The Relationship between Perception and Satisfaction for National Health Insurance in Korea (국민건강보험 인식수준과 만족도의 관계)

  • The purpose of this study was to identify the relationship between the health insurance perception level and health insurance satisfaction for national health insurance subscriber. The collected data were analyzed by $X^2$-test, t-test, ANOVA using the SPSS/WIN (ver.14.0). Research results can be summarized as follows. The relationship between the health insurance perception level and health insurance satisfaction was relatively high. The study showed significantly high levels in the score of health insurance satisfactions according to perception of security level, health insurance premium, positive thinking, and fair in premium charge. Also health insurance satisfaction scores were significantly low levels according to perception of raise health insurance premium and burden on households. This study investigated the relationship between the detail perception and satisfaction for national health insurance. Therefore, we are setting the overall direction of national health insurance perception, and look forward to utilizing the basis data as help to improve the system of health insurance for people.

Analysis of changes National Health Insurance Policy and Claim Data of PET (양전자단층촬영 건강보험 적용 정책 및 이용량 변화에 관한 연구)

  • In this study, the health insurance application of PET and the change in the pay standard were examined, and the amount of health insurance use over the past 10 years was analyzed. Positron tomography was applied as health insurance in 2006, and after 18F-FDG was first applied as health insurance, positron tomography tests using various radioactive isotopes have been applied as health insurance. As of 2019, the number of positron emission tomography tests was 198,651 cases, and the treatment amount was about 88.3 billion won, and the number of tests according to general characteristics was higher in men than in women, and by age, the number of tests was the highest in 60s. The number of outpatient examinations was higher than that of inpatient examinations, and the number of examinations in tertiary hospitals(68.2%) was significantly higher than that of general hospitals and hospitals. As for the test site, torso test was the most common at 86.6%, and radioisotope was the most at 93.6% using 18F-FDG. The change in the use of PET for 10 years increased steadily from 2010 to 2014, but the amount of use decreased sharply afterwards as the recognition of asymptomatic long-term follow-up tests was deleted due to the government's change in health insurance application standards in 2014. As changes in health insurance standards have a great influence on changes in health insurance usage, continuous monitoring will be required in the future.

The Management Strategies of National Health Screening Patients in Health Examination center (건강검진센터의 국민건강보험 검진환자 관리방안)

  • This study aims to develop the methods for effective managing national health screening patients in the health examination center using digital data from national health screening in Dae-Jeon health examination center. To achieve this, we collected about national health screening for 10 years from 2002 to 2011 in Dae-Jeon health examination center and developed re-examination predictive model, private examination predictive model and stomach cancer examination predictive model for national health screening patients by using this data. According to the predictive model results, age, residence, group or individual health examination and the previous number of national health screening were statistically associated with re-examination, private examination, stomach cancer examination. We came up with a plan for health examination center system based on the predictive model and logic in Dae-Jeon. Customized service based on patient management system for national health screening will contribute to efficiency in health examination center.

말 못할 고민 치질

  • 치질은 전형적인 한국인의 질병이다. 국민건강보험공단과 건강보험심사평가원 등 정부기관이 집계한 자료에 따르면 입원환자수가 가장 많은 질병이 다름 아닌 치질인 것으로 나타났다. 이를 증명하듯 건강보험 통계연보를 살펴보면 최근 6년 사이 치질 입원 환자 수는 12만 명에서 22만 명으로 2배 정도 급증했다. 그만큼 치질은 우리 주변에서 흔히 볼 수 있는 대표적 질환인데 가끔씩 배변 후 명미한 출혈까지 포함한다면 거의 대부분의 사람들이 치질 증상을 경험해 봤다고 하겠다.

  • 학업 스트레스와 불규칙한 식습관으로 인해 10대 위염환자가 늘고 있다. 이는 국민건강보험공단의 '건강보험 진료비 지급' (2008~2012) 자료를 통해서도 드러나고 있는데, 그 결과를 보면 10대 위염환자 증가율이 전체 평균 3%의 두 배가 넘는 7.3%에 이르고 있다. 위염을 방치할 경우 위궤양 등 더 큰 병으로 발전할 수 있기 때문에 다음의 내용을 참고하여 위염으로부터 건강을 지킬 수 있도록 하자.

  • Health insurance is the main instrument to protect the people against sickness. To examine the task of in the future it would be necessary to extract and understand the components formed in its formation and development, benefit related and normative characteristics of the health insurance itself. The health insurance oriented itself to the universal coverage at a law level. Paradoxically this worked positively for the universal development of the health insurance. The task of the health insurance is on the one hand universal, positive and open. On the other hand it has to shape type of allowed method, art and content of the medical treatments into the regulation to ensure the equal benefit as well as the financial stability. That is, the health insurance should check the aberrant medical treatment, and at the same time should be compensated for the their necessity and effectiveness. However there are always some structural differences between both requirements. This article aims to restate and analyse the development of the health insurance, based on the characteristics formed hitherto show the way to reform the health insurance. The problem to enhance the coverage of health insurance, its institutional as well as financial crisis, its peculiar governance would be handled.

Convergence effect of dental insurance knowledge and oral health knowledge of adults on dental health insurance recognition satisfaction (성인의 치과보험지식 및 구강건강지식이 치과건강보험인식만족도에 융합적으로 미치는 영향)

  • The purpose of the study was to analyzed the convergence effect of dental insurance knowledge and oral health knowledge on dental insurance recognition satisfaction of adults. Dental health insurance satisfaction recognition, "Need expand the number of treatment subjects of dental treatment" was high, "satisfied cost of dental treatment covered by dental insurance" was low. Dental insurance knowledge was high teeth whitening, orthodontic treatment. Oral health knowledge was high, "the food you eat affects your teeth.". Dental health insurance recognition satisfaction was high when enrolled in private dental insurance. Dental health insurance recognition satisfaction showed a positive correlation with dental health insurance knowledge. Collectively in order to increase the dental health insurance recognition satisfaction, systematic education on dental insurance knowledge and oral health knowledge should be provided.

The Demand of Microinsurance: a Case of Health Insurance (소액보험의 수요: 건강보험을 중심으로)

  • This study explains the characteristics of micro-insurance based on the theoretical model of health insurance, such as the low demand of low-income people and the lower demand of higher risk aversion. In particular, these characteristics contradict the existing insurance theory which states that the lower the income, the higher the risk aversion, and the higher the demand for insurance. This study postulates a two-period model focusing on health insurance, contrary to a one-period model assumed in existing studies. As a result, first, we show that the decrease in income leads to a decrease in the preventive effort for illness. Second, we offer a model for micro-insurance in which the individual chooses a partial insurance under an actuarially fair insurance premium, while full insurance is optimal in existing studies. Third, we also show that the insurance demand decreases when the outlook for the future improves. Fourth, we finally show that the lack of trust and default risk of the insurer decrease the insurance demand as risk aversion increases.

Analysis of Health Insurance Standards and Utilization of MRI in Korea: Based on Health Insurance Claim Data (한국의 MRI 건강보험 급여기준 및 진료이용에 관한 연구)

  • This study analyzed the current status of MRI (frequency, amount of treatment) based on the history of application of the MRI health insurance benefit standard and health insurance claim data. MRI examinations began as a health insurance benefit in 2005. In 2005, the indications were restricted for some diseases, but coverage for benefits in 2010, 2013, 2016, and 2018 was expanded. In 2021, the Ministry of Health and Welfare decided to apply health insurance for all MRI examinations. From 2010 to 2017, the number of MRI examinations increased by 86.7% in 2017 compared to 2010, and the amount of treatment increased by 53.5%. According to general characteristics, the number of MRI examinations was higher in women than in men. By age, the number of examinations was the highest among ages 70-79. Outpatient examinations were more frequent than inpatient examinations, and the number of examinations in the tertiary hospitals was the highest among the types of hospitals. The number of brain MRI examinations was the highest in each exam site. In December 2013, the standard of MRI was expanded for heart disease and Crohn's disease, the number of cardiac MRI and abdominal MRI examinations increased in 2014 compared to 2013. However, the number of examinations is small and not associate with the disease, it would be difficult to say that it affected the increase in the total number of MRI examinations. To assess health insurance sustainability and policy effectiveness, monitoring will be necessary.

Problems of National Medical Expenses Management in Korea (한국 국민의료비 관리의 문제점 분석 : 건강보험, 산재보험, 자동차보험을 중심으로)

  • The purpose of this study is suggesting proper management methods for the national health expenditures by considering advanced countries and analysing the problems of national health expenditures management in korea. The majors results of the research are as follows. First, most advanced countries is integrating the management of national health expenditures about health insurance, workmen's accident compensation insurance and auto insurance etc, and medical prices and benefit standards are same regardless of insurance type. Second, national health expenditures has been managing separately by national health systems in korea, and there are many problems like the differences medical expenditure review and payment, medical prices and benefit standards etc. Although same symptoms and disease, there is great difference in health service utilization. Hereafter, management system of national health expenditures must be integrated, and must change same medical prices and benefit standards.
