The fabrication of abutment crowns for existing removable partial denture using CAD/CAM: A clinical report (CAD/CAM을 이용하여 기존 국소의치에 맞는 지대치 보철물 제작 증례)

  • Abutment teeth supporting removable partial denture could be faced a number of problems including development of dental caries. If the existing removable partial denture is in clinically acceptable state and the patient does not want to replace the existing removable partial denture, then a new prosthesis for abutment teeth need to be made. The procedure of fabricating a new prosthesis of abutment teeth for existing removable partial denture is complicate and technically challenging. To fabricate the abutment crown, the original cast of patient obtained before any complication to the abutment teeth is required. The original cast should also contain teeth other than the abutment teeth as a reference point. Once the cast is prepared, CAD/CAM could be used to produce retrofitting prosthesis effortlessly and efficiently. This clinical report presents fabricating a crown to fit existing removable partial denture using CAD/CAM for a patient with post and core failure and dislodged prosthesis. The prosthesis had high stability with minimum adjustment yielding satisfying result.

  • In this case, a CAD-CAM complete denture that can be easily remanufactured and reduced the number of visits was decided in consideration of the fact that it was difficult to visit the dental hospital and many existing dentures were lost because the patient was inpatient in a nursing hospital. In order to reflect the shape, vertical dimension, and maxillomandibular relationship of the existing provisional dentures adapted by the patient to the fabrication of the final denture, the existing provisional dentures were scanned and closed mouth impression was taken with the printed individual tray. After digital facebow transfer using UTS CAD and arranging artificial teeth in CAD software, the maxillary and mandibular complete dentures were fabricated by milling the denture base and artificial teeth at once with the monolithic disc.

  • In this report, Replicated denture was fabricated by 3d printing after scanning the existing denture. It was used as an individual tray in closed-mouth technique. It reduced laboratory process and clinical time. Also it helped in facial evaluation and tooth arrangement. Compared to open-mouth technique, the number of visits was reduced, and the patient's adaptability to new denture was enhanced by utilized various information of the existing denture, including vertical dimension.

  • In the case of complete edentulous patients, as the mandibular alveolar bone absorption progresses, the maintenance and stability of the existing dentures, which had satisfactory functions in the past, are deteriorated. Despite of the need to fabricate new dentures, they often hesitate due to physical burdens on the duration and intensity of future treatment progress due to the effects of aging and systemic diseases. In the case of these completely edentulous patients, it is necessary to consider the treatment goals that can reduce the number of visits and the adaptation period for new dentures before starting the treatment. This case is a case of producing complete dentures of elderly patients with deteriorated physical ability. In addition to producing suction dentures through preliminary and definitive closed mouth functional impression suggested by Sato, CAD / CAM technique was used to transfer occlusal functional information of existing dentures to facilitate adaptation to new dentures.

  • It often happens that a removable partial denture needs to be repaired due to tissue changes in the remaining alveolar ridge, fracture of the denture, or fracture of the abutment tooth. There are several advantages to retrofitting a customized surveyed crown under the existing RPD. Retrofitting a crown to the RPD decreases the economic burden to the patient and avoids the need for several appointments to fabricate a new RPD. It is difficult for artificial teeth used to repair dentures due to fractured natural teeth to have a shape similar to that of natural teeth, and to repair aesthetic artificial teeth, it is necessary to manufacture customized artificial teeth similar to the shape of each patient's teeth. Recently, CAD/CAM technology has been used to fabricate customized prosthetics on existing RPD to achieve high retention and fitness accuracy, and by manufacturing customized artificial teeth, more aesthetic and harmonious artificial tooth repair is possible. This is a case in which a denture was repaired using a digital method to fabricate a customized prosthesis on an existing partial denture and customized artificial teeth that mirrored the adjacent dentition, saving time and cost, simplifying the process, and achieving aesthetically and functionally satisfactory results.

  • Removable partial denture wearers are exposed to the risks that remaining teeth get damaged by caries, attritions, erosion, and fracture. In the case of damaged abutment tooth which should fit to Removable partial denture (RPD), the fabrication of surveyed crown is followed by the making of RPD. However, making new denture takes a long time, and needs several processes and costs. Also, patients should get used to new denture. If other abutment teeth and edentulous ridges provide the existing denture with support, retention, and stability, use of existing denture is considered clinically acceptable. In this situation, fabricating retrofit crowns to an existing removable partial denture makes patient use existing denture, cuts costs, and reduces discomfort. In this case, severely worn teeth were restored using monolithic zirconia crown which fit to an existing removable partial denture by CAD-CAM. Moreover, support, retention, and stability of the denture were improved, and both doctor and patient were satisfied with the result.

  • Statement of problem: As the number of elders is growing with the advancement of medicine, partially or fully edentulous patients have increased. Medically compromised conditions are common in the older population so that it should be taken into account in prosthetic treatment planning as well as their economic conditions. In the older patients, removable prosthesis has been preferred to implant prosthesis. However, cast metal based removable partial dentures also has several limitations. Purpose: In this report, we present several cases of Valplast$^{(R)}$ flexible denture which were fabricated in patients who had medically compromised conditions or whose remaining teeth showed a relatively poor prognosis. Results & Conclusion: This article describes an alternative treatment for a partially edentulous patient with mouth opening limitation, after cancer surgery, compromised general condition and questionable remaining teeth. In these patients, Valplast$^{(R)}$ flexible denture was used because of its unique characteristics and the results were all satisfactory. Patients had 1-2 check-up and there were no postoperative pain or fracture of denture up to now.

  • Loss of multiple teeth by multiple dental caries, traumatic influence or hereditable disease can cause many problem in children. Using removable appliance can be used for treatment of this problem. But conventional removable appliance has limits of retention by wire-clasp, non esthetic appearance, difficulty in pronunciation by its volume. Flexible denture using $Valplast^{(R)}$ system could be a excellent partial denture for restorarion in child because of various advantage such as retention from soft tissue, natural esthetic aspect, biocompatibility, durability to compare with conventional acrylic appliance. The present case report describe properties and consideration of flexible denture using Valplast $system^{(R)}$ in comparison with conventional acrylic appliance and report successful restoration of child using Valplast $system^{(R)}$.

Clinical considerations for complete denture relining (의치상 첨상재의 종류와 임상적 고려사항)

  • The residual alveolar ridge below the denture base undergoes physiologic changes over time, which results in the existing dentures becoming less accurate with the residual alveolar ridge. In addition, changes of the occlusal plane, decreasing in vertical dimension and loss of denture retention and facial support can occur. Consequently, denture relining may be required to accommodate these changes and ensure an ongoing close fit. Relining a denture can be performed directly on the chairside using autopolymerizing relining materials or indirectly in the laboratory using heat-cured relining materials. A direct relining method is not only simple but also time and cost effective. However, irritation or burning sensation of the mucosa can occur, and poor bonding of the relining material to the denture base can be cited as disadvantages. The indirect relining method exhibits relatively high bonding strength between the relining material and the denture base, but the patient might experience discomfort during relining process period. This report will examine the characteristics of relining materials, including those used in the relining of CAD-CAM dentures, and explore the clinical considerations for relining procedures.
