EMG Signal Analysis of Upper Extremity Motor Function using Balance-handle Device (밸런스 핸들 장치를 이용한 상지 운동 기능의 근전도 신호 분석)

  • Rehabilitation of upper limb motor function of hemiplegic patient must maintain interest and demand a device for a quantitative evaluation of rehabilitation training. In this paper, we developed the device that is composed of arm cradle, handle, and balance ball for rehabilitation exercise. We have performed experiment for validity as to whether to use the rehabilitation device when tilting the upper extremity training device developed to measure changes in the EMG signal to the main upper limb muscles for 7 healthy volunteers. We have analyzed muscle activation signals on agonist and antagonist as a reference in the muscle contraction and relaxation in the upper limb extension and flexion when the balance-handle device is tilted to front-rear and left-right. The experimental results showed that a tendency of muscle activation of biceps, triceps, and deltoid used in upper limb motor function of hemiplegic patients from extension and flexion evaluation items of Fugl-Meyer Assessment(FMA). These results may be helpful for rehabilitation training for upper limb motor function of hemiplegic patients by utilizing a developed unit.

Lower Extremity Biomechanics while Walking on a Triangle-Shaped Slope (삼각경사면 보행 시 하지 관절 생체역학적 분석)

  • Gait analysis has been conducted in various environments, but the biomechanics during the transition from uphill walking to downhill walking have not been reported. The purpose of this study is to investigate the knee and ankle joint kinematics and kinetics during walking on a triangle-shaped slope compared with those during level walking. Kinematic and kinetic data of eighteen participants were obtained using a force plate and motion capture system. The greater peak ankle dorsiflexion angle and moment and the peak knee extension moment were observed (p<0.05) during both uphill and downhill walking on the triangle-shaped slope. In summary, uphill walking on a triangle-shaped slope, which showed a peak knee flexion of more than $50^{\circ}$ with greater peak knee extension moment, could increase the risk of patellofemoral pain syndrome. Downhill walking on a triangle-shaped slope, which involved greater ankle dorsiflexion excursion and peak ankle dorsiflexion, could cause gastrocnemius muscle strain and Achilles tendon overuse injury.

Comparison of Modified-Modified Schober Test with Range of Motion in Evaluating Visual Analog Scale of Patients with Low Back Pain (요통 환자의 시각 통증 등급 평가에 있어서 Modified-Modified Schober Test와 관절 운동 범위의 비교)

  • 목적 : 요통 환자에서 Modified-Modified Schober Test (MMST)와 관절 운동 범위를 비교하여 시각 통증 등급 평가에 있어서 타당도의 우위를 추정하여 보고자 하였다. 방법 : 요통으로 입원한 31명의 환자들을 대상으로 MMST, 관절 운동 범위 및 시각 통증 등급을 주 3회씩 입원 기간 동안 측정하였다. 각 측정 방법의 시행 순서는 무작위로 선택되었으며, 환자들은 맨발로 환자복을 입은 채 측정하였다. 결과 : 각 자료의 상관성을 분석하기 위하여 피어슨 상관계수가 사용되었다. 요추의 MMST 측정값(r=-0.61 ; 99%CI)이 요추의 신전 및 굴곡의 관절 운동 범위 측정값보다 시각 통증 등급에 통계적으로 의미 있게 나타났다. 또한 요추의 MMST 측정값(r= -0.77 ; 99%CI)은 요추의 신전 관절 운동 범위 측정값보다 요추의 굴곡 관절 운동 범위 측정값과 통계적으로 의미 있게 나타났다. 결론 : 이상의 결과를 통하여 요통 환자의 평가에 있어서 MMST가 관절 운동 범위보다 높은 타당도를 가지는 것으로 추정된다.

Biomechanical Analysis of Arm Motion during Steering Using Motion Analysis Technique (동작분석기법을 이용한 조향동작에 대한 팔의 생체역학적 특성분석)

  • Biomechanical analysis of arm motion during steering was performed using a motion analysis technique. Three-dimensional position data for each part of arm are fed into an interactive model combining a musculoskeletal arm model and the mechanical steering system to calculate joint angles and torques using inverse kinematic and dynamic analyses, respectively. The analysis shows that elbow pronation/supination, wrist flexion/extension, shoulder adduction/abduction, and shoulder flexion/extension have significant magnitudes. Sensitivity analysis of the arm joint motion with respect to seating posture and steering wheel configuration is carried out to investigate the qualitative influence of the seating posture and driver's seat configuration on the steering behavior.

Comparative Study on Axes of Rotation Data by Within-Subjects Designs (피험자내 설계에 의한 회전축자료의 비교연구)

  • The axis of rotation in biomechanics is a major tool to investigate joint function; therefore, many methods to estimate the axis of rotation have been developed. However, there exist several problems to describe, estimate, and test the axis statistically. The axis is directional data(axial data) and it should not be analyzed with traditional statistics. A proper comparative method should be considered to compare axis estimating methods for the same given data ANOVA (analysis of variance) is a frequently used statistical method to compare treatment means in experimental designs. In case of the axial data response assumed to come from Watson distribution, there are a few ANOVA method options. This study constructed ANOVA models for within-subjects designs of axial data. Two models (one within-subjects factor and two within-subjects factors crossed design) were considered. The empirical data used in this study were instantaneous axes of rotation of flexion/extension at the knee joint and the flexion/extension and pronation/supination at the elbow joint. The results of this study can be further applied to the various analysis of experimental designs.

The Kinematics Analysis of Abscission Handicap Wheelchair Table Tennis (절단장애 휠체어 탁구 스트로크의 운동학적 분석)

  • The objective of this study is to kinematically analyze forehand & backhand strokes of abscission handicap wheelchair table tennis athletes. The participant of this study were picked out of national athletes who have abscission handicap. Forehand stroke movement was expected to show a throw like motion. However, external rotation and internal rotation of the right arm created a backswing and an impact. Backhand stroke was expected to show a push like motion, and as expected, proximal part of the body didn't participate in the stroke motion, but the distal part, the right arm, rotated internally to backswing and external rotated to impact to form a push like motion. Forehand swing was expected to show throw like motion according to the Link Segmental System principle. However, abscission handicap athlete didn't show clear linking structure connecting proximal and distal parts. Successful strokes were dependant only on the angle of arm rotation.

Cervical Range of Motion Associations with Sub-clinical Neck Pain (경미한 경부 증상이 있는 대상자의 경추 관절 운동 범위 연구)

  • 목적: 병원치료가 필요하지 않을 만큼의 경부통증/증상과 관절가동범위, 경흉추의 다각적 면에서 관계를 조사하기 위해서이다. 방법: 연령은 19세에서 42세(평균연령 28세로 실험에 참여하기를 원하는 건강한 성인 40명을 대상으로 하였다. 경흉추의 척추 자세, 경추 능동 관절 가동범위, 경부 분절 길이 등의 다각적인 면을 측정하였다. 모든 측정들은 동일시에 각 대상자에게 서로 다른 측정자에 의해 두 번 실시되었다. 경부 근 지구력은 수정된 Biering-Sorensen 검사법에 의해 측정되었다. 마지막으로 대상자들에게 경부 통증/증상의 재발에 대한 질문을 하였다. 결과: 14명의 대상자들은 경미한 재발성 경부 통증/증상을 보고하였다. 경부 근 지구력 시간(F(1,38)=6.75, p=0.01)과 좌측 회전 가동 범위(F(1,38)=4.56, p=0.04)가 경부 통증을 가진 대상자들에서 유의하게 감소하였다. 신전 가동범위가 재 측정에서 특정군 변화 즉, 정상군 증가, 경부 통증/증상군 감소(F(1,38)=4.67, p=0.04)가 보였다. 경부 통증/증상군은 정상군과 비교 시 후인 가동 범위의 증가하였다(F(1,38) =4.56, p=0.04). 통증 유무에 관계없이 모든 대상자들에서 우측 회전보다 좌측 회전에서 가동범위가 더 크게 나타났고(F(1,38) =4.34, p=0.04), 반복 측정에서 좌측 측방굴곡 (F(1,38) =5.10, p=0.03)과 우측 측방 굴곡(F(1,38) = 5.27, p=0.03)의 감소가 나타났다. 결론: 경미한 증상의 경부통증 대상자군과 정상적인 대상자군을 비교할 때 그룹 간 차이는 경부 근 지구력 시간의 감소, 좌측 회전 가동 범위 감소와 특히, 두 번 째 측정에서 신전범위 감소가 나타났으나 후인의 가동범위는 증가로 관찰되었다. 이러한 결과들은 경부 통증의 발생과 관련된 초기 가동범위 변화를 제안한다.

Effects of Cervical Joint Mobilization on the Forward Head Posture and Neck Disability Indexes (경부관절가동술이 두부전방자세와 경부장애지수에 미치는 영향)

  • This paper tries to examine whether the application of joint mobilization to subjects who have the forward head posture due to malalignment in the cervical joint has influence on posture changes and functions in the cervical joint. The subjects were 39 students from G University in Gyeongsangbuk-do. The cervical joint mobilization was applied to 20 subjects and not to 19. The students with a cervical lordosis angle of $21^{\circ}C$ or less, an anterior weight bearing (AWB) of 15mm or greater, and a cervical extension ROM of $70^{\circ}C$ or less in terms of radiography were selected as subjects under their voluntary agreement. The patients actively performed the joint mobilization slowly 8 times per session while therapists continuously applied sustained accessory glide to their painful joints 3 times per week for 4 weeks along with the cervical expansion and flexion in SNAGS among other Mulligan's (1995) techniques. The measurement was carried out in terms of radiographic inspection and neck disability indexes. As a result of the experiment, it turned out that the subjects with the forward head posture had changes in the cervical AWB and ARA, the ranges of expansion and flexion, and the NDI(Neck Disability Index) after the intervention for the experimental group by applying cervical joint mobilization. There were no changes observed in the control group. In conclusion, the application of joint mobilization turned out to have influence on the improvement of cervical joint postures, and craniocervical region functions.

Clinical Case study of Conservative Caring Method to Abnormal Knee Flexion in Standing Position (기립상태에서 비정상적 무릎관절 굴곡에 대한 보존적 치료 임상사례연구)

  • Objectives: There are many reasons to restrict extension of knee. Unless we can find any structural deformities from patients, we don't have any clue of helping this condition except give them some instruction of exercise of strengthening anterior thigh muscles and releasing extensor muscle group of lower extremities. In this study, the author reports new case, using technique of releasing restriction of knee movement based on Sacro Occipital Technique. Method: From the day of notifying 14yrs. old patient's extension restriction of knee, it had have 7 trials of having S.O.T category Ⅰ. technique procedure. Result: The patient can stand without extension restriction of knee. Conclusion: In many case, muscle soft tissue work helps releasing hamstring tension so that we can make patient stand with straightly extended knee position but if this method is not working then S.O.T category Ⅰ. technique procedure may be considerable to improve this condition.

Effects of Energy System Contribution on Isokinetic Muscle Strength in Various Sport Events Athletes (무산소, 유산소 운동종목별 엘리트선수의 등속성 근기능에 미치는 영향)

  • This study was conducted to compare the muscle strength, muscle power, and H/Q ratio according to energy system contribution in athletes participating in various sports. Subjects of the study were assigned into an Anaerobic Exercise Group (AEG, n=60; Short-Distance, Weight Lifting, Jumping, Throwing, Bowling, Golf) and an Endurance Exercise Group (EEG, n=60; Modern Pentathlon, Field Hockey, Handball, Cycle, Boxing, Rowing) groups. Isokinetic peak torque/body weight% and flexor/extensor ratio at 60, 180 deg/sec of knee extension and flexion were measured using an cybex 770. Data analysis was conducted using an independent t-test and one-way ANOVA. Based on the results of this study, there was higher extension, flexion strength and flexion power in the AEG than the EEG (p<0.05). We also confirmed higher muscle strength and muscle power in short distance and jumping athletes than other athletes participating in other events (p<0.05). However, there was no significant difference within the endurance exercise group. The HS ratio was within a stable range of 50% to 60% in all events. Collectively, the outcomes of this study indicate that routine physiological and performance testing can provide measurable benefits for elite athletes and their coaches.
