  • The soft palate of carcinoma limited to the uvular region is infrequent among oropharyngeal cancers. The oropharynx regulates swallowing and speech through dynamic motions. Failure to reconstruct after surgical resection of the oropharynx structure can lead to permanent velopharyngeal insufficiency. Therefore, suitable reconstruction is important in establishing proper functional outcomes while maintaining oncological safety. We present a case of a 66-year-old male who was diagnosed with oropharynx cancer limited in the uvula accompanied by lymph node metastasis. After surgical resection, reconstruction was performed with the united arrangement of bilateral palatal mucomuscular flap and superiorly based posterior pharyngeal flap. There was no aspiration or reflux after feeding and epithelialization completely occurred after 1 month postoperatively. We report a successful case that the reconstruction with the local flap described above could preserve proper oropharyngeal function after primary surgery in small-sized oropharyngeal cancer.

  • The aim of this study was to compare the morphological characteristics of two types of stainless steel permanent molar crowns using three-dimensional scanners and the morphological changes of these crowns after crimping. Two types of stainless steel permanent molar crowns, PO-96 and PERMACROWN were scanned using three-dimensional scanner. Crown size, crown index (ratio of buccolingual diameter to mesiodistal diameter at height of contour), cervical convergency of crown were measured. Stainless steel crowns were crimped and re-scanned with three-dimensional scanner. Morphological changes of stainless steel permanent molar crowns were analyzed. As for the crown index, maxillary PERMACROWN was larger buccolingually and smaller mesiodistally than maxillary PO-96 and mandibular PERMACROWN was smaller buccolingually and larger mesiodistally than mandibular PO-96. Maxillary PO-96 was more convergent to cervical mesiodistally than maxillary PERMACROWN and mandibular PO-96 was more convergent to the cervical mesiodistally, buccolingually than mandibular PERMACROWN. Both types of stainless steel permanent molar crowns showed reduction of cervical circumference after crimping. Two products were morphologically different in crown size, shape and cervical convergence. Although both types of stainless steel permanent molar crowns are pre-contoured type, additional crimping is needed to achieve better marginal adaptation.

  • The hardening effect by ice-quenching after oxidation of a Pd-Ag-Sn-Au metal-ceramic alloy during porcelain firing simulation was investigated by means of hardness test, field emission scanning electron microscopic observations, and X-ray diffraction analysis. The hardness decreased by ice-quenching after oxidation, which was induced by the homogenization of the ice-quenched specimen. The decreased hardness by ice-quenching after oxidation was recovered from the wash stage which was the first stage of the remaining firing process for bonding porcelain. After wash stage, the hardness of the ice-quenched specimens decreased during the subsequent porcelain firing process. But the final hardness of the ice-quenched specimens after oxidation was higher than that of the specimens cooled at stage 0 after oxidation. The increase in hardness of the specimens during the first firing process was caused by the lattice strains generated at the interface between the face-centered cubic Pd-Ag-rich matrix and the face-centered tetragonal Pd3(Sn, Ga, In) precipitate. The decrease in hardness of the specimens during the remaining firing process was caused by the microstructural coarsening.
