간호간병통합서비스 병동 간호사의 이직의도에 영향을 미치는 요인 : 감정노동, 역할갈등, 호혜성을 중심으로 (Factors Influencing Turnover Intention of Nurses in Comprehensive Nursing Care Wards : Focused on Emotional Labor, Role Conflict, and Reciprocity)

  • 본 연구의 목적은 간호간병통합서비스 병동 간호사의 감정노동, 역할갈등, 호혜성이 이직의도에 영향을 미치는 요인을 확인하는 것이다. 연구대상자는 G및 M시 소재 13개 병원에서 근무 중인 간호간병통합서비스 병동 간호사 209명이다. 자료분석은 기술통계, t-test, one-way ANOVA, Pearson 상관관계, 위계적 다중회귀분석을 시행하였다. 연구모델은 이직의도의 52.9%를 설명하였고(F=14.458, p<.001), 이직의도에 가장 큰 영향을 준 변수는 감정노동(β=.311)이었고, 호혜성, 역할갈등 순으로 영향을 주었다. 따라서 감정노동을 수행하는 과정 중 경험하는 다양한 사례에서 간호사를 보호할 수 있는 장치와 지침 마련이 필요하며, 우호적이고 평등한 인간관계를 유지할 수 있는 인적자원 관리 시스템의 꾸준한 시행이 필요하다.

간호·간병통합서비스병동 간호사의 위임준비성과 직무만족 간의 관계 (Relationship between Delegation Preparedness and Job Satisfaction of Nurses in Comprehensive Nursing Care Service Units)

  • 본 연구의 목적은 간호·간병통합서비스병동 간호사를 대상으로 위임준비성과 직무만족 간의 관계를 파악하기 위함이다. 자료 수집은 2017년 5월 26일부터 2017년 6월 7일까지 서울과 경기 지역에 위치한 5군데 종합병원의 간호·간병통합서비스병동에서 근무하는 간호사 126명을 대상으로 시행되었다. 자료는 independent t-test, one way ANOVA, Pearson's correlation coefficients을 이용하여 분석되었다. 연구결과 간호사의 위임 교육 여부(t=-2.77, p=.006)가 위임준비성에 유의한 차이를 나타냈고, 위임준비성과 직무만족 간에 유의한 양적 상관관계(r=.43, p<.001)가 있는 것으로 나타났다. 위임준비성 하부 영역과 직무만족에서는 업무 내용에 대한 이해(r=.26, p=.003), 위임업무구분(r=.45, p<.001), 위임 내용에 대한 이해(r=.35, p<.001), 위임 기술(r=.34, p<.001) 간에 유의한 양적 상관관계가 있는 것으로 나타났다. 위임준비성과 직무만족의 하위 영역 중에서는 상호작용(r=.46, p<.001), 직무과업(r=.36, p<.001), 전문직업적 수준(r=.33, p<.001), 행정(r=.31, p<.001), 보수(r=.20, p=.026)가 유의한 양적 상관관계가 있는 것으로 나타났다. 이상의 결과는 간호·간병통합서비스병동 간호사가 효과적인 위임을 위해서는 학교와 간호현장에서 위임 기술을 포함한 교육을 강화시킬 필요성을 제시한다.

  • 본 연구는 간호·간병통합서비스 병동 간호사의 직무만족도에 미치는 영향을 파악하기 위해 수행된 서술적 조사연구이다. 본 연구는 3개 도시 250병상이상 종합병원 2개와 800병상이상 상급종합병원 3개로 총 5개 병원에서 수행하였다. 연구 대상자는 간호·간병통합서비스 병동에 근무하고 있는 간호사로150부를 배포하여 최종 146부를 회수하였다. 자료수집 기간은 2019년 8월 1일부터 2019년 11월 15일까지였고, x2-test, t-test, ANOVA, Pearson 상관관계, Multiple linear regression으로 분석하였다. 감정노동과 강인성은 음의 상관관계(r=-.20, p=.018), 감정노동과 직무만족도는 양의 상관관계가 나타났다(r=.25, p=.004). 대상자의 직무만족도에 영향을 미치는 예측변인은 병동업무의 적성여부(β=-.35, p=<.001)와 강직성(β=.21, p=.009)으로 나타났다. 직무만족도의 설명력은 18.2%였다(F=3.54, p=.009). 본 연구 결과 간호·간병통합서비스 병동 간호사의 직무만족도에 영향을 미치는 예측변인은 강직성과 병동업무의 적성여부로 나타났다. 따라서 해당 병동 간호사의 직무만족도를 높이기 위해서는 간호사의 강직성 향상과 적성을 고려한 인력배치 등의 행정적인 뒷받침이 요구된다.

  • Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.

  • Purpose: This study aims to examine the relationships between the self-leadership, role conflict, nursing work environment, and quality of nursing service in comprehensive nursing care service wards and identify the factors that affect the quality of nursing service. Methods: The data were collected from 158 nurses working in comprehensive nursing care service wards from three general hospitals with 200 beds or more in Seoul, Korea. The data were analyzed by descriptive statistics, t-test, ANOVA, scheffe test, Pearson's correlation coefficient, and multiple linear regression analysis of enter method using SPSS/WIN ver 22.0 program. Results: The factors of quality of nursing services were self-leadership (β=.44, p<.001), nursing work environment (β=.17, p=.014), and the work experience in comprehensive nursing care service wards (β=-.15, p=.035) explaining 32% of the total variance. Conclusions: The results indicated that self-leadership, work experience in the comprehensive nursing care service wards, and nursing work environment affect the quality of nursing services of nurses in the comprehensive nursing care service wards. It is necessary to make efforts for seeking various intervention strategies and improving nursing work environment.

간호⋅간병통합서비스병동 간호사의 조직 내 의사소통과 셀프리더십이 환자안전역량에 미치는 영향: 중소병원을 중심으로 (The Influence of Nurses' Organizational Communication and Self-Leadership on Patient Safety Competence in Comprehensive Nursing Service Units: Focusing on Small and Medium-sized Hospitals)

  • Purpose: This study investigated the influence of nurses' organizational communication and self-leadership on patient safety competence in comprehensive nursing care units of small and medium-sized hospitals. Methods: A descriptive design was used and self-reported questionnaires were used to collect data from 165 nurses in Seoul from February to March, 2020. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple linear regression with SPSS/WIN 22.0 program. Results: A mean score of organizational communication was 3.20±0.49, self-leadership 3.58±0.50, and patient safety competency 4.01±0.49 out of 5. A significant positive correlation was found between patient safety competence, organizational communication and self-leadership. Self-leadership and combined ward explained 32% of the variance of patient safety competence (Adjusted R2=.32, p<.001). Conclusion: Patient safety competence in comprehensive nursing care units are associated with self-leadership and organizational communication. Systemic education to improve patient safety competence in small and medium sized hospitals should be implemented to promote organizational communication as well as self-leadership.

국민건강보험 간호·간병통합서비스의 전면 도입을 위한 간호인력 및 재정비용 추계 (Optimal Nursing Workforce and Financial Cost to Provide Comprehensive Nursing Service in the National Health Insurance System)

  • 본 논문은 병원에서 간호 간병통합서비스가 전국에 확대 적용되었을 경우 필요한 간호인력과 재정비용을 추계한 연구이다. 연구자료는 건강보험심사평가원으로부터 받은 2012년 기준 간호사 수, 간호조무사 수, 의료기관 수, 환자 수를 이용하였다. 간호 인력의 규모는 결정론적 방법으로 개발된 작업부하모델에 의해 추정되었다. 간호간병통합서비스 연간 총 재정비용을 계산하는데 상향식 추정방법이 사용되었다. 간호사 및 간호조무사 수는 각각 81.8%, 83.2% 더 증가되어야 간호 간병통합서비스가 전국 규모로 적용가능한 것으로 추정되었다. 전국의 모든 일반병동에 간호 간병통합서비스가 적용되기 위해 필요한 재정적 비용은 110.4% 더 증가해야 할 것으로 추정되었다. 이 새로운 시스템을 성공적으로 구현하려면, 인력공급의 양적확대 전략뿐만 아니라 숙련된 간호사와 신규간호사의 이직을 최소화해야 한다. 또한 간호사-환자 비의 타당성을 지속적으로 확인할 필요가 있다. 새로운 시스템을 모든 병원에 단계적으로 확대하여야 국민건강보험의 재정부담을 최소화할 수 있을 것이다.

  • Purpose: The purpose of this study is to provide patient-centered nursing by comparing the patient needs, patient satisfaction, and communication type of nurses for patients admitted to integrated nursing care service ward and general ward. Methods: Data were collected by self-report questionnaires between September 7 and October 9, 2021. Patients were patients recruited from a hospital in D city, 100 each from comprehensive nursing care service ward and general ward. The data were analyzed using descriptive statistics, 𝜒2-test, t-test, ANOVA, and Pearson's correlation. Results: There was no significant difference in patient needs between the patients of integrated nursing care service ward and the general ward (t=-1.12, p=.263). However, patient satisfaction was higher among the patients in integrated nursing care service ward than in the general ward (t=-3.55, p<.001). Pertaining to the communication type of nurses, the informational communication type (t=-4.02, p<.001) and the friendly communication type (t=-3.92, p<.001) were stronger in the integrated nursing care service ward than the general ward. It was, hence, confirmed that the educational requirements were positively correlated with patient satisfaction in the reliability domain and technical-professional domain, and that the nurse informational communication type and friendly communication type had a significantly positive correlation with patient satisfaction. Conclusion: The expansion of the integrated nursing care service ward may be considered in the future, with an informational and friendly communication method as an effective response that can be adapted in practice as a reflection the type of communication among nurses.

간호·간병통합서비스 병동 간호사의 우울 예측 요인 (Factors Influencing Depression of Nurses among Comprehensive Nursing Care Service Ward)

  • Purpose: The purpose of this study was to identify depression and its influencing factor of nurses among comprehensive nursing care service ward. Methods: The research was cross-sectional descriptive study. The subjects of the study were 173 nurses of comprehensive nursing care service ward in Gyeonggi-do. Data collection was done using self-reported structured questionnaires asking about job stress, emotional labor, job burnout, somatic symptoms, turnover intention, depression and general characteristics from May 16 to June 3, 2016. Data were analyzed with SPSS/WIN 19.0 program. Results: Mean scores of job stress were 159.15(range 43~215), emotional labor 30.90(range: 9~45), job burnout 59.45 (range: 0~132), somatic symptoms 24.03 (range: 12~60), turnover intention 5.61 (range 0~18) and depression 19.25 (range: 0~60) and reported as depression in 63.6%. Somatic symptoms (${\beta}=.26$, p<.001), job burnout (${\beta}=.37$, p<.001) had significant associations with depression and the most important variable was job burnout. Conclusion: It is necessary to apply the developed program to reduce job burnout in hospitals and to prevent and/or control depression. And also, it needs to improve working conditions and increase the nursing staff to reduce somatic symptoms for nurses among comprehensive nursing care service ward.

간호·간병 통합서비스 병동 교육전담간호사 운영 효과 (Effect of Educational Nurse Practitioners at Comprehensive Nursing Care Service Units)

  • Purpose: This study aimed to investigate the effect of educational nurse practitioners at comprehensive nursing care service units by comparing the educational satisfaction of general nurses and the occupational stress of preceptor nurses before and after education by educational nurse practitioners. Methods: Participants were 71 general nurses and 71 preceptor nurses working at comprehensive nursing care service units of G University hospital in I City. The level of educational satisfaction of general nurses and occupational stress of preceptor nurses were measured and compared. The differences in educational satisfaction and occupational stress before and after the education program were verified by paired t-test and one-way ANOVA using SPSS/WIN 27.0 statistical program. Differences before and after the education program according to the general characteristics were analyzed with repeated measure analysis of variance after Sapiro-Wilks normality test, and post-hoc analysis was conducted using Scheffétest. Results: It was inferred that there was a significant difference (t=-9.32, p<.001) in the educational satisfaction of general nurses, with an average of 3.72±0.77 before the education and 4.47±0.60 after the education. However, there was no significant difference in occupational stress in the preceptor nurses. Conclusion: It could be concluded that introduction of educational nurse practitioners at comprehensive nursing care service units has a positive effect on the educational satisfaction of general nurses. This result will serve as basic data to implement educational nurse practitioners for general nurses in all medical institutions as well as comprehensive nursing care service units.
