Elderly Suicide and It's Related Factors : Focused on the Role of Social Support and Mastery in the Effects of Hopelessness and Depression on Suicidal Ideation (노인 자살관련 요인: 무망감과 우울증이 자살생각에 미치는 영향에 대한 사회적 지지와 자아통제감의 효과를 중심으로)

  • This study examined the role of social support and mastery in the effects of hopelessness and depression on suicidal ideation in order to identify elders' suicide and it's related factors. Especially, it focused to find out whether the hopelessness excluding depression has an independent effect on elders' suicidal ideation, and psychosocial factors such as social support and mastery has an interaction effects with elders' hopelessness variables and depression on suicidal ideation. This study recruited 297 elders over 65(older persons in the elder education programs of senior welfare center in Masan, Changwon and Jinhae) and employed a self-administered survey method during January, 2007. The following are the major results of the study. First, the level of hopelessness, depression and suicidal ideation related elders' suicide indicated differences according to their educational level, marital state, income, and their chronical disease numbers. Especially, the lower income and higher chronical disease numbers showed higher level of hopelessness, depression and suicidal ideation among the elder subjects. Second, interaction effects of social support and depression were significant in suicidal ideation, the more level of social support in elder' subjects, the lower the suicidal ideation. While, interaction effects of social support and hopelessness, and mastery and depression/hopelessness in suicidal ideation were not statistically significant. Third, psychosocial factors such as social support and mastery on elders' suicidal ideation have a significant effect when other relevant variables are controlled. From these results, hopelessness is other significant factor predicting suicidal ideation(suicide) among elders. Also, social support and mastery is very significant factor influencing to prevent or decrease suicidal ideation among elders.

The Mediating Effects of Family Solidarity on Depression and Suicidal Ideation of Older Adults in Korea (노인의 우울이 자살생각에 미치는 영향에 대한 가족연대감의 매개효과)

  • This study, we analyzed the following in order to verify the effects of depression on suicidal ideation. First, correlations among depression, family solidarity and suicide variables were analysed. Second, the hierarchical regression analysis was conducted to examine the effects of suicidal ideation. Third, family solidarity Sobel-test was performed to investigate whether family solidarity mediates the link between depression and suicidal ideation. The data used in the study comes from the survey conducted by Hallym Aging Research Institute in 2014. Through stratified multi-stage random sampling, 2,034 individuals aged 65 and over and living in Chun-cheon city were analysed. The result with depression and family solidarity together shows the higher the level of depression is, the more suicide ideation is. which means depression has a direct impact on suicide ideation. In addition, it indicates the higher family solidarity is, the less suicide ideation is. Sobel-test which examines indirect effects of depression on suicidal ideation through family solidarity produces a statistically meaningful result. The result is family solidarity mediates the link between depression and suicidal ideation.

Conjunctive Predictors of Suicidal Ideation in Korean Elderly : Panel Data Analysis (한국 노인의 자살생각에 관한 융복합 예측요인 : 패널자료분석)

  • In 2016, the number of suicides per 100,000 population in Korea was 24.6, which is the highest record of OECD countries. The number of suicide deaths increased with age. Elderly people have a higher risk of completed suicide than any other age group. The purpose of this study was to analyze the panel data of the Korean Welfare Panel Study to identify the factors affecting the suicide of Korean older people. This study analyzed the data of the 11th Korean Welfare Panel Study, which was constructed in 2016. The mean age of the participants was 75.55 years and 37% were man and 63% were women. The annual prevalence of suicidal ideation was 3.4%. The effects of depression(Exp(B)=1.113) and subjective health status((Exp(B)=.767) on suicidal ideation was statistically significant by stepwise logistic regression analysis(Nagelkerke $R^2=.248$). Therefore, for the effective application of suicide prevention program for the older people, assessment of subjective health status and depression screening should be preceded.

A study on predictive factors of Elderly Suicidal Ideation (노인의 자살생각 예측요인)

  • This study describes predictive factors for elderly suicidal ideation by analyzing the survey data from 359 people who were over 65 years old in Daegu, Korea. The mean score of social support(3.35), self-esteem(3.38), depression(8.64) and suicidal ideation(1.65) of elderly residence. Social support, self-esteem and suicidal ideation had negative correlation. Depression and suicidal ideation had positive correlation. Suicidal ideation was significantly different according to age, education, spouse, living arrangement, type of medical guarantee, pocket money, past job, leisure(time), health status, number of chronic disease, smoking. Others support, self-esteem, pocket money, education, age were influencing factors of suicidal ideation(48.2%). Among them, 41.8% of others support was.

  • 본 연구의 목적은 후기 고령노인의 자살시도 경험의 의미와 본질을 탐구하여 이들의 경험에 대해 보다 심층적으로 이해하는 데에 있다. 이를 위해 서울지역 ${\bigcirc}{\bigcirc}$구에 거주하는 자살시도를 경험한 75세 이상 노인 6명을 연구참여자로 의도표집하여 심층면접을 실시하였다. 수집한 자료를 현상학적 연구방법을 적용하여 분석하였다. Giorgi가 제시한 4단계 분석 절차에 따라 자료를 분석한 결과 6개의 구성요소와 20개의 하위 구성요소가 도출되었다. 연구를 통해 후기 고령노인의 자살시도 경험의 4가지 구성요소는 <모진 세월의 회한; 죽도록 고생한 모진 인생, 조실부모와 가난의 대물림, 도무지 잊을 수 없는 자식들의 외면, 가족갈등에서 온 깊은 원망>, <절망적이며 처량한 신세; 경제력을 상실과 절망, 마음대로 죽을 수 없는 처절함, 차라리 죽고 싶은 죄책감>, <쓸모없는 무가치한 삶; 아무 쓸모없는 사람, 자녀들 및 신세지기 싫은 강박관념에 괴로움, 자살충동이 일어남>, <참을 수 없는 신체의 고통과 무기력: 죽음과 삶의 두려움, 우울증, 정신병, 치매, 무기력>, <의지할 곳 없는 외로움; 하루 종일 고립됨, 숨쉬고 있지만 마지 못해 살아감, 죽은 것처럼 살고 있는 나, 하루가 지루함>,<마지막 선택; 세상이 싫어 떠날 것임, 먼저 간 배우자 따라.., 아무것도 바라지 않음>으로 나타났다. 이러한 연구결과를 바탕으로 후기 고령노인의 자살시도 경험에 대한 의미와 본질을 논의하고 노인자살예방을 위한 실천 현장의 구체적 개입전략, 생명존중문화의 실질적인 대처 방안에 대한 제언을 제시하였다.

A Comparative Study on Elderly Suicide Rates in Korea and the United States and Its Implications (한·미 노인자살률의 비교 연구와 정책적 시사점)

  • Any comparative study has not been done between Korea and a certain country about the social economic factors of elderly suicide rates, even though there has been comparative studies among several nations. Therefore, this study is going to reveal how the macro-social economic factors are related to elderly suicide rates based on Korea and the United States, which show great gap of elderly suicide rate and have completely different aspect of increasing and decreasing rate of elderly suicide, as members of OECD. For this, trend analysis has been done between elderly suicide rates of both Korea and the United States for the past twenty years(1990-2009) and social economic factors, and correlation analysis also has been done at the same time. After all, this study is going to suggest policy implications for the prevention of elderly suicide in our society based on the results of the analysis.

Suicide among the Elderly in Korea: A Meta-Analysis (노인의 자살생각에 영향을 미치는 요인군에 대한 메타분석)

  • Elderly suicide is a major public health issue in South Korea. The aim of this study was to systematically examine the current knowledge about suicidal ideation among Korean older adults with specific focus on risk and preventive factors. In order to achieve this aim, a meta-analysis was conducted using Korean academic peer-reviewed journals published since 2001. A total of 97 articles were selected that met the research criteria (e.g., original study findings and key words of suicidal ideation, suicidal thought, and suicide). Three domains were identified and used for further analysis: individual, family, and society. Results showed that among individual factors, depression and burden or stress increased the risk of suicidal ideation, while better mental health reduced the risk of suicidal ideation. Among family factors, living alone was a risk factor for suicidal ideation, while family cohesion was a preventive factor for suicidal ideation. Among social factors, elderly discrimination, social isolation, and negative relationships were significant risk factors, while social support, social environment, and social activities were significant preventive factors affecting suicidal ideation. The results suggest several practical implications for developing suicide prevention programs and counseling approaches to address suicidal ideation. For example, depression and stress can be reduced by MBSR (Mindfulness-Based Stress Reduction) program where meditation is used as a coping strategy. In addition, counseling programs specifically focused on improving family and social relations should be implemented. Government should continuously support for these programs to prevent suicide among older Koreans.

Convergence Study on the Related Risk Factors of Elderly's Suicidal Ideation Based on the 6th Korea National Health and Nutrition Examination Survey (제6기 1차년도 (2013) 국민건강영양조사로 추정한 노인의 자살생각 관련 요인에 관한 융합연구)

  • The study was conducted to identify related risk factor of elderly's suicidal ideation. This study used a cross-sectional design with secondary analysis of the $6^{th}$ 2013 Korea National Health and Nutrition Examination Survey (KNHANES). Data for 1,201 elderlies who aged over 65 years old from the KNHANES were included. Suicidal ideation, general characteristics, physical health, psychological health, EuroQoL, and health behavior were measured. Chi-square test, t-test, and multiple logistic regression were used to analyze this data through the SPSS win 22.0 program. In the results, 7.7% of the elderly had suicidal ideation. The odds ratios of suicidal ideation were significantly higher among those with restriction of activity (OR=1.31, CI=1.40~3.82), those with depression (OR=4.44, CI=1.66~7.40), and those with high levels of stress (OR=9.24, CI=3.48~24.5). Therefore, the activity level, depression level, and stress level need to be regularly assessed in order to prevent suicidal ideation among the elderly.

The Effects of Alcohol Use, Depression, and Self-esteem on Suicidal Ideation of the Aged (노인의 알코올 사용, 우울 및 자존감이 자살생각에 미치는 영향)

  • The purpose of this study was to examine the effects of alcohol use, depression, and self-esteem on suicidal ideation of older adults. Community-dwelling older adults aged 65 years and older were recruited from three welfare centers. A total of 245 older adults was assessed with the Alcohol Use Disorder Identification Test-Korean (AUDIT-K), Geriatric Depression Scale Short Form-Korea Version (GDSSF-K), Self-Esteem Scale (SES), and Scale for Suicidal Ideation (SSI). Descriptive statistics, Pearson's correlation, independent t-test, one-way ANOVA, and multiple regression analysis were performed using the SPSS WIN 19.0 program. Higher depression, lower self-esteem, no religion, and poorly perceived health were statistically significant determinants of higher level of suicidal ideation in older adults. To prevent or reduce suicidal ideation of older adults, health care providers should consider the determinants identified in this study.

The Effect of Past Acceptance on Suicidal Ideation in Old Age: Verification of Double Mediation of Attitude to aging and Depression (노인의 과거수용과 자살생각의 관계: 노화태도와 우울의 이중매개효과)

  • This study examined the effect of past acceptance on suicidal ideation in older adults and verified double mediation effects of attitude to aging and depression. The responses of 329 aged 65 years or older who participate in senior welfare centers in Seoul and Gyeonggi-do were used. As an analysis method, the double mediation effect was confirmed through serial multiple mediation model verification using the SPSS PROCESS macro program. The followings are the results of the study. First, the past acceptance had a significant effect on the aging attitude, the aging attitude on the depression, and the depression on the suicidal ideation, but the direct effect of past acceptance on the suicidal ideation was not significant. Second, double mediation effects of attitude to aging and depression were significant in the relationship between past acceptance and suicidal ideation. Based on these results, practical and policy interventions to prevent suicide in old age were suggested.
