An Analysis of Clinical Research Trends on Interventions of Oriental Medicine for Postpartum Disease and Postpartum Care (산후병 및 산후관리에 대한 국내 한의학 임상 연구 동향 분석)

  • Objectives: This study was performed to analyze the interventions of Oriental Medicine which had been commonly used for postpartum disease and postpartum care. Methods: We searched research on the interventions for postpartum disease and postpartum care in 4 domestic search engines. After that, we conducted eligibility screening based on inclusion and exclusion criteria. Results: 1. We selected total 50 studies. There were 2 randomized controlled trial (RCT), 5 non-RCT, 35 case reports, 8 case series within the 6~8 weeks after childbirth. 2. Of the 35 case reports, several interventions were used : acupuncture (22), moxibustion (11), cupping therapy (7), pharmacopuncture (5), chuna manipulation (4), herbal medicine (34). The most common symptoms were musculoskeletal symptoms (8), followed by postpartum depression (7). Various prescriptions and acupoints of oriental medicine were used depending on the diseases or symptoms. 3. Of the 8 case series, 382 subjects in 5 case series had taken Saenghwa-tang-gagam. And Acupuncture, moxibustion, cupping therapy (5), pharmacopuncture (1) were used as an intervention. 4. The most commonly used acupoint is 腎兪 (BL23) in the pain including postpartum back pain and 三陰交 (SP6), 關元 (CV4) in the postpartum care. 關元 (CV4) is the most commomly used moxibustion point not only the postpartum disease but also the postpartum care. Conclusions: In clinical studies of oriental medicine related to postpartum disease and postpartum care, pain-related clinical studies that belong to or progress to Sanhupung were the most common (30%), and among them, postpartum low back pain studies were the most common (20%). Based on this, we believe that large-scale clinical studies with high quality using oriental interventions including chuna and pharmacopuncture are needed to establish guidelines for the management of pain treatment including postpartum back pain.

A retrospective study of mothers who received herbal medicine treatment using the Jeollabuk-do postpartum health support project at one korean medicine hospital (일개한방병원에서 전라북도산후건강지원사업을 이용해 한약치료를 받은 산모에 대한 후향적 연구)

  • Objectives: This study analyzed medical records and online survey of mothers who received herbal medicine treatment using the Jeollabuk-do Postpartum Health Support Project. Through this, we would like to discuss the proposal of a postpartum questionnaire of korean medicine for the initial examination, and the project direction. Methods: This study investigated the medical records of 46 mothers who received herbal medicine treatment using the Jeollabuk-do Postpartum Health Support Program at one korean medical hospital. The mother's general characteristics, past history, symptoms, and herbal medicine prescription were analyzed. In addition, through the 33 mothers who responded to the online survey questionnaire, 13 items including medication satisfaction, lactation, and side effects were investigated. Results: The average age of the subjects was 34.46±4.27 years. Musculo-skeletal pain was the most common in both medical records and online surveys. The date of visit from the date of delivery was 47.56±57.15 days. Boheo-tang was prescribed the most, and 84.8% of mothers answered positively about their health after taking herbal medicine. Conclusions: The satisfaction of mothers who received herbal medicine treatment through this project was very high. There were no significant side effects on the mother and the breastfeeding baby who took herbal medicine. In the future, support projects for postpartum korean medical treatment at the national or local level will be necessary.

Definition, Role and Method of Yinyang Pattern Differentiation (음양변증(陰陽辨證)의 정의와 기능 및 판별방법 연구)

  • In order to ensure the fact that eight principle pattern differentiation is used clinically as a basic guideline for Korean medicine practice, the definition, role and method of yin-yang pattern differentiation with its case report were explored at first. Yinyang Pattern Differentiation is a method of discriminating human tendencies or morbidity based on the yin and yang characteristics expressed in living bodies. And yin and yang are the two contrasting characteristics and aspects of the interaction when certain physical conditions that have a lasting effect on the human physiological metabolic function are correlated with the morbidity. Specific methods of yinyang pattern differentiation can be divided into several types of yin and yang indicators. First, time and space factors like day and night, hot and cold seasons, above and below, topographical districts. Second, colors and pulse and their/or relative clearness and muddiness, hardness and softness, moving and resting. Third, diagnose yin and yang patterns through distinguishing the true and false of a fever and cold in an emergency phase such as increase of brain pressure and shock state. Fourth, general characteristics of the propensity and constitution of a subject such as body type, speech, behavior, and physiological metabolism. And for clinical use, these were summarized again as a symptom indicators of physical signs and color, pulse, tongue and questionnaire indicators of propensity, body type, and space-time characteristics. Conclusively, it was confirmed that yinyang pattern differentiation has its own diagnostic significance which is distinct from exterior-interior, cold-heat and deficiency-excess pattern differentiation.

Clinical Significance of FDG PET-CT Scan at 12 Weeks after Curative Radiation Therapy in Patients with Head and Neck Cancer (두경부암 환자에서 근치적 방사선치료 후 12주 시행한 양전자방출촬영의 임상적 중요성에 대한 연구)

  • Background/Objectives: To evaluate clinical significance of FDG PET-CT for detection of residual cancer cells after curative radiation therapy or chemoradiotherapy for patients with squamous cell carcinoma (SCC) of Head and Neck Materials & Methods: A retrospective analysis of patients with SCC of Head and neck with curative radiotherapy or chemoradiotherpy between June 2011 and Jan. 2019 was performed. Sixty patients were treated with Intensity-modulated radiotherapy (IMRT). The Metabolic responses were evaluated on the post-treatment FDG PET-CT at 12 weeks after curative radiotherapy completion. Results: Median follow up was 51.5 months (3-102). The overall survival (OS), disease free survival (DFS), local control rate (LCR), and Distant metastasis free survival (DMFS) at 5 years were 80.5%, 80.1%, 87.7% and 89.1%. Metabolic CR was found in 43 (71.7%) and partial metabolic response (PR) was noted in 17 (14.6%). Metabolic CR was significantly correlated with OS, DFS, LCR, and DMFS. On multivariate analysis, Metabolic CR remained significant for DFS and LCR. Conclusion: Metabolic CR on post-radiotherapy FDG PET-CT is highly predictive of increased DFS and LCR in patients with head and neck cancer.

Analysis of the Active Compounds and Therapeutic Mechanisms of Yijin-tang on Meniere's Disease Using Network Pharmacology(I) (네트워크 약리학을 활용한 메니에르병에 대한 이진탕(二陳湯)의 활성 성분과 치료 기전 연구(I))

  • Objectives : This study used a network pharmacology approach to explore the active compounds and therapeutic mechanisms of Yijin-tang on Meniere's disease. Methods : The active compounds of Yijin-tang were screened via the TCMSP database and their target proteins were screened via the STITCH database. The GeneCard was used to establish the Meniere's disease-related genes. The intersection targets were obtained through Venny 2.1.0. The related protein interaction network was constructed with the STRING database, and topology analysis was performed through CytoNCA. GO biological function analysis and KEGG enrichment analysis for core targets were performed through the ClueGO. Results : Network analysis identified 126 compounds in five herbal medicines of Yijin-tang. Among them, 15 compounds(naringenin, beta-sitosterol, stigmasterol, baicalein, baicalin, calycosin, dihydrocapsaicin, formononetin, glabridin, isorhamnetin, kaempferol, mairin, quercetin, sitosterol, nobiletin) were the key chemicals. The target proteins were 119, and 7 proteins(TNF, CASP9, PARP1, CCL2, CFTR, NOS2, NOS1) were linked to Meniere's disease-related genes. Core genes in this network were TNF, CASP9, and NOS2. GO/KEGG pathway analysis results indicate that these targets are primarily involved in regulating biological processes, such as excitotoxicity, oxidative stress, and apoptosis. Conclusion : Pharmacological network analysis can help to explain the applicability of Yijin-tang on Meniere's disease.

A Literature Review on the Application of the Propensity Score Matching Method in the Field of Asian Oncology (한의 종양학 연구 분야에서의 Propensity Score Matching Method 적용에 대한 문헌 고찰)

  • The Randomized Control Trial (RCT) is the most well-established and widely used statistical methodology in clinical research; however, applying thorough RCT to cancer patients presents challenges such as ethical concerns, high costs, short clinical periods, and limitations in collecting various side effects. To address this issue, the propensity score matching method, which takes advantage of the benefits of observational research while compensating for the drawbacks of randomized control trials, is used in a variety of fields. In recent years, 28 studies on the effectiveness of Korean medicine on tumors have been conducted abroad using the Propensity Score Matching Method, but none have been conducted in Korea. The majority of studies have focused on liver cancer, colon cancer, lung cancer, and stomach cancer, with endpoints such as survival time, incidence rate, quality of life, and treatment outcomes revealing statistical differences in how Korean medicine intervention affects treatment outcomes. As a result, well-established studies using the propensity matching score methodology should be useful in evaluating the impact of Korean medicine in oncology treatments.

A Study on the Trend in Korean Medicine Research of Irritable Bowel Syndrome in Korea, Japan and China (한국, 일본, 중국 3국의 과민성 대장 증후군에 대한 한의학적 연구동향 분석)

  • Objectives : The purpose of this study was to analyze the papers from 2011 to 2020 to study on the trends of irritable bowel syndrome (IBS) in Korea, Japan and China. Methods : In order to analyze the research trends on IBS in Korea, Japan and China, domestic and foreign databases were selected to provide academic resources to the common use of telecommunication lines represented by the Internet. The latest research trends were determined by limiting the 10-year period from January 2011 to December 2020. Results : In the past decade, six IBS-related papers have been searched in Korea, six in Japan and 458 in China. Although not many studies have been found in Korea and Japan to understand the research trends, there have been many clinical reports, and researchers are more interested in herbal medicine than acupuncture treatment. In China, where many papers were searched, there were more papers related to herbal medicine than acupuncture treatment. Acupuncture treatment was highly interested by researchers in certain acupoints and stimulation methods, and herbal medicine was found to seek diversity in drug prescription and use. Conclusions : In this study, we were able to understand the research trends of Korea, Japan, and China, and provided information that could be useful as a basic data for establishing the direction of Korean medical research on IBS in the future.

Assessing Spatial Accessibility to Pediatric Dental Clinics Using the Geographic Information System (지리 정보 시스템을 활용한 소아 전문 치과의 공간적 접근성 평가)

  • The purpose of this study was to evaluate the accessibility of pediatric dental clinics and quantitatively compare the regions with the best and worst accessibility to pediatric dental clinics in South Korea using the geographic information system. Seoul city showed the highest density of pediatric dental clinics, while Gyeongbuk and Jeonbuk provinces showed the lowest density. Seoul city had a service area ratio of 100% in all districts on a 10 km basis. In Gyeongbuk and Jeonbuk provinces, more than half of the districts had a service area ratio of 0%. In Seoul, the average distance from elementary school to the nearest pediatric dental clinic was 3.45 km. The average distance values were 43.66 km and 40.57 km in Gyeongbuk and Jeonbuk provinces, respectively, which were 12.66 times and 11.76 times longer than the average distance in Seoul city.

Orostachydis Herba and Fermented Orostachydis Herba Enhances Anti-Inflammatory and Anti-oxidant Effect against Lipopolysaccharide-Induced Acute Liver Injury in Mice (Lipopolysaccharide로 유발한 급성 간손상 마우스 모델에서 와송과 발효 와송의 항산화 조절과 염증 예방 효과 비교 연구)

  • Objectives : This study aimed to evaluate the protective effect of Orostachydis Herba (OH) and Fermented OH (OHF) against the acute liver injury by lipopolysaccharide (LPS). Methods : OHF by 4 lactic bacteria such as (Lactobacillus hilgardii (OHF1), Leuconostoc mesenteroides (OHF2), Pediococcus acidilactici (OHF3), Saccharomyces cerevisiae (OHF4)) were prepared. Samples were selected to OHF0, OHF2, OHF3 based on UPLC analysis, DPPH, ABTS radical scavenging activities. To evaluate the protective effect of OHF on liver injury mice, ICR mice were divided into 5 groups: Normal mice (Nor), LPS (20 mg/kg) treated mice (Veh), administrated OHF0, OHF2 OHF3 200 mg/kg body weight during 8 days before LPS injection. Serum and liver were collected 24 hours after LPS injection. Results : The activity was high in order of OHF0 and OHF3 in DPPH and ABTS radical scavenging activities. The quercetin contents for bioactive ingredient of OH was 5.39, kaempferol contents was 9.94 by UPLC analysis. The LPS-treated vehicle group significantly increased liver weight, and aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels in serum. In contrast, administrated OHF3 group decreased liver weight, AST, ALT. In addition, OHF3 groups reduced the elevated levels of reactive oxygen species (ROS) in serum and tissues. Moreover, AP-1, iNOS and COX-2 were significantly decreased in OHF2 and OHF3. But $NF-{\kappa}B$ p65 and $TNF-{\alpha}$ only showed a significant reduction in OHF3. Conclusions : Therefore, these results suggest that fermented Orostachydis Herba might be protective effect on liver injury through anti-oxidant effect.

  • Ankylosing spondylitis causes ankylosis of the spine due to ossification of the articular cartilage and ligaments around the vertebral body as well as the sacroiliac joint. This pathophysiology limits joint movement and, in many cases, causes pain and deformity of the spine. If this disease is left untreated, it ultimately causes ankylosis and ossification of the whole-body joints. The symptoms generally develop before age 30 years, and the gradual progression of the disease adversely affects the physical function, professional ability, and quality of life. This increases the likelihood of developing psychiatric disorders, such as depression. The authors are aware of this severity and introduce recent trends and studies to prevent surgical treatment with various medications before systemic ossification. This paper presents various surgical treatments and complications in patients who were unable to prevent progression and underwent surgical treatment.
