AN EXPERIMENTAL STUDY ON THE EFFECT OF CONDYLAR OSTEOPLASTY WITH PRESERVATION OF ARTICULAR COVERED SOFT TISSUE ON THE HEALING PROCESS IN RABBIT (가토 하악과두 연조직 피개가 과두골성형술시 치유과정에 미치는 영향에 관한 실험적 연구)

  • During the condylar shaving procedure, the articular soft tissue cover can be removed. Author compaired the histological healing process of the articular soft tissue cover between the preservative and unpreservative group group with 45 New Zealand rabbits(Average wt. : about 2.5kg). In unpreservative group, the usual high condylar shave with the removal of soft tissue cover was performed. In the preservative group, the underlying bone, replaced in its original position and sutured. The animals were sacrified 1, 2, 3, 4, 6 weeks interval after operation. The specimens were fixed in 10% neutral formalin and decalcified, paraffin embedded and stained by Hematoxylin & Eosin, and Masson's trichrome. The obtained results were as follows. 1. The condyles of the both group were covered with an articular sop tissue layer. 2. The cartilage cells in subarticular layer has regular continuous patterns in the preservative group but frequently interrupted in the unpreservative group. 3. The incision made in the posterior part of the articular surface for the elevation of the articular soft tissue frequently caused a deformity such as the interruption of the subarticular layer of cartilage. 4. By the above findings, the preservation of articular sop tissue cover may be the effective operation method on concept of bone remodelling.

  • Loss of mandibular continuity due to neoplasm, trauma, or infection results in major esthetic and biologic compromise. The use of costochondral grafts for reconstruction of temporomandibular joint, described first by Gillies in 1920, has been accepted as a suitable method for replacing the mandibular condyle, especially in growing children. Autogenous iliac bone graft has been a satisfactory source of mandibular reconstruction since Sykoffs report in 1900. Autogenous bone grafts from the posterior aspects of the ilium provide large amount of PMCB with acceptable donor site morbidity. In timing of reconstruction, initial disease, age, medical history, growth and development, esthetic and psychologic factors should be considered. We present a case of osteosarcoma in the mandible that was treated by a hemimandibulectomy and the defect was reconstructed 20 months later with composite method of costochondral and posterior iliac bone graft.

Full mouth rehabilitation in edentulous patient with unstable mandibular position using flat table treatment dentures and CAD-CAM technology (치료의치와 CAD-CAM 기술을 이용한 불안정한 하악위를 가진 완전 무치악 환자의 치료 증례)

  • For edentulous patients with unstable mandibular movements and abnormal facial features due to condylar fractures and morphological abnormalities, it is important to find a stable mandibular position. In this case, the patient's facial improvement, mandibular movement stability, and denture stability were improved by using flat table treatment dentures. In addition, computer-aided design/manufacturing (CAD-CAM) technology was used in denture fabrication to maintain the vertical dimension and lip support of flat table treatment dentures, we report good results in improving the patient's discomfort.

Effect of conservative therapy and Mandibular condylar bone change on Adolescents with osteoarthritis of TMJ (청소년 측두하악관절 골관절염의 보존적 치료효과 및 관절면의 변화 비교)

  • This study is designed to evaluate the treatment outcome of occlusal stabilizing splint and to assess follow-up study of condylar bony changes using cone beam computed tomography(CBCT) in adolescents patients (12-19 years) with TMJ osteoarthritis(OA). 167 eroded condyles in 149 subjects were chosen among the patients who presented to the Department of Oral Medicine of Pasan National University Hospital, diagnosed as TMJ osteoarthritis by clinical exam, x-ray and CBCT from 2009 to 2012. They were treated conservatively with physical therapy, medication, behavioral therapy and occlusal stabilizing splint therapy. After average 9 months, CBCT was retaken and subjective symptoms and clinical findings were investigated. Condyle bony changes were classified by unchanged, less severe and more severe. The obtained results were as follow: 1. Pain, Noise, LOM(Limitation of motion) and MCO(Maximum comfortable opening) measurement of TMJ OA patients were markedly improved after conservative treatment. 2. In the occlusal stabilizing splint therapy group, Pain and LOM were statistically significant improved than non-occlusal stabilizing splint therapy group. 3. In the acute occlusal stabilizing splint therapy group, Pain and LOM were remarkably improved. 4. In comparison of CBCT1 and CBCT2 images, the transition of bone changes to lesser severe was most commonly in joint with erosive change. 5. In the non-occlusal stabilizing splint therapy group, the transition of condylar bone changes from erosion to more severe was many than occlusal stabilizing splint therapy group.

EVALUATION OF MANDIBULAR CONDYLE POSITION IN TMJ TRANSCRANIAL RADIOGRAPH (악관절의 경두개 방사선사진에서 하악과두위 평가에 관한 연구)

  • Transcranial radiograph is widely used in the diagnosis of craniomandibular disorder because it can be used by the dentist with relative ease in spite of image distortion. But correct information can be obtained only when one know the image change by projection angulation. The author compared the condyle position in the 3 groups of transcranial radiographs (horizontal angulation - 0°, 5°, individualized) with that in the individualized corrected lateral tomogram by objective and subjective evaluation methods. The results were as follows: 1. Among 3 transcranial groups, the condyle position in individualized group showed the highest concordance rate with that in the tomogram group in objective and subjective evaluation methods. 2. Correlation coefficient between individualized transcranial group and tomogram group in objective evaluation method was highest (P≤0.01). 3. Image clarity in individualized transcranial group was worse than that of the other two transcranial groups.

  • The osteochondroma, also known as osteocartilagenous exostosis, is one of the most common benign tumors of the axial skeleton, but is rarely found in the facial bones. When present, the tumor is most often reported to affect the mandibular coronoid process. Osteochondroma of the mandibular condyle is extremely rare and may cause signs and symptoms like those seen in patients with temporomandibular joint dysfunction. Sometimes, differentiation between osteochondroma and condylar hyperplasia is not possible on histologic grounds alone, but the radiographic and intraoperative findings together are usually sufficient to establish a definite diagnosis. This report reviews the literature concerning osteochondroma, especially of the maxillofacial region, and describes a case of osteochondroma of the condyle.

  • Purpose: To evaluate the condylar movement at maximal mouth opening on MRI in patients with internal derangement. Materials and Methods: MR images and transcranial views for 102 TMJ s in 51 patients were taken in closed and maximal opening positions, and the amount of condylar movement was analyzed quantitatively and qualitatively. Results: For MR images, the mean condylar movements were 9.4 mm horizontally, 4.6 mm vertically and 10.9 mm totally, while those for transcranial views were 12.5 mm, 4.6 mm, and 13.7 mm respectively. The condyle moved forward beyond the summit of the articular eminence in 41 TMJs (40.2%) for MR images and 56 TMJs (54.9%) for transcranial views. Conclusion: The horizontal and total condylar movements were smaller in MR images than in transcranial views.

  • This study was aimed to evaluate the changes in condylar position when severe anterior open bite patients were treated with MEAW. The subjects consisted of 11 patients(21 TMJs) who visited the departement of orthodontics in SNUH, having severe anterior open bite as a chief complain. They were supposed to wear the up and down elastics and MEAW after finishing the leveling. The condylar position was evaluated with individualized corrected tomography in centric occlusion taken before and after treatment. The results were as follows; 1. In the change of condylar position after treatment, there were no statistically significant differences in right and left TMJs. 2. In the change of condylar position after treatment, there were no statistically significant differences in each malocclusion groups. 3. There were no statistically significant differences in percent dispacement of condyle between before and after treatment 4. There were no statistically significant differences in the amount of change in condylar change.

  • 본 연구는 측두하악관절장애의 증상이 있는 악관절의 통상적인 방사선 단층촬영술에 대한 해석과 관절강 측정에 대한 조사자내, 조사자간 신뢰도를 조사하였다. 신뢰도 검사는 조사자들 간에 판독의 일관성이 있는지와 일정 시간이 지난 후 반복 측정 시 판독의 정확성이 있는지를 알아보기 위하여 시행하였다. 조사자내 상관계수는 과두 형태에 대해서는 각각 0.812와 0.619, 골변화 형태에 대해서는 모두1.00, 골변화에 대해서는 0.846과 0.991, 전방관절강에 대해서는 0.919와 0.966, 상방관절강에 대해서는 0.864와 0.955, 그리고 후방관절강에 대해서는 0.718과 0.672였다. 통상적인 방사선 단층촬영술이 신뢰도가 있음을 보여주는 이 연구의 결과로 보아, 측두하악 관절을 평가하는데 있어서 통상적인 방사선 단층촬영술은 높은 진단학적 정확성과 조사자간 일치성을 가진다고 할 수 있다.

  • The purpose of this study was to investigate the range of condylar movement of children with young permanent dentition. Materials included 33 roentgenograms of 4 serial projections of TMJ by modified transcranial projection with Accurad 100. Four serial projections were taken in the centric occlusion, rest position, 1 inch open, and maximum open position. The results obtained from this study were as follows. 1. The condyle was located anteriorly in articular fossa at centric occlusion. 2. The condyle was moved forwardly and downwardly during mandibular movement from centric occlusion to maximum open position. 3. The pattern of condylar movement was similar in both sexes, but there were no sex differences in distance of condylar movement.
