관상동맥 조영술 후 불편감에 영향을 미치는 요인 (Factors Predicting Patient Discomfort after Coronary Angiography)

  • Purpose: The purpose of this study was to identify the factors that predict discomfort after coronary angiography or percutaneous coronary intervention (PCI) among hospitalized patients. Methods: A total of 203 patients who underwent coronary angiography or PCI were recruited from C hospital located in S city, J province, from June through August 2008. The level of discomfort was measured and standardized by two instruments, discomfort questionnaire and the Visual Analogue Scale (VAS). Results: Stepwise multiple regression showed that the factors predicting the level of discomfort were type of angiography, gender, previous angiography, dysuria, pre-information, and sleep satisfaction, which together explained 30.6% of the total variance of the level of discomfort. Conclusion: Patients who had previous experience with these procedures, received a pre-information about the upcoming procedure, had no dysuria, and had slept well after the procedure were less likely to complain discomfort. Pre-informed education should be given by nurses to patients who will have an angiography or PCI to reduce their physical and emotional discomforts.

성인과 노인 관상동맥증후군 초발 환자의 임상적 특성 및 치료추구행위 비교 (Comparison of Clinical Manifestations and Treatment-Seeking Behavior in Younger and Older Patients with First-time Acute Coronary Syndrome)

  • Purpose: This study was conducted to examine and compare clinical manifestations and predicting factors for treatment-seeking delay among patients <65 and ${\geq}65\;yr$ with first-time acute coronary syndrome (ACS). Methods: A total of 288 patients who were diagnosed with ACS were individually interviewed at C university hospital in G-city from November 2007 to December 2008. Results: Median pre-hospital delays for younger and older patients were 5 and 12 hr, respectively. Younger patients were more likely to be current smokers, heavy drinkers, obese, stressed, and have an unhealthy diet and family history, and to complain of chest pain, left shoulder and arm pain, perspiration, and nausea. Older patients were more likely to have hypertension and diabetes, and to complain syncope and dyspnea. Logistic regression analyses showed that after adjustment for age, gender and education, progressive onset of symptom and no attribution to cardiac problem significantly predicted pre-hospital delay >3 hr in both younger and older patients. Low perceived health status was a significant independent predictor in older patients only. Conclusion: Health care providers should be concerned with different manifestations between younger and older adults, and educate people at risk for heart attack about symptoms and actions to get immediate help.

경피적 관상동맥 중재술을 받은 노인환자의 자가간호 교육프로그램 개발 및 평가 (Development and Evaluation of a Self Care Education Program for Elderly Patients with Percutaneous Coronary Intervention)

  • Purpose: The purpose of this study was to identify the effects of the education program to promote self care for elderly patients with percutaneous coronary intervention. Method: The research was a quasi-experimental study using a nonequivalent control group non-synchronized design. The participants was 56 patients, 28 in the experimental group and 28 in the control group. A pretest and a posttest were conducted to measure main variables. The self care education program, consisting of flash animation, leaflet and telephone counseling, was given for the experimental group. Data were analyzed using ${\chi}^2$, Fisher's exact test, and independent t-test with SAS statistics program. Results: There were statistically significant increase in knowledge and self care behavior, and decrease in anxiety by difference between a pretest and a posttest in the experimental group compared to that in the control group. Conclusion: The result of this study indicate that the self care education program is effective in increasing knowledge and self care behavior, and in decreasing anxiety for elderly patients with percutaneous coronary intervention. Therefore, it can be usefully utilized in the field of nursing for elderly patients with percutaneous coronary intervention.

관상동맥질환자의 금연변화단계와 관련된 요인 및 장애요인에 대한 전향적 연구 (Prospective Study on the Relating Factors to the Stages of Change in Smoking Cessation and Barriers in Coronary Artery Disease Patients*)

  • Purpose: The main purpose of this study was to investigate that the stages of change in smoking cessation behavior among coronary artery disease patients for six months progressed following the stages of change suggested by the transtheoretical model. Method: Subjects for this descriptive survey were 59 coronary disease patients who were smoking or who had stopped smoking for less than six months. Result: In the baseline, the distribution of the subjects’ stages of change was as follows: pre-contemplation stage 25.4%, contemplation stage 25.4%, preparation stage 22%, and action stage 27.1%. After six months, more subjects in the contemplation(33.3%) and preparation stages(30.8%) progressed to the action stage than those of the pre-contemplation stage(0%). Eighty-one percent of the subjects in the action stage at baseline progressed to the maintenance stage. The relationship between the numbers of smoking cessation attempts for six months and stages of change at baseline was significant(p=.001). However, the relationships between self-efficacy and nicotine dependence at baseline and progression in stages of change after six months were not significant. Conclusion: Progression in the stages of change for six months among subjects corresponded to the stages of change suggested by the transtheoretical model. Hence, future development and evaluation of intervention programs should be tailored individually considering each patient's stage of change.

노인 관상동맥질환자의 스트레스 측정도구 개발 (Development of a Stress Scale for Elderly Patients with Coronary Artery Disease)

  • Purpose: The purpose of this study was to develop a scale to evaluate stress in elderly patients with coronary artery diseases (CAD) and to examine validity and reliability of the scale. Methods: The development process for the preliminary scale included construction of a conceptual framework and initial items, verification of content analysis, sentence correction, and pilot study. This study was conducted using a questionnaire survey with one-to-one interviews during January and February, 2012. Participants were 240 elderly patients with CAD. Data were analyzed using item analysis, factor analysis, criterion related validity, and internal consistency. Results: The developed scale consisted of 32 items and 6 factors - aging and disease (7 items), family relations (5 items), anxiety and withdrawal (9 items), management of daily living (3 items), compliance of medical regimen (4 items), poverty and finance (4 items), and explained 68.5% of total variance. The scale had significantly positive correlation with the Korean Perceived Stress Scale (KPSS). Cronbach's alpha was .96, and Guttman split half coefficient was .91. Conclusion: Results indicate that the Stress Scale for Elderly Patients with CAD has validity and reliability, and is a suitable scale in health care settings to assess stress in elderly patients with CAD.

급성관상동맥증후군 남성 환자의 흡연과 스트레스에 영향을 미치는 요인 (Predicting Factors of Smoking and Emotional Stress among Male Patients with Acute Coronary Syndrome)

  • Purpose: This study was conducted to identify the factors that predict a current smoking behavior and higher emotional stress among male patients with acute coronary syndrome (ACS). Methods: The study was approved by an institutional review board from a university hospital, 2010. A face to face interview using questionnaires was performed with 185 first-time ACS male patients who were undertaken a percutaneous coronary intervention at a cardiovascular care unit. Data were analyzed using SPSS/WIN 15.0. Results: About 54% of the study subjects were currently smoking. The current smokers had dyslipidemia and reported bad eating habits compared to the non-smokers. The current smokers were younger, living alone, and reported lower perceived benefit on smoking cessation than the non-smokers, and 15% of them did not consider quitting (precontemplation stage). Smoking status was not significantly related to emotional stress. Logistic regression analysis revealed that being employed including professional or labor increased the odds of current smoking four or three times compared to the non-employed or retired. Low income or dyslipidemia also increased the likelihood of current smoking 2.8 and 2.1 times, respectively. Blue collar workers or heavy drinkers had 2.9 and 2.8 times more risks of having higher level of stress. Conclusion: An occupational background and health habits should be considered to develop an effective educational strategy for smoking cessation and stress reduction among male patients with ACS.

관상동맥질환자의 마음챙김 명상에 대한 경험: 현상학적 연구 (Mindfulness Meditation Experiences among Patients with Coronary Artery Disease: A Phenomenological Study)

  • Purpose: This study was aimed to explore and describe the experiences of mindfulness meditation (mindfulness- based stress reduction, MBSR) among patients with coronary artery disease (CAD) as a method of cardiac rehabilitation. Methods: In-depth interviews was conducted with 7 participants with CAD post percutaneous coronary intervention. Main question was 'what is your experiences with MBSR?' Colaizzi's seven steps of phenomenological analysis was used for eliciting embedded meanings. Results: Five theme clusters were found; rediscovery of mindfulness meditation, obtaining self-control for stress management, self-worth, feeling of happiness, and looking into myself. Conclusion: This study provides a profound picture on the experiences of mindfulness meditation among participants with coronary artery disease. MBSR can help patients with CAD in reducing stress and to enhance their cardiac rehabilitations. It is hoped that mindfulness meditation can be used in clinical setting as a part of cardiac rehabilitation program.

요골동맥을 통한 경피적 관상동맥 중재술 환자의 손마사지 효과 (The Effects of Hand Massage in Patients Who Underwent Transradial Percutaneous Coronary Intervention)

  • Purpose: This study aimed to determine the effect of hand massage in patients who underwent transradial percutaneous coronary intervention. Methods: This was a quasi-experimental study with a nonequivalent control group and non-synchronized design. The study included 30 patients in the experimental group and 30 in the control group. Hand massage was performed 2 times for 5 minutes each in the experimental group and the control group only received usual nursing interventions. Pain, level of discomfort, and vital signs were defined as key outcome measures, and the data were analyzed using the chi-square test, an independent t-test, Mann-Whitney U test, repeated-measures analysis of variance, and Friedman test. Results: Significant differences were observed between the 2 groups in the pain score (F=7.91, p=.003), discomfort score (F=18.15, p<.001), pulse (F=12.92, p<.001), and respiration rate (${\times}^2=19.35$, p<.001). Conclusion: Hand massage can be a helpful nursing intervention for transradial percutaneous coronary intervention by reducing pain and discomfort to a considerable degree.

관상동맥질환자의 건강행위이행에 영향을 미치는 요인 : 건강신념 변수를 중심으로 (The Influencing Factors on Health Behavior of Patients with Coronary Artery Disease)

  • The purpose of this study was to investigate the influencing factors on health behavior among patients with coronary artery disease. Method: The subjects were 95 patients who visited the out-patient department of a university hospital for follow-up. The four health belief concepts (motivation, benefit, barrier, seriousness), general self-efficacy, health behaviors on medication, diet, exercise, stress management, smoking, and drinking were measured. Result: There were significant differences in the health behavior scores of subjects according to family support and the experience of surgical procedure. Subjects were found to have a high degree of compliance in taking medication. However subjects reported the lowest degree of compliance in regular exercise. In the multiple regression analysis, surgical procedure and motivation were significant predictors to explain diet. Motivation and barrier were significant predictors to explain exercise. Self-efficacy, motivation and family support were significant predictors to explain stress management. Family support and seriousness explained 16% of variance in drinking. Also, family support explained 30% of variance in smoking. Conclusion: Since predicting factors on each health behavior indicator were different, then nurses should consider these differences to construct strategy enhancing patient's recovery.

  • The purpose of this study was to examine the level of intention to quit smoking and to identify factors influencing intention to quit among patients with coronary heart disease. Method: The subjects consisted of 80 male patients with coronary heart disease (angina pectoris, myocardial infarction) at three hospitals in Seoul. The data were collected with self reporting in a structured questionnaire. Stepwise multiple regression was used to identify predictors of intention to quit. Included variables were attitudes toward smoking cessation, subjective norms, perceived behavioral control, usefulness of smoking cessation, and previous attempts to quit. Result: 1. The mean score for intention to quit was 11.1($\pm$6.1) which was lower than median score of the scale. 2. There were significant correlations between the all predictive variables and the intention to quit(r=.24-.48, p<.05). 3. usefulness of smoking cessation, perceived behavioral control, and previous attempts to quit explained 34.6% of the variance for intention to quit. Conclusion: usefulness of smoking cessation, perceived behavioral control, and previous attempts to quit were identified as important variables in explaining the intention to quit smoking among patients with coronary heart disease. Thus, it is necessary to try to enhance this factors for increasing intention to quit among patients with coronary heart disease.
