Adult Tracheoesophageal Fistula Incidentally Found on General Endotracheal Anesthesia - A Case Report - (전신마취 중 우연히 발견된 성인에서의 고립성 기관식도루 - 1례 보고 -)

  • We describe a case of adult tracheoesophageal fisula incidentally found during laparotomy. A 41 year old male came to the emergency room due to multiple injuries from a car accident. An emergent laparotomy was given to the patient to rule out hemoperitoneum, and progressive distension of the stomach was noted with each positive pressure ventilation. The diagnosis of tracheoesophageal fistula was made via an intraoperative esophagogram. Detailed inquiry of the patient's history from his mother together with extensive destructive changes over the right upper lung field on the patient's chest X ray suggested that the fistula was longstanding and not of traumatic origin, obviating the need of urgent operation. The fistula was divided via the right thoracotomy 24 days later. Postoperative course was uneventful.

Direct Closure of Post-Incubation Tracheoesophageal Fistulas, -Report of 3 Cases- (기관내 삽관 후 발생한 기관식도루의 직접 봉합에 의한 수술적 치료 -3례 보고-)

  • Post-intubation tracheoesophageal fistula is rare, and its management causes a serious problems to surgeons. We have experienced 4 cases in 3 patients. Simple ditcision and closure of the fistulas were done by trans-cervical approach after weaning of ventilator. The tracheal defect was closed by simple suture, and the esophageal defect was closed in two layers before a viable muscle flap was interposed between the two suture lines in order to prevent recurrence. There was one delayed tracheal stenosls and one recurrent fistula, and these complications were also managed success ully.

  • Esophageal atresia with tracheoesophageal fistula is a common form of congenital malformation of the esophagus, which was usually overlooked during the physical examination after delivery. The first report of esophageal atresia with tracheoesophageal fistula was done by Thomas-Gibson in 1696. We have performed primary repair by extrapleural approach through the right 4th intercostal space in 4 cases of congenital esophageal atresia with distal racheoesophageal fistula. End to end anastomosis was performed by Haight method in all cases. Postoperatively, two patients showed severe stenosis and one patient showed mild stenosis at the anastomotic site. Ballots dilatation was performed two cases with severe anastomotic stenosis.

  • Acquired tracheoesophageal fistula is a rare but serious condition which is usually a result of prolonged intubation or tracheostomy statc, and is difficult to treat. A fifty-seven year old woman who was in a state of prolonged intubation and tracheostomy following a traf%c accident, presented with recurrent aspirati n. A tracheoesophageal fistula was demonstrated ) cm above the carina by csophagogram. We confirmed a subglottic web and tracheoesophageal fistula by bronchoscopic examination. Fistulectomy was performed with collor incision and partial sternotomy. The esophagus was repaired by two-layer interrupted suture using 4-0 Vicr)1, and the trachea was repaired by single layer suture using a 4-0 PDS. The sternohyoid muscle was interposed between the trachea and the esophagus. A T-tube was inserted through the previous tracheostomy site for easy tracheal suction and maintenance of the tun:on. The T-tube was removed on the 14th postoperative day, and the patient recovered well without any complications.

  • Acquired bronchoesophageal fistula rare. Conservative treatment such as endoscopy is widely used. The recurrence rate after endoscopic treatment, however, is not well known. We report here on a case of a 54-year-old female who presented with recurrent bronchoesophageal fistula after endoscopic treatment that filled the fistula tract with $Histoacryl^{(R)}$.

  • The common cause of tracheoesophageal fistula(T-I fistula) after tracheal intubation is ulceration and necrosis of the posterior wall of trachea by compression pressure generated by cuff. We experienced a young woman sustaining a T-I fistula which was found on the 12th day of intubation for cardiopulmonary resuscitation. Because spontaneous closure of the fistula is far uncommon, operative closure should be aimed for and should be done as soon as diagnosis is conformed. We delayed ope ative closure because of poor general condition of the patient. In spite of delayed reconstruction, the tracheal reconstruction itself was successful, but the patient died of peritonitis induced sepsis on the postoperative 41th day.

  • There are still some controversies in treatment strategy for the very low-birth-weight baby with esophageal atresia even though the result of primary repair has been improving. We report a successful end to end anastomosis with staged approach in one of twin weighing 1,270 g at birth.

  • Acquired tracheoesophageal fistula, a life threatening lesion, is rare but occurs most frequently alter prolonged mechanical ven ilation using a cuffed endotracheal tube. The mechanism of injury seems to be ischemia and inflammation of compressed trachea and esophagus by cuffed endotracheal tube. The patient was a 25 years old pregnant woman who was on prolonged mechanical ventilation for bacterial meningitis secondary to untreated otitis media. 40 days after mechanical ventilation, sudden subcutaneous empysema and pneumomediastinum ocurred and these were due to tracheoesophageal fistula. It was diagnosed with bronchoscopy and CT We performed tracheal repair with TA 60mm stapler and esophageal repair by interruted two layer suture with 410 vicryl and 510 prolene. A flap of sternocleidomastoid muscle was inserted between trachea and esophagus. Postoperative course was uneventful and the result of operation was acceptable by esophagography.
