Arthroscopic Distal Clavicle Resection for Chronic Stage 2 Acromioclavicular Dislocation - Preliminary Report of 6 Cases Study - (관절경하 원위 쇄골 절제술을 이용한 만성 제 2형 견봉쇄골 손상의 치료 - 6례에 대한 예비 연구 -)

  • Purpose : To evaluate effectiveness of arthroscopic distal clavicle resection for chronic stage 2 acromioclavicular dislocation. Material: 6 patients who had failed to respond to physical therapy were evaluate more than 6 months prospectively, which were treated with arthroscopic distal clavicular resection from february 1998 to January 2001. Result : The median preoperative Constant score changed from 32 of 100 to 37 of 100 at 3 to 6 weeks, to 49 of 100 at 3 months and to 69 of 100 at 6months. Improvement was achived at mean follow up more than 6 months after surgery. Overal $83\%$ of patient were satisfied with this procedure. but the remained case was unsatisfied because symptom was not improved more than 6 months Conclusion : Arthroscopic distal clavicle resection is useful method to treatment for chronic stage 2 acromioclavicular dislocation.

Adaptive control of flexible joint manipulators based on the singular perturbation theory (특이 섭동 이론에 의한 유연성 관절 매니퓰레이터의 적응제어)

  • The adaptive control of flexible joint manipulator is the focus of this paper. The full order flexible joint manipulator dynamic system does not allow the determination of a feedback linearization control as for rigid manipulators. This drawback is overcome by a model order reduction based on a singular perturbation strategy. The full order flexible joint manipulator dynamic model is adopted for derivation of the adaptive control law to damp out the elastic oscillations at the joints. It is shown that the joint position error will converge to zero asymptotically and that other signals remain bounded without precise knowledge of parameters of the manipulator and its joint flexibility.

  • This paper presents an adaptive control scheme for flexible joint robot manipulators. This control scheme is based on the Lyapunov direct method with the arm energy-based Lyapunov function. The proposed adaptive control scheme uses only the position and velocity feedback of link and motor shaft. The adaptive control system of flexible joint robots is asymptotically stable regardless of the joint flexibility value. Therefore, the assumption of weak joint ealsticity is not needed. Also, joint flexibility value is unknown. Simulation results are presented to show the feasibility of the proposed adaptive control scheme.

  • We experienced rare cases of Snapping plicae on radiohumeral joint which arthoscopic plicae resection improved symptom of painful snapping elbow. We report that Arthroscopy can be used as a effective therapeutic and diagnostic method for snapping plicae on radiohumeral joint with a review of its literature.

The Relationship between Physical Characteristics and Walking Ability in Elderly: A Cross-Sectional Study (노인들의 보행 능력과 신체적인 특성 간의 상관관계: 단면 연구)

  • This study aimed to investigate the relationship between physical characteristics and walking ability in the elderly population. Subjects were 77 elderly (38 men and 39 women) who are capable of walking independently with and without walking aids. Correlation and stepwise multiple linear regression analyses were used to analyze the relationship between physical characteristics (age, gender, height, weight, body mass index, muscle mass, waist/hip ratio, heart rate, vital capacity, flexibility, maximum oxygen consumption, one-leg standing time, and strength of knee flexor and extensor) and walking velocity of subjects. Age, height, vital capacity, one-leg standing time, and strength of knee flexor and extensor showed significant correlations with walking velocity of subjects (p<.05). Further, the strength of knee flexor explained 27% of the variance, and up to 32% of the walking velocity could be explained when the strength of knee extensor were added to the model. The findings suggest that walking velocity of elderly depends on the strength of lower limb's strength and a variety of physical characteristics.

Assessment of Elderly's Isokinetic Muscle Function, Flexibility and Balance in a Region of Seoul (서울 일부 지역 노인들의 등속성 근기능, 유연성, 균형성 측정 및 평가)

  • The aim of the present study was to compare the strength, balance, and flexibility across two age groups (older adults vs. young adults). The isokinetic muscle function, ankle ROM, trunk forward flexibility, stability, and antropometric data for the elderly and 46 university students were collected. The results indicated that male older adults possessed relatively low flexibility (2.97cm) among the groups and showed better stability in the right foot than in the left foot and $35^{\circ}$ of plantar flexion ROM, which was not in the normal range. Their peak strength at the knee joint was below 50% of their counterpart. They revealed a hamstring deficit of 18.55%, ankle eversion deficit of 23.08%, and ankle inversion deficit of 19.19%. The results indicated that female older adults possessed comparable flexibility compared to female young adults. The reciprocal muscle strength ratio of both knees was under 50%, and the deficit was 14.32% (extension) and 19.73% (flexion). The ankle plantar flexion peak torque was approximately 62% (left) and 73% (right) of WS's. The ankle dorsi flexion deficit was 25.05% and the plantar flexion was 26.86%. The eversion deficit was 19.97% and the inversion was 21.09%. These results will be significant in establishing an elderly fitness enhancement program and policy.

Effect of the Prevention Programs for Musculoskeletal Disorders in one Farming Village (일부 농업인에서의 근골격계 질환 관리 프로그램의 효과)

  • 일개 농촌 마을에서 농한기를 활용하여 시행된 근골격계 질환 예방사업을 소개하고 주요 사업결과를 분석하여 농업인의 건강증진을 위한 기초자료로 활용하고자 본 연구를 수행하였다. 2006년 12월 5일부터 2007년 1월 26일까지 사곳리 마을회관을 12회 방문하여 4번의 기초교육과 5번의 심화교육 등 9번의 근력 및 유연성 강화교육을 실시하였다. 교육 전(사전평가), 기초교육과 심화교육 사이(중간평가), 교육 후(최종평가) 등 3회에 걸쳐 의사 2인에 의한 근골격계 질환 평가, 시각통증척도에 의한 근골격계 통증 평가, 생활체육 전문가 3인에 의한 좌우의 악력, 배근력, 하지근력, 몸통, 어깨, 하지, 무릎유연성 측정을 통한 근골격계 근력 및 유연성 평가, 설문지를 사용한 어깨, 허리, 무릎의 기능 평가를 실시하였다. 총 12회의 일정 중 1회 이상 참석한 농업인은 총 57명이었으며 남자가 20명(35.1%), 여자가 37명(64.9%)이었으며 평균 연령은 60.7(±8.1)세였다. 근골격계 질환 검진에서 한 번 이상의 검진을 받았던 43명중 32명(74%)이 한 가지 이상의 근골격계 질환을 가지고 있었으며, 퇴행성관절염이 26명(60.5%), 근막통 증후군이 19명(44.2%), 허리디스크가 10명(23.3%)이었다.목어깨, 허리, 무릎다리의 통증정도는 1차, 2차 평가 간, 2차, 3차 평가 간, 1차, 3차 평가 간의 비교에서 모두 감소하는 경향을 보였으며 3차 평가에서 1차 평가에 비해 목어깨, 허리, 무릎다리 모두 유의한 통증의 감소를 보였다(p<0.05). 근골격계의 기능을 1차, 2차 평가 간, 2차, 3차 평가 간, 1차, 3차 평가 간 비교한 결과 모두 기능점수가 우수하게 변화했으나 통계적으로 유의하지 않았다. 근력 및 유연성 정도는 좌우의 악력, 배근력, 하지근력, 몸통, 어깨, 하지, 무릎유연성을 측정하였으며 1차 평가에 비해 3차 평가에서 모든 항목에서 근력 및 유연성 정도가 통계적으로 유의하게 증가하였다(p<0.05).연구대상자의 수가 적었고, 농한기에 의한 근골격계 질환의 개선 효과를 배제할 수는 없었지만 이번연구를 통하여 일부 농업인에서 근골격계 질환 관리 프로그램 실시 후 근골격계 질환의 단기적인 개선효과를 볼 수 있었다. 근골격계 질환의 중장기적인 개선효과를 기대할 수 있는 추가적인 연구와 농번기를 통한 관리 프로그램에 대한 추가적인 연구가 필요하다.

Effects of Lower Extremity Exercise Program on Muscle Strength and Flexibility in Women with Knee Osteoarthritis (슬 골관절염 여성을 위한 하지 운동프로그램이 근력과 유연성에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of a lower extremity exercise program on muscle strength and flexibility in women with knee osteoarthritis. Method: This study was conducted using a non-equivalent control group pretest-posttest design. The participants were 35 women with knee osteoarthritis. The experimental group performed the lower extremity exercise program, which was an adapted BeHaS exercise program developed by Kim(2006). It consisted of approximately 65-130 minutes per one session, two days a week, for twelve weeks. Lower extremity muscle strength was measured by the Seated Leg Press Machine (TY-H2217, made in Korea), back muscle strength by back muscle strength scale (TKK 5402, made in Japan) and flexibility, by Sit and Reach tester (01285, U.S.A). Data were gathered from September 2005 to March 2006. Frequency, $X^2$ test and t-test were used to analyze the data. SPSS Win 11.0 was used to assist analysis. For normal distribution of data, Kolmogorov-Smirnov test was done. Results: There were statistical differences in the mean strength of both legs together, strength in leg with more pain(p=.044; p=.046), sit and standing time (p=.007), and back strength (p=.021). Conclusion: Based on these results, it can be concluded that a lower extremity exercise program can help improve lower extremity muscle strength in women with osteoarthritis.
