The Implementation Status of Dental Treatment Infection Control Standards of Dental Hygienists (치과위생사의 치과진료 감염방지기준에 관한 연구)

  • This study was conducted to find out about the implementation status of dental treatment infection control standards of dental hygienists. The subjects of research were the dental hygienists working at dentist offices in Chungnam, Cheonan-si using self-input method from July 25th, 2012 to August 1st 2012. The collected data was analyzed after computerized statistical processing using SPSS 18.0. For hepatitis B vaccination, the results were high with 75.0% of answers being 'yes' or auto-active immunity, but for the latest hepatitis vaccination period showed highest results in '5 years or more ago' with 48.0%. Although 93.0% answered that vaccination was important, the percentage of replies that they had vaccination education was relatively low with 41.0%. For the use of personal protection tools the use of protective goggles was low compared to the use of masks and medical gloves. The percentage of subjects that answered that they always wash their hands before treatment was relatively low with 56.0% compared to 82.0% of subjects that answered that they always washed their hands after treatment. Dental treatment equipment washing before sterilization, use of packing and re-sterilization of tools with damaged packing showed high results for 'yes', but the ratio of subjects that answered 'no' to water line management was low with 39%.

A Study of Occurrence of Secondary Complications and Chronic Diseases due to Aging of Spinal Cord Injury (척수손상환자의 고령화에 따른 2차합병증 발생과 만성질환발병에 관한 연구)

  • The purpose of this study is to provide basic data for effective management and prevention of chronic diseases and secondary complications as elderly spinal cord injuries(SCI). The subjects were 200 spinal cord injuries admitted to S hospital from April, 2013 to April, 2018. We investigated the occurrence of chronic diseases and secondary complication through medical records. The results showed that SCI were affected chronic diseases and secondary complications over 50years. The prevalence of chronic diseases and secondary complications over 50 years of age had the odds ratio 11.8 times higher in hypertension and 6.7 times diabetes mellitus. Secondary complications had the odds ratio Osteoporosis 7.5 times, Pneumonia 5.2 times, and central pain 0.4 times. We suggest that continuous management and service of chronic diseases and secondary complications of elderly SCI are necessary. It will be necessary to expand the target population and to study various characteristics including.

A Study on Awareness and Degree of Practice about Infection Control by Dental Hygienics Student's in Some Ares (일부지역 치위생과 학생의 감염관리 인식 및 실천도에 관한 조사)

  • This research surveyed 324 college students majored dental hygiene in Gwangju and Chonnam province to analyze their awareness and degree of practice about infection control. Through the study for recognition about infectious disease related with the grade, the sophomore students are the group who is the most aware. In addition, the study shows students who experienced teeth cleaning have high degree of awareness (p<0.05). The research of perception about infectious disease based on experiences for vaccinations and education about infection prevention says hepatitis B has the highest rate when it comes to level of occurrence risk and tuberculosis and acquired immune deficiency syndrome are the lowest (p<0.05). According to the research, the group who had vaccination, knowledge about infection prevention and experience for scaling shows high practice rate for hand washing according to whether students receive protective inoculation or not. Depending on what the result were, since student learned about infection control has high degree of recognition and practice about infection management if we could emphasize the importance to students through regulative education about infection control and then increase the degree of practice, it would make big contributions to the effective infection control.

Relationships with Alanine Aminotransferase Activity, and Anthropometric and Biochemical Measures in Obese Children (단순 비만아에서 간효소치와 신체 계측 및 생화학 측정치와의 관련성)

  • Purpose: To investigate the physical and biochemical parameters related with elevated serum alanine aminotransferase (ALT) levels in obese children. Methods: One hundred forty-two obese or overweight children who visited the out-patient clinics of Chungnam National University Hospital between January 2006 and August 2008 were enrolled. Physical measures and biochemical tests were performed in all patients. Liver sonography was performed in 43 patients. They were divided into the following 2 groups based on ALT levels: group I, normal ALT levels (n=65); and group II, elevated ALT levels (n=77). We compared the physical measures, biochemical results, and ultrasonographic findings of the livers in both groups. Other causes of elevated serum ALT levels were ruled out. Results: The male-to-female ratios were 1.6:1 in group I and 7.6:1 in group II. Among physical parameters, the waist circumference-to-height ratio and hip circumference-to-height ratio were significantly higher in group II (p=0.001 and 0.046, respectively). Among biochemical parameters, aspartate aminotransferase (AST), ${\gamma}$-glutamyltransferase (${\gamma}$-GT), and total cholesterol levels were significantly higher in group II (p<0.001, 0.001, and 0.001, respectively). The AST/ALT ratio was <1 (mean, 0.55) and statistically lower in group II. There was a positive correlation between the serum ALT level and ${\gamma}$-GT (p<0.001, r=0.750), and a positive correlation between the serum ALT level and the waist circumference-to-height ratio in group II (p<0.001, r=0.401). Conclusion: The results suggest that the waist circumference-to-height ratio and ${\gamma}$-GT may be associated with elevated alanine aminotransferase activity in obese children.

Hepatic Cirrhosis Secondary to chronic Hepatitis in an English Cocker Spaniel (ECS) Dog (잉글리쉬 코커스파니엘 견에서 발생한 만성 간염 및 간경화 증례)

  • A 1-year-old, female English cocker spaniel (ECS) dog was presented with 3-month history of vomiting and retaking of the vomitus, and chronic weight loss. The client had noticed mild abdominal distension 10 days before. The dog was diagnosed as chronic hepatitis with hepatic cirrhosis based on complete blood count (CBC), serum chemistry profiles, radiography, ascites assessment, bile acid evaluation, and liver biopsy through exploratory laparotomy and necropsy. CBC and serum chemistry profiles revealed mild anemia, slightly elevated hepatic enzymes (ALT and AST), increased creatinine kinase (CK), hyperammonemia, and hypoproteinemia with hypoalbuminemia. Ascites was transudate according to analysis of components. Bile acid assessment (fasting; $174.4{\mu}mol/L$ and postprandial; $198.4{\mu}mol/L$) showed strongly suspected hepatic insufficiency. On radiological findings, ascites was evident. Atrophied liver (especially left side lobes) and distended mesenteric vasculatures were observed by exploratory laparotomy. Histopathological examination of marginal lesion of left lateral lobe of liver by biopsy revealed the necrosis of hepatic cells, dilation of sinusoids, infiltration of neutrophils in sinusoids, and vacuolation of hepatic cytoplasm. The patient had been managed with careful low protein diet and specific supportive therapy (ursodeoxycholic acid, prednisolone, vitamine E, and interferon). Vomiting and ascites disappeared with medical management. The dog was monitored periodically by CBC, serum chemistry and radiographic examination. The dog survived more 18 months with medical therapy. After spontaneous death, necropsy and histopathologic examination were performed.

A research into perceptionality of students of the dental hygiene department on hepatitis B virus (치위생과 학생들의 B형 간염 바이러스에 대한 인지도 조사연구)

  • This study aims to furnish basic data about prevention and infection control of Hepatitis B Virus(HBV) for those who, working in dental offices, are particularly exposed to a high risk of HBV infection. A survey was conducted to 310 students including freshmen, sophomores and juniors enrolled in the dental hygiene department in order to examine their knowledge about infection routes of HBV, clinical history of their family members and their own health. The outcomes of the survey showed following facts; 1. Students were found to lack knowledge about the present conditions of their HBsAg and HBsAb of HBV(PF0.05), conduct of preventive vaccination(PE0.05), completion of 3 required vaccinations(PF0.05) and formation of antibody(PF0.05). 2. Students named "blood"(88.6%) and "infected needles"(82.5%) as most likely infection routes of HBV(PE0.05 and PE0.01). These replies came mostly from sophomores(65.6% and 92.1%), followed by juniors(89.2%, 82.5%) and freshmen(81.1%, 73.0%). Least knowledge about infection routes of HBV was sensed with the reply "infection through breast-feeding of positive mother(27.9%)"(PE0.05). Generally, sophomores seemed to have much knowledge about infection routes, followed by juniors and freshmen in order. 3. As to clinical history of family members, 10 students(3.5%) replied that any of their family members is suffering from HBV now, 8(2.6%) revealed that some of their family members once suffered from it and 10(3.2%) reported cases of death of their family members from liver diseases. 4. Ninety-four point seven percent of respondents believed their health to be better than normal.

A Survey of the Basic Protection Manners of Dental Personnels Against Hepatitis B in and around Seoul (수도권 치과의료 종사자들의 B형 간염에 대한 기초방호실태 및 수행에 관한 조사)

  • The purpose of this study was to examine dental personnels' concern for health, their personal protection manner, and whether or not they were inoculated against hepatitis B and made an inquiry of patients about that. It's ultimately meant to serve as a basis for the preparation of an anti-infection guide for the protection of dental personnels against all sorts of infectious diseases. After a survey was conducted from March 29 through May 9, 2001, the following findings were obtained: (1) The dental personnel group that had worked for 5 to less than 7 years took health examination the most, with 71.4%, before joining the hospital, and 61.5% of those who had served for 9 years or more, the greatest percentage, took medical checkup after joining the hospital(P<0.05). (2) The largest percentage of being inoculated against the anti-B type hepatitis after employment was 71.4% of the dental personnels over 35, and 57.7% of those with 9-year or more working experiences. Therefore, their age and service term made a significant difference(P<0.05). (3) There was no significant gap in personal protection practices between the dental hygienists and nurse aids(P>0.05). But the use of safety goggles for treatment was not properly done, with just 1.48 on the basis of 3 points, whereas the use of mask was scored 2.40 and the use of gloves was scored 1.96. (4) After a narcotic was used once, the leftovers were wasted by 89.7% of the dental hygienists and 70.0% of the nurse aids. More dental hygienists wasted them(P<0.05). (5) For hand washing, antibiotic liquid soap was used by 19.3% of the dental hygienists and 10.0% of the nurse aids, which were both very low percentage. And just 37.2% of the dental hygienists utilized a disposable paper towel to dry their hands, and only 36.0% of the nurse aids used the same(P<0.05).

Control of Human Clonorchiasis at Gokseong-gun and Sunchang-gun near the Sumjin River in Korea (섬진강 유역 곡성군, 순창군 지역 주민의 간흡충증 관리)

  • Objectives: This study was carried out to decrease the prevalence of human clonorchiasis and to evaluate the control effect in two Clonorchis sinensis-endemic area of Gokseong-gun and Sunchang-gun adjacent to the Sumjin River in Korea. Methods: The formalin-ether concentration method for stool egg examination or ELISA was applied for the diagnosis of clonorchiasis. As a primary survey, according to the non-probability sampling, a total of 1,2.13 inhabitants at Gokseong-gun were screened through the stool examination, and 1,004 inhabitants at Sunchang-gun were screened through the ELISA. The humans infected with C. sinensis were medicated with praziquantel and educated for the prevention of reinfection with the fluke. After 9 months, as a secondary survey, each prevalence of 616 inhabitants at Gokseong-gun and 2.637 inhabitants at Sunchang-gun was followed-up for the decrease of human clonorchiasis. Results: The prevalence before the mass control was 39.0% at Gokseong-gun and 30.1% at Sunchang-gun in average from 61.5% to 8.9% according to the villages (Myeon) of the survey. In the riverside villages to the Sumjin River the prevalences were higher than other villages located far from the river. The prevalence after the control was decreased to the level of 22.4% at Gokseong-gun(P<0.0001) and 16,3% at Sunchang-gun (P<0.0001). Conclusions: These results suggested that human clonorchiasis was still highly endemic in riverside area of the Sumjin River and could be decreased through the control activities such as diagnosis, medication and education. It was highly recommended that a integrated control such as those of the present study must be adopted in other localities along the Sumjin River for the eradication of human clonorchiasis.

The effect of chronic diseases and oral health behavior of economic workers on oral diseases (경제활동자들의 만성질환과 구강건강행태가 구강질환에 미치는 영향)

  • This study investigated 2,088 economically active workers aged 20 to 65 years selected from the sixth Korea National Health and Nutrition Examination Survey to evaluate the relationship between chronic diseases and oral health behaviors among economic workers and examine the risk factors for oral diseases. The results of the study were as follows. Dental caries were related to subjective oral health status, toothache experience, orthodontic treatment experience, chewing problems, speaking problems, oral examination, and dental clinic use. Periodontal disease was associated with oral health status, toothache experience, orthodontic experience, and chewing and speaking problems. The risk factors for chronic diseases and oral health behavior were as follows. Hypertension, 1.37 times; obesity, 1.48 times; diabetes, 1.5 times; low HDL-cholesterol, 1.31 times; hepatitis A antibody, 1.53 times higher. Health status was found to be 1.70 times higher in dental caries and 2.10 times higher in periodontal disease. The tooth experience was 1.30 times higher in dental caries and 1.35 times higher in periodontal disease. Problems chewing were 1.76 times for dental caries and 1.78 times for periodontal disease. The possibility of dental caries and periodontal disease was 1.44 times higher. Smokers were 1.61 times more likely to have dental caries and 1.63 times more likely to have periodontal disease. These results suggest that periodontal diseases increase the risk of chronic diseases and oral health behavior increases the risk of dental caries and periodontal disease. Although dental caries seem to have a lower risk of becoming a chronic disease if only oral health care is good, it is considered that both chronic disease and oral health care are important for periodontal disease.

Development of Rule-based Expert System for Interpretative Report with Health Screening Tests (건강검진자를 대상으로 해석적 보고를 위한 전문가 시스템의 개발)

  • Background : Interpretative reporting is an important aspect of laboratory medicine. The large menu of laboratory tests available today makes it increasingly difficult for the non-specialist to order and interpret all laboratory tests. The aim of this study was to determine the usefulness of an expert system to interpret laboratory tests and help physicians order the appropriate tests. Materials and Methods : In order to interpret laboratory tests, a rules-based expert system was developed. In this module, if-then rules were used to interpret the given test result patterns (e.g. urinalysis, anemia, hepatitis B virus, hypercholesterolemia, glucose, syphilis, and tumor markers) and select matching text elements. The system was used to evaluate 535 subjects who visited a health-check program. Results : The overall abnormal rate was 50.5% in the expert system; 34% for cholesterol, 9.9% for urinalysis, 8.0% for anemia, 7.7% for thyroid function tests, 4.5% for tumor marker study, 4.7% for hepatitis virus antigen, 4.3% for serum glucose, and 1.1% for syphilis. Conclusion : These results indicate that the application of the expert system for the interpretation of laboratory tests may provide a useful method for the interpretation of reports. However more rules are needed for the application to in-patients.
