Two Cases of Acute Renal Failure Caused by Acute Paraquat Poisoning (급성 Paraquat 중독 후 발생한 급성 신부전 환자 2예)

  • Paraquat is a very potent herbicide which causes fatal toxicity when ingested, and there is no specific antidote against it. Human ingestion induces acute renal failure, hepatic dysfunction and progressive respiratory failure with high mortality rate. Clinical investigation and medical treatment were done on two cases of acute renal failure caused by paraquat poisoning admitted to the Department of Internal Medicine, Wonkwang University Oriental Chonju Medical Hospital. We report two cases of patients who survived after acute paraquat intoxication, by means of oriental medicine such as Gamdutang, a typical antidote of toxins, chinese ink as an absorbent and burned powder of Rhei Radix et Rhizoma for laxative and so on, western medicine such as gastric lavage, diuretics and fluid therapy. We suggest more experiments and studies related to such treatment for paraquat poisoning be conducted.

The Protective Effects of Kamdootang on Accumulation of Cadmium in Rats (랫트의 신장 내 카드뮴 축적에 대한 감두탕(甘豆湯)의 방어 효과)

  • To find out the protective effects of Kamdootang against accumulation of cadmium in rats, the experimental rats were divided into 2 groups. One group was administered with cadmium alone and the other group administered with cadmium mixed with Kamdootang. Each group has been orally administered with different doses of cadmium such as 1.7 ${\mu}g/g/day$, 3.4 ${\mu}g/g/day$ and 6.8 ${\mu}g/g/day$, respectively, for 1 to 8 weeks. As a results, body weight gained has a tendency to decrease more in the Kamdootang treated groups and non-treated groups than in control group. Cadmium accumulation in kidney showed a significant difference between the Kamdootang treated group and Kamdootang non-treated group. As the experiment period is longer and longer, both groups have got more significantly cadmium accumulation amounts. The kidney system was researched through histopathological observation that Multiple foci of necrosis, hemorrhagic necrosis in glomeruli and cloudy swelling of tubules in kamdootang non-treated group. but tissue lesion of kidney was showed less kamdootang treated group than Kamdootang non-treated group. As remarked above results, when dose low concentrated Cd in rat, Kamdootang was reduced accumulation of Cd. in kidney.

Antitumor activities of Gamdutang aqua-acupuncture solution (감두탕 약침액의 암예방 효과에 관한 연구)

  • Gamdutang aqua-acupuncture solution(GAS), Gamdutang water-extracted solution(GWS) and Degamdutang aqua-acupuncture solution(DGAS) were prepared and tested for potential antitumor activities. It was used three biomarkers (quinone reductase, omithine decarboxylase, glutathione) to test chemopreventive potentials of GAS, GWS, DGAS. GAS was potent inducer of quinone reductase activity in Hepalclc7 murine hepatoma cells in culture, whereas GWS is less potent. GAS, GWS and DGAS were significantly induced quinone reductase activity in cultured rat normal liver cell, Ac2F. Glutathione levels were increased about 1.8-fold with GAS, 1.0-1.1 fold with GWS, DGAS in cultured murine hepatoma hepaiclc7 cells. In addition glutathione s-transferase levels were increased with GAS, GWS and DGAS. The effects of GAS, GWS and DGAS were tested on the growth of Acanthamoeba castellanii. Proliferation of Acanthamoeba castellanii was inhibited by GAS, GWS and DGAS at concentradons of $1{\times}$ and $5{\times}$. These results suggest that GAS has chemopreventive potential by inducing quinone reductase and quinone reductase activities, inhibition of ornithine decarboxylase activity, and increasing glutathione levels.

A Clinical Study on the Protective Effects of Gamdu-tang on Intoxication of Organophosphate Insecticides (감두탕(甘豆湯)의 농약중독 예방효과에 대한 임상적 연구 (I) - 살충제(유기인제)를 중심으로 -)

  • Objectives: We have accomplished a clinical research about counteracting the pesticides intoxication using oriental medical method and treatment. And we observed the preventive effect of Gamdu-tang(甘豆湯) to intoxication of Organophosphorus Insecticides. Material and Methods: The subjects were 8 volunteers who were healthy and not taking medications. we observed change of cholinesterase(ChE) activity in serum of them after exposed to pesticides Results and Conclusions : In this study of the preventive effects of Gamdu-tang(甘豆湯) on the intoxication of Organophosphate Insecticides, we evaluated that the decreasing value of ChE activity after spraying insecticides of Gamdu-tang medication group was smaller than non-medication group, but it was less significant(P>0.05 P<0.1). We had a difficult to gather participant in this study, so it's necessary for us to get together more larger group in the next study.

Case Report of Treatment for Paraquat Poisoning with Gamdutanggami (감두탕가미(甘豆湯加味)를 이용한 파라콰트 중독 환자 치험 1례)

  • Paraquat is a nonselective contact herbicide that may induced damage to many organs poisoned with it. But there is no effective treatment modality. This report is about one case of treatment for paraquat poisoning. The patient was hospitalized of Wonkwang university Gwangju Oriental Medical Hospital at the department of Internal Medicine. For 26 days of hospitalization period, the authors supplied gamdutanggami(甘豆湯加味), which consists of Radix glycyrrhizae(甘草), Semen Glycine(黑豆), burned powder of Rhizoma rhei(大黃沙炭末), Succus phyllostachyos(竹瀝) and chinese ink(墨汁), decoction of Galla chinensis(五倍子) and Radix glycyrrhizae(甘草), and fluid. And then he showed improvement of condition and returned to a daily life. This report has a limitation for proof of oriental medicine remedial value. Because this is the only one case of a small quantity of paraquat poisoning patient less than 20cc. Actually there are a few paper regarding the Oriental medicine remedial value in paraquat poisoning patient, so we hope luther study and report ensue.

Effects of Gamdutang Aqua-acupuncture Solution on Tumor Cell Lines in vitro (감두탕(甘豆湯) 약침액(藥鍼液)이 종양세포(腫瘍細胞)에 미치는 영향(影響))

  • Gamdutang aqua-acupuncture solution(GAS) and Gamdutang water-extracted solution(GWS) were prepared and tested for potential antitumor activites. It was shown to possess considerable toxicity toward various tumor cell lines. Concentration of $5{\times}\;and\;10{\times}$ of GAS resulted in more than 70% inhibition of growth in Ehrlich ascites tumor cells(EATC), hepa1c1c7 and A549. GAS at the concentration of $10{\times}\;and\;5{\times}$ revealed that more than 60% inhibition in HeLa. GWS showed more than 50% in hibition of growth with EATC, HeLa at the concentration of $5{\times}\;and\;10{\times}$. Toxicity assay with GWS in hepa1c1c7 and A549 revealed that more than 80% inhibition in growth at the concentration of $5{\times}\;and\;10{\times}$. In morphological study, the number of cells were decreased, and the shape of cells was round-form. Most of cells in detached in EATC, Hepa1c1c7, HeLa, and A549 with GAS. These results suggest that GAS has antitumor activity in vitro.

One Case of Gamdutanggamibang-treated Acute Hepatitis Caused by Acute Paraquat Poisoning (감두탕가미방(甘豆湯加味方)을 이용한 급성 파라콰트 중독후 급성 간염의 한방 치료 1예)

  • Paraquat is one of the widely used herbicides. But it is fatal, if it is consumed by people. Paraquat poisoning causes acute renal failure, hepatic dysfunction, and progressive respiratory failure. There are no effective antidotes to paraquat. This report is about one case's treatment for acute hepatitis caused by paraquat. The patient was hospitalized in the Department of Internal Medicine, Wonkwang University Oriental Medical Hospital in Chonju. The patient received the following treatments while in an acute stage : Gamdutanggamibang(甘豆湯加味方), which consists of Radix glycyrrhizae(甘草), Semen mungo(綠豆), burned powder of Rhizoma rhei(大黃炒炭末), Succus phyllostachyos(竹瀝), chinese ink(墨汁) and fluid therapy. The patient received Sagunjatanggamibang(四君子湯加味方) while in a chronic stage. The patient improved faster with the above treatments than with the conventional treatment. We hope that this report will help widening the clinical range of oriental medicine, and improve systemic efforts in treating paraquat poisoning cases.

The Case of Toxicity Caused by Scopolia Japonica Ingestion Treated with Gamigamdu-Tang. (낭탕근(狼菪根) 중독 환자의 가미감두탕(加味甘豆湯) 투여 증례 1례)

  • The prevalence rate of plant poisoning is growing up gradually. However, it could be difficult to get proper treatment without detail medical history taking due to the diversity of poisonous plants and their various symptoms. Scopolia Japonica, which grows naturally in Korea, is reported to cause severe toxicity which could be lethal. A few therapeutic measures have been reported in Traditional Korean Medicine (TKM), but it has not been reported yet that those measures have been successfully proven in the clinical practice in modern times. This is a case report of toxicity caused by scopolia japonica ingestion. A 65years-old male patient with cognitive impairment, behavioral disorder, gait difficulty, dysarthria, dysphagia and abnormal pupil reflex recovered from those symptoms during taking Gamigamdu-tang and getting acupuncture. This study shows the clinical aspects of Scopolia Japonica toxicity and the possibility of TKM treatments as one of plant poisoning treatments.
