Comparison of Inpatient Medical Use between Non-specialty and Specialty Hospitals: A Study Focused on Knee Replacement Arthroplasty (전문병원과 비전문병원 입원환자의 의료이용 비교 분석: 인공관절치환술(슬관절)을 대상으로)

  • Background: The purpose of this study was to determine the effectiveness of the specialty hospital system by comparing the medical use of inpatients who had artificial joint replacement surgery in specialty hospitals and non-specialty hospitals. Methods: This study utilized 2021-2022 healthcare benefit claims data provided by the Health Insurance Review and Assessment Service. The dependent variable is inpatient medical use which is measured in terms of charges per case and length of stay. The independent variable was whether the hospital was designated as a specialty hospital, and the control variables were patient-level variables (age, gender, insurer type, surgery type, and Charlson comorbidity index) and medical institution-level variables (establishment type, classification, location, number of orthopedic surgeons, and number of nurses). Results: The results of the multiple regression analysis between charges per case and whether a hospital is designated as a specialty hospital showed a statistically significant negative relationship between charges per case and whether a hospital is designated as a specialty hospital. This suggests a significant low in charges per case when a hospital is designated as a specialty hospital compared to a non-specialty hospital, indicating that there is a difference in medical use outcomes between specialty hospitals and non-specialty hospitals inpatients. Conclusion: The practical implications of this study are as follows. First, the criteria for designating specialty hospitals should be alleviated. In our study, the results show that specialty hospitals have significantly lower per-case costs than non-specialty hospitals. Despite the cost-effectiveness of specialty hospitals, the high barriers to be designated for specialty hospitals have gathered the specialty hospitals in metropolitan and major cities. To address the regional imbalance of specialty hospitals, it is believed that ease the criteria for designating specialty hospitals in non-metropolitan areas, such as introducing "semi-specialty hospitals (tentative name)," will lead to a reduction in health disparities between regions and reduce medical costs. Second, it is necessary to determine the appropriateness of the size of hospitals' medical staff. The study found that the number of orthopedic surgeons and nurses varied in charges per case. Therefore, it is believed that appropriately allocating hospital medical staff can maximize the cost-effectiveness of medical services and ultimately reduce medical costs.

A Study on Clinical Reasoning Ability and Academic Achievements in Nursing Students (간호대학생의 임상추론 능력과 학업성취도)

  • The purpose of this study was to verify clinical reasoning of nursing students as well as clarify the relevance between clinical reasoning and academic achievement. 87 students broke into 30 groups of two or three. The students performed post operative care for adult patients in the simulation practicum course of their final semester. Data were collected by evaluating the video-recorded nursing practicum using the clinical reasoning rubric. Clinical reasoning of 61(70.1%) students was limited to the beginning level and that of 6(6.9%) students of the highest scoring was at the third level among the four levels of ability. There was no significant correlation between clinical reasoning and the final cumulative GPA, while there was low level of significant correlation between clinical reasoning and GPA of adult health nursing course or adult health nursing practicum course. Therefore, it is necessary to improve the nursing curriculum to enhance nursing students' clinical reasoning.

Experiences and Process of Patients' Healing Relation with Nurses (환자-간호사간의 치유적 관계형성 경험과 과정)

  • The purpose of this study is to know experiences and process of patients' healing relation with nurses. This study had been conducted from July to September in 1994. The subjects were 7 patients who had hospitalized within 10 days at general surgery unit in C university hospital. The data were collected through in-depth interview and observation on the basis of Grounded Theory. In-depth interview were performed 3-6 times for each patient and taken 10-15 minutes at once. The result were as follows : (1) The main concepts are worry to operation, worry to hospital environment, worry to their children, worry to nurses, general appraisal, appraisal of appearance, appraisal of attitude, appraisal of nurses' response, appraisal of nurses' help, interest to patients, appraisal of explanation, appraisal of direct nursing, appraisal of nursing role, appraisal of nursing fairness, empathy, feeling of jung, discontent exposure, content, rapport. (Jung is a unique feeling that appeared in only korean culture) (2) The main categories are worry, general appraisal, concrete appraisal, empathy, and rapport. (3) These process is worry, appraisal, empathy, and rapport in the order. (4) The core category is the need of relation formation. The hospitalized patients have had need of relation formation with nurses. In order to satisfy this need, patients in the early of hospitalization had the worries to nurses, hospital environment, their children and themselves. The more patients have information about themselves, the more they have worries. In addition to, patients have general appraisal through the first impression or feeling to nurses. A time goes by, general appraisal has changed concrete appraisal. High educated group (above bachelor degree) have expressed concretely and variousely their needs. The patients who have empathy to nurses are content with nurses and form rapport to be shown frankly thier discontents. Therefore, patients' healing relation formation is the process of being filled up the need of relation formaton with Nurses. As mentioned above, researcher suggests that nurses need study nursing strategies to make earlier the last phase of the healing relation formation, rapport.

A Study on the Hull-dimension of 89 ton class Stow-net Vessel with Stern-fishing (89톤급 선미식 안강망어선의 선형치수에 관한 연구)

  • This paper presents the optimum dimension of 89 ton class stow-net vessel with stern-fishing. The model of basic design is developed by using the optimization techniques referring to objective function and numerous constraints as follows; speed, fishing quantity, fishing days, catch per unit effort(CPUE), and weight/ratio of main dimensions, etc. Thus, the basic design of stow-net fishing vessel is built up by using the optimization of the design variables called the economic optimization criteria, and the objective function represents the criterion which is cost benefit ratio(CBR). The main conclusions are as follows. 1. S/W for decision of optimum hull size is developed in 89 ton class stow-net fishing vessel which is constructed with optimization of the design variables called the economic optimization criteria. 2. For optimum ship dimensions in 89 ton class stow-net fishing vessel, the hull dimensions can be obtained in the range of L= 27.3m, B = 6.6m, D = 2.80m, Cb = 0.695, T/D = 0.80, $\Delta$(displacement)=281.7ton with 10 knots.

The Effects of Verbal Violence in Clinical Practice, Academic Stress Coping, and Depression on Professional Self-concepts among Korean Nursing Students (간호대학생의 임상실습 시 경험한 언어폭력, 학업스트레스 대처 및 우울이 전문직 자아개념에 미치는 영향)

  • The aims of this study were to investigate the effects of verbal violence experienced during clinical practice, emotional responses, academic stress coping, and depression on professional self-concepts of nursing students. Data were collected through a self-reported structured questionnaire form given to nursing students in grades 3-4 using convenient sampling methods from October 25 to November 1, 2017. As a result, 41.9% of the subjects experienced verbal violence during clinical practice. The frequency of experiences for verbal violence was '1 time' at 16.9%, '2 times' at 15.5%, and '3 times' at 4.7%. The verbal violence attackers were patients (42.9%), nurses (37.7%), and protector (15.5%). Emotional response was positively correlated with depression and frequency of verbal violence. Academic stress coping had a negative correlation with depression and a positive correlation with professional self-concept. Depression was negatively correlated with professional self-concept. Multiple regression analysis showed that academic stress coping and depression were statistically significant variables in the professional self-concept, and these variables had a 28% explanatory power. Therefore, in order to improve the professional self-concept of nursing students, it is necessary to develop a strategy to raise the level of academic stress coping and to reduce depression.

Functional Status, Hostility, and Social Support in Patients with Percutaneous Coronary Intervention (관상동맥중재술 대상자의 신체적 기능상태, 적대성향, 사회적지지에 관한 연구)

  • The purpose of this study was to determine the relationships among the functional status, hostility, and social support in patients with percutaneous coronary intervention (PCI). The subjects were comprised of 135 patients who had received PCI from two general hospitals in Seoul, Korea. The functional status differed significantly according to gender (t=5.880, p<0.001), age (F=23.620, p<0.001), education (F=17.718, p<0.001), occupation (t=-6.498, p<0.001), monthly income (F=7.237, p<0.001), and smoking (t=2.327, p=0.025). The hostility differed significantly according to age (F=6.150, p=0.001), period after PCI (F=6.141, p=0.001), and family history (t=2.514, p=0.013). Differences were observed in the scores for social support according to age (F=2.866, p=0.039) and education (F=5.136, p=0.002). Significant positive correlations were found among functional status and social support (r=.20, p=0.025), friends support (r=.22, p=0.010) and a significant negative correlation between functional status and hostility (r=-.24, p=0.005). A significant negative correlation was found between hostility and friends support (r=-.17, p=0.046). Nurses should consider the characteristics of patients who have undergone PCI and develop nursing intervention programs to improve their functional status, hostility, and social support.

The Effects of Emerging Infectious Disease Knowledge and Clinical Practice Stress on Nursing Students' Coping with Stress (신종감염병지식과 임상실습스트레스가 간호대학생의 스트레스대처방식에 미치는 영향)

  • The purpose of this study is to inquire into the effect on the coping with stress by the knowledge of Emerging infectious disease, clinical practice stress. A research was held to the nursing students living in Seoul and Chungbuk from September 10 to October 10, 2022, 259copies of the data were used for the final analysis, Pearson's correlation coefficient and Multiple linear regressions was used. As a result of the study, there was a positive correlation between clinical practice stress due to burden of work, practice education environment stress, and active coping with stress. Clinical practice stress due to interpersonal conflicts, conflicts with patients, burden of work, and undesirable role models was positively correlated with passive coping with stress. Satisfaction of clinical practice, practical educational environment stress and gender accounted for 15.0% of the total variance in the active stress coping, and burden of work accounted for 7% of the total variance in the passive stress coping. Consequently, this study could be suggested as a basis for counseling and developing practical education program for active coping with stress.

Effects of Mobile based-Healthcare Service using Human Coaching to the Self-care of Diabetes (당뇨병 환자의 자가관리를 위한 모바일 기반의 휴먼코칭 헬스케어서비스의 효과)

  • This study aims to investigate the impact of a mobile-based human coaching healthcare service provided for diabetic patients by nurses on their self-management ability. This study collected data from 311 persons who were diagnosed with diabetes from all over the country, and the collected data were analyzed through SPSS Win 23. The results of the study are summarized as follows: First, it was found that the utilization of a glucose meter by the provision of the mobile-based human coaching healthcare service was men group, and there was a statistically significant difference (${\chi}^2=6.059$, p= .048*). Second, there was a positive correlation in the utilization between the glucose meter and an activity tracker band by the provision of the mobile-based human coaching healthcare service, which was statistically significant (r=.660, p< .001). Third, as a result of checking the changes of the utilization of the glucose meter by the provision of the mobile-based human coaching healthcare service, it was maintained between 79% and 41% from Week 1 through 49. In conclusion, this study partially confirmed that the mobile-based human coaching health service was easily accessible and cost-effective in enhancing the participants'self-healthcare ability.

The survey on the awareness towards academic misconduct required in the Convergence era : Focusing on Nursing Students (융복합시대에서 요구되는 학습윤리인식 실태조사: 간호대학생을 중심으로)

  • The purpose of this study was to investigate the awareness towards academic misconduct in nursing students. A descriptive survey was performed from November to December in 2015 with self-reporting questionnaires for awareness toward academic misconduct. Participants were 355 nursing students who was in attendance at department of nursing in three university. Data were analyzed using SPSS 22.0. As a result of the study, nursing students' average point of awareness towards academic misconduct is about 1.71 out of 4. The study showed that irresponsibility on the class was the lowest level, on the other hand, disrespectful behavior in patient care was the highest among awareness towards academic misconduct. This has significant differences according to the their interpersonal relationship and motivation of admission. This study may provide basic data for enhancement of nursing students' recognition of academic improvement in academic integrity. Further studies should be needed to assess academic integrity policies, violations, and the results of the measures in nursing students.

The Influence of Ethical Values and Ethical Sensitivity on Character of Nursing Students (윤리적 가치관과 윤리적 민감성이 간호대학생의 인성에 미치는 영향)

  • The purpose of this study was to examine the influence of ethical values and ethical sensitivity on character in junior and senior nursing students. A total of 356 surveys were analyzed using SPSS version 20. T-test, ANOVA/Kruskal-Wallis test, Pearson correlation coefficients, and stepwise multiple regression analysis were used. The results of the study showed that the main factors influencing character was patient oriented care in ethical sensitivity (${\beta}=0.258$), followed by nurse-colleague relationship among ethical values (${\beta}=0.153$), religion (${\beta}=0.131$), professional responsibility in the ethical sensitivities (${\beta}=0.101$), and the above variables accounted for 43.3% of character (F=44.960, p<.001). This study confirmed that ethical values and ethical sensitivity are important variables affecting character and convergence course integrating above factors needs to be provided for nursing students.
