Overview of Mitochondrial Encephalomyopathy with Lactic Acidosis and Stroke-like episodes (MELAS) syndrome (멜라스 증후군의 개요)

  • Mitochondrial Encephalomyopathy with Lactic Acidosis and Stroke-like episode (MELAS) is a rare maternally inherited disorder primarily caused by mutations in mitochondrial DNA, notably the m.3243A>G mutation in the MT-TL1 gene. This mutation impairs mitochondrial function crucial for cellular energy production, particularly in high-energy-demanding organs such as the brain and muscles. MELAS manifests as recurrent stroke-like episodes, seizures, diabetes mellitus, cardiomyopathy, and other multisystemic symptoms that are often present in childhood. The diagnosis combines genetic testing, clinical evaluation, and neuroimaging, with elevated lactate levels and characteristic magnetic resonance imaging (MRI) findings as key indicators. Treatment focuses on symptomatic management and enhancement of mitochondrial function through L-arginine, coenzyme Q10, high-dose vitamins, and taurine supplementation. Studies have identified additional genetic variants linked to MELAS, including mutations in POLG and other mitochondrial genes, further complicating the genetic landscape. Emerging therapies, particularly gene therapy and mitochondria-targeting drugs, offer promising avenues for addressing the underlying genetic defects and improving mitochondrial functioning. Furthermore, ongoing studies continue to enhance our understanding and management of MELAS, with the aim of reducing its burden and improving patient outcomes and quality of life. This review summarizes the current knowledge on the genetics, clinical features, diagnosis, and treatment of MELAS, highlighting the latest advancements and future directions for therapeutic interventions.

Implant-assisted removable partial denture restoration in small number of residual teeth in mandible: A case report (하악 소수 잔존치 환자에서 임플란트 보조 국소의치 수복 증례)

  • The patient in this case presented with a desire to have new dentures due to discomfort with existing ones. At the initial visit, all of teeth were missing except for the mandibular left second molar. As the patient was 65 years old, treatment with dentures and implant-supported prostheses was possible under the national health insurance system, and the patient opted for the mandibular denture using implant. Temporary prostheses were initially provided for patient adaptation, and following successful adaptation period, the treatment progressed. A maxillary complete denture and a mandibular implant-supported denture using two implants in the canines were fabricated. The mandibular denture is a Kennedy Class II removable partial denture which consists of a six-unit porcelain fused to metal fixed dental prostheses supported by the implant in the canines on both sides and left second molar serving as the abutments. Despite severe bone resorption and insufficient abutment teeth, the patient expressed satisfaction with the treatment results. In cases with economic and anatomical constraints affecting the feasibility of complete denture, implant-supported overdenture, and implant-supported fixed dental prostheses, an implant-assisted removable partial denture using implant surveyed crowns proves to be a viable and effective alternative treatment option. Nevertheless, the current dearth of scientifically rigorous studies underscores the necessity for meticulous regular check-up and occlusal assessment.

Single-unit fixed restoration using the automated crown shaping artificial intelligence program (자동 치관 형성 인공지능 프로그램을 이용한 단일 고정성 보철물 수복 증례)

  • Recently, several attempts have been made to integrate AI into the field of dentistry. To overcome the limitations of traditional fixed prosthetic fabrication methods such as CAD-CAM (computer-aided design-computer-aided manufacturing), AI programs are being developed for automated crown fabrication, and various studies are underway to applicate in clinical situation. In these case studies, single-unit fixed prostheses were fabricated using an AI program (Dentbird Crown, Imagoworks Inc, Seoul, Korea) in both the anterior and posterior regions and the fabrication time and accuracy were compared with previously used CAD-CAM method. The first case is a 44-year-old woman who presented for re-fabrication of a zirconia prosthesis due to a prosthesis fracture on the lingual side of the upper right lateral incisor. The second case is a 53-year-old male patient who presented for a crown restoration on an upper left first molar following root canal treatment, where he received a final zirconia restoration. In both cases, the first prosthesis was designed manually using a CAD program, the second prosthesis was designed using AI alone, and the third prosthesis was designed using AI and then modified by CAD program, and the three designs were superimposed to compare suitability. When evaluated after temporary placement, the final prosthesis demonstrates adequate stability, retention and support, resulting in functional and esthetic satisfaction.

  • This study aims to evaluate application of a heat recovery ventilator(HRV) using photovoltaic(PV) system. To this end, we analyzed performance of a PV system, which it was evaluated by monthly power wattage and conversion efficiency according to design capacity of a HRV. The results of this study can be summarized as follows. (1) A conversion efficiency of the PCS was evaluated about 86% in rated power. (2) A maximum, minimum and average output power were respectively analyzed 49.2W, 47.3W, and 48.8W. (3) Total power wattage of 200W PV system was 211kW and it was 316kW in case of 300W PV system. (4) Insufficient electrical power of a duct and window type ventilation system was respectively calculated 133.5kW and 147.7kW.

  • 현장조사학습은 학습자로 하여금 생생한 현실에 직면하게 함으로써 보고 듣고 느끼는 구체적인 학습을 통하여 생활경험과 시야를 넓히고 사회사상 간의 관계를 보다 잘 이해할 수 있게 하며 사회과 교수, 학습과정에서의 문제해결을 위해 교실을 벗어나 고장을 중심한 우리도의 현지학습을 위주로 하는 것을 말한다. 그러나 21세기 정보화시대를 맞아하여 우리는 수많은 정보를 일일이 다 경험하기에는 한계가 있으며 현실적으로도 매우 어려운 실정이다. 또한 초등학교 사회과 편성의 주요방향인 정보화능력을 바탕으로 학습자가 정보화 사회에 나아가 다양한 정보를 취득하고 활용하여 현대사회의 문제를 창의적이며 합리적으로 해결하는 능력을 길러야 한다. 이러한 능력을 기를 수 있는 기틀을 마련해 주기위해 학습자가 원하는 시간과 장소에서 자기 학습력과 속도에 맞춰 자기 주도적인 학습을 할 수 있도록 학습자에게 다양한 미디어를 결합시킴 형태의 WBI를 설계하고 구현하여 학습효과를 극대화 하고자 한다. 본 연구에서는 사회과 4학년 1학기 보조교과서 "우리 경상남도의 생활"을 재구성하여 서부경남의 7개 시 군의 교통, 관광, 인구, 특산물, 문화재, 등에 대하여 정보를 탐색, 분석, 활용할 수 있는 능력을 길러주고자 한다.

A Study for Role and Effects of Atopic Dermatitis Care Program with Korean Medicine in Public Health Center (보건소 건강증진 사업 중 한방아토피교실의 역할과 효과에 관한 연구)

  • Objectives: The purpose of this study was to evaluate the effects and develop the role and contents of the atopic dermatitis care program with Korean medicine in a public health center. Methods: The data were collected from 180 parents and 43 kindergarten teachers who participated in a child care program with Korean medicine. The program ran from April to October of 2012. The atopic dermatitis conditions of children were reported by SCORAD index; the questionnaires to measure effect and satisfaction of the program were composed of demographic items, children's life habit change checklist, etc. The questionnaires were executed before and after the program. Results: The SCORAD index total scores decreased (1.3908) after the program, especially itching scores (1.1) and sleep disorder scores (0.5693) significantly. The satisfaction with the program was 88% and 77% of parents and teachers groups, respectively. Making food and treatment were mentioned as the best program. The Children's food preference for junk food, irritability, and anger also decreased after the program. Conclusions: From this study, the atopic dermatitis care program with Korean medicine is worthwhile for treating atopic dermatitis.

A study on the optimal task-based instructional model: Focused on Korean EFL classroom practice (효율적인 과업중심 교수.학습모형 연구: EFL 교실 상황을 중심으로)

  • The purpose of this study is to present the task model that is the most effective in English language methodology based on the investigation of task-based performance in Korean EFL classroom practice. The subjects were 538 high school students and 126 high school teachers, each of whom had common experiences using the materials of task-based activities for more than one year. To analyze the data, the program SPSS WIN 11.0 including frequency distribution and chi-square analysis was used. The results of the questionnaire analysis showed that both teachers and students had a comparatively high level of satisfaction in task rationale, but that they had some mixed responses in the fields of input data, settings, and activity types. To conclude, a few suggestions are made to provide some meaningful considerations for the EFL teachers and material developers: a) task goals and rationale that encourage the learner's positive motivation; b) authenticity of input data based on the real-world context; c) collaborative learning environment that enhances communicative interaction; d) proportional representation of the creative problem-solving activities related to discussions and decision-making processes; e) systematic introduction of integrated language skills. It also suggests that the multi-lateral task model, which has some positive assets compared to previous task models, be newly introduced and applied to the second language learning classrooms.

  • This study aims to investigate the relationship between various motivational factors and to identify other causative components affecting English learners' learning motivation(From March to Jun, 2012). In case of integrative motivation and instrumental motivation, there was a statistically significant difference among those who have studied in other countries. However, in the case of negative motivation, there was a statistically significant difference among those who have no learning experience in other countries. English learners' learning motivation between the students majoring in Liberal Arts and Others, learning contents and teaching methods were significant factors.

A Study on Teacher's Perceptions and Practice of the Early Childhood Classroom as Play space (놀이 공간으로써 유아교실에 대한 교사의 인식과 실천 연구)

  • Objective: The purpose of this study is to find out how teachers organize and operate early childhood classroom, as play areas. Methods: Four teachers who had been aware of importance of play and were motivated to form an early childhood classroom as a play area were selected as research participants. Semi-structured interview and participatory observations were conducted to collected data. Results: The findings of the study are as follows. First, in regard to the role of the early childhood classroom as a play space, teachers supported the play of children and wanted to organize and operate the classroom as a play space. Second, as, play space, the teachers wanted to construct the space according to their own perception with children. Conclusion/Implications: The conclusions of this study are as follows. First, the teachers attempted to create play space by recognizing the contradictions that the classroom had. Second, the teacher organized the classroom into a play area and allowed the children to transform and create the space in their own.

Study on Development of Environmental Attention Assessment Applied in the Elementary Classroom (초등학교 교실에서 적용한 환경주의력 평가기법 개발에 관한 연구)

  • Attention plays vital role either students academic performance in classroom or work performance of workers. This study was accomplished among elementary school students of elementary school classroom for two years. Three experiment cases were designed based on the Predicted Mean Vote (PMV) indexes. Thermal environment and PMV were directly monitored; Thermal Sensation Vote (TSV) and Comfort Sensation Vote (CSV) were analyzed based on survey data; and attention was analyzed for different comfort level using FAIR program. PMV, TSV and CSV were varied with the change in thermal environment and there was not noticeable gender influence on impact. The good correlation between thermal environment, PMV, TSV and CSV confirmed the impact of thermal environment on indoor comfort. There were different impacts on attention with comfort conditions. Academic attention of low attention group i.e. weak students, can be improved by providing the comfort environment. Thermal environment influences the comfort and the comfort influences the attention, it is possible to assess the impact of thermal environment on attention in further.
