Clinical Results of the Radial Tear of Posterior Root of Medial Meniscus (내측 반월상 연골 후각부 방사형 파열의 임상적 결과)

  • Purpose: To examine the clinical results after arthroscopic meniscectomy of radial tear of medial meniscus. Materials and Methods: We studied 45 cases with the radial tear of medial meniscus which follow up more than 2 years(range 2 to 7 years). Arthroscopic surgery was performed to the patients with grade 0-2 according to the Kellgren and Lawrence classification. Evaluation of cartilage damage was performed on surgical photos according to Outerbridge classification. Evaluation of clinical result was used the modified Lysholm score. Results: The mean pre-operation Lysholm score was 79.1, 71.2, 68.5, 67.9, 67.2, 61.5 and post-operation Lysholm score was 86.3, 75.1, 73.0, 73.1, 73.2, 66.2 and 61.5%, 62.5%, 60.0%, 50.0%, 50.0%, 25.0% improved knee pain and 69.2%, 75.5%, 70.0%, 66.6%, 75.0%, 75.0% were satisfied knee surgery and 30.7%, 20.5%, 20.0%, 50.0%, 25.0%, 50.0% required further surgery in patients respectively. According to Kellgren and Lawrence classification, 7 cases(15.5%) progress grade 3 osteoarthritis. Conclusions: The radial tear of medial meniscus showed the poor results with arthroscopic meniscectomy even if the grade 0-2 osteoarthritis. For the improvement of the clinical results, consider the technique to restore the hoop stresses or use the high tibia osteotomy for preventing the osteoarthritis.

Factors Affecting Clinical Results of the Meniscal Repair (반월상 연골 손상의 봉합치료 결과에 영향을 미치는 인자)

  • Purpose : Clinical outcome of meniscal repair fur meniscal injury was examined and the factors affecting the outcome were determined. Materials and Methods : We examined 55 patients with meniscal injury who underwent arthroscopic meniscal repair and could be followed-up for more than 1 year. Clinical outcome according to Tapper & Hoover classification was examined in relation with age, gender, injury site, the presence of bucket-handle tear, method of repair, the period between injury to repair, and the concomitant reconstruction of anterior cruciate ligament. Results : According to Tapper & Hoover classification, the outcome was excellent in 29 cases, good in 23 cases and fair in 3 cases. Age and method of repair have significant relationships with clinical results according to Tapper & Hoover classification, but gender, injury site, the presence of bucket-handle tear, the period between injury to suture, and the concomitant reconstruction of anterior cruciate ligament have no significant relationships. Conclusion : A satisfactory results was obtained in $95\%$ of the patients with meniscal injury with meniscal repair. Age at injury and method of repair have significant relationships with clinical result statistically.

Arthroscopic treatment of septic arthritis of the knee in adults (성인의 화농성 슬관절염의 관절경적 치료)

  • Purpose : The purpose was to assess the result of arthroscopic management of the septic arthritis on the knee in compromised patients. Materials and Methods : Fourteen patients with septic knee were analyzed. The mean age was 55 years and the mean follow-up period was 14.6 months. Underlying diseases included 4 cases of diabetes, and history of direct acupuncture in 4 cases. Clinical stage of septic arthritis was judged by $G\ddot{a}chter's$ classification, which was determined by arthroscopic findings. After arthroscopic irrigation and debridement, we observed the results of laboratory data and improvement of clinical findings. Results : Causative organism was identified in 7 cases and no organism was detected in the remaining 7 cases. Stage I was 1, stage II 8, stage III 4, and stage IV 1, respectively. Eleven of 14 cases were improved by one stage operation. Two cases of stage III were recurred and additional arthroscopic management was done. In 1 case of stage IV, symptom was not improved and needed arthrotomy. The result was unsatisfactory in patients with stage III and IV. Serum erythrocyte sedimentation rate and C-reactive protein were normalized after 29.3 and 20.8 days following the operation, respectively. Clinical symptoms disappeared average 2 days following the operation. Conclusion : Arthroscopic management of acute septic arthritis of the knee would be an effective and satisfactory treatment modality in that its postoperative pain and complications are minimal, and it can be done with ease repeatedly.

Arthroscopic Management for the Treatment of Ankle Gutter Syndrome (족 관절 구 증후군에 시행한 관절경적 치료)

  • Purpose : The purpose of this study was to evaluate the gutter pathologies and the clinical results of arthroscopic treatment for ankle gutter syndrome. Materials and Methods : Seventy six patients(77 ankles) who had lesions in the ankle gutter were treated by arthroscopy. The gutter pathology could be divided into 3 categories; hypertrophic scar tissue, loose bodies and bone spurs. Fifty two patients were evaluated subjectively and functionally with authors' criteria. The follow-up period averaged 44 months(range, 2.9 year-8.5 year). Results : The incidence of the gutter lesion was $21\%$(77 ankles) among 366 ankles undergoing arthroscopy. The most common pathology was hypertrophic scar tissue. In 31 ankles$(40\%)$, the lesions were found only in gutter, and 46 ankles$(60\%)$ were associated with pathologies in other areas. All of parameters for subjective and functional evaluation were improved with statistical significance(p<0.001). Seventeen patients$(33\%)$ could returned to competitive or contact sports activity with same level of premorbid period. Conclusion : Ankle gutter is an important site of pathologies which cause chronic ankle pain, and should be explored in ankle arthroscopy. The ankle arthroscopy is a very effective treatment method fur the removal of pathologies in ankle gutter syndrome.

Prognostic Factors after Arthroscopic Treatment of Infectious Knee Arthritis (감염성 슬관절염의 관절경적 치료 이후 예후 인자에 대한 분석)

  • Purpose: This study examined the effects of gender, age, underlying disease, duration after onset of symptoms, preoperative invasive procedures, bacterial culture of joint fluid, and stage of infection by the Gachter classification on the prognosis of patients with infectious knee arthritis who underwent arthroscopic surgery. Materials and Methods: From June 2014 to December 2016, 51 patients who underwent arthroscopic surgery for infective knee arthritis were enrolled in this study. The average follow-up period was 14.2±2.1 months (range, 12-20 months). The subjects were 27 men (52.9%) and 24 women (47.1%), with an average age of 55.1±17.6 years (range, 13-84 years). A preoperative evaluation of the joint aspiration with a count of more than 50,000 leukocytes and a polymorphonuclear leukocyte count of 95% or more was performed. All patients underwent arthroscopic surgery and postoperative continuous joint irrigation. Results: The initial mean value of the C-reactive protein decreased from 9.55±6.76 mg/dl (range, 1.51-31.06 mg/dl) to a final mean of 0.74±1.26 mg/dl (range, 0.08-6.77 mg/dl); the mean duration of C-reactive protein normalization was 27.6±18.9 days (range, 8-93 days). Among the 51 patients who received arthroscopic surgery and antibiotics, 44 patients (86.3%) with infectious knee arthritis completed treatment with improved clinical symptoms, such as fever, pain, and edema, and the C-reactive protein decreased to less than 0.5 mg/dl. Finally, 5 cases were treated with two or more arthroscopic operations, and 2 cases were converted to arthroplasty after prosthesis of antibiotic-loaded acrylic cement. Conclusion: The duration of surgery after the onset of symptoms and the stage according to the Gächter classification are important prognostic factors for predicting the successful treatment of infectious knee arthritis. On the other hand, the other factors were not statistically significant. Nevertheless, patients with bacteria cultured from the joint fluids appear to reflect the treatment period because the period of normalization of the C-reactive protein is shorter than that of the control group.

Arthroscopic Outside-in Repair of Tears of the Triangular Fibrocartilage Complex (삼각 섬유연골 복합체의 파열에 대한 관절경적 Outside-in 봉합술)

  • Purpose: We repoted the clinical results and introduction of surgical techniques for the patients with tears of the triangular fibrocartilage complex (TFCC) who had arthroscopic management. Materials and Methods: According to Palmer classification, nine patients(9 wrists) with type 1B lesions of the triangular fibrocartilage complex from 2001 to 2004 were included in this study. The mean follow-up period was 28 months. Six were male and three were female, right side was in seven cases and left side in two cases. Nine patients received arthroscopic debridement and repair by outside-in technique. To assess the clinical outcomes, the visual analogue pain scale(VAPS), grip strength, range of motion were investigated. Results: The average of VAPS improved from 6.2 to 1.4. The grip strength of wrists of all nine patients improved after surgery and the range of motion of all wrists reached full range. Conclusion: Arthroscopic repair of type 1B tear of the triangular fibrocartilage complex has satisfactory results. The repair by outside-in technique was simple and we could have secure fixation.

MRI Follow-up Study After Arthroscopic Repair of Multiple Rotator Cuff Tendons (다발성 회전근 개 파열에서 시행한 관절경적 회전근 개 복원술 후 MRI 추적 검사)

  • Purpose: This study investigated the rate of retear and related factors after arthroscopic repair of rotator cuff tears involving more than one tendon. Materials & Methods: Arthroscopic repair of 22 rotator cuff tears (average size 3.2cm: average age 58 years old) involving the supraspinatus and part or all of the infraspinatus were investigated using MRI on average 10 months after repair. The status of the repaired cuff was investigated using Sugaya's classification, and the change in muscle was evaluated with Goutallier's classification. Results: Retear (Sugaya grade IV, V) was found in 7cases(32%). Tears larger than 3cm had a higher retear rate(67%) than smaller tears(8%). Retear cases had Goutallier grade II or higher muscle changes preoperatively and showed aggravation of muscle atrophy postoperatively. Even without retear, reversal of muscle change was not seen Conclusion: Rotator cuff tears not confined to the supraspinatus had a 32% retear rate after arthroscopic repair. The size of the tear was the most crucial factor influencing retear. Retear was frequent in tear over 3cm. Atrophy of the cuff muscle worsened when the repair failed but did not improve even without retear.

Clinical results of arthroscopic meniscal repair according to joint stability (반월상 연골의 관절경적 봉합시 관절안정성에 따른 결과)

  • Purpose : The purpose of this study was to evaluate the results of arthroscopic meniscal repair according to joint stability. Materials and Methods : Twenty cases were reviewed, which had underwent arthroscopic meniscal repair. The mean age was 32.3 years old. The mean follow-up period was 59.7 months. Menisci that had underwent complete repair of associated ligament injuries (8cases, Sa) and menisci that had no associated ligament injury (9 cases, Sb) were classified as stable group (S), and the others (3 cases) as unstable group (U). Also stable group was divided into acute and chronic group. The result was evaluated with Lysholm score and IKDC method. The statistical analysis was done using Wilcoxon rank sum test and Fisher's exact test (p<0.05). Results : Group Sa had $87.5\%$ satisfactory and Lysholm score was 90.9. Group U had $66.7\%$ satisfactory, and Lysholm score was 77.7. Group Sb had $89.9\%$, satisfactory and Lysholm store was 91.4. In acute group $91.7\%$ was satisfactory, Lysholm score 92.5, in chronic group $80\%$ was satisfactory, Lysholm score 88.6. Conclusion : Joint stability was important factor for the outcome of meniscal repairs. So, it is desirable to repair meniscus injury early and the repair of associated ligament injuries should be performed together.

Arthroscopic Reduction and Internal Fixation of Intra-articular Fractures of Lateral Tibial Plateau (관절면을 침범한 경골 외과 골절의 관절경적 정복 및 내고정술)

  • Purpose: This study is to analyze the clinical and radiological results after arthroscopic reduction and internal fixation of intra-articular fractures of lateral tibial plateau. Materials and Methods: The subject of the study are the 13 cases of the patients visited orthopedics surgery during March year 2000 to August year 2004 because of intra-articular fractures of lateral tibial plateau and were treated with arthroscopic reduction and internal fixation. X-rays and CT or MRI were both carried out to identify the precise pattern of fracture and the degree of depression which showed out to be all type 2 by Schatzker fracture classification. And in 9 of the cases, autogenous and allogenous bone grafts were given as bone loss were severe. The average age was 48, age group between 31 and 66, and average follow up period of about 38 months ($13{\sim}65months$). Radiological ratings were given by comparing the X-rays of degree of joint congruency before and after the operation, functional ratings by analyzing IKDC score and Lysholm score. Combined injuries observed after arthroscopy were posterior cruciate ligament injury in 1 case, meniscus injury in 4 cases and medial collateral ligament in 2 cases. Results: During follow up, X-rays showed well-maintained reduction of articular surface in all cases and no complications such as joint depression, fracture reduction loss, angular deformity or malunion were found. Average Lysholm score at last follow up was 87 points ranging from 65 to 97, in 8 of the cases excellent, 3 good, 1 fair and 1 poor according to Lynsholm classification. Average IKDC score was 92 (from 82 to 99). Conclusion: Not only does arthroscopic reduction of lateral tibial plateau fracture bring exact reduction of articular surface, but also, is considered to be a good way of operation to diagnose and treat combined injuries of knee joint using arthroscopy.
