Effects Of Oral Health Awareness and Oral Health Behavior on Preventive Behavior of Cardiocerebrovascular disease in Cardiocerebrovascular Disease Risk Group (심뇌혈관질환위험군에서의 구강건강인식과 구강건강행위가 심뇌혈관질환 예방행위에 미치는 영향)

  • This study was conducted to investigate the effects of oral health awareness and oral health behavior on cardiocerebrovascular prevention behavior in a cardiocerebrovascular diseases risk group and provide basic data for a nursing intervention program for the group. Data were collected from 131 people in the risk group of cardiocerebrovascular diseases living in J Province in February and March of 2018. The obtained data were analyzed using descriptive statistics, t-tests, Pearson's correlation, and stepwise multiple regression analysis. The results revealed that preventive acts were positively correlated with subjective oral health state (r=0.261, p=0.003), importance of oral health (r=0.250, p=0.004), and practice of oral health (r=0.303, p<0.001). Moreover, acts to prevent cardiocerebrovascular disease were influenced by oral health (${\beta}=0.29$, p<0.001), age (${\beta}=0.27$, p=0.001), and subjective oral health state (${\beta}=0.24$, p=0.003). Specifically, scores of preventive acts for cardiocerebrovascular diseases increased with higher degree of oral health behavior, older age, and better subjective oral health state. These variables had an explanatory power of 19.3%. The results of this study imply a need to prepare measures to enhance the oral health level of the risk group of cardiocerebrovascular diseases and conduct ongoing follow-up studies of the perception, attitude, and behavior of individuals toward oral health, as well as the state of oral health.

Relationship between White Matter Changes and Homocysteine Concentration in Healthy Adults (건강한 성인에서 대뇌 백질 변성과 호모시스테인 농도의 연관성)

  • This study was conducted to analyze the correlation between changes in white matter and homocysteine concentration through brain computed tomography of healthy 50-75 year old subjects without stroke or dementia history. We studied 722 out of 900 patients who underwent health screening at one hospital from 2016 to 2017. Based on the medical records, retrospective studies were conducted and analyzed using SPSS. A chi-square test, T-test and univariate logistic regression analysis were used for analysis. After the subjects were divided into the group with and without white matter changes, the population characteristics were analyzed. The mean age, homocysteine concentration and prevalence of hypertension and diabetes were higher and the duration of education was shorter in the group with white matter changes. In the group with white matter changes, the population increased as homocysteine concentration increased. When the odds ratio was compared based on the lowest group (Q1), age [p<0.001], hypertension [p<0.001] and hyperhomocysteinemia [p=0.021] were risk factors for white matter changes. We also identified modifiable risk factors such as hypertension and hyperhomocysteinemia to prevent complications of white matter changes. However, there has been no report of risk for the each causes of hyperhomocysteinemia and relationship between white matter changes and homocysteine concentration in Koreans. Therefore, large scale prospective studies are needed to better understand this topic.

The Experiences of Nursing Student's Introductory Clinical Practice (간호대학생의 임상입문실습 경험)

  • This study was conducted to discover the meaning and the essential structure of the experiences of nursing students' introductory clinical practice. The participants were eight nursing students who had carried out the introductory clinical practice at a hospital. The data was collected from July 15 2019 to July 31, 2019 through focus group discussions and in-depth individual interviews using non-structured questionnaires. The data was analyzed by Colaizzi's phenomenological analysis methodology. The introductory clinical practice experienced by nursing students was categorized as followed: 'Feel worried and concern about expectations at the same time', 'Hospital experience as reality', 'Becoming accustomed to complexed emotion's, 'The first step of becoming a nurse', and 'Preparation for the future'. The five categories were expanded in the same context according to time. The essential structure of the clinical introductory practice experiences of the nursing students revealed by the study is that they begin to practice feeling worried and concerned about expectations, and concern about experience and adaption to various situations, emotions and preparation for the future. Based on the results obtained from this study, it is necessary to develop a realistic and effective education program before starting clinical practice.

Influence of Social Support and Health Literacy on Treatment Adherence in Hemodialysis Patients (혈액투석환자의 사회적 지지와 건강정보이해능력이 치료순응도에 미치는 영향)

  • This study was a descriptive research study for hemodialysis patients to survey the effects of social support and health literacy on treatment adherence. The subjects of this study were 140 hemodialysis patients aged over 40 years who had been receiving treatment for more 1 year in artificial kidney rooms at two general hospitals in Y city. The data were collected from November 1, 2019 to December 31, 2019 and were analyzed using descriptive statistics, t-tests, one-way ANOVA, Scheffe test, Pearson's correlation coefficients, and multiple regression with the SPSS/WIN 26.0 statistical program. The results of this study show that social support (family, friends, medical staff) and health literacy (functional, communication, critical) were positively correlated with treatment adherence. The variables affecting treatment adherence in hemodialysis patients were identified by social support and health literacy with 69.6% explanatory power. To improve the treatment adherence of hemodialysis patients, it is necessary to develop education programs to improve health literacy based on social support.

Music for Pediatric Patients in Medical Settings: A Systematic Review of Randomized Controlled Trials (소아환자를 위한 음악: 무작위 임상연구의 체계적인 문헌고찰)

  • The aim of this study was to systematically review the latest clinical trials in music medicine and medical music therapy for pediatric patients. Thirteen databases were searched to obtain randomized controlled/crossover design studies published between the year 2000 and 2012 in English language. Out of 1012 articles retrieved in the initial search, fifteen studies were identified based on an exclusion criteria. Overall, selected articles involved children 1 month to 18 years, sample size of 11 to 150, and total participants of 987. Studies were classified and compared as music medicine or music therapy studies through a systematic synthesis assessing general characteristics, methodological quality, measured outcomes, types of interventions and the study results. Seven music medicine and eight music therapy studies measured seven dependent variables using thirty-six different measurement tools with a large heterogeneity in the selection, type, and method of music interventions. Evaluation of the methodological quality revealed that many studies did not provide a full report of the research method, and did not meet some or most methodological standards, such as randomization, allocation concealment, double or partial blinding, and intention to treat analysis. Although overall research results were positive if not significant, poor methodological quality and heterogeneity in design and intervention strategies raise the question of research bias and trustworthiness issues. The systematic review concluded that music may have a valuable clinical effect in addressing the physical and psychosocial needs of hospitalized children, although more rigorous, homogeneous and replicable studies are greatly needed.

Analysis of Influential Factors for the Quality of Life Among Dental Hygienists (일부 지역 치과위생사의 삶의 질에 영향을 미치는 관련요인 분석)

  • The purpose of this study was to examine the influential factors for the quality of life of 151 dental hygienists. Average score were job stress $2.84{\pm}0.60$, social support $3.97{\pm}0.52$ and quality of life $3.18{\pm}0.35$ In terms of the quality of life, there were significant gaps according to age, marital status, the type of workplace, education and economic status. In social support significant difference were found according to age, the type of workplace and work system. In job stress, economic status made significant differences to that. Among the sub-factors of the quality of life, the dental hygienists who worked in public dental clinics led a life of higher quality than those who worked in hospitals in terms of physical and psychological health. In terms of social relationships, the dental hygienists who received graduate-school education led a life of better quality than the college graduates. In terms of environments, the 30-34 age group led a life of better quality than the 25-29 age group, and those who worked in public dental clinics led a life of better quality than the others. Those who received graduate-school education led a life of better quality than the junior-college graduates and the college graduates, and the group whose annual income was between 30 and 39 million won led a life of higher quality than the others. In terms of the overall quality of life, the group that worked for five days a week was ahead of the other that worked for five days a week. As a result of analyzing what factors affected the quality of life, emotional support, material support were identified as the variables to have a significant impact on that.

A Study on the Interest in Hospice According to the Perception of Happiness (행복 인식에 따른 호스피스 준비의 관심에 관한 연구)

  • The purpose of this study is to examine whether there is a difference in the awareness of death based on happiness (since attitude or way of life differs among the general public, depending on the perception of death), and to investigate whether there is a difference in the interest in hospices between a group with a high happiness index and a group with a low happiness index. A survey was conducted on 200 men and women aged over 20 who participated in a walking event held in a metropolitan city on April 22, 2017. The survey consisted of eight demographic items: gender, age, education, occupation, marital status, family cohabitation, religion, and health status. It included 29 questions regarding happiness and nine questions regarding the perception of death. For the statistical tests, a chi-square test, an independent sample t-test, correlation analysis, and canonical correlation analysis were performed on the items of the survey based on a happiness index. The results of the study showed that, for a suitable place for death, medical institutions was the highest score (46.3%) for the group with a low happiness index, and own house was highest (59.4%) for the group with a high happiness index. This was a significant difference. It is thought that management systems should be supplemented by professional manpower, educating people so as to increase the utilization rates of home-based hospice as a way for patients to die in a comfortable and familiar place because, for Korea, death is much more common in hospitals. It is also necessary to increase the awareness of death and to make effective use of hospices through individual and various other approaches, considering the demographics of the subjects.

Application and development of radiation worker management program (방사선종사자 관리프로그램의 개발 및 적용)

  • The O2O service is an efficient way to operate based on systematic connection of the offline and online states. At present, education, health checkups, and exposure management of radiation workers are separated from offline and online at nuclear-related workplaces. Therefore, we developed a radiation workers management program at the workplace and tried to link online and offline states. After applying the program to the local network, we developed a written employee management system as a computer program and applied it to radiation workers and radiation safety managers at a university hospital in Daejeon using a questionnaire. As a result of the survey, the results of this program can enhance the awareness of safety management and work convenience. There is also a demand for radiation closed workers as well as radiation relative workers. It will be necessary to collect opinions regarding the characteristics of each workplace for the generalization of the radiation workers management program. As a result, real-time radiation safety management and work improvement can be expected through network connections between the workplace, supervisory authority, and related organizations in the future. If a circle structure is formed using the employee management program, development of radiation safety management can be expected.

A study on the utilization of complementary and alternative medicine for elementary children (학동기 소아에서 보완대체요법의 이용실태)

  • Purpose: Recently, complementary and alternative medicine (CAM) has been increasingly used in children. Studies have shown that 34% of adults and 11% of children use CAM in the USA and Canada. The purpose of this study was to investigate the prevalence and patterns of CAM use in elementary children in Korea. Methods:From July to August 2007, parents of elementary children completed a questionnaire survey at Gwang-ju. In all, 794 questionnaires were analyzed. Results:Of the 794 respondents, 278 answered that their pupils (35%) had experienced CAM. The following types of CAM therapy were used: herbal medicine, 62.5% dietary supplements, 31.2% vitamins, 30.2% and acupuncture, 11.1%. CAM therapies were used for the following diseases: nutritional deficiency, 33.3% atopic dermatitis, 31.3% arthralgia, 31.3% allergic rhinitis, 28.8% obesity, 26.3% and asthma. The following were the motives to use CAM: prevention of diseases (33.5%), dissatisfaction with modern medicine (21.2%), and complementary therapy to modern medicine (20.5%). People gained information about CAM through neighbors (65%) and mass media (21%). Moreover, 83 parents (30%) were satisfied with CAM because of its effectiveness. Conclusion:Many parents have advocated the use of CAM in their children. However, most of them used CAM without any prescription or adequate knowledge. Further studies are required to determine the efficacy of CAM.

Development of Nursing Process Information System for the Home Health Care (가정간호를 위한 간호과정 정보시스템 개발)

  • We carried out this study to reduce the gaps between medical institutes and between medical personnels and help to improve medical service quality, by classifying diagnoses and related intervention through the development of standard nursing intervention and by computerizing protocols. We considered two processes: one is the development process of home nursing standard intervention, and the other is the process of computerizing its related protocols. For the former, research covered analysis of home health care practices, development of client assessment protocol, of patients diagnosis protocols, and of patients intervention protocol. For the latter, strategies for home health care information systems should be set up and it constituted four research contents of analysis, design, management and evaluation of the systems. We also trained and educated home nurses who work at home health service center, by making them use the manual of home health care information systems at a certain city of P. In this study, therefore, we developed elements of standard home health care mediation so that they could be included in the forms of home health information note, home health progress note, and home health progress summary, home health discharge summary. Because standard home health care intervention has been developed, it became easier to exchange information between different home heath service center offices, can prevent from missing or redundant information, and contribute to standardization of hospital terminologies when EMR and HMR are developed.
