ICF Tool applied Intervention Strategy and Charting on Upper Extremity Functional Recovery of the Frozen Shoulder Patient - Case Study (동결견 환자의 상지 기능 회복에 대한 ICF Tool을 적용한 중재 전략과 문서 기록-사례연구)

  • Background: This study was to suggest the process of making strategy for effective intervention and evaluation on functional problems of the frozen shoulder patient applied International Classification of Functioning Disability and Health (ICF) Tools. Methods: The patient was 48years old woman with right shoulder adhesive capsulitis and calcific tendinitis. In first phase, therapists could list the information relating to functional problems used by the ICF Core set and then could confirm the interaction among the problems using the ICF assessment sheet. In second phase, therapist is needed to make the hypothesis and hypothesis testing and then set a primary functional goals and therapeutic goals in detail after prioritizing the problems to be managed based on the problem list. Lastly, after setting the confirmed problems as the purpose of intervention through the hypothesis testing, therapist could do some intervention after making a plan to solve these problems. Results: This report illustrates how to apply the process based on ICF concept into physical therapy practice. The decided hypothesis and goal that are to solve the problems the client faced were remarkably meaningful. Conclusions: Clinical decision making for the most effective intervention requires that therapists use the clinical reasoning process based on ICF concept.

Cold hardiness of Spodoptera litura Fabricius (Lepidoptera: Noctuidae) (담배거세미나방(Spodoptera litura Fabricius)의 내한성)

  • Supercooling points (SCP) and cold tolerance of the tobacco cutworm, Spodol~rera litura Fabricius. were measured in response to subzero temperatures. SCPs were varied among developmental stages. Eggs showed the lowest SCP (-27$^{\circ}$C). Pupae and adults had the intermediate SCP(- 18$^{\circ}$C). The SCPs (- I0 to - 16$^{\circ}$C) of larvae increased with their ages. Lethal low temperature of each stage was higher than its SCP. Preexposure of eggs and larvae to a sublethal low temperature increased their survival capacities under lethal low temperatures. The sublethal temperature also induced the fifth instar larvae to increase hemolymph osmolality and to produce cold-induced proteins of apparent molecular weights of 20 and 27 kD. These results indicate that this species is classified into a freeze-susceptible insect.

Autoradiographic Verification of Transdermal Penetration of Oleic Acid-conjugated Peptide Nanosomes (자가방사법에 의한 올레산이 결합된 펩타이드의 피부침투 확인)

  • Short peptides are potentially effective materials as cosmeceuticals, but their delivery across the skin can be problematic due to the ionic nature of peptides and the structure of the skin. For short peptide to be utilized as cosmeceuticals, its ability to penetrate the skin must be altered. In this study, we conjugated the widely used procollagen type I signal peptide, KTTKS, with oleic acid to improve the lipophilic properties of the peptide, and used the oleic acid-conjugated peptides to construct cosmeceutical nanosomes. Then we examined the penetration of cosmeceutical nanosomes prepared from isotope-labeled peptide into the skin after transdermal application using autoradiography. Because of its hydrophilic property of penta-peptide, the penta-peptide itself was not able to be penetrated through the stratum corneum of the skin. In contrast, nanosomes made from olecic acid conjugated penta-peptide were able to be penetrated through the stratum corneum effectively. Autoradiography showed the precise penetration points to dermal layer, demonstrating the appropriateness of this method for clarifying the mechanism of penetration of transdermal delivery systems.

Interrelationship of Matrix Metalloproteinase and $TNF-{\alpha}$ in Human Gingiva with Chronic Periodontitis associated to Type 2 Diabetes Mellitus (단순 만성 치주염 환자 및 2형 당뇨병을 가진 만성 치주염 환자의 치은조직에서 Matrix Metalloproteinase와 $TNF-{\alpha}$ 의 발현 양상 비교)

  • 치주질환의 병원균은 세포벽의 항원에 의하여 조직내 존재하는 mononuclear phagocytes가 활성화되어 cytokine들이 생성됨 으로써 치주 결체조직의 파괴를 진행시킨다. 이런 관련된 cytokine들은 순차적으로 상주하는 치은세포 및 대식세포가 Matrix metalloproteinase 합성을 하도록 유도하여 조직파괴를 시작한다. 이들 Matrix metalloproteinase중 MMP-2, MMP-9 (Gelatinase A,B)는 type IV collagen 및 변성된 interstitial collagen을 파괴하며 치주환자의 치은 열구액, 치은조직, 타액 네에서 높게 보고 되어왔다. 당뇨병은 치주질환의 위험요소중 하나로 달뇨 환자에서는 치주질환의 유병율이 일반인에 비해 높고 치주질환의 중증도도 더 심하여 진행도 빠르다고 알려져 있다. 그 병리 기전 중 하나로는 당뇨 환자에서는 치은 열구액 내 중성구 유래의 Matrix metalloproteinase의 활성 증가 및$TNF-{\alpha}$ 의 활성 증가가 추정되고 있다. 본 실험에서는 제2형 당뇨병 환자와 비당뇨 환자들에서 만성 치주염 부위의 치은 및 건강한 치은에서 염증매개체 중 하나인 MMP-2, MMP-9 및 $TNF-{\alpha}$ 의 발현에 대해 상호 비교 분석함으로서 염증, 혈당이 미치는 영향을 밝히고 제 2형 당뇨병 환자에서 심한 치주조직 파괴의 기전을 연구하고자 하였다. 경북대학교병원 치주과 내원환자 중 제2형 당뇨병 환자와 비당뇨 환자들 및 치주질환이 없는 건강인 대조군을 대상으로 여러 가지 환자요소, 임상 치주상태를 기록하고, 전신적으로 건강한 환자의 건강한 부위(n=8,Group 1), 전신적으로 건강한 환자으 만성 치주염 부위(n=8, Group 2), 제2형 당뇨병 환자의 만성 치주염 부위 (n=8,Group 3)에서 각각 변연치은을 채득하고 액화질소에 급속 동결하였다. Western blotting을 이용하여 각 조직 내 MMP-2, MMP-9 및 $TNF-{\alpha}$ 의 발현을 관찰, densitometer를 이용하여 상대적 발현을 정량, 각 조직의${\beta}-actin$을 이용하여 표준화하여 실험군과 대조군들의 평균치를 비교하였다. 비당뇨 환자들의 만성 치주염 부위 및 제2형 당뇨병 환자의 만성 치주염 부위에서 모두 건강 대조군에 비해 MMP-2와 MMP-9 의 발현이 증가되었다. 또한 MMP-2와 MMP-9는 2형 당뇨 환자의 만성 치주염 부위가 비당뇨 환자의 만성 치주염 부위보다 증가된 발현양상을 보였으며, $TNF-{\alpha}$ 발현 비교시 각 군간 유의성 있는 변화는 없었으나 2형 당뇨환자군에서 MMP-2 및 MMP-9의 증가와 함께 다소 증가 양상을 보였다. 결론적으로 본 실험에서 MMP-2 및 MMP-9의 증가가 만성 치주염 및 2형 당뇨 환자에서의 만성치주염에서 비당뇨환자 보다 MMP-2, MMP-9의 증가양상을 보여 주었으며 $TNF-{\alpha}$ 가 2형 당뇨환자의 만성치주염 진행과정에 기여인자로써 역할을 하는 것으로 생각된다.

  • 표고(Lentinula edodes)에 다량 함유되어 있는 아미노산은 면역효과와 밀접한 관련이 있는 것으로 알려져 있다. 표고 특유의 향미에 대한 소비자들의 선호도가 상이하여, 표고를 맥아로 당화하여 소비자들의 섭취 편이성을 향상시키고, 아미노산을 포함한 당화액을 제조하여 면역효과가 증강된 제품을 개발하고자 본 연구를 수행하였다. 또한 버섯류를 비롯한 담자균류에는 다양한 약효성분이 함유되어 있어 민간에서도 자주 사용된 기록이 있다. 최근 고등담자균류인 버섯의 성분 및 유용성에 관심이 크게 증가하고 있는 추세이며, 이에 따라 버섯류에 대한 식품학적 및 약리학적 측면에서 연구가 활발히 진행되고 있다. 표고(L.edodes)와 당화효소로 사용한 맥아의 비율에 따라 A (표고 1 kg : 맥아 50 g), B (표고 1 kg : 맥아 100 g), C (표고 1 kg : 맥아 200 g), D (표고 1 kg : 맥아 300 g), E (표고 1 kg : 맥아 300 g) 5개의 시험구를 설정하였으며, 제조된 표고당화액을 농축 후 동결건조 하여 분말로 분쇄 후 시료로 사용하였다. 표고당화액의 면역효과를 검증하고자, Raw 264.7에 표고당화추출분말 A, B, C, D, E를 각각 10, 50, 100 및 $500{\mu}g/mL$의 농도로 처리하여 각 시료에 대한 세포 독성과 항염증효과를 확인하였다. 세포독성 시험결과, E 시험구의 경우 $100{\mu}g/mL$의 농도에서 75.3%, $500{\mu}g/mL$의 농도에서 66.3%의 세포생존율을 나타내 독성을 보였다. 또한 A, B, D를 각각 $500{\mu}g/mL$의 농도로 처리 했을 때, 94.1%, 83.1%, 80.2%의 NO 생성율을 나타내어 E 시험구를 제외하고는 세포독성이 없는 것으로 확인되었다. 표고당화추출분말의 구성아미노산 함량은 C 시험구에서 19,699.01 mg%로 가장 높게 나타났으며, A 시험구에서 17,231.14 mg%, B 시험구에서 17,152.38 mg%순으로 높게 나타났다. E 시험구에서 13,367.98 mg%로 가장 낮게 나타났다. 모든 표고당화추출분말에서 총 16종의 유리아미노산 이 검출 되었으며, C 시험구에서 2,562.06 mg%로 가장 높게 나타났고, B 시험구에서 2,243.16 mg%, D 시험구에서 1,895.29 mg%, A 시험구에서 1,672.82 mg%순으로 높게 나타났다.

On-site Conservation Treatment of the Beaker-shaped Pottery from Yori, Hyangnam, Hwaseong (화성 향남 요리 출토 심발형 토기 수습과 보존처리)

  • Ten historic sites (denoted as A- J in this study) of a tomb were found during the construction of the east-west expressway in District 2 of Hyangnam, Hwaseong, which is implemented by the Gyeonggi-do headquarters of the Korea Land & Housing Corporation. Thetombswere first detected at siteH, and further investigations revealed various tombs from the Three Kingdoms period; artifacts such as gilt-bronze shoes and caps were excavated from wooden coffins in the tombs. The pottery examined in this study was the only pottery artifact excavated from the site. Its raw clay was soft and loose, reddish brown, and had quartz and feldspar particles of < 1 mm, which appeared to have been added as reinforcing agents. The firing temperature of the pottery was estimated to be under 800-870 ℃ as the mica remains and tridimite, which is the phase transition mineral of quartz, was not produced; a slight endothermic peak was also detected because of the hydration of sericite at 800 ℃. The condition of the artifact was severely weakened because of various factors, such as soil pressure from the stratum formed over the site and repeated freezing and thawing. The artifact could not be collected alone, and thus, surrounding soil that had attached to the artifact was also collected; the artifact was transported to the laboratory and conservation treatment was conducted in a safe and systematic manner.

The comparison of inflammatory mediator expression in gingival tissues from human chronic periodontitis patients with and without type 2 diabetes mellitus (단순 만성 치주염 환자 및 2형 당뇨병환자의 만성 치주염 치은조직에서 염증성 매개인자의 발현 양상 비교)

  • Gingival tissue samples were obtained during periodontal surgery or tooth extraction. According to the patient's systemic condition & clinical criteria of gingiva, each gingival sample was divided into three groups. Group 1 (n=8) is clinically healthy gingiva without bleeding and no evidence of bone resorption or periodontal pockets, obtained from systemically healthy 8 patients. Group 2 (n=8) is inflamed gingiva from patients with chronic periodontitis. Group 3 (n=8) is inflamed gingiva from patients with chronic periodontitis associated with type 2 diabetes. Tissue samples were prepared and analyzed by Western blotting. The quantification of $IL-1{\beta}$, MMP-13 and TIMP-1 were performed using a densitometer and statistically analyzed by one-way ANOVA followed by Tukey test. 1. The expressions of MMP-13 and TIMP-1 showed increasing tendency in group 2 & 3 compared to group 1. 2. The expressions of $IL-1{\beta}$ & MMP-13 were showed increasing tendency in group 3 compared to group 2. 3. As $IL-1{\beta}$ levels were increasing, MMP-13 showed increasing tendency in group 3, and although $IL-1{\beta}$ , MMP-13 levels were increasing, TIMP-1 levels were similar expressed comparing to group 2. In conclusion, this study demonstrated that the expression levels of MMP-13 and TIMP-1 had increasing tendency in inflamed tissue. It can be assumed that $IL-1{\beta}$ and MMP-13 may be partly involved in the progression of periodontal inflammation associated to type 2 DM.

The expression of elastase showed increasing tendency in group 2 and 3, and It was highest in group 3. 3. The expression of ${\alpha}_1-PI$ showed increasing tendency in group 3 compared to group 1. 4. The ${\alpha}_1-PI$/elastase ratio was decreased in group 2 and 3 compared to group 1, especially most decreased in group 3. 5. As IL-6 levels were increasing, elastase showed increasing tendency in group 3, and although IL-6 and elastase levels were increasing, ${\alpha}_1-PI$ level in group 3 showed slightly increasing pattern comparing to group 1. In conclusion, this study demonstrated that the expression levels of IL-6 and elastase will be inflammatory markers of periodontal inflammed tissue and DM. The ${\alpha}_1-PI$/elastase ratio also may be important measuring inflmmatory factors in the progression of periodontal inflammation associated to type 2DM.

MMP-2, MMP-8 Expression in gingival tissue of Chronic Periodontitis associated to Type 2 Diabetes Mellitus (2형 당뇨병을 동반한 만성 치주염 환자의 치은조직에서 MMP-2, MMP-8의 발현 양상 비교)

We investigate whether expression of MMP-2, MMP-8 is increased by chronic periodontitis associated with type 2 DM. Based on patient's systemic condition & clinical criteria of gingiva, each gingival samples were divided into three groups. Group l(n=8) is clinically healthy gingiva without bleeding and no evidence of bone resorption or periodontal pockets, obtained from 8 systemically healthy patients. Group 2(n=8) is inflammed gingiva from patients with chronic periodontitis. Group 3(n=8) is inflammed gingiva from type 2 diabetic patients with chronic periodontitis. Tissue samples were prepared and analyzed by Western blotting. The quantification of MMP-2, MMP-8 was performed using a densitometer and statistically analyzed by ANOVA. MMP-2, MMP-8 was expressed in all samples including healthy gingiva and increased in group 3 compared to group 1 and 2, and showed that significant variation was observed between group 1 & 3 in MMP-8 results. In conclusion, this study demonstrated that human gingival tissue with chronic periodontitis associated to type 2 diabetes showed slightly elevated MMP-2, MMP-8 levels compared to healthy gingiva and non-diabetic inflamed gingiva.

Comparative Analysis of Cold Tolerance and Overwintering Site of Two Flower Thrips, Frankliniella occidentalis and F. intonsa (꽃노랑총채벌레와 대만총채벌레의 내한성과 월동처 비교 연구)

  • Two dominant thrips in hot pepper (Capsicum annuum) cultivating in greenhouses are Frankliniella occidentalis and F. intonsa in Korea. This study investigated their overwintering physiology. These two thrips were freeze-susceptible and suppressed the body freezing temperature by lowering supercooling point (SCP) down to -15~-27℃. However, these SCPs varied among species and developmental stages. SCPs of F. occidentalis were -25.7±0.5℃ for adults, -17.2±0.3℃ for pupae, and -15.0±0.4℃ for larvae. SCPs of F. intonsa were -24.0±1.0℃ for adults, -27.0±0.5℃ for pupae, -17.2±0.8℃ for larvae. Cold injuries of both species occurred at low temperature treatments above SCPs. Thrips mortality increased as the treatment temperature decreased and its exposure period increased. F. occidentalis exhibited higher cold tolerance than F. intonsa. In both species, adults were more cold-tolerant than larvae. Two thrips species exhibited a rapid cold hardening because a pre-exposure to 0℃ for 2 h significantly enhanced the cold tolerance to a lethal cold temperature treatment at -10℃ for 2 h. In addition, a sequential exposure of the thrips to decreasing temperatures made them to be acclimated to low temperatures. To investigate the overwintering sites of the two species, winter monitoring of the thrips was performed at the greenhouses. During winter season (November~February), adults of the two species were not captured in outside of the greenhouses. However, F. occidentalis adults were captured to the traps and observed in weeds within the greenhouses. F. occidentalis adults were also emerged from soil samples obtained from the greenhouses during the winter season. F. intonsa adults did not come out from the soil samples at November and December, but emerged from the soil samples obtained after January. To determine the adult emergence due to diapause development, two thrips species were reared under different photoperiods. Adult development occurred in all photoperiod treatments in F. occidentalis, but did not in F. intonsa especially under short periods. Tomato spotted wilt virus, which is transmitted by these two species, was detected in the weeds infested by the thrips during the winter season. These results suggest that F. occidentalis develops on weeds in the greenhouses while F. intonsa undergoes a diapause in the soil during winter.
