Analysis of Treatment Period on the Intraoral Removable Appliance Utilizing Vertical Facial Growth on Class III Malocclusion (얼굴의 수직성장을 이용하여 III급 부정교합을 치료하는 구강내 가철식 장치의 치료기간분석)

  • Vertical facial growth triggers the rotation of mandible to move the chin point to the downward and backward direction, which showed remarkably effective result making the less prominent chin. Recently, the intraoral removable appliance utilizing class III elastic demonstrated the vertical growth trigger mechanism. The treatment change was very fast and wearing was quite easy, compared to extraoral appliances. The purpose of this study was to verify the duration of the treatment on class III malocclusion using intraoral removable appliances, which designed to accelerate vertical facial growth. 56 patients were selected with the complaint of the protruded mandible and class III malocclusion (overjet : -3 - 0 mm, overbite : 0 - 4 mm). Information like; age at start, duration of the treatment events, type of the treatment, overjet, overbite etc. was collected and calculated. The average age of the patients delivering the initial brace was $8.75{\pm}1.10year$. Most of the anterior crossbite was resolved within 6 months. The total treatment period was $21.79{\pm}10.73months$ with the additional procedures like the alignment of anterior teeth and torque control using additional removable and fixed orthodontic appliances. The correlation study showed that patient's cooperation (p = 0.000) and the use of fixed appliance (p = 0.032) were significantly influenced on treatment duration.

Correlation between Dental and Skeletal Maturity in Korean Children (한국 어린이의 골 성숙도와 치아 석회화 단계 간의 상관관계)

  • The aim of this study was to investigate the relationships between the stages of calcification of various teeth and skeletal maturity stages among Korean subjects. The samples were derived from hand-wrist, panoramic radiographs, and lateral cephalograms of 743 subjects (359 males and 384 females) with ages ranging from 6 to 14 years. Calcification of seven permanent mandibular teeth on the left side were rated according to the system of Demirjian. To evaluate the stage of skeletal maturation, hand-wrist radiographs were analyzed by skeletal maturity indicators (SMI) system of Fishman and lateral cephalograms by cervical vertebral maturation (CVM) method of Baccetti. Statistically significant relationships were found between dental calcification and skeletal maturity stages according to Spearman rank-order correlation coefficients (r = 0.40-0.84, p < 0.001). The second molar showed the highest correlation and central incisor showed the lowest correlation for female and male subjects. For both sexes, canine stage G and second molar stage F were related to SMI 6 and CS 3. Because of the high correlation coefficients, this study suggests that tooth calcification stages from panoramic radiographs might be clinically useful as a maturity indicator of the pubertal growth period in Korean patients.

Immunomodulatory Effects of Euglena gracilis Extracts (Euglena gracilis 추출물의 면역조절 및 생리활성 분석)

  • Euglena gracilis is a microalga of great biotechnological interest that can create high levels of bioactive compounds, such as tocopherol, paramylon, and folic acid. The objective of this study was to investigate the biological activities of extracts from E. gracilis, especially those focused on immunological activity. E. gracilis biomass was extracted with hot water (HWE) and the remaining pellet was continuously extracted with methanol (HWME). First, we examined the effect of two extracts from E. gracilis on the production of nitric oxide (NO) and the expression of pro-inflammation cytokines, including IL-1β, IL-6, and TNF-α in murine macrophage RAW 264.7 cells. HWE treatment dose-dependently increased the production of IL-1β and TNF-α. On the other hand, treatment with HWME significantly decreased the generation of NO and pro-inflammatory cytokines (IL-6 and TNF-α) in lipopolysaccharide (LPS)-stimulated macrophage cells. In addition, other biological activities of the extracts were further analyzed: α-glucosidase inhibition, protein tyrosine phosphatase (PTP1B) inhibition, tyrosinase inhibition, xanthine oxidase (XO) inhibition, and angiotensin-converting enzyme (ACE) inhibition. Analysis of these biological activities showed that HWE has more inhibitory effects than HWME against α-glucosidase, tyrosinase, and XO agents. However, the inhibition of PTP1B and ACE with HWME were higher than with HWE. Taken together, the results suggested that E. gracilis possesses various biological activities―especially immunological capabilities―through regulation of cytokine production. Therefore, E. gracilis extract may be potentially useful for food material with immune-regulating effects.

Clinical characteristics of sleep bruxism patients with idiopathic facial pain (특발성 안면 통증 환자에서 수면 이갈이의 임상적 특징)

  • Purpose: To profile various clinical characteristics of sleep bruxism (SB) patients with idiopathic facial pain (IFP) in the orofacial region. Materials and Methods: We analyzed 28 SB patients among 210 patients with IFP complaints. The profiles were evaluated using patient charts including gender, age, pain duration, pain location, pain intensity and affected areas by pain. Results: SB with IFP occurred more often in females (85.7%) than males (14.3%). The mean age at presentation was 48.9 years. The most common IFP sites of SB patients were the right maxilla (28.6%) and the right mandible (25.0%). The pain complaints occurred mostly in 2 teeth or areas (50.0%), followed by 1 area (28.6%) and then in ≥ 3 teeth or areas (21.4%). The mean pain intensity was 5.9 on a visual analogue scale from 0 to 10. The pain was spontaneous in 20 patients (71.4%), and the mean pain duration was 24.4 months. Conclusion: Identification of clinical characteristics of SB patients with IFP could be useful in the diagnosis of various IFP patients and beneficial in decreasing unnecessary care to reduce IFP. Further studies with larger number of subjects and extended duration are required for more systemized diagnostic methods and development of future treatment guidelines.

Urinary Arsenic Species Concentrations and Related Factors among Residents Living near Abandoned Metal Mines (폐금속광산 지역 주민들의 요 중 비소종별 농도와 관련요인 평가)

  • Objectives: The purpose of this study was to evaluate urinary arsenic concentrations by arsenic species and to identify related factors among local residents near abandoned metal mines in Korea. Methods: Among the subjects of the Health Survey of Residents Near Abandoned Metal Mines for 2013-2017, 664 people were enrolled in this study. Urinary arsenic species analysis was performed using ICP/MS. Result: The geometric means (95% Confidence Interval) by urinary arsenic species were 0.15 (0.13-0.17) ㎍/L for AsIII, 0.64 (0.55-0.75) ㎍/L for AsV, and 1.21 (1.05-1.40) ㎍/L for inorganic arsenic. The geometric means of urinary MMA and DMA were 1.58 (1.35-1.86) ㎍/L and 77.93 (72.61-83.63) ㎍/L, respectively, and that of organic arsenic was 83.15 (77.80-88.88) ㎍/L. The concentration of inorganic arsenic in the group using groundwater as drinking water was 1.36 (1.13-1.64) ㎍/L, which was statistically significantly higher than the 1.00 (0.80-1.25) ㎍/L in the other drinking water groups. Regarding rice consumption, the concentration of inorganic arsenic in urine in the group whose consumption was more than half rice produced in the residential area was 1.32 ㎍/L, which was statistically significantly higher than that of the 1.12 ㎍/L for the group whose consumption was less than half. Conclusion: In the analysis of the factors affecting the urinary inorganic arsenic concentration of the residents of the abandoned metal mine area, the use of groundwater as drinking water and consumption of rice produced in the residential area were considered related factors.

Effect of applying adhesive after enamel etching on the shear bond strength of orthodontic brackets using light curing resin cements (광중합형 레진시멘트를 사용한 치열교정용 브라켓 접착 시 접착제 사용 유무가 산 부식한 법랑질의 전단접착강도에 미치는 영향)

  • Purpose: The purpose of this study is to compare the shear bond strength of resin cement for orthodontic brackets without applying an adhesive primer, to the case of applying an adhesive primer. Materials and Methods: The specimens were divided into three experimental groups, Transbond XT, GC Ortho Connect and Orthomite LC, and the enamel surface was divided into two sections, one with 37% phosphoric acid and the other with 37% phosphoric acid and an adhesive primer or universal adhesive. Each of three types of cement was applied to orthodontic bracket, and after bonding, the shear bond strength was measured. Results: Transbond XT and Orthomite LC significantly increased shear bond strength when orthodontic brackets were bonded after applying an adhesive primer and universal adhesive, respectively. Conclusion: It is expected that application of an adhesive primer or universal adhesive after acid etching will improve shear bond strength of orthodontic brackets in Transbond XT and Orthomite LC.

Difference in Bonding Strength of RMGIC according to Type of Hemostatic Agent in Primary Tooth (지혈제의 종류에 따른 레진 강화형 글라스아이오노머 시멘트 결합력의 차이)

  • The purpose of this study was to compare the effect of the hemostatic agent containing aluminum chloride with hemostatic agent containing ferric sulfate on the shear bond strength of resin-modified glass ionomer cement(RMGIC) to dentin in primary tooth. Twenty extracted non-carious human primary teeth were collected in this study. The specimens were cut to expose dentin and polished. The specimens were randomly seperated into 3 groups for treatment; group I: polyacrylic acid(PAA), RMGIC; group II: aluminum chloride, PAA, RMGIC; group III: ferric sulfate, PAA, RMGIC Ten specimens from each group were subjected to shear bond strength test. The mean shear bond strength of each group was as follows: 10.07 ± 1.83 MPa in Group I, 7.62 ± 0.78 MPA in group II, 5.23 ± 0.78 MPa in group III. There were significant differences among all groups(p < 0.001). In conclusion, both aluminum chloride hemostatic agent and ferric sulfate hemostatic agent decreased the shear bond strength of RMGIC to dentin. And ferric sulfate hemostatic agent decreased the shear bond strength of RMGIC more than the aluminium chloride hemostatic agent.

Effect of Aluminum Chloride Hemostatic Agent on Bonding Strength of RMGIC in Primary Tooth (염화알루미늄 지혈제가 유치와 레진강화형 글라스아이오노머 시멘트의 결합강도에 미치는 영향)

  • The purpose of this study was to evaluate the effect of a hemostatic agent containing aluminum chloride on the shear bond strength of resin-modified glass ionomer cement (RMGIC) to the dentin of primary teeth. Thirty-six extracted non-carious human primary teeth were collected in this study. Dentin surfaces were cut and polished. The specimens were randomly divided into 4 groups; group I: RMGIC without conditioning; group II: polyacrylic acid (PAA), RMGIC; group III: aluminum chloride, RMGIC; group IV: aluminum chloride, PAA, RMGIC. All teeth were thermocycled between 5.0℃ and 55.0℃ for 5000 cycles. Fifteen specimens from each group were subjected to shear bond strength test and 3 specimens from each group were inspected using scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy. The mean shear bond strength of each group was as follows: 4.04 ± 0.88 MPa in group I, 8.29 ± 1.40 MPa in group II, 1.39 ± 0.47 MPa in group III, 6.24 ± 2.76 MPa in group IV. There were significant differences among all groups (p < 0.001). SEM image of the dentinal tubules were partially exposed in group III and group IV. Fully exposed dentinal tubules were found in group II. In conclusion, aluminum chloride decreased the shear bond strength of RMGIC to dentin, regardless of PAA conditioning.

Investigation of the listening environment for lower grade students in elementary school using subjective tests (주관적 평가법을 이용한 초등학교 저학년 교실의 청취환경 조사)

  • The present study was conducted as a pilot investigation to suggest the standards of acoustic performance for classrooms suitable for incomplete hearing people such as children under 9 years of age. Subjective evaluations such as questionnaire and speech intelligibility test were conducted to 264 students at two elementary schools in Cheong-ju in order to analyze the characteristics of the listening environment in the classrooms of the lower grades in elementary school. The survey was undertaken with a total of 264 students at two elementary schools in Cheong-ju, and investigated their satisfaction with the classroom listening environment. As a result, students responded that the most helpful information type for understanding class content is the voice of teacher. In addition, the volume of the current teacher's voice is normal, and the level of clarity is highly satisfactory. As for the acoustic performance of the classroom, the opinion that the noise was normal and the reverberation was very short was found to be dominant in overall satisfaction with the listening environment. Meanwhile, as a result of speech intelligibility test using the word list selected for the lower grade students of elementary school, it could be inferred that the longitudinal axis distance from the sound source in the case of 8-year-olds is a factor that affects speech recognition.

A Study on Factors Related to Sleep Disordered Breathing in Children (어린이의 수면 호흡 장애 관련 위험인자)

  • The aim of this study was to investigate the risk factors associated with sleep disordered breathing (SDB) by comparing intraoral factors, body mass index (BMI), and medical history with pediatric sleep questionnaire (PSQ) findings. Seven hundred eighty-seven subjects aged between 7 to 11 years old were included. Their caregivers were asked to complete questionnaires. Oral manifestations including Angle's classification, overjet, and Brodsky tonsil grade were examined. Children with PSQ scores of more than 0.33 points were classified into the SDB high-risk group. Among the 787 subjects, 34 (4.3%) were classified into the SDB high-risk group. Children with allergic rhinitis, atopic dermatitis, excessive overjet, or large tonsil size had a significantly higher risk for SDB versus those without. Also, there was a significant difference in SDB risk according to BMI status. Gender, gestational age, breastfeeding, and Angle's classification were not associated with SDB. Children at high risk for SDB were predisposed to tonsillar hypertrophy, allergic rhinitis, obesity, and atopic dermatitis. Children with these factors could be candidates for early intervention to prevent the progression of SDB.
