ELECTROMYOGRAPHIC INVESTIGATION OF DURATION OF THE MASSETERIC SILENT PERIOD

  • 정상교합인 179명과 , 악관절이상환자 12명, 그리고 악관절이상환자중 국소마취를 한 6명, 총 197예의 대상에 교근의 Jaw-Jerk Reflex를 야기시키기 위하여 Spring Triggered Tapping Device를 하악골유간부에 장치하고, 교근에 쌍극은판전극을 부착하여 Functional Switch Assembly 를 통하여 Preamplifier(461 B형, Beckman)에서 증폭된 활동전압을 Dynograph (RS형 9853A, Beckman)에서 직접 기록하였다. 기록된 근전도는 Computer (Univac 9400 Cobol Complier)로 분석하여 다음과 같은 결론을 얻어 이에 보고하는 바이다. (1) 정상교합인 ; 남자 평균 21msec, 여자 20msec, 남여평군 20±5.99msec (S.D.)였다. (2) 악관절이상환자 : 남자평균 39msec, 여자38msec, 남여평균 39±13.57msec (S.D.) (3) 국소마취를 시행한 악관절이상환자 ; 남자평균 21msec, 여자 18msec, 남여평균 19msec (4) 179명의 정상교합인중 34명은 Masseteric Slient period가 나타나지 않았다. (5) 정상교합인과 악관절이상환자가 중복될 수 있는 기간은 31~33.5msec 였다. (6) 정상교합인과 악관절이상환자와의 경계는 32.2msec 였다.

Effect on the Management of Postherpetic Neuralgia (대상포진 후 동통에 대한 치료 효과)

  • Fifteen patients was analyzed on effect of the management of postherpetic neuralgia by local anesthesia on the special region at pain clinic in Youngnam University Hospital. The results were on follows : 1) The frequency of occurrence of sex and the lesion side were similar in all patients. 2) The age of incidence was between 50 and 70 years old. 3) The most frequent site of lesions was the neck. 4) There was no relationship between age and treatment time. 5) Whole patients was done average 7-10 time local injection.

  • 본원의 비데오 흉강경술 경험과 문헌에서 볼 때, 흉막강 질환에서 흉강경술의 역활은 수기가 국소 또는 경막하 마취 혹은 전신 마취에서 가능하고 작은 구경의 흉강경으로 전 흉막강을 관찰할 수 있으므로 진단 및 치료 효과가 우수하였다. 흉막강 질환에서의 주요 적응증은 흉수, 농흉, 기흉, 흉막 종양, 흉막의 외상, 흉막강내 이물의 진단 및 치료되고 있다. 흉막강내 각종 수기에 사용하는 기구의 개발과 수기의 발전으로 흉막강내 질환에 대한 치료 성적은 전통적인 개흉수술에 비교하여 우수한 결과를 나타내고 있다.

Epidural Administration of Local Anesthetics and Steroids for Low Back Pain Management (요통 치료를 위한 경막외 국소마취제 및 스테로이드 주입 -제 2 보-)

  • Low back pain and/or lumbosacral radiculopathy have become one of the most common pain problems in our pain clinic center. There are several kinds of conservative management for low back pain and/or lumbosacral radiculopathy. We as pain clinicans, focused on epidural administration of local anesthetics and steroids. The rationale for epidural steroid administration is to reduce inflammation and to inhibit the action of nociceptive agents. Eighty mg of methylprednisolone acetate in 10 ml of 0.25% bupivacaine was infected, into epidural space 3 times at one week intervals for 1 year, to 921 patients(male: 422, female: 499) seen in the period between March 1986 and December 1989. The effectiveness was evaluated a month after the final injection. The results were as follows: Excellent pain relieved group 122 patients(13.25%) Good pain relieved group: 485 patients(52.66%) Fair pain relieved group: 184 patients(19.98%) No effect group: 130 patients(14.11%) We recommend the epidural steroid inject to the patients following failure of conservative management of discogenic pain.

Effects of Local Anesthetics on the Fluidity of Synaptosomal Plasma Membrane Vesicles Isolated from Bovine Brain (국소마취제가 Synaptosomal Plasma Membrane Vesicles의 유동성에 미치는 영향)

  • To elucidate the mechanism of action of local anesthetics, the effects of local anethetics on the microenvironment of the lipid bilayers of synaptosomal plasma membrane vesicles (SPMV) isolated from bovine brain and dimyristoylphosphatidylcholine (DMPC) multilamellar liposomes were investigated employing the intermolecular excimer fluorescence technique and differential scanning calorimetry (DSC). The relative intensities of excimer and monomer fluorescence of pyrene are a simple linear function of the viscosity of a homologous series of solvents. The microviscosity(${\eta}$)of the hydrocarbon region of SPMV was measured by this method and the value was $57.3{\pm}5.3\;cP$ at $37^{\circ}C$. In the presence of lidocaine-HCl and procaine-HCl, the values decreased to $46.5{\pm}5.1\;cP$ and $54.7{\pm}4.8\;cP$, respectvely. The differential scanning thermograms of DMPC multilamellar liposomes showed that local anesthetics significantly lowered the phase transition temperature, broadened the thermogram peaks, and reduced the size of the cooperative unit. These results indicate that local anesthetics have significant fluidizing effects on biomembranes and perturbation of membrane lipids may produce some, but not all, of their pharmacological actions.

Effectiveness of Drain Insertion and Irrigation in the Treatment of Septic Arthritis of the Knee under Local Anesthesia (국소 마취하 배액관 삽입 및 세척을 통한 화농성 슬관절염의 치료의 효용성)

  • Purpose: Septic arthritis of the knee is an orthopedic emergency that requires early diagnosis and surgical treatment. This study examined the effectiveness of drain insertion and irrigation in the treatment of septic arthritis of the knee under local anesthesia. Materials and Methods: A retrospective study was conducted on nine cases (eight patients) diagnosed with septic arthritis of the knee from September 2017 to February 2020 and treated with drain insertion and irrigation under local anesthesia. After penetrating through the superolateral portal to the superomedial portal and inserting the drain, daily irrigation of approximately 3 L of normal saline was done. The following were investigated: age, sex, underlying disease, cause, degree of osteoarthritis, time from diagnosis to surgery, duration of hospitalization, duration of normalization of C-reactive protein, and smear and culture. Results: The initial white blood cell count of joint fluid was 71,472±51,667/mm3 (32,400-203,904/mm3), and polymorphic leukocytes were 91.1%±2.6% (86%-95%). The average time from diagnosis to surgery was 8.3±1.3 hours (6-10 hours), and the irrigation period was 8.2±3.2 days (4-15 days). The average length of hospitalization was 20.8±8.7 days (9-37 days). There was no reoperation or recurrence. Smear and culture tests were not identified. Conclusion: In the treatment of septic arthritis of the knee, the insertion of a drain tube and irrigation under local anesthesia is a relatively fast and simple method to reduce pain by repetitive draining of purulent joint fluid and can be used as an alternative treatment for patients with a risk of general or spinal anesthesia.

  • Local pain management is the most critical aspect of patient care in dentistry. Local anesthesia is a reversible blockade of nerve conduction in an applied area that produces loss of sensation. The chemical agents used to produce local anesthesia stabilize neuronal membranes by inhibiting the ionic fluxes required for the propagation of neural impulses. Proper local anesthesia permits the dental surgeon to perform the necessary surgical procedure in a careful, gentle fashion that will be less stressful for both the operator and the patient. The improvements in agents for local anesthesia are probably the most significant advances that have occurred in dental science. Today's anesthetics are safe, effective, and can be administered with insignificant soft tissue damage and minimal concerns for allergic reactions. This article reviews the widely used local anesthetic agents for obtaining local anesthesia, and also discusses some frequently seen complications.

Duty of Care on Medical Accidents related to Anesthesia - Focused on Court Decisions - (마취 관련 의료사고 시 주의의무 - 법원 판결 사례를 중심으로 -)

  • Medical practices such as surgery often need to accompany anesthesia, which frequently causes medical accidents. In order to determine whether a medical accident related to anesthesia was caused by a doctor's fault, it is necessary to understand what is the duty of care required for the medical staff such as a doctor through all stages of anesthesia. This paper analyzed Supreme Court decisions since 1990s and recent lower courts' decisions in order to understand standard of care with respect to anesthesia. While numerous medical accidents were related to inhalation anesthesia in the past, it turned out that recent medical accidents were often related to the use of intravenous or local anesthetics. In particular, legal disputes with respect to medical accidents related to propofol have considerably increased since 2007. However, because Supreme Court decisions as to anesthesia accidents are mostly related to inhalation anesthesia, they seem to be insufficient to set standard of care as to other types of anesthesia accidents. In light of the fact that medical accidents related to the use of propofol have been increasing, it is critical to establish and maintain clinical guidelines on the use of each anesthetic in the medical field. However, The Courts can present the standard of care suitable for medical reality to serve as a compass for medical practices.
