CONSTRUCTION PROCEDURE OF TOOTH POSITIONER (Tooth Positioner의 제작에 관해서)

  • Tooth Positioner는 band를 제거한 후 가능한 빨리 장착시켜야 susceptible하며 무엇보다도 환자의 생리학적 jaw movement를 설정해서 제작해야 하고 적어도 hingeaxis relation은 찾아서 제작해야한다. Positioner의 장착시간은 하루에 $3\~4$시간의 active biting exercise와 수안시간동안 사용하게 한다. 처음 장착시에 가장 tight하고 이 tightness 는 치아가 요망되는 위치로 근접함에 따라서 점차 감소되고 이 tightness의 감소가 작용효과의 가장 좋은 지침이 되며 자는 하루 하루 그 변화를 느껴야 한다. Positioner가 느슨하게 적합되고 좋은 교합관계를 보이면 장착을 수안시간동안으로 제한시키거나 혹은 깨어 있을 동안 한 두시간 장착하게 한아. 대개 $3\~4$주 후에 predetermined pattern과 유사하게 되며, 일반적으로 장착 후 $8\~10$주 후에 더 장착할 것인가를 결정하고 필요성이 없다면 conventional retainer로 바꾸어 준다. Tooth Positioner는 기능적인 구호흡, thumb sucking, snoring을 해소시켜 줄 수 있으며 jaw relationship이나 overbite문제를 해결할 수 있다. 그리고 chair time을 감소시켜 줄 수 있으며 tissue tone을 자극해서 치아위치를 증진시키는데 끊임없이 작용한다. 그러나 제작에 많은 시간이 걸리고 치아를 배열하는 술자의 능력에 따라 치료효과가 많이 좌우되며 너무 bulky하므로 이물감이 커서 유용하게 장착할 수 있는 시간이 제한적이고 natural muscle balance에 대해 간헐적인 교정력을 가함으로써 치아를 loosening 시킬 수 있다. 이 장치물은 다른 removable appliance와 같이 환자의 협조가 무엇보다 중요하므로 환자에게 장착동기를 유발시켜 주는 것이 좋으며 True Blocked Nasal Airway가 있는 환자에게는 금기증이 된다.

The Relationships between Orthodontic Treatment, Oral Health-Related Quality of Life, and Happiness of among Some High School Students (일부 고등학생들의 교정치료와 구강건강 관련 삶의 질, 행복감과의 관련성)

  • The purpose of this study was to provide basic data on counseling, education, and treatment related to orthodontic practices through an understanding of the relationships between orthodontic treatment, oral health-related quality of life, and happiness of high school students. The subjects in this study were 500 students in Grades 1~3 of a high school in Jeollabuk-do. We conducted a survey targeting this group; the results revealed the following: in the social support item, peer support showed significant differences in the matter of orthodontic treatment (p<0.05). In oral health-related quality of life, the students without orthodontic treatment showed 14.09 scores that were higher than the students with treatment (p<0.05). Regarding happiness, the students with orthodontic treatment showed higher scores which were significantly different from the those of the non-orthodontic treatment group (p<0.05). Additionally, upon examining the correlations between self-esteem, social support, oral health-related quality of life, and happiness, oral health-related quality of life, peer support, and happiness showed the negative (-) correlations while the rest of the variables showed the positive correlations (p<0.05). The factors having effects on oral health-related quality of life included the stress, matter of orthodontic treatment, and teachers support (p<0.05). The factors having effects on happiness included self-esteem, peer support, and family support (p<0.05). Based on the above results, even though dental orthodontic treatment had negative effects on oral health-related quality of life, it had positive effects on happiness. Consequently, it would seem necessary to seek active methods to increase oral health-related quality of life. This improvement could be achieved by minimizing inconveniences, such as mastication, in the process of dental orthodontic treatment, and developing programs that could enhance happiness.

Evaluation of Physical Properties of Resin Containing Zinc Nanoparticle. (아연나노입자함유 교정용 레진의 물리적 특성 평가)

  • Polymethyl methacrylate (PMMA), a self-polymerizing resin for removable orthodontic devices, has been used as a dental orthodontic device for many years because of its advantages such as color stability, volume stability, and tissue compatibility. However, such a removable orthodontic device has a disadvantage that the longer the use in the oral cavity due to the low strength of the PMMA fracture of the orthodontic device resin in use. In this study, zinc nanoparticles (ZNP) were mixed with orthodontic PMMA to introduce strength effect. Rectangular samples ($1.4{\times}3.0{\times}19.0mm$) of orthodontic PMMA (0, 0.5, 1.0, 2.0 and 4.0%) containing ZNP were prepared. The finished specimen was tested for three-point bending strength at a speed of 1 mm / min, and the Vickers hardness was measured three times using a hardness tester. The surface roughness was measured with a surface roughness. As a result, the 3-point bending strength did not change significantly (p>0.05). Surface energy increased significantly. As a result, we successfully synthesized ZNP in this study and prepared the dispersed resin specimen for calibration. It will be possible to develop high-density dental orthodontic resins.

A Study on Titanium Miniscrew as Orthodontic Anchorage : An experimental investigation in dogs (성견에서 교정적 고정원으로서의 티타늄 미니스크류에 대한 연구)

  • Titanium miniscrews we being used increasingly as an anchorage for tooth movement, because they ate easy to place and to remove, increase the number of sites available, give minimum strain to patients regarding surgical procedures, and offer uneventful healing alter removal. The use of titanium miniscrews as an orthodontic anchorage has been reported in clinical case reports, but clinicians have experienced screw loosening when using such screws.' To our knowledge, there are no published reports evaluating the stability of miniscrews. Information about the length of miniscrews used in relation to the location is of some importance, as stability will vary depending on bone duality The purpose of this study was to evaluate a variety of Lengths of miniscrews (dimeter: 2mm) which were inserted in maxilla or mandible and to demonstrate in a dog model which miniscrew provides fundamental stability in the jaws. 10 mm long miniscrews in the maxilla and 8mm long: miniscrews in the mandible showed no clinical mobility and retained their position throughout an 8 weeks force (200g) application. The mucosal condition around the screws was healthy in cases in which miniserews were inserted in the alveolar bone between the roots and the head of the screws emerged into the attached gingiva. When the force application was terminated, radiographic analysis revealed neither rent resorption not periodontal pathology around the miniscrews that remained stable during the entire treatment period. This study suggests that if titanium miniscrews with adequate length are properly used depending on the location, they provide sufficient stability for orthodontic anchorage.

  • The purpose of this study is to discuss about problems exposed as operating of bibliotherapy program and to find way to vitalize bibliotherapy as a correctional education for juvenile delinquents. The institutionalized youths of juvenile correctional school at P city which have been chosen as the sample representatives for planning, implementing, accessing and analyzing this program. The analysed results of the program show that points are to be considered: (1) Priority should be given to self-motivated youths when selecting participants. (2) Setting up as a regular program at juvenile correctional school. (3) To be necessary active participation, individual program should be provided in abreast of group program. (4) Creating adequate environment in other to ensure continuous self-healing for bibliotherapy (5) To create of professional bibliotherapy programs specialized for juvenile delinquents (6) Bases for Cooperation between the Ministry of Justice and The National Central Library, and between local juvenile corrective institutions and local libraries (7) Ultimately, To found a library at juvenile correctional schools.

  • Recently orthodontics in elderly people is continually increasing due to aging of the society arising from decrease of birth rate as well as death rate. As the elderly population grows, needs for oral health care increases, and elderly patients with poor teeth alignment need more attention with orthodontic treatment. Our study analyzes various treatment protocols for different cases of aged orthodontic patients, and following are considerations in orthodontic treatment of elderly patients. 1. Periodontal treatment should be preceded before orthodontic treatment. 2. Periodontal condition should be considered when selecting teeth for extraction 3. In dealing with the residual prosthesis, condition of prosthesis, anchorage availability, and need for size reduction, patient seconomic status should be taken into consideration. 4. Fixed retainers are recommended for retention.

THE PREVALENCE OF CLEFT LIP AND/OR CLEFT PALATE IN KOREAN MALE ADULT (한국인 성인 남자에게 구순열 및 구개열의 유병률에 관한 연구)

  • Cleft lip and/or palate(CLP) is the most common congenital deformity in the craniofacial region. For a practical public health system for these patients it is necessary to have an understanding on the prevalence of CLP. However, it was difficult to estimate the exact number of CLP patients due to problems in sample selection, higher miscarriage and still birth rate, difficulty in classification, and adoptions to foreign countries. Therefore this study was to estimate the prevalence of CLP and the rate of orthodontic treatment, which is usually necessary in cleft lip and/or palate patients. The samples consisted of 218,322 Korean male adults from Seoul, Kwangju, Taegu and Pusan, all born in 1979. The screening method for recognizing the CLP patients was proceeded in steps and the results are as follows. 1. The prevalence of cleft lip and/or palate in Korean male adults born in 1979 was 0.65 out of the 1000 samples. 2. In the anteroposterior aspect of the 1000 samples, the prevalence of cleft lip, cleft lip and palate, and cleft palate was 0.26, 0.36 and 0.03 respectively. 3. In the transverse aspect of the 1000 samples, the prevalence of left, right and bilateral cleft was 0.35, 0.16, 0.12 respectively. The cleft in the left showed a much higher prevalence than in the rirht, while bilateral cleft showed a lower prevalence than unilateral cleft. 4. The orthodontic treatment rate of Korean male adults among cleft lip and/or palate Patients was $35\%$, and it was in the order of cleft lip and Palate, cleft lip, and cleft Palate, being $67\%,\;29\%\;and\;29\%$ respectively. The orthodontic treatment rate in patients with the more severe cleft lip and palate was higher than in patients solely having cleft lip or cleft palate.

  • The evaluation of malocclusion has to be done quantitatively and qualitatively. This will be lead toward an analysis of malocclusion severity as well as treatment difficulty. The method of proper evaluation of malocclusion severity and treatment difficulty is necessary to assess treatment effect and efficiency for the orthodontists and to establish fundamentals for planning and executing the health-related policies in private and public institutions. The purposes of this study as the first part of the objective and quantitative analysis of malocclusion were 1) to measure treatment difficulty based on the opinions of several orthodontists. and 2) to investigate the relationships between objective malocclusion severity and subjective treatment difficulty 100 pairs of dental casts that had various types and severity of malocclusion were selected from the orthodontic departments of Kyurghee University and Samsung Medical Center The objective malocclusion severity was measured with the PAR (Peer Assessment Rating) index and the subjective treatment difficulty was evaluated by 8 experienced orthodontists. The relationships between objective malocclusion severity and subjective treatment difficulty were statistically evaluated. There were significant relationships between objective malocclusion severity and subjective treatment difficulty especially in the measurements of the upper anterior alignment, the buccal occlusion. the overjet, the overbite and the midline discrepancy en the malocclusion components. The results of this study can provide the background knowledge to develop a new occlusal index. which contains both the malocclusion severity and treatment difficulty for Korean orthodontists.

The changes of root length and form in immature teeth after orthodontic treatment (교정치료시 발생하는 미완성 치근의 길이와 형태변화)

  • Previous studies have focused on the causes of root resorption after orthodontic treatment and treatment methods to reduce this phenomenon, and have been mainly associated with developed, mature roots. As parents become increasingly interested in their children's' dentition, orthodontists are performing fixed orthodontic treatment on patients of less than 10 years and before the completion of the immature root. Thus, the author evaluated the changes of root length and root form of maxillary immature incisors after orthodontic treatment, compared with those of mature teeth, and investigated the correlation according to gender, treatment duration, and displacement of incisors. The sample consisted of an immature root group of twenty-eight persons (between 8 and 10 years old) and a mature root group of thirty-one persons (between 11 and 15 years old). The crown and root length of the maxillary four incisors were measured with a periapical radiograph, changes in root length and crown-root ratio were calculated, and root form was classified according to a scoring system. The results were as follows. 1. The development of immature roots was not affected by orthodontic treatment and mostly showed normal root length and apical form. 2. Root length of immature teeth was sustained or became shorter, partially in long treatment duration or with open bite patients. Even though the teeth reached their normal root length, they demonstrated a blunt form. 3. Most of the mature roots showed mild resorption, and the form of mature roots was more blunt than the developed form of the immature roots (p<0.05). 4. The developed form of the immature roots was statistically related to treatment duration, while the form of the mature roots was significantly related to the displacement of incisors (p<0.05). 5. In contrast, other variables such as gender, classification of malocclusion, changes in overbite, and changes of U1 to SN showed no correlation with the root resorption of both groups.

Malocclusion with Congenital Missing of Lower Incisors (하악 전치의 선천적 결손을 동반한 부정 교합의 치험례)

  • Positions, angulation and mesiodistal dimension of lower incisors are important in esthetics, occlusion and post-treatment stability of tower arch. When lower incisor is congenitally missing, problems such as increased overjet and overbite, closing in of adjacent teeth and size/space discrepancies may occur. When creating treatment plans, incisor position and angulation, lip support, anteroposterior skeletal relationship canine-molar relationship, overjet overbite, remaining growth potential, crowding and anterior tooth ratio have to be considered. For an accurate analysis of incisal size discrepancy, diagnostic model set-up may be helpful. The two patients in this presentation both had two lower incisor missing, but the degree of crowding, skeletal relationship, lip support, molar relationship are different and therefore treatment plan was different as well. Long term follow-up may be necessary for stability and retention.
