Purpose: The usability, user satisfaction, and impact of electronic nursing record (ENR) systems were investigated. Methods: This mixed-method research was performed as a time-motion (TM) study and a survey which were carried out at six hospitals between August and November 2013. The TM study involved 108 nurses from medical, surgical, and intensive care units at each hospital, plus an additional 48 nurses who served as nonparticipating observers. In the survey, 1879 volunteer nurses completed the Impact of ENR Systems Scale, the System Usability Scale, and a global satisfaction scale. Qualitative and quantitative analyses were performed. Results: The mean scores for the ENR impact, system usability, and satisfaction were 4.28 (out of 6), 58.62 (out of 100), and 74.31 (out of 100), respectively, and they differed significantly between hospitals (F=43.43, p<.001, F=53.08 and p<.001, and F=29.13 and p<.001, respectively). A workflow fragmentation assessment revealed different patterns of ENR system use among the included hospitals. Three user characteristics-educational background, practice period, and experience of using paper records-significantly affected the system usability and satisfaction scores. Conclusion: The system quality varied widely among the ENR systems. The generally low-to-moderate levels of system usability and user satisfaction suggest many opportunities for improvement.
Purpose: This study was done to examine changes towards positive ethics in registered nurses working in hospitals who took part in performing 'theatre of situations' in terms of issues of nursing ethics. Methods: This research was conducted with one-group pretest-posttest design. The sample, 232 registered nurses, voluntarily participated in a contest of 'Theatre of Situations' with themes of nursing ethics hosted by Hospital Nurses Association of Korea on September 8, 2011. For this contest, eight nursing situational scenario regarding ethical issues or dilemma were created. The theatre of situation was performed by volunteer nurses over a period of 160 minutes. Data were collected before and after the performance using 10 items to measure Positive Ethics. Item internal consistency had a Cronbach's alpha in this study of .716. Results: Before the theatre of situations was performed, the mean (${\pm}$standard deviation) level of positive ethics was 2.34 (${\pm}.37$). For the nurses who participated, there was a significant increase in the degree of positive ethics after the theatre of situations ($2.62{\pm}.36$, p<.001). Conclusion: This result implies that indirect experiences such as 'theatre of situations' in terms of ethical issues, created as well as performed, is effective for registered nurses in changing their ethical perspective positively and in realizing ethical ideals.
Purpose : The 2020 Dietary Guidelines Advisory Committee specifically noted that meal frequency is associated with risks for cardiovascular disease, type 2 diabetes, and all-cause mortality, although the current evidence on meal frequency is conflicting. As meal frequency itself is affected by various factors, the aim of the study was not only to examine its relationships with cardiometabolic risk but also to identify the mediating effects of dietary quality. Methods : This study used a descriptive correlational design. In all 8,141 healthy adults participated in the study. Measurements included meal frequency, cardiometabolic risk, and diet quality. Data were analyzed using descriptive statistics, Pearson's correlation coefficient analysis, and process macro bootstrapping model 4. Results : The meal frequency was 3.52±0.61 times per day, the risk of cardiovascular metabolic diseases was 0.01±0.61 points, and the diet quality was 62.08±13.87 points. In mediation analysis, the effect of meal frequency on cardiometabolic risk score was completely mediated by diet quality. Conclusion : Improved diet quality in healthy adults should be considered when designing meal frequency interventions aimed at reducing their cardiometabolic risk, as the effect of meal frequency support on cardiometabolic risk was found to be mediated by diet quality.
Background: This study purposed to examine how the incidence of the central line-associated bloodstream infection (CLABSI) in a operating room (OR) is affected by bundle application on central line insertion (CLI) practice. Methods: The study design was a pretest-posttest experimental trial. The subjects were 83 patients before the bundle application on CLI and 70 patients after. Results: The compliance(%) of bundle on CLI of among those who observed all of the five items increased from 7.2% before the intervention to 72.9% after. By items, compliance with the maximal barrier precaution was 100% for the use of a mask and cap before and after the intervention, but increased from 73.5% before the intervention to 88.6% after for the hand hygiene, from 73.5% to 88.6% for the use of a sterile gown, and from 9.6% to 75.7% for the use of a sterile large drape covering the whole body. CLABSI did not happen on CLI either before or after the application of the bundle intervention. Conclusion: Bundle application increased compliance with the use of a sterile gown and the use of a sterile large drape. However, its effect in the prevention of CLABSI was not clear probably due to the short period of intervention in a single hospital.
본 연구는 지역사회 노인복지관 이용 노인의 주관적 건강상태, 의료인과 의사소통, 약물오남용이 약물복용이행에 대한 융합 영향요인을 파악하기 위한 서술적 조사연구이다. 본 연구 대상자는 지역사회 노인복지관 이용 노인 179명이었다. 자료는 2020년 1월13일부터 2020년 1월17일까지 수집하였다. 자료분석은 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, 사후검정은 Scheffe test, Pearson's correlation coefficients, 다중회귀분석을 실시하였다. 본 연구 회귀식은 약물복용이행을 24%를 설명하였다. 대상자의 약물복용이행에 가장 큰 영향을 미치는 요인은 약물오남용(𝛽=-4.32, p=<.001)이었고 다음은 만성질환 유,무(𝛽=-3.04, p=.003), 결혼상태(𝛽=2.64, p=.009), 의료인과 의사소통(𝛽=2.26, p=.025 ) 순으로 나타났다. 따라서 지역사회 노인복지관 이용 노인의 약물복용이행을 위한 융합적 시스템 구축이 요구된다.
본 연구의 목적은 노인복지관 여가활동 참여자들의 자아통합감과 전두엽 인지기능간의 관계를 파악하기 위한 서술적 조사연구이다. 자료 수집은 K도, M시 소재한 복지관 이용하는 65세 이상 노인 120명을 대상으로 2018년1월 16일 ~1월 30일까지 실시하였다. 자료 분석은 실수와 백분율, 평균 및 표준편차, t검증, ANOVA, Pearson's correlation coefficient, multiple regression analysis로 분석하였다. 본 연구의 결과는 노인복지관 여가활동 참여대상자들의 현재 만족이 자아통합감의 '생에 대한 태도', '지나온 일생 수용', '현재 삶의 만족'과 통계적 차이를 보였고, 자아통합감은 현재 만족정도에 28.3%정도 설명력을 보였다. 동거인에 따라 전두엽 인지기능에 미치는 효과는 16.1% 설명력을 보였다. 본 연구는 노년기의 자아통합감과 전두엽 인지기능 향상을 위한 노인 공동체 생활을 함께 할 수 있는 정책과 프로그램 개발의 필요성을 시사한다.
본 연구는 간호대학생들의 대인관계, 대처 및 임상실습 만족도가 전문직 자아개념에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 연구자료는 M시와 S시의 2개 대학교, 2개 학과 간호대학생 355명을 대상으로 구조화된 설문지로 조사하였다. 수집된 자료는 SPSS WIN 14.0을 이용하여 Independent t-test, One-way ANOVA, Pearson correlation coefficients, Stepwise multiple regression으로 분석하였다. 연구결과, 대상자의 종교유무, 가정의 경제적인 수준, 학교성적에 따라 전문직 자아개념에 차이가 나타났다. 전문직 자아개념은 대인관계. 대처, 임상실습만족도와 모두 순상관관계를 나타냈으며, 대인관계(${\beta}=.45$), 적극적 대처(${\beta}=.30$), 종교(${\beta}=.15$), 학교성적(${\beta}=.11$), 소극적 대처(${\beta}=-.12$)가 간호대학생들의 전문직 자아개념에 영향을 미친 요인으로 총 설명력은 45.2%였다. 따라서 간호대학생들의 전문직 자아개념을 향상하기 위해서는 대인관계, 대처, 종교, 학교성적이 고려된 교육적 프로그램의 고려가 필요할 것으로 생각된다.
본 연구는 전화코칭을 병행한 운동프로그램이 지역사회 거주 독거노인의 균형능력, 악력 및 우울에 미치는 효과를 조사하기 위한 단일집단 사전사후 원시실험연구이다. 연구대상자는 경기도 S시에 위치한 노인복지관에 등록된 독거노인 20명이 참여하였으며, 자료수집기간은 2021년 7월 03일부터 2021년 10월 14일까지 총 12회기 프로그램을 시행하였다. 프로그램 전후에 대상자 가정에 직접 방문하여 4단계 균형검사, 근력검사, 악력 및 우울을 측정하였다. 중재 프로그램은 탄력밴드와 악력볼을 이용하여 구성하였으며 매주 전화코칭 중재를 제공하였다. 연구결과, 전화코칭을 병행한 운동 프로그램은 대상자의 4단계 균형기능(3단계)(t=-2.37, p=.029), 4단계 균형기능(4단계)(t=-2.46, p=.024), 우측 악력(t=-2.18, p=.042), 우울(t=2.82, p<.001)에서 통계적으로 유의한 차이가 나타났다. 본 연구 결과를 바탕으로 전화코칭을 병행한 운동프로그램은 독거노인의 균형능력, 악력 향상 및 우울 감소를 위한 간호중재로 활용될 수 있을 것으로 기대된다.
Purpose: This study was conducted to identify the clinical characteristics and risk factors on the occurrence of variant angina, and to examine the predicting factors on the vascular endothelial dysfunction of the patients with variant angina. Methods: A total of 134 patients diagnosed with variant angina were recruited from 2006 to 2008. The degrees of endothelial dysfunction were measured and recorded by the researcher using the values of flow-mediated vasodilation of their brachial arteries and Nitroglycerine-mediated dilation. Subjects' demographic data and risk factors were gathered after obtaining informed consent, and their electronic medical records were reviewed to collect laboratory data. Results: The mean age was $54.2{\pm}9.6$ years and 52% was male patients. More than 50% of the male patients were cigarette smokers and had hypercholesterolemia. 84% of the male patients and 70% of the female patients had more than one risk factor of cardiovascular disease. A stepwise multiple regression analysis showed that smoking and hypercholesterolemia predicted the decrease of flow-mediated vasodilation (Adjusted $R^2$ = .204, p < .001). Conclusion: Tailored educational interventions for smoking cessation and cholesterol management are needed to prevent recurrence of angina attack for patients with variant angina and to prevent cardiovascular disease for middle-aged workers.
Purpose: The purpose of this study was to verify the validity and reliability of the Type D Scale-14 (DS14). Methods: The participants were 288 patients who were diagnosed with angina pectoris, myocardial infarction, and hypertension. DS14 was developed by Denollet(2005) and consists of two domains, 7 items on negative affectivity (NA) and 7 items on social inhibition (SI). The Korean version of DS14 was developed through translation-reversed translation and a preliminary test. Data were collected using a self-report questionnaire. Cronbach $\alpha$ and Guttman split-half were used to test reliability and item analysis and factor analysis for validity. The SPSS program was used. Results: 35.8% of the participants were classified as Type D. Mean score for NA was 16.80 and for SI, 14.10 in Type D participants. For reliability of NA, Cronbach $\alpha$=0.771, and for SI, 0.707. Factor analysis on 12 items(numbers 1 & 3 were excluded as the corrected item-total correlations were below r=0.3) yielded two factors for NA (6 items) and SI (6 items). Number 7 in the NA domain was sorted into the SI domain. Conclusion: The results indicate, the cultural differences were between Europeans and Koreans. Repetition of the research is needed for generalization of DS14.
