 직원직무수행은 청년기와 중고령기에 공통적으로 유의하지만, 활동피로감, 이용자 선택, 사회활동 참여 등은 중고령기에만 유의한 것으로 정리된다. 이상의 결과는 그룹홈 중고령 발달장애인의 생활만족도 증진을 위해서는 다른 연령층에 대비되는 이들의 차별적 욕구를 고려한 특화 건강지원, 사회활동지원, 서비스 공급노력이 요구된다는 점을 보여준다. 이는 그룹홈 발달장애인 당사자의 견해에 기초한 생활만족도를 증진하려는 노력과 그 맥을 같이한다. 본 연구는 향후 조기노화의 위험이 높은 다운증후군장애인, 뇌성마비 및 지적 중복장애인 등을 대상으로 이들의 차별적 욕구와 현황을 살피려는 연구로 확장되기를 제언한다.

시각장애청소년을 위한 3D 프린팅 촉각수학교재 모델 개발 연구 - 함수 지도와 관련하여 - (A Study on 3D Printed Tactile mathematics textbook for Visually Impaired Students)

  • 시각장애를 극복하고 주어진 환경에서 찾을 수 있는 장점을 활용하여 학문적인 중요한 기여를 해온 수학자들의 사례에 대한 연구 결과가 최근에 소개되었다. 이 연구를 통해 한국에서 소아마비나 뇌성마비 등의 신체적 장애를 극복하고 수학자로 성장한 예는 확인할 수 있었으나 시각장애인이 수학을 전공하여 수학 교사나 교수 또는 수학과 관련된 전문직에 진출한 경우가 전무하다는 것을 확인할 수 있다. 따라서 연구자는 선진국과 달리 한국에서 시각장애인 학생들이 수학을 전공하여 전문직에 진출할 수 없었던 이유에 대하여 생각해 보고, 이런 상황을 극복하기 위하여 시각장애청소년들이 좀 더 직관적으로 수학에 접근할 수 있는 방안을 연구한다. 그리고 본 연구에서는 시각장애인 학생들이 수학을 만지고 느끼면서 수학에 대한 관심과 실제 학습 내용의 직관적 이해를 돕는 하나의 도구로 3D 프린팅을 활용한 촉각수학교재와 교사용지도서 개발(제작 보급)을 위한 모델을 제안 한다. 이는 시각장애청소년 특수교사 부모의 교재활용을 수학교육 측면에서 지원함으로써 시각장애청소년의 수학에 대한 학습력을 신장시키고, 수학에 대한 자신감을 향상시키며, 수학을 이미지화하여 상상하는 수학교육 환경을 도모 할 것으로 본다. 특히 수학적 재능을 갖는 시각장애청소년들이 앞으로 수학을 전공하여 수학 관련 전문직으로 진출하는 모델을 만드는데 도움이 될 것으로 여겨진다.

  • In treatment of dentigerous cyst, complete enucleation, histopathologic examination and postoperative care are important to prevent the potential complications (mural ameloblastoma, squamous cell carcinoma). On the other hand, a maxillary impacted supernumerary anterior tooth are removed surgically, owing to the possibility of the cyst formation in future. After the cyst enucleation and extraction of the involved tooth, the wound area sutured and removable resin plate is then applied. In this operation, the postoperative bleeding and infection is likely to occur owing to postoperative accumulation of hematoma & seroma, psychologic stress and other contaminated factor. So, the authors established the immediate rubber & iodoform gauze drainage into the sutured wound of cyst enucleation & tooth extraction for the prevention of postoperative bleeding and infection. The removable resin splint are not used because of the poor cooperation and economic factor. The results were more favorable without the postoperative bleeding & wound infection in a cerebral palsy patient.

뇌성마비 장애인의 구강보건 실태 (THE ORAL HEALTH STATUS OF DISABLED PEOPLE WITH CEREBRAL PALSY)

  • The objective of this study was to assess oral health status such as prevalence of dental caries, gingivitis and other combined disorder, orofacial dysfunction in patients with cerebral palsy(CP), as well as the state of tooth brushing, possibility of dental treatment. Sixty-six subjects with CP aged 9 to 37 years were included in the study. Clinical caries status(DMFT index) and other data were evaluated and statistically analyzed using SPSS program (SPSS 17.0). The results were as follows: The DMFT index and prevalence of gingivitis of study subjects were 2.82(male), 3.33(female) and 89%, respectively. DMFT index classified into four groups according to age as follows: DMFT index were 1.14(ages 6-11), 1.40(ages 12-14), 2.16(ages 15-24), 4.15(ages 25-37). In addition to the physical disorder, speech difficulty(86%), epilepsy(35%) and visual impairment(14%) were associated and the epilepsy medication was the most common medication. And orofacial dysfunction such as the eating difficulties(79%), drooling(36%), swallowing disorder(30%), breathing difficulty(15%), bruxism(30%) and snoring(33%) was shown. Most people with cerebral palsy can't brush alone and didn't use oral care adjunctive supplies. Moreover, Fifty-nine percent of them were noncooperative to dental treatment.

뇌성마비환자의 전신마취 하 치과치료 후 혀 깨물기 손상 관리 (THE MANAGEMENT OF TONGUE BITE IN A PATIENT OF CEREBRAL PALSY AFTER DENTAL TREATMENT UNDER GENERAL ANESTHESIA -CASE REPORTT-)

  • Trauma to the lips and tongue can occur by accidental self-biting after dental treatment. After local anesthesia, it is likely that the patient may feel painless even in biting the tongue. In case of young children and disabled patients, the dentists should be careful not to bite the tongue. In this report, we present a case of deep lingual laceration due to biting the tongue in the course of dental treatment under general anesthesia. A 33 year-old male was transferred to our hospital to treat tongue laceration. Before 2 hour on arrival, he had received dental care under general anesthesia at a dental hospital for the disabled because of cooperation difficulty and cerebral palsy. During recovery from general anesthesia, he tried to bite his own tongue involuntary. The doctors and nurses tried to prevent the patient from being injured. Despite these efforts, massive bleeding occurred from the injured sites of the tongue. Because we could not communicate with him, we decided to evaluate the extent of the injury and treat the injured sites under general anesthesia. The laceration wound was sutured for nearly 1 hr general anesthesia. During recovery we inserted mouth prop into the oral cavity to prevent further injuries from tongue biting. After full recovery from general anesthesia he didn't try to bite his tongue. After 4 hour admission, he was discharged without other complications.

  • A 33-years-old female pateint with cerebral palsy showing spastic quardriplegia and severe mental retardation was scheduled for dental implant restorations. Before implant surgery we had to take implant CT. But, because of her involuntary motion and communication difficulty, sedation was needed in order to take CT. After 8 hour NPO, propofol infusion sedation with TCI (target controlled infusion) system was administered. The propofol blood concentration of the patient was maintained 2-3 ${\mu}$/ml to keep deep sedation to prevent uncontolled movement. During sedation, we monitored ECG, pulse oximetry, blood pressure, capnometry for patient safety. Oxygen was administered via nasal prong for preventing hypoxemia and to keep airway during sedation some bands were applied to lift mandible. Total duration was 20 minutes for taking CT, and she was discharged from hospital after 30 minute rest without complication.

국내 뇌성마비 환자에서의 장애인보조기구 지원현황 (Current Situation of Assistive Devices and Appliances Provision for Persons with Cerebral Palsy in Korea)

  • Background: The aim of this study was to investigate the current state of the assistive devices and appliances provision system for cerebral palsy patients registered with brain disability. Methods: From 2003 to 2013, we analyzed the records of cerebral palsy patients who had assistive devices and appliances provisioned at least once in their lives based on National Health Information Database. Patients with cerebral palsy were divided into three groups: infants and toddlers, school age and adolescence, and adults. Results: Data on short leg plastic orthoses, ankle joint orthoses, and wheelchair were mainly analyzed. The types of ankle joint orthoses divided into three categories: limited, $90^{\circ}$ limited, and Klenzac. Limited ankle joint orthoses was most frequently supported of the three in all age groups. Powered wheelchair and scooter were most supported to adult patients. When the re-supply duration was evaluated, the duration was suitable to the duration on guideline of regulation of re-supplement according to the related laws in adult patients but not in infants/toddlers and school age/adolescence as the actual re-supplement duration was much shorter than the reference value. Conclusion: This study confirmed the pattern of assistive devices and appliances supply differed depending on the age of cerebral palsy patients.
