Compliance Level with Therapeutic Regimen of Medication and Life Style among Patients with Hypertension in Rural Communities (일 농촌지역 고혈압 환자의 치료적 요법의 이행수준 - 약물복용과 생활습관을 중심으로 -)

  • Purpose: To identify the compliance level with therapeutic regimen among patients with hypertension residing in rural communities. Method: A descriptive-retrospective research design was employed. One hundred patients with hypertension using 8 Primary Health Care Posts under W Public Health Center were randomly recruited on the basis of being over 35 years of age. After obtaining written consent, the patients underwent direct interviews with a structured questionnaire carried out by 8 public health practitioners. Descriptive statistics and binary logistic regression were utilized. Results: In a binary logistic regression model adjusted for age, sex, education, income, and occupation, those who were receiving medication (OR=5.34), were undergoing a weight control program (OR=4.45), restricted alcohol (OR=9.93), or smoking cessation (OR=25.59) as recommended by medical or health professionals were more compliant (p<.05) while those under a low salt diet, exercise, and stress management were not significant statistically (p>.05). Conclusions: Further research should be conducted to validate these findings so as to facilitate the development of nursing intervention strategies for improving the compliance of hypertensive patients in respect to medication and life style modification.

  • The clinical evaluation was done in eight eyes of four patients with bilateral branch retinal vein occlusion. The onset of age was from 36 to 65 years, and three patients were women. Hypertension was the most common associated systemic disease. Retinal neovascularization was complicated in six eyes, and five eyes had vitreous hemorrhage. The vitrectomy was performed in three eyes. We believe that branch retinal vein occlusion associated with hypertension should be followed carefully in the fellow eyes.

Effects of a Qi Massage Program on the Physio-psychological State of Industrial Employees with Hypertension (기마사지요법이 고혈압 환자의 생리·심리적 상태에 미치는 효과)

  • Purpose: This research aims to identify the physio-psychological effects of applying a qi massage program as an independent complementary intervention for employees with hypertension in a workplace. Methods: A quasi-experimental design was employed using a nonequivalent control group with pre- and post-tests. The study was performed from August 18 to October 10, 2008 on hypertensive employees being treated with a qi massage program in a company. The control group was recruited first to prevent treatment diffusion. The experimental group was subsequently recruited and received individual treatment for 30 minutes. Collected data was analyzed using SPSS16.0 version. ${\chi}^2$-test, Mann-Whitney U test, Wilcoxon signed ranks test, Fisher's exact test and paired t-test were implemented in order to examine the effects of individual intervention on patients who are treated with the qi massage program. Results: The experimental group showed a greater improvement in systolic blood pressure, diastolic pressure, stress, and quality of life than did the control group. Conclusion: A qi massage program may be implemented as a complementary intervention to effectively treat hypertensive persons.

The Effects of Pneumoperitoneum on Heart Rate, Mean Arterial Blood Pressure and Cardiac Output of Hypertensive Patients during Laparoscopic Colectomy (복강경 대장절제술에서 기복이 고혈압 환자의 심박수, 평균동맥압 및 심박출량에 미치는 영향)

  • Purpose: This study was performed to identify effects of pneumoperitoneum on hemodynamic changes of hypertensive patients undergoing laparoscopic colectomy under general anesthesia. Methods: Data collection was done from January 2 to June 10, 2008. Seventy-six patients, including 38 hypertensive patients, who had taken antihypertensive drugs more than 1 month and 38 normotensive patients undergoing laparoscopic colectomy were enrolled in this study. The hemodynamic parameters were heart rate (HR), mean arterial pressure (MAP) and cardiac output (CO) which were measured 7 times from before induction of anesthesia to 5 min after deflation of the pneumoperitoneum. Collected data were analyzed using Repeated Measures ANOVA and Bonferroni comparison method. Results: HR in the hypertensive group was significantly decreased at deflation of the pneumoperitoneum and 5 min after deflation of the pneumoperitoneum (p=.012). MAP in the hypertensive group was not different from the normotensive group (p=.756). CO in hypertensive group was significantly lower than normotensive group (p<.001) from immediately after pneumoperitoneum to 5 min after deflation of the pneumoperitoneum. Conclusion: The results indicate that pneumoperitoneum during laparoscopic surgery does not lead to clinically negative hemodynamic changes in heart rate, mean arterial pressure or cardiac output of hypertensive patients, who have taken antihypertensive drugs for more than 1 month.

Predictors of Compliance in Hypertensive Patients (고혈압 환자의 치료지시 이행에 영향을 미치는 예측요인)

  • Purpose: The purposes of this study were to identify knowledge, health belief and compliance in patients with hypertension and to identify the most important predictors for compliance of hypertensive patient. Method: The participants in this study were 117 patients who were receiving treatment for hypertension at E. university hospital or one of three local clinics in D-city. Data were collected using a knowledge measurement instrument, health belief scale, and an instrument on compliance. Collected data were analyzed using $X^2$ test, ANOVA, multiple linear regression with PASW statistics 18.0 program. Results: There were statistically significantly positive correlations between knowledge of hypertension and health belief, health belief and compliance. But there was no correlation between knowledge of hypertension and compliance. In the multiple regression analysis, perceived barriers, perceived severity, perceived benefits were significant predictors to explain compliance and accounted for 54.1% of the variance in compliance. Conclusion: The results of the study indicate that health belief and compliance are significantly strongly correlated. Thus it is suggested that nursing interventions to improve compliance should include nursing care plans to increase health belief, perceived severity, perceived benefit and to decrease perceived barrier.

The Life Style and Quality of Life according to the Pattern of Type D Personality in Patients with Hypertension (고혈압 환자의 D유형 성격 양상에 따른 생활습관과 삶의 질)

  • Purpose: The purposes of this study were to describe the pattern of type D personality, to compare the life style and quality of life between type D personality and non-type D personality patients, and to investigate the factors influencing quality of life in patients with hypertension. Methods: A cross sectional, descriptive study was used. The participants in this study were 193 outpatients who were diagnosed with hypertension at two university hospitals in urban area, Korea. The data was collected from December, 2006 to January, 2007. Type D personality was measured by the DS-14 scale. Results: The prevalence of type D personality was 83.9%. Patients of type D personality were significantly different in educational status, monthly income, fat intake and exercise, and had a lower overall quality of life than patients of non-type D personality. Under controlled general characteristics and life style factors, multiple linear regression analysis was performed. The most significant factor influencing quality of life in hypertensive patients was type D personality, and this factor explained their quality of life with a variance of 14.8%. Conclusions: Various programs for psychological intervention are required to control for the distressed personality of patients with hypertension. Further studies should be conducted prospectively on a larger patient population.

The Coping Experience in Hypertensive Clients (고혈압 환자의 대처경험)

  • To find the experience about clients with hypertension and to help them to care for themselves in the community. Method: All data was collected from August 1999 to October 1999, through in-depth interviews, observation, and telephone interview with 7 participants who have been diagnosed with hypertension for 1 to 10 years. According to Strauss and Corbin's Methodology, the data was continuously coded into concepts and categories, and then new data was analyzed simultaneously by a constant comparative method. Results: There are 171 concepts, and then they were grouped into 34 the lower categories and 15 to the upper categories. The course of the coping of fear in hypertensive client consisted of 6 processes. The awareness of seriousness was context, and the fear was core phenomenon about the coping experience. We also found that hypertensive clients have 3 patterns, depending on the awareness degree of seriousness and the fear about hypertension. Conclusion: Our nurses should recognize the importance of effective management and seriousness about hypertension, offer clients the importance of family support and the information of etiology, symptoms and signs of hypertension, and provide the correct information on hypertensive medication. We should be able to guide their fear about hypertension to positive self-management, so that they may manage their disease thoroughly and effectively.

Creatinine and microalbuminuria levels are increased in type 2 diabetic patients with hypertension (고혈압을 동반한 제2형 당뇨병 환자에서의 creatinine과 미세 단백뇨 증가)

  • The purpose of this study was to identify clinical characteristics of type 2 diabetic patients with hypertension. The subjects were 209 type 2 diabetic patients who visited at the endocrine center at Kangnam St. Mary's Hospital of Catholic University in Seoul from beginning of March through the end of April in 2001. The patient's clinical laboratory data were assessed at medical record review. The data were analyzed using for t-test, $x^2$ test. The results were as follows: 1) There were no significant differences in age, body mass index, sex, family history of diabetes and oral hypoglycemic agents between hypertensive group and normotensive group, However, percentage of patients receiving insulin treatment was higher significantly in the hypertensive group. 2) Creatinine and microalbuminuria levels were higher significantly in the hypertensive group. However, fasting blood glucose levels were lower significantly in the hypertensive group. There were no significant differences in $HbA_1c$, 2-hour postprandial blood glucose, total cholesterol, triglyceride, high density lipoprotein cholesterol, lipoprotein(a) and blood urea nitrogen between two groups. Our present study supports that Creatinine and microalbuminuria levels were higher significantly in the hypertensive group.

The Effects of an Internet Based Coaching Program for Obesity Management in Hypertensive Patients (고혈압 환자의 비만관리를 위한 인터넷 기반 코칭 프로그램의 효과)

  • Purpose: The purpose of this study was to investigate the effects of the internet based coaching program for obesity management on weight control related knowledge, self-efficacy, eating habits, physical activity and obesity related physiological indexes in hypertensive patients. Methods: The study design was non-equivalent control group pre-test-post-test design. A total of 43 subjects were overweight or obese hypertensive patients aged 30 or older, who were divided into experimental group (n=23) and control group (n=20). This program lasting 12 weeks consisted of a weight control related education, internet based individual coaching for diet, exercise and behavior modification, offering internet community and health counseling. The variables were measured three times (before the treatment, after 6 weeks, and after 12 weeks) and the collected data were analyzed by SPSS/WIN 17.0 program. Results: The experimental group had significantly improved weight control related knowledge, self efficacy, eating habits, body composition, and systolic blood pressure with time. Conclusion: The internet based coaching program was effective in improving obesity management, also this program will help to prevent cardiovascular disease of obese hypertensive patients.

The Experiences of Young Adult with Hypertension (성인 전기 고혈압 환자의 경험)

  • Purpose: The purpose of this study was to explore the experiences of young adults with hypertension, using phenomenology in order to describe the essence of such experiences and to understand them from the patients' point of view. Methods: Participants in this study were 4 hypertension patients with ages ranging from 20 to 35 years old, living in Seoul city. Data were collected from in-depth interviews done from October to November in 2005. Colazzi method(1978) was used to analyze the data. Results: The 12 themes were extracted in this study ; 'Discovery by accident', 'Denial of the diagnosis', 'A disease unworthy of the young men', 'Guilty feeling toward their parents', 'Fear of death', 'Unconvinced reason', 'Worry about the future', 'Attribute all symptoms to the disease', 'Will of not life-long medication', 'Concern for fertility problem', 'Not convinced with current treatment method', 'Becoming unimportant'. Conclusion: The results of this study show that the young adult with hypertension will be in a high risk group of non compliance, so helpful program for them should be developed and the nurses need to pay more attention to the psychosocial aspect of nursing.
