Although many PCR-based assays have been developed, the majority of rapid detection of Toxoplasma gondii in animal and their meat product has been dependent on immunogenic assays. Thus, there is still a need for more reliable PCR based detection method for T. gondii in retail meats. Recently, a 529-bp repeat element that exists in 200-300 copies per genome of T. gondii genome had been spotlighted for its usefulness as potential detection targers. In this study, the 529-bp repeat element was selected for real-time PCR to detect three types of T. gondii (type I, II and III). A primer pair targeting 82-bp of the 529-bp element detected all three types of T. gondii and showed high level of specificity against 14 different food-borne pathogens as well as 3 protozoan parasites such as Giardia lamblia, Cryptosporidium parvum and Entamoeba histolytica. Application of the new real-time PCR assay in meat samples showed improved detection sensitivity compared to the B1-gene targeted method suggesting potential new target for Toxoplasma gondii screening in retail meats.
To evaluate the sanitary conditions of four species of commercial shellfish (Crassostrea gigas, Tapes japonica, Mytilus coruscus and Anadara granosa), the psychrotrophic coliforms and the bacterial flora were monthly examined from January to December, 1978. Viable cell counts of the samples submitted were $1.3\times10^3\~2.5\times10^6/g$ and the numbers of coliform and E. coli were above $0\~5.5\times10^2/g\;and\;2\~9.2\times10^2/100g$ respectively. Among the 373 coliform bacteria isolated from the samples submitted, 298 strains were identified as coliform by IMViC test, $44^{\circ}C$ culture and gelatin liquefaction test. E. coli type I and K. aerogenes type I were the most predominant ones among the identified coliform as 138 strains and 71 strains respectively. The psychrotrophic coliform growing at $5^{\circ}C$ were 186 $strains(50\%)$ among 373 strains of isolated coliform. Vibrio and Pseudomonas were the most predominant groups. Among 453 strains of isolated bacteria, Vibrio was 240 $strains(53\%)$ and Pseudomonas was 91 $strains(20\%)$. Moraxella, Acinetobacter, Enterobacteriaceae and Flavobacterium-Cytophage were scantly detected. In seasonal variation of general bacterial flora, a lot of Vibrio appeared from May to October, did Moraxella during the winter season and the other genuses showed no seasonal change.
Gitelman's syndrome is an autosomal recessive disorder characterized by hypokalemic metabolic alkalosis, hypomagnesemia, and hypocalciuria that has recently been reported to be linked to thiazide- sensitive Na-Cl cotransporter gene mutation. In this study, we performed renal clearance studies to differentiate Gitelman's from Bartter's syndrome and to confirm the diagnosis in two patients clinically diagnosed with Gitelman's syndrome. Each patient was hydrated by 20 mL/kg body weight of oral water within 30 minutes, which was followed by intravenous half saline. When urinary flow reached 10 mL/min, samples of urine and serum were obtained to calculate the osmolar clearance, free water clearance, chloride clearance, and distal fractional chloride reabsorption. Subsequently, furosemide or hydrochlorothiazide was administered. Samples were collected and the same parameters were calculated. In our patients, chloride clearance was increased more than 10 times after furosemide administration(2.1 : 25.7 and 2.2 : 27.4 mL/min/100 mL GFR), but not increased after hydrochlorothiazide treatment(2.1 : 1.6 and 2.2 : 2.6 mL/min/100 mL GFR). And the distal fractional chloride reabsorption was significantly decreased by furosemide injection (73% : 15% and 75% : 4.6%), whereas hydrochlorothiazide had no effect on it(73% : 63% and 75% : 78%). These findings indicate that our patients have a defect in thiazide-sensitive Na-Cl cotransporter in the distal tubule, which is compatible with the pathophysiology of Gitelman's syndrome.
This studies were carried out to investigate the high infection sites from various specimens, cultural isolation on the susceptible media and specific antibody titres of M. pulmonis in experimental 310 mice and 330 rats obtained from two breeding facilities. Efficiency of complement faxation test (CF test) for detection of M. pulmonis antibody in mice and rats were compared directly with the diagnostic cultural isolation method. 1. Isolation rates of M. pulmonis among infection sites were about 30% from the oral cavities and $40\%$ from the middle ear of mice. The rates were $100\%$from the nasal cavaties and $90\%$ from the oral cavities of rats. 2. The infection rates were $12\%$ to A group and $16\%$ to B group of mice. The rates in the rats were $55\%$ to A group and $70\%$ to B group. 3. The M. pulmonis antibody titres by CF test were $73\%$ of total 100 mice in serum dilution below 1:5 (< 1:5), and $24\%$ of total samples in antibody titres above 1:5 (> 1:5), but 3 samples were not showed anticomplementary activities. The antibody titres in rats were $35\%$ of 120 rats in below 1:5 (< 1:5), and $61\%$ of total samples in above 1:5 (> 1:5), but the remained were not showed anticomplementary activities.

Objectives: A group consisted of 32 Korean rural people had been travelled to Cambodia and Vietnam. During their travel for 7 days, first diarrheal case had occurred on 3rd day of travel and overall 8 people had diarrhea during travel. So we investigated the case for determination of the cause and route of infection. Methods: We interviewed all 32 people by visiting or telephone and took 23 stool samples from them, which are examined by Gyeungsangbuk-do Government Public Institute of Health & Environment in Korea. We interviewed travel agency for travel route and schedule. Results: The attack rate was 25.0% (8 cases) among 32 people. Of them, 3 cases were microbiologically confirmed to have the infection of Shigella flexneri (Serotype 3a). The result of this epidemiological investigation showed that the infection of shigellosis was happened in foreign countries. We assumed the water from Angkor-Watt was contaminated by S. flexneri. We could not able to test environmental specimens because of the infection is thought to be occurred in foreign countries. Conclusions: Most common infectious diseases from foreign countries are transmitted by water or food. Travellers to foreign countries must take caution for communicable diseases and must do report and get appropriate treatment when symptoms developed after travel.
In order to analyze the positive rate of hepatitis B surface antibody by age before and after hepatitis B vaccination, 13,855 serum specimens who were referred for the hepatitis B surface antibody test at the General Hospital of Jeju Hospital were examined by CMIA method. The positive rate of HBs Ab was 5,176 (37.36%). The positive rate according to gender was 40.13% for female and 34.77% for male. The age group with the highest HBs Ab average was in their 40's (399.86 mIU/mL), while the lowest age group was in the 90's (211.50 mIU/mL). It is remarkable that the age group with the highest HBs Ab positive rate is in the 30's, and that teenagers (age group of 10-20 years) had the lowest positive rate. Especially 15, 18, 19 years old was statistically significant. Consideration should be given to determining the titer of vaccination and to clarify the timing of vaccination.

Establishment of simultaneous analysis method and monitoring for individually analyzing residual eight ergosterol biosynthesis inhibitors, EBI (difenoconazole, diniconazole, fenarimol, fenbuconazole, hexaconazole, myclobutanil, nuarimol and paclobutrazol) pesticides in commercial agricultural products, were conducted. The simultaneous analysis method for the pesticides was established using a GC/MS/MS for EBI pesticides. Residual amount of those pesticides were investigated in 989 commercial agricultural products (fifteen kinds of cereal grains, vegetables, beans, nuts, fruits and mushrooms) from seven metropolitan cities and eight provinces. In EBI pesticides analysis, linearity of GC/MS/MS analysis was 0.9974-0.9992, and that of recoveries were 86-135% with relative standard deviations (RSD) <20%. The limit of quantification (LOQ) of the method ranged from 0.5 to 5.0 mg/kg for eight EBI pesticides. According to the monitoring of the EBI pesticides in commercial agricultural products, difenoconazole, fenarimol, hexaconazole showed various residual levels (total frequency of 8/989 detection, 0.8%). Paclobutrazole showed in excess levels of the MRLs (maximum residue limits) for pesticides in one chard sample by the Korea Food Code. As a result of exposure assessment on the detected 8 individual pesticides, all pesticides (difenoconazole, fenarimol, hexaconazole, paclobutrazole) were evaluated as safe level in comparison to toxicologically acceptable daily intake.

Purpose : The prevalence of Reye syndrome has decreased since late 1980's. But we report that recently there were concentrative attacks of Reye syndrome after acute enteritis during the neonatal period. Methods : Clinical symptoms and laboratory results(quantitative organic acid analysis, routine chemistry, arterial blood gas analysis, serum ammonia) of seven patients admitted at the Samsung Medical Center, Sanggye Paik Hospital, Wonju Christian Hospital and Chuncheon Sacred Heart Hospital, referred from Jan. 2005 to Apr. 2005, were analysed retrospectively. The major clinical symptoms were derived from the patients' clinical records sended with urine samples and quantification of organic acids were done with gas chromatography and mass spectrometry. Results : The mean age of seven cases is 18 days and the major preceding symptoms were gastrointestinal symptoms(vomiting, diarrhea, refusal to feeding). The major clinical symptoms were clouded conciousness, repiratory difficulty, vomiting, seizures, and diarrhea. One patient died; that patient's serum ammonia was twenty times higher than normal. Conclusion : The seven patients were neonates. Reye syndrome has been known to be closely related with upper respiratory infections as a preceding disease and to internal use of aspirin, but in our study, the major preceding disease of the seven cases was gastrointestinal infection and none of these used aspirin.

Purpose : Acute gastroenteritis is a frequent childhood disease. The purpose of this study is to determine if certain pathogens are related to more severe clinical disease in children with acute gastroenteritis. Methods : Two hundred seventy one stool samples were obtained from pediatric patients with acute gastroenteritis that was associated with virus, bacteria and protozoa infection between January 2006 and December 2006. The clinical severity was assessed by using the 24 point scale Clark score. We evaluate the Clark score according to the pathogens. Results : One hundred thirty seven children were noted to have virus, bacteria or protozoal infection. Rotavirus was the most common pathogen found in the children with gastroenteritis (20.6%). Other frequent pathogens were norovirus (10.7%) and E. coli (12.9%). Vomiting (P=0.049) and diarrhea (P=0.003) symptoms were more frequent in the rotavirus group than that in the rotavirus negative group. There were no statistical differences in the Clark score between the virus positive group and the virus negative group. Conclusion : We concluded that rotavirus was the most common pathogen in children with gastroenteritis, yet the clinical severity was not dependent on the presence of viral or bacterial pathogens.

Purpose: The Immunotech AFP is a widely used test in 14 hospitals(RIA QC establishments: 46) as a liver tumor marker. Normally, the test takes 45 minutes including complicated 2 step method. We evaluated the possibility of 1 step method and suitable test time. Materials and Methods: Samples of 31 patients in SNUBH were used as subject. We evaluated the sensitivity, intra and inter assay precision, recovery rate, linearity between 1 step and 2 step method (45 min. and 60 min.). Results: Both of 45 minutes and 60 minutes test showed 0.1 ng/mL sensitivity. In 45 minutes test, the intra assay coefficients of variation were 3.05%, 3.43%, 1.68%. Also, inter assay coefficients of variation were 3.91%, 2.38%, 0.82%. In 60 minutes test, the intra assay were 5.00%, 3.69%, 1.97%, inter assay were 3.14%, 3.71%, 1.85%.The correlation coefficient of each 2 step and 1 step (45 min.), 2 step and 1 step(60 min.), 1 step (45 min.) and 1 step (60 min.) were y=0.9293x+2.7356 ($R^{2}=0.9999$), y=0.9193x+4.1002 ($R^{2}=0.9993$), y=0.9894x+1.3805 ($R^{2}=0.9997$). Conclusion: Compared 1step method to 2 step method, both method showed very reasonable precision, recovery rate and correlation coefficient. Also, 1 step method(45 min.) of correlation coefficient ($R^2$) was 0.999. We suggest that 1 step method test can reduce the test time and is useful for AFP test.
